Laryngeal neuroma in a MEN-2B paediatric patient.

Multiple endocrine neoplasia type 2B (MEN-2B) syndrome is a genetic disorder that predisposes to the development of tumours in the mouth, eyes and endocrine glands. It is associated with mucosal neuromas; however, laryngeal mucosal neuromas are very rare. We describe an adolescent with MEN-2B who presented with dysphonia, nocturnal stridor and exertional dyspnoea following a total thyroidectomy for an invasive medullary thyroid carcinoma with right recurrent laryngeal nerve sacrifice.Following initial assessment, laryngeal stroboscopy was performed, followed by a microlaryngobronchoscopy under

Bamboo Nodes as an Autoimmune Disease Laryngeal Manifestation. Case Series.

INTRODUCTION: Bamboo nodes are transverse creamy-yellow subepithelial nodes in the vocal folds (VF) midpoint, usually bilateral, resembling a bamboo stem. They appear almost exclusively in females, and are associated with underlying autoimmune diseases. CASE SUMMARY: Six female patients, 45.5 years median age, with underlying autoimmune diseases, consulted due to dysphonia. The laryngeal stroboscopy showed bilateral VF bamboo nodes in four patients, and unilateral in the remaining two. VF mobility was normal in all patients, while the mucosal wave was impaired in four of them. Treatment with

Managing dysphonia in paediatric patients with complex airway conditions.

OBJECTIVE: To suggest a phonosurgical management strategy that can be used for children who have previously undergone laryngotracheal reconstruction. METHODS: This cases series describes three children who presented with complex, multi-level airway stenosis and marked dysphonia. Phonosurgical intervention involved endoscopic and open approaches, and was combined with voice therapy. A phonosurgical reconstruction management algorithm is suggested for evaluating and treating these complex conditions. RESULTS: Pre-operative assessment is critical, and should involve voice analysis and glottal a

The role of glottal gap in predicting aspiration in patients with unilateral vocal paralysis.

Aspiration has been frequently noted among patients suffering from vocal-fold paralysis. The association between a defect of glottal closure and aspiration in patients with unilateral vocal paralysis appears to have been rarely reported. The current study reviews collected data consisting of videotaped flexible nasoendoscopy and stroboscopy for patients with a unilateral vocal-fold paralysis. Mean normalized glottal gap areas for patients suffering vocal paralysis with associated aspiration are significant greater than that for the non-aspiration group. Subsequent to the surgical correction of

Botulinum toxin: helpful adjunct to early resolution of laryngeal granulomas.

Treating laryngopharyngeal reflux is the most accepted treatment for laryngeal granulomas. However, response to this treatment is prolonged and in some cases persistent. The authors hypothesize that this is due to the continuous trauma to the mucoperichondrium from adduction of the vocal folds thus preventing regeneration of healthy mucosa. A prospective study was performed on six patients with laryngeal granulomas failing prolonged laryngopharyngeal reflux treatment, speech therapy, and surgical excisions. Botulinum toxin A was injected into the affected thyroarytenoid to reduce adduction tra

Hoarseness as the initial manifestation of systemic lupus erythematosus.

A 51-year-old woman complained of hoarseness of two years duration. The patient's past medical history was significant for autoimmunological hepatitis and arthritis for which she had not received treatment. Laryngoscopy and laryngeal stroboscopy revealed 'bamboo joint-like nodules' on both true vocal folds. These nodules resembled rheumatoid nodules and were suggestive of a collagen disease. Previous reports have documented that the treatment for such conditions related to collagen diseases is surgical resection. However, we initially attempted to treat the laryngeal lesions systemically with

Vocal dysfunction following cricothyroidotomy: a prospective study.

Voice dysfunction is a known consequence of cricothyroidotomy, but few detailed analyses have been published. This study reports an investigation of voice function in 19 patients who underwent cricothyroidotomy during a 12-month period (54 were patients operated on, 29 survived the primary disease or injury, and 19 agreed to participate in the investigation). The patients were interviewed regarding signs of dysfunction of the cricothyroid muscles, or a diminished external tensor effect on the vocal folds. Laryngoscopy with a magnifying-angled endoscope, stroboscopy, phonetography, determinatio

A critical review of electroglottography.

The technique of electroglottography is reviewed from the perspective of a laboratory instrument for assessing laryngeal function, a device to assist speech and speaker recognition, and as a potential diagnostic aid in the clinic. A description of the electronic functioning of the electroglottograph (EGG) is provided. Considerable emphasis is given to contemporary research which has focused on laryngeal assessment using the EGG. Methods for validating and aiding the interpretation or reading of the EGG are discussed, including photoglottography, stroboscopy, ultrahigh-speed laryngeal cinematog

Iron Pill-Induced Chemical Laryngitis.

OBJECTIVE: To discuss the presentation and management of pill-induced chemical laryngitis by illustrating a rare case. METHODS: We report a unique case of a patient with iron pill-induced laryngitis. RESULTS: A 71-year-old male presented for evaluation of dysphonia. Five weeks prior, the patient had reportedly aspirated an iron pill. The pill was lodged in his throat for several hours before being coughed up, soft but still intact. Since that event, the patient noted complete voice loss and in clinic was found to have a very breathy and asthenic voice. Stroboscopy revealed aperiodicity with

Stroboscopic assessment of unilateral vocal fold paralysis: a systematic review.

PURPOSE: To review the relevant basic stroboscopic evaluations in unilateral vocal fold paralysis (UVFP). Our aim was twofold: (1) to determine the frequency of use of stroboscopic parameters in outcome evaluation after surgical treatment of UVFP using a Pareto diagram; and (2) to select the most relevant parameters in terms of a significant difference between pre- and post-surgical intervention for UVFP. METHODS: A systematic review in PUBMED includes studies on stroboscopic evaluation in combination with UVFP and surgical treatment. The review was limited to English studies published betwee

Stroboscopic light stimulation safety within and beyond laboratory settings: Observational evidence and practical guidance.

Stroboscopic light stimulation (SLS) reliably evokes geometric visual hallucinations and, in some contexts, altered-state experiences. Its increasing use in research, public, recreational and exploratory clinical settings creates a need for proportionate safety guidance. The main concern is a visually triggered seizure in a susceptible individual; reported non-epileptic responses include discomfort or distress. We combined a retrospective survey of safety practices and reported events across four laboratories, provider-reported operational data from two commercial SLS platforms, and a focused

[Comparison of cepstral coefficients to other voice evaluation parameters in patients with occupational dysphonia].

UNLABELLED: BACKGROUND Special consideration has recently been given to cepstral analysis with mel-frequency cepstral coefficients (MFCCs). The aim of this study was to assess the applicability of MFCCs in acoustic analysis for diagnosing occupational dysphonia in comparison to subjective and objective parameters of voice evaluation. MATERIALS AND METHODS: The study comprised 2 groups, one of 55 female teachers (mean age: 45 years) with occupational dysphonia confirmed by videostroboscopy and 40 female controls with normal voice (mean age: 43 years). The acoustic samples involving sustained v

Vocal-fold leukoplakia and dysplasia. Mini-review by the French Society of Phoniatrics and Laryngology (SFPL).

Vocal-fold leukoplakia and dysplasia are together designated "epithelial hyperplastic laryngeal lesions" (EHLL). Work-up and follow-up are founded on optical examination with high-definition imaging, stroboscopy and narrow-band imaging. Diagnosis is based on pathology, using the new 2017 WHO classification, dichotomizing "low grade" and "high grade". Statistically, the risk of cancerous progression is 20% within 5 to 10 years of diagnosis, or more in over-65 year-old males; risk for any given patient, however, is unpredictable. Research focuses on the genetic criteria of the lesion and charact

Case 223: Arytenoid Dislocation.

A 71-year-old man with a history of atrial fibrillation refractory to medical therapy and lung cancer status after left upper lobectomy presented to our hospital for elective cardioversion and rate control with tikosyn. Overnight, the patient became unresponsive and was found to be in a state of cardiogenic shock. A code was called, and he was stabilized after cardioversion and bedside intubation. His stay in the intensive care unit was complicated by ventilator-associated pneumonia. The patient subsequently underwent multiple failed extubation attempts, requiring two additional reintubations.

Quantum molecular resonance-assisted phonomicrosurgery: preliminary experience.

The objective of this study was to evaluate the use of quantum molecular resonance (QMR) energy in phonomicrosurgery. Quantum molecular resonance energy (QMRE) is an innovative technology that provides low temperature cutting and coagulation of tissues and causes minimal tissue damage during the procedure. Because of these features, this technology may offer new possibilities in phonomicrosurgery. Twelve patients with vocal fold polyps underwent QMR-assisted phonomicrosurgery. The patients were evaluated before and after surgery at 1 and 3 months postoperatively by using the voice handicap ind

A wavelet-based approach for a continuous analysis of phonovibrograms.

Recently, endoscopic high-speed laryngoscopy has been established for commercial use and constitutes a state-of-the-art technique to examine vocal fold dynamics. Despite overcoming many limitations of commonly applied stroboscopy it has not gained widespread clinical application, yet. A major drawback is a missing methodology of extracting valuable features to support visual assessment or computer-aided diagnosis. In this paper a compact and descriptive feature set is presented. The feature extraction routines are based on two-dimensional color graphs called phonovibrograms (PVG). These graphs

Diagnosis and treatment of persistent dysphonia after laryngeal surgery: a retrospective analysis of 62 patients.

Sixty-two patients with persistent or recurrent dysphonia after laryngeal surgery underwent interdisciplinary voice evaluation, laryngostroboscopy, and objective measurements of vocal function. The causes of persistent dysphonia were attributed to vocal fold scarring (n = 22), residual mass lesion (n = 8), residual inflammation (n = 13), recurrent mass (n = 4), and hyperfunctional voice disorder (n = 7). Laryngoscopy often showed excessive ventricular compression and anterior-to-posterior laryngeal compression. Ventricular dysphonia was often a compensatory gesture in response to poorly mobile
