[Stuttering: effects of genes and early treatment].

As stuttering by most persons resolves after a time, an initial wait-and-see policy has been maintained up to now. Two recent important developments in the area of stuttering have occurred: the discovery of gene mutations that appear to be of relevance to the developmental-neurological abnormality and growing evidence that early intervention helps recovery. The mutated genes found are GNPTAB, GNPTG and NAGPA. They are involved in lysosomal decomposition. Published study results show that early treatment using either the Demands and Capacities Model (indirect treatment aimed at the child's surr

Normal interhemispheric inhibition in persistent developmental stuttering.

Imaging studies suggest a right hemispheric (pre)motor overactivity in patients with persistent developmental stuttering (PDS). The interhemispheric inhibition (IHI) studied with transcranial magnetic stimulation is an established measure of the interplay between right and left motor areas. We assessed IHI in 15 young male adults with PDS and 15 age-matched fluent-speaking subjects. We additionally studied the ipsilateral silent period (iSP) duration. We found no significant between-group difference for IHI or for iSP duration. We conclude that the interplay between the primary motor cortices

Jaw-phonatory coordination in chronic developmental stuttering.

UNLABELLED: A deficiency in sensorimotor integration in a person who stutters may be a factor in the pathophysiology of developmental stuttering. To test oral sensorimotor function in adults who stutter, we used a task that requires the coordination of a jaw-opening movement with phonation onset. The task was adapted from previous limb coordination studies, which show that movement coordination depends on intact proprioception. We hypothesized that adult stutterers would show deficient jaw-phonatory coordination relative to control participants. The task required initiation of phonation as a j

Differential diagnosis of stuttering for forensic purposes.

PURPOSE: This case study demonstrates the application of an assessment protocol for differential diagnosis of psychogenic stuttering, neurogenic stuttering, developmental stuttering, and malingering. METHOD: A male in his late 30s, accused of armed robbery, was evaluated for stuttering at the request of his defense attorney. The speech assessment included 4 main sections: collection of speech samples, observation in multiple speaking conditions, evaluation of communication attitudes, and consideration of case history and background information. RESULTS: The defendant stuttered severely in al

A phenomenological understanding of successful stuttering management.

UNLABELLED: The purpose of this investigation was to understand, from the perspective of the speaker, how seven adults have been able to successfully manage their stuttering. Individual experiences were obtained across the three temporal stages (past, transitional, and current). Recurring themes were identified across participants in order to develop an essential structure of the phenomena at each stage. The ability to make the transition from unsuccessful to successful management of stuttering was associated with six recurring themes of: (1) support, (2) successful therapy, (3) self therapy a

Statistical power in stuttering research: a tutorial.

The capacity to make reliable inductive statements about populations is critical for the advancement of scientific knowledge. An important contribution to that advancement of knowledge is determining that effects are either present or not present in populations. Statistical power is an important methodological qualification for any research that presents statistical results, and particularly so for research that presents null results. In this paper we describe the statistical concept of power, outline parameters of research that influence it, and demonstrate how it is calculated. With referenc

Administering stuttering modification therapy in school settings.

Stuttering modification techniques can be used effectively in the school setting. In fact, speech-language pathologists can use this environment to their advantage to address not only disfluency but also the emotional and behavioral aspects of stuttering. Toward this end, stuttering modification goals and techniques are outlined, with a discussion of how to adapt them to school environments. Specific topics include writing clear, measurable IEP goals, taking advantage of the school setting to implement these goals, administering stuttering modification within a group therapy mode, involving pa

Prediction of outcome after treatment for stuttering.

Predicting who will relapse after behavioural or dynamic psychotherapy is important. A search for variables likely to predict individuals at risk of relapse was conducted in two groups of successfully treated stutterers. The most powerful predictors were the attainment of three goals by the last day of treatment; namely, skill mastery as evidenced by no stuttering, normal attitudes to communication, and an internalisation of the locus of control. Of the subjects who achieved these three goals, 97% maintained their improved speech in the long term. No subject who failed to achieve any of these

Decreased Cerebellar-Orbitofrontal Connectivity Correlates with Stuttering Severity: Whole-Brain Functional and Structural Connectivity Associations with Persistent Developmental Stuttering.

Persistent developmental stuttering is characterized by speech production disfluency and affects 1% of adults. The degree of impairment varies widely across individuals and the neural mechanisms underlying the disorder and this variability remain poorly understood. Here we elucidate compensatory mechanisms related to this variability in impairment using whole-brain functional and white matter connectivity analyses in persistent developmental stuttering. We found that people who stutter had stronger functional connectivity between cerebellum and thalamus than people with fluent speech, while st

Is covert articulation impaired in developmental stuttering?

Adults who stutter (AWS) have been reported to show difficulties in nonword repetition tasks, where covert articulation supports the maintenance of phonological information during the retention interval. However, it remains unclear whether these difficulties reflect impaired covert articulation, auditory-phonological encoding, overt articulation, or aspects of executive function/attention. It is also unknown whether such difficulties characterize any, some, most, or all AWS. To address these questions, we administered spoken and written versions of a phonological memory task-both using printed

Large-scale genome-wide analyses of stuttering.

Developmental stuttering is a highly heritable, common speech condition characterized by prolongations, blocks and repetitions of speech. Although stuttering is highly heritable and enriched within families, the genetic architecture is largely understudied. We reasoned that there are both shared and distinct genetic variants impacting stuttering risk within sex and ancestry groups. To test this idea, we performed eight primary genome-wide association analyses of self-reported stuttering that were stratified by sex and ancestry, as well as secondary meta-analyses of more than one million indivi

Drug-induced stuttering: A comprehensive literature review.

Drug-induced stuttering (DIS) is a type of neurogenic stuttering (NS). Although DIS has not been reported as frequently as other cases of NS in the literature, it is not a negligible adverse drug reaction (ADR) which can significantly affect the quality of life if not treated. This literature review aims to evaluate the epidemiological and clinical characteristics of DIS and suggests some pathophysiological mechanisms for this ADR. Relevant English-language reports in Google Scholar, PubMed, Web of Science, and Scopus were identified and assessed without time restriction. Finally, a total of 6

Weak Vestibular Response in Persistent Developmental Stuttering.

Vibrational energy created at the larynx during speech will deflect vestibular mechanoreceptors in humans (Todd et al., 2008; Curthoys, 2017; Curthoys et al., 2019). Vestibular-evoked myogenic potential (VEMP), an indirect measure of vestibular function, was assessed in 15 participants who stutter, with a non-stutter control group of 15 participants paired on age and sex. VEMP amplitude was 8.5 dB smaller in the stutter group than the non-stutter group (p = 0.035, 95% CI [-0.9, -16.1], t = -2.1, d = -0.8, conditional R 2 = 0.88). The finding is subclinical as regards gravitoinertial function,

Stuttering: Understanding and Treating a Common Disability.

Childhood-onset fluency disorder, the most common form of stuttering, is a neurologic disability resulting from an underlying brain abnormality that causes disfluent speech. Stuttering can lead to significant secondary effects, including negative self-perception and negative perception by others, anxiety, and occasionally depression. Childhood-onset fluency disorder affects 5% to 10% of preschoolers. Early identification of stuttering is important so that therapy can begin while compensatory changes to the brain can still occur and to minimize the chances of the patient developing social anxie

Acquired stuttering due to recurrent anaplastic astrocytoma.

Acquired (neurogenic) stuttering is a rare phenomenon seen after cerebral infarction or brain injury. Aetiology of this symptom is unclear, but recent evidence supports that it is a disturbance in the left hemispheric neural network involving the interplay between the cortex and basal ganglia. We present the case of a patient who develops acquired stuttering after a recurrence of a right temporoparietal anaplastic astrocytoma (WHO grade III). We also review other cases of acquired stuttering and known anatomical correlates.

Novel tactile feedback to reduce overt stuttering.

Stuttering is generally considered to be a speech disorder that affects ∼1% of the global population. Various forms of speech feedback have been shown to reduce overt stuttered speaking, and in particular, second speech signal through speech feedback has drastically reduced utterances of stuttered speech in adults with persistent stuttering. This study reports data for increased overt fluency of speech in an adult stuttering population, whereby the vocalization of the speaker is captured by a microphone or an accelerometer, signal processed, and returned as mechanical tactile speech feedback t

Stuttering, cluttering, and phonological complexity: case studies.

UNLABELLED: The phonological complexity of dysfluencies in those who clutter and/or stutter may help us better understand phonetic factors in these two types of fluency disorders. In this preliminary investigation, cases were three 14-year-old males, diagnosed as a Stutterer, a Clutterer, and a Stutterer-Clutterer. Spontaneous speech samples were transcribed, coded for dysfluent words which were then matched to fluent words on grammatical class (i.e., function vs. content), number of syllables and word familiarity. An Index of Phonological Complexity was determined per word, and word frequency
