Ruptured middle cranial fossa arachnoid cysts after minor trauma in adolescent boys presenting with subdural hygroma: two case reports.

BACKGROUND: Intracranial arachnoid cysts are common, cerebrospinal fluid-filled, innocent lesions that are usually detected incidentally on brain imaging. They may rupture and complicate due to subdural hematoma or hygroma after minor trauma. Authors present two cases of ruptured middle cranial fossa arachnoid cysts in adolescent (12-year-old and 15-year-old) Afghan boys presenting with subdural hygroma after minor trauma. CONCLUSION: Imaging work-up is necessary for symptomatic patients following minor head trauma as incidentally detected ruptured intracranial arachnoid cysts can be responsi

Spontaneous rupture of an arachnoid cyst with subdural hygroma formation: a rare case report and literature review.

BACKGROUND: Arachnoid cysts are congenital, cerebrospinal fluid-filled sacs, predominantly asymptomatic and often detected incidentally. Spontaneous rupture of arachnoid cysts is an exceedingly rare complication, particularly in pediatric patients, and may present with acute neurological symptoms, including cranial nerve palsies. CASE PRESENTATION: We report a 7-year-old male with a large, left middle fossa arachnoid cyst, initially asymptomatic and followed conservatively for five years. The patient developed resistant headache, vomiting, and right sixth cranial nerve palsy. Neuroimaging rev

Non-contrast Enhancing Group A Streptococcus Subdural Empyema: An Illustrative Case Report of a Potential Radiographic Pitfall.

Subdural empyemas (SDEs) are an uncommon complication of intracranial infection, typically presenting as a hypodense collection with peripheral contrast enhancement. Herein, we report two rare cases of SDE without contrast enhancement, both secondary to group A streptococcus. The first is a 27-year-old previously healthy female, at 27 weeks of gestational age who presented with fever, headache, neurological decline, and blood cultures positive for gram-positive cocci. The second case is a previously healthy 48-year-old male who presented with left-sided otalgia, fever, headache, and precipitou

Are ventriculopleural shunts the second option for treating hydrocephalus? A meta-analysis of 543 patients.

BACKGROUND: Some centers utilize ventriculopleural shunt (VPLS) for treating hydrocephalus when conventional approaches are not feasible. Nonetheless, the literature regarding this approach is scarce. PURPOSE: Evaluate the outcomes of VPLS through a single-arm meta-analysis. METHODS: Following PRISMA guidelines, the authors systematically searched for articles utilizing the VPLS in a cohort with more than four patients. Outcomes included: mortality, pleural effusion, number of patients who underwent revisions, obstructions, shunt migration, emphysema, and subdural hematoma. RESULTS: A total

Middle cranial fossa Sylvian fissure arachnoid cysts in infants younger than 1 year: clinical characteristics, preoperative growth patterns, and outcomes of endoscopic fenestration.

BACKGROUND AND PURPOSE: Middle cranial fossa Sylvian fissure arachnoid cysts represent the most common type of intracranial arachnoid cysts in children, yet their natural history and optimal management strategies remain controversial. In infants younger than 1 year, the number of reported cases is limited, and these patients are often analyzed together with older children, limiting our understanding of cyst growth patterns and treatment responses during infancy. This study was aimed at characterizing the clinical features and preoperative growth patterns of middle cranial fossa Sylvian fissure

Risk factors for chronic hydrocephalus in patients with intracerebral hemorrhage complicated by coma after emergency hematoma removal.

BACKGROUND: The cerebrospinal fluid circulation of patients with intracerebral hemorrhage (ICH) can be blocked by blood clots, resulting in acute hydrocephalus. However, current research on chronic hydrocephalus (CH) is lacking. METHODS: A total of 253 patients with ICH combined with coma treated at The Third People's Hospital of Gansu Province after emergency hematoma removal from January 2018 to January 2020 were included. Patients were divided into the CH group (n=48) and the control group (n=205) depending on whether hydrocephalus occurred or not within 3-12 months after operation. The ma

A randomised trial of high and low pressure level settings on an adjustable ventriculoperitoneal shunt valve for idiopathic normal pressure hydrocephalus: results of the Dutch evaluation programme Strata shunt (DEPSS) trial.

BACKGROUND: In treating idiopathic normal pressure hydrocephalus (INPH) with a shunt there is always a risk of underdrainage or overdrainage. The hypothesis is tested whether patients treated using an adjustable valve preset at the highest opening pressure leads to comparable good clinical results with less subdural effusions than in a control group with an opening pressure preset at a low pressure level. METHODS: A multicentre prospective randomised trial was performed on a total of 58 patients suspected of INPH. Thirty patients were assigned to (control) group 1 and received a Strata shunt

Occult cervical (C1-2) dural tear causing bilateral recurrent subdural hematomas and repaired with cervical epidural blood patch.

The authors report the case of a 56-year-old previously healthy man who presented with a 4-month history of postural headache accompanied by nausea and vomiting. The results of initial imaging studies of the brain were normal. Repeated MR imaging demonstrated bilateral subdural hematomas which were drained and reaccumulated over a period of time. Spinal myelography revealed a cerebrospinal fluid leak at the C1-2 level. A cervical epidural blood patch, with repeated injections of 10 ml autologous blood at the site of the leak, dramatically improved the headache within 24 hours and eliminated th

Posterior fossa subdural hygroma with supratentorial chronic subdural haematoma.

Posterior fossa subdural hygromas are very rare. They tend to occur following direct occipital trauma. We present an unusual case of complex subdural hygroma of the posterior fossa, which was associated with a supratentorial chronic subdural haematoma. This developed after an apparently minor injury to the head. The unusual features of our case are discussed. We also review the literature and discuss the natural history and pathogenesis of subdural hygroma.

Paradoxical postural headaches in cerebrospinal fluid leaks.

Two patients with cerebrospinal fluid (CSF) leak, one at the level of fourth thoracic spine and another with undetermined level of leak, presented with paradoxical postural headaches in that the headaches were present when in a horizontal position and resolved if the patients were upright. One patient improved spontaneously and the other responded to a targeted epidural blood patch. Paradoxical postural headache is yet another headache type that can be associated with CSF leak and CSF volume depletion. Its mechanism is uncertain, but it could be related to congestion and dilatation of cerebral

Neonatal lead poisoning. An unusual clinical manifestation.

A case of lead encephalopathy in a 2-month-old child is reported. Modes of poisoning are discussed and the unusual clinical manifestations of metallic brownish discoloration of nails and subdural effusion are presented. The possibility of lead poisoning as a cause of convulsions in neonates should be considered by doctors caring for these patients. Detailed history of lead exposure in prenatal and postnatal periods aids in early diagnosis and treatment which, thus, prevent severe neurological sequelae.

Subdural hygroma as a rare complication after revision spine surgery.

Spinal fusion surgery is the most commonly performed orthopaedic surgical procedure. However, subdural hygroma occurrence is a very rare complication after revision spinal fusion surgery. Here, we report a case of revision lumbar fusion surgery at the L3-4 level. The patient developed acute conus medullaris syndrome at 10 days postoperatively. MRI showed a subdural, extra-arachnoid area fluid collection following the T12-L2, cephalad to the area of revision spinal fusion. When patients have a decreased motor grade, difficulty in voiding urine and neurological abnormalities after lumbar spine s

Enlarged cerebrospinal fluid spaces in infants with subdural hematomas.

Computed tomography in 16 infants with subdural hematomas showed enlarged basal cisterns, a wide interhemispheric fissure, prominent cortical sulci, and varying degrees of ventricular enlargement. Radionuclide cisternography in eight of the 16 patients showed findings consistent with enlargement of the subarachnoid space rather than those of communicating hydrocephalus. Clinical findings and brief follow-up showed no convincing evidence for cerebral atrophy in 13 patients. These findings suggest that the enlarged subarachnoid space, which is encountered in some infants and may be a development

Use of confirmatory imaging studies to illustrate adequate treatment of cerebrospinal fluid leak in spontaneous intracranial hypotension.

Spontaneous intracranial hypotension (SIH) is a syndrome with serious neurological sequelae. As demonstrated by the following report, recurrent episodes of SIH can be difficult to diagnose when associated with other neurosurgical procedures, such as craniectomies. In this paper, the authors demonstrate SIH presenting as a subdural hematoma with recurrence of CSF leaks. Spontaneous intracranial hypotension was further complicated by paradoxical herniation following a craniectomy. Treatment of SIH necessitated multiple epidural blood patches for CSF leaks at different spinal levels and at differ
