Brain edema in liver failure: basic physiologic principles and management.

In patients with severe liver failure, brain edema is a frequent and serious complication that may result in high intracranial pressure and brain damage. This short article focuses on basic physiologic principles that determine water flux across the blood-brain barrier. Using the Starling equation, it is evident that both the osmotic and hydrostatic pressure gradients are imbalanced across the blood-brain barrier in patients with acute liver failure. This combination will tend to favor cerebral capillary water influx to the brain. In contrast, the disequilibration of the Starling forces seems

Effect of thermal stress on cardiac function.

Whole-body heating decreases pulmonary capillary wedge pressure and cerebral vascular conductance and causes an inotropic shift in the Frank-Starling curve. Whole-body cooling increases pulmonary capillary wedge pressure and cerebral vascular conductance without changing systolic function. These and other data indicate that factors affecting cardiac function may mechanistically contribute to syncope during heat stress and improvements in orthostatic tolerance during cold stress.

Interstitial fluid pressure in the pulp of the rat incisor.

This was measured in inactin-anaesthetized rats, using a cotton-wick technique. Access to the pulp was through an opening produced by repeated shortening of the incisor under anaesthesia. The pressure was subatmospheric and behaved as predicted by Starling's hypothesis. In teeth which had not been opened to the atmosphere, the pressure measured through a hole drilled during the experiment was approx. 300 Pa higher than in the repeatedly shortened teeth, but was still lower than the pressures recorded in teeth of limited eruption.

Extracorporeal cephalic blood autoperfusion method in the rat. Hemorrheological evaluation and proposed pharmacological uses.

We propose a constant cephalic blood perfusion method especially suitable to the rat. Cerebrovascular blood pressure is artificially regulated by an extracorporeal circuit with a perfusion pump and a Starling valve. A hemorrheological evaluation allows one to demonstrate an extracorporeal circuit that shows a growing pathological evolution, which can be purposely accelerated by iterative ischemic episodes. This method can be used (as shown by some examples) as a model for cerebrovascular failure or to study deleterious effects of the extracorporeal circuit.

A single exposure to an organophosphate insecticide disrupts developmental patterns of hippocampal cell proliferation and overall brain size in a migratory songbird.

Developmental exposure to neurotoxic insecticides, such as the organophosphate chlorpyrifos (CPF), can cause deleterious effects on the brain and behaviour of non-target organisms. Migratory birds are at an increased risk of exposure due to their extensive space use across seasons. However, there is limited understanding of how acute sublethal exposure to neurotoxic insecticides affects neural development that underpin fitness-related behaviours such as migration. Successful migration relies on spatial cognition, which is controlled in part by the hippocampus, a brain region on the dorsal surf

Endemic infection of the common mynah Acridotheres tristis with Trichomonas gallinae the agent of avian trichomonosis.

Avian trichomonosis is an architypal disease of wild columbids and those birds that predate them. Increasingly though, it has been reported in passerines; a recent and ongoing epidemic in the chaffinches and greenfinches of Europe and outbreaks amongst house finches, American goldfinches and purple finches in North America. The parasite, Trichomonas gallinae, causes lesions in the upper respiratory tract which can cause mortality associated with dehydration and emaciation. This paper reports for the first time, the widespread, endemic and often asymptomatic infection of common mynah (Acridothe

["Intravascular space contracted-tissue space expanded": interstitial edema and fluid management in intensive care medicine].

In the second phase of shock therapy, we regularly find fluid overload with edema in our patients, which not only involves the skin and interstitial tissue but can also impair kidney, liver and pulmonary function. Revision of the Starling principle and new insights into physiology of the endothelial glycocalyx have important implications for fluid therapy in intensive care medicine. Determination of fluid overload and an appropriate therapy with either diuretics or ultrafiltration are the focus of "late goal-directed fluid removal" management.

The alveolar edema equation.

Pulmonary edema is excessive liquid accumulation in the alveolar air spaces and interstitium. This reduces gas exchange leading to increased morbidity and mortality. Clearance of pulmonary edema fluid relies on reabsorption into the alveolar interstitium and then either to the alveolar capillaries or to the lung lymphatics. How it reaches the lymphatics has been a puzzle since 1896. Our 2D model of an interstitial strip resolves that puzzle. We also show that alveolar interstitial pressures differ from values used in traditional medical textbooks. Our approach employs detailed fluid mechanical

Normovolemia defined according to cardiac stroke volume in healthy supine humans.

BACKGROUND: Both hypovolemia and a fluid overload are detrimental for outcome in surgical patients but the effort to establish normovolemia is hampered by the lack of an operational clinical definition. Manipulating the central blood volume on a tilt table demonstrates that the flat part of the Frank-Starling curve is reached when subjects are supine and that finding may be applicable for a clinical definition of normovolemia. However, it is unknown whether stroke volume (SV) responds to an increase in preload induced by fluid administration. METHODS: In 20 healthy subjects (23 +/- 2 years, m

A model of human microvascular exchange.

A compartmental model consisting of the circulation, a general interstitium, and the lymphatics, is formulated to describe the transport and distribution of fluid and plasma proteins (albumin) in the human microvascular exchange system. Transcapillary mass exchange is assumed to occur via a coupled Starling mechanism. Unknown or poorly quantified model parameters are estimated by statistical fitting of simulation predictions to five different sets of experimental data. The data consist of steady-state and transient plasma and interstitial volumes and colloid osmotic pressures measured under la

Better the devil you know: avian predators find variation in prey toxicity aversive.

Toxic prey that signal their defences to predators using conspicuous warning signals are called 'aposematic'. Predators learn about the toxic content of aposematic prey and reduce their attacks on them. However, through regulating their toxin intake, predators will include aposematic prey in their diets when the benefits of gaining the nutrients they contain outweigh the costs of ingesting the prey's toxins. Predators face a problem when managing their toxin intake: prey sharing the same warning signal often vary in their toxicities. Given that predators should avoid uncertainty when managing

Ectoparasite Activity During Incubation Increases Microbial Growth on Avian Eggs.

While direct detrimental effects of parasites on hosts are relatively well documented, other more subtle but potentially important effects of parasitism are yet unexplored. Biological activity of ectoparasites, apart from skin injuries and blood-feeding, often results in blood remains, or parasite faeces that accumulate and modify the host environment. In this way, ectoparasite activities and remains may increase nutrient availability that may favour colonization and growth of microorganisms including potential pathogens. Here, by the experimental addition of hematophagous flies (Carnus hemapt

[Oncotic pressure and hemodilution].

The appropriate fluid therapy in neurosurgical patients remains an area of disagreement between neurosurgeons and anaesthesiologists. Fluid restriction has long been practiced in patients with brain pathology, in order to reduce or prevent the formation of cerebral oedema. This grows from a fear that rapid administration of fluids, particularly noncolloidal fluids, can enhance cerebral oedema, although there is a lack of experimental evidence to substantiate this belief. On the other hand, fluid restriction can lead to relative hypovolaemia, causing haemodynamic instability during anaesthesia

Experience-dependent neuronal specialization and functional organization in the central auditory area of a songbird.

The effect of early experience on brain development was investigated in the central auditory area of a songbird, the field L complex, which is analogous to the mammalian auditory cortex. Multi-unit recordings of auditory responses in the field L complex of adult starlings raised without any experience of adult song during development provide strong evidence of developmental plasticity both in the neuronal responses and in the functional organization of this area. Across the entire area, experimental birds, separated from adults from the age of 1 week old until they were 2 years old, had a much

Dynamics of pleural fluid effusion and chylothorax in the fetus and newborn: role of the lymphatic system.

Pleural fluid effusion particularly chylothorax is a relatively rare occurrence in the newborn, but when it occurs it is often life-threatening. In this article, we describe and illustrate the morphologic features of the visceral and parietal pleura including pleural lymphatics and the physiology and pathophysiology of pleural fluid balance. The role and function of the lymphatic system in controlling the volume and composition of pleural liquid are detailed and a conceptual scheme presented. Finally, the crucial role of inadequate lymphatic drainage (either functional overload from an imbalan

Physiological consequences of normal atrioventricular conduction: applicability to modern cardiac pacing.

Properly timed atrial systole may alter systolic performance by the mechanism of Starling's law of the heart, which states that the extent of systolic myocardial fiber shortening is dependent on the degree of diastolic fiber stretch, or preload. However, the atrial contribution to physiological need and thus is critical in developing the proper cardiac output response to exercise. Current generations of permanent pacemakers, which may have a combination of dual-chamber and sensor technologies, will permit the restoration of normal cardiac physiology (AV synchrony and/or rate response) in most

Crystalloid and colloid therapy.

Fluid therapy is a cornerstone of emergency medicine, but equine practitioners should be aware of recent developments that have modified previous recommendations. First, new emphasis on the avoidance of hyperchloremia suggests that crystalloids with a lower chloride concentration may be more appropriate for use. Second, modifications to the understanding of the Starling equation suggest that the benefits of colloids may be more limited than previously thought. In addition, the negative effects of fluid overload on morbidity and mortality are becoming increasingly recognized. Although more spec

Upper airway collapsibility: measurement techniques and therapeutic implications.

Several techniques are currently available that aim to characterize upper airway function/mechanics during wakefulness or sleep. Based on the concept of a Starling resistor, we propose a standardized protocol to measure the critical pressure (Pcrit) (an indicator of upper airway collapsibility) during sleep. The effect of therapeutic interventions such as weight loss, positional changes or uvulopalatopharyngoplasty on Pcrit is illustrated by data from the literature. We propose that measurement of Pcrit become implemented in the diagnostic work-up of selected patients with sleep-related breath

Lung heparan sulfates modulate K(fc) during increased vascular pressure: evidence for glycocalyx-mediated mechanotransduction.

Lung endothelial cells respond to changes in vascular pressure through mechanotransduction pathways that alter barrier function via non-Starling mechanism(s). Components of the endothelial glycocalyx have been shown to participate in mechanotransduction in vitro and in systemic vessels, but the glycocalyx's role in mechanosensing and pulmonary barrier function has not been characterized. Mechanotransduction pathways may represent novel targets for therapeutic intervention during states of elevated pulmonary pressure such as acute heart failure, fluid overload, and mechanical ventilation.
