Long-Term Electromyographic Monitoring of the Stapedius Reflex via Implanted Electrodes in Sheep: Toward Objective Autonomous Cochlear Implant Fitting.

Objective fitting measures offer a means to circumvent the subjectivity of cochlear implant programming, with the stapedius reflex representing one robust predictor of the maximum comfortable loudness level. With the present study, it was investigated whether long-term electromyographic measurements of the stapedius muscle using implanted electrodes are feasible. In nine sheep, myoelectrical activities were recorded intraoperatively and synchronized with middle ear admittance as a reference signal. For acoustic stimulation pure tones with different frequencies were used. The electrodes were pl

Persistent Stapedial Artery, Facial Nerve Overhang, and Calcified Stapedius Tendon Leading to Aborted Stapedotomy: An Unusual Middle Ear Anatomy.

This report describes a rare medical case of a 16-year-old girl with progressive bilateral hearing loss over nine years. Despite normal external ear anatomy, tests revealed moderate conductive hearing loss. Surgery uncovered an unusual persistent stapedial artery and ossified stapedial tendon with an overhang of the facial nerve over the oval window obscuring it and preventing the planned stapedotomy. Post-surgery, the patient had mild earache but no serious complications and was treated with antibiotics and steroids. She is scheduled for a hearing aid trial. This is the first known report of

Accessing the stapedius muscle via novel surgical retrofacial approach during cochlear implantation surgery: Intraoperative results on feasibility and safety.

Human stapedius muscle (SM) can be directly and safely accessed via retrofacial approach, opening new approaches to directly measure the electrically evoked stapedius reflex threshold (eSRT). The measurement of the SM activity via direct surgical access represents a potential tool for objective eSRT fitting of cochlear implants (CI), increasing the benefit experienced by the CI users and leading to new perspectives in the development of smart implantable neurostimulators. 3D middle-ear reconstructions created after manual segmentation and related SM accessibility metrics were evaluated before

The Relationship Between the Electrically Evoked Stapedius Reflex Threshold and Stimulus Burst Duration in Pediatric Cochlear Implant Users: Preliminary Data.

INTRODUCTION: The electrically evoked stapedius reflex threshold (eSRT) is an objective measure that helps to fit cochlear implants (CIs). The primary aim of this study was to investigate the relationship between different burst durations to elicit eSRT in pediatric CI users. MATERIALS AND METHODS: Different stimuli burst durations (100-500 ms) were applied to observe the effect in the reflex determination in 11 pediatric CI users. Reflexes were measured in electrode contacts #2, #6, #9, and #11. Testing was conducted at 9 months after CI activation. RESULTS: Burst duration and electrode con

The use of a surgical planning tool for evaluating the optimal surgical accessibility to the stapedius muscle via a retrofacial approach during cochlear implant surgery: a feasibility study.

PURPOSE: During cochlear implant (CI) surgery, visual detection of the stapedius reflex as movements of the stapes tendon, electrically elicited via the CI, is a standard measure to confirm the system's functionality. Direction visualization of the stapedius muscle (SM) movements might be more reliable, but a safe access to the small SM is not defined. A new surgical planning tool for pre-operative evaluation of the accessibility to the stapedius muscle (SM) during a cochlear implantation (CI) via a retrofacial approach was now evaluated. METHODS: A surgical planning tool was developed in MAT

Effect of the acoustic reflex on inner ear damage induced by industrial noise.

Shipyard noise is a variable noise which induces permanent threshold shift (PTS) in exposed workers. In humans, the stapedius reflex has been found to be very stable in this type of exposure. Temporary threshold shift (TTS) in the absence of stapedius reflex has been found to extend downwards through the speech-frequencies instead of showing a high frequency dip as when the stapedius reflex is normal. The features of PTS produced by the same type of noise was investigated in rabbits with and without functioning middle ear muscles. The auditory sensitivity was measured by auditory brain stem re

[Examinations of the thresholds of discomfort of the ears treated by stapedectomy (author's transl)].

Ears treated by stapedectomy often are extremely irritated by noise. To clear up this phenomenon were plotted the thresholds of discomfort by tone- and speechaudiograms in the operated ears as well as in the non-operated. All our patients being examined had the stapedius muscle tendons cut. There was no clinical otosclerosis in the non-operated ears and the audition was the same on either side. A significant increase of the discomfort threshold was seen in the operated ears (93.8% in the toneaudiograms and 78.1% in the speech audiograms). In the tone audiograms the frequencies of 500, 1000 and

The biphasic acoustic reflex: a new perspective.

The anatomic, neurologic, and physiologic characteristics of the middle ear structures have created confusion concerning the nature of the intraaural muscle reflex. After reviewing the relevant literature, we correlated this information with our studies of human temporal bones and computer analyses of the acoustic reflex responses of individuals with normal hearing. Our hypothesis is that although the stapedius is the initiator of the reflex and the primary contributor to ossicular chain fixation, the tensor tympani is responsible for the major observed response. The negative deflection presen

Voluntary eardrum movement: a marker for tensor tympani contraction?

HYPOTHESIS: Voluntary eardrum movement (VEM) and resultant tympanometric changes reflect tensor tympani (TT) contraction. BACKGROUND: TT contraction has been hypothesized to cause symptoms of aural fullness, tinnitus, clicking, and even vertigo despite the lack of understanding of how it functions or what causes it to contract. Identifying tympanometric changes unique to TT contraction can provide a diagnostic tool for identifying its role in pathologic conditions. METHODS: Various tympanometric measurements were performed on human subjects who could voluntarily move their eardrums. These we

[Contralateral stapedial reflex in children, adults and high-frequency loss hearing impaired patients (author's transl)].

Contralateral stapedius reflexes were measured in 332 patients. Results were analysed and statistically checked with regard to amplitude, slope of reflex in the recovery phase and partial and total reflex area. The results seem to indicate that none of the measure parameters have any direct diagnostic value. Optimal results measuring contralateral stapedius reflex are to be expected at 1000 Hz.

[Otoneurologic testing in multiple sclerosis].

The values of the individual audiological and vestibular examination methods in multiple sclerosis (MS) diagnosis are discussed. Our experience shows that the most accurate indications are provided by acoustic stapedius reflex, brainstem auditory evoked potentials (BAEPs) and vestibular investigation. Other testing processes play only a minor part. Using the three methods mentioned, brainstem injuries can be shown to be present at an early stage. Hence one should always include them in MS-diagnosis as a matter of routine. Of the 85 patients we examined, 72 had definite MS and 13 probable (sub-

Mechanics of the middle ear in otosclerosis and stapedoplasty.

Seventy-three otosclerotic patients, 138 subjects who had undergone stapes surgery (50 total stapedectomies, 43 stapedotomies with stapedius tendon section and 45 stapedotomies with stapedius tendon preservation) and 70 normal controls were investigated by susceptance multiple frequency tympanometry. The aims were to evaluate the efficiency of this procedure in diagnosing otosclerosis and to assess the resonance properties of middle ears undergoing different types of stapedectomy. Tympanometry was performed using sweep frequencies ranging from 226 to 2,000 Hz for each pressure step of 12.5 daP

Feasibility of endoscopic treatment of middle ear myoclonus: a cadaveric study.

Stapedius and tensor tympani tenotomy is a relatively simple surgical procedure commonly performed to control pulsatile tinnitus due to middle ear myoclonus and for several other indications. We designed a cadaveric study to assess the feasibility of an entirely endoscopic approach to stapedius and tensor tympani tenotomy. We performed this endoscopic ear surgery in 10 cadaveric temporal bones and summarized our experience. Endoscopic stapedius and tensor tympani section is a new, minimally invasive treatment option for middle ear myoclonus that should be considered as the first line surgical

CPA tumors with normal routine audiometry and positive reflex and BSER tests.

Recently, in a ten-day-period, three patients whose hearing was within normal limits with good speech discrimination were found to have large cerebellopontine angle tumors. There were neurologic symptoms in only one patient. In these three cases, stapedius reflexes were absent with the affected ear stimulated. Brain stem evoked responses (BSER) were clearly abnormal in the three cases. Postoperative brain stem testing showed essentially no change in the nonoperated ear owing to brain stem manipulation. In two cases hearing was preserved. In one patient, the brain stem recordings returned to ne
