Triazolam--an "abused drug" by the lay press?

For the second time in the past ten years adverse reports in the lay press have questioned the safety of triazolam. Anecdotal reports of central nervous system adverse reactions can be separated into four general categories: (1) delirium in psychiatric patients; (2) delirium in geriatric patients; (3) withdrawal reactions; and (4) anterograde amnesia. Of the four, the amnesia reactions are regarded as serious enough by the Food and Drug Administration that it is requiring hypnotic drug manufacturers to revise the labeling of temazepam and flurazepam along with triazolam to emphasize the potent

Anxiety disorders and the use and abuse of drugs.

Substance abuse continues to be a growing problem--both in the United States and worldwide. Moderate alcohol consumption reduces anxiety, but alcohol abuse actually increases anxiety and impairs judgment and performance. Research data indicate that phobic anxiety is more common than general anxiety as a prelude to alcohol dependence. Benzodiazepines are widely used throughout the world in the treatment of anxiety disorders. Patients who take benzodiazepines for clinical anxiety rarely exceed the prescribed dosage: benzodiazepine abuse is more likely to occur in those who abuse other drugs. For

Management of benzodiazepine dependence.

Dependence is a recognized problem in a significant number of patients taking therapeutic doses of benzodiazepines. A characteristic withdrawal syndrome has been described. These patients are best managed as outpatients with gradual dosage reduction over 4-12 weeks. Pharmacological adjuncts such as propranolol and non-benzodiazepine hypnotics are useful for certain specific symptoms, but so far no single substance has proved useful in attenuating the majority of symptoms. Psychological and social support should be maximized, but here again no single technique has been shown to be more than mod

Neurological complications of alcohol and misuse of drugs.

The nature of many of the symptoms associated with substance and alcohol use means that patients often present to neurologists. The frequently catastrophic consequences of overlooking these patients makes this an important cause to identify. Here I will discuss various acute and non-acute substance misuse associated presentations, with particular emphasis on the neurology. As neurological sequelae are particularly common in alcohol use, there will be an emphasis on this drug while other substances are included when relevant, extending to the recently notorious 'legal highs'. I hope this review

Standardizing the Clinical Definition of Opioid Withdrawal in the Neonate.

OBJECTIVE: To standardize the clinical definition of opioid withdrawal in neonates to address challenges in clinical care, quality improvement, research, and public policy for this patient population. STUDY DESIGN: Between October and December 2020, we conducted 2 modified-Delphi panels using ExpertLens, a virtual platform for performing iterative expert engagement panels. Twenty clinical experts specializing in care for the substance-exposed mother-neonate dyad explored the necessity of key evidence-based clinical elements in defining opioid withdrawal in the neonate leading to a diagnosis o

Dopamine agonist withdrawal syndrome - what do we really know?

INTRODUCTION: Dopamine agonist withdrawal syndrome (DAWS) comprises a wide range of neuropsychiatric and autonomic symptoms that occur in temporal relation with the reduction or withdrawal of dopamine agonists (DAs) in a dose-dependent manner. This syndrome was described as an important complication in Parkinson's disease (PD) and restless legs syndrome (RLS) treatment. STATE OF THE ART: Dopamine agonist withdrawal syndrome affects even up to 24% of PD patients treated with DAs in whom the medication was tapered. Predominant symptoms include the following: anxiety, depression, irritability, p

Challenges in the postsurgical recovery of cushing syndrome: glucocorticoid withdrawal syndrome.

Glucocorticoid withdrawal syndrome is a challenging clinical phenomenon that can complicate the postsurgical recovery of Cushing syndrome. It is characterized by physical tolerance and dependence to supraphysiologic glucocorticoid exposure during active Cushing syndrome followed by the abrupt decline in cortisol levels after surgical treatment. The symptoms of glucocorticoid withdrawal often overlap with those of postoperative adrenal insufficiency and can be difficult for patients to cope with and for clinicians to treat. This mini review will discuss the clinical characteristics, pathophysio

Prevention and treatment of malignant syndrome in Parkinson's disease: a consensus statement of the malignant syndrome research group.

We report a consensus statement of the collaborative research group on the prevention and treatment of malignant syndrome (MS) in Parkinson's disease. The syndrome is quite similar to neuroleptic MS. Although sudden withdrawal of levodopa was the most frequent cause, many other precipitating events were found such as intercurrent infections, dehydration, hot weather, discontinuation of other anti-parkinsonian drugs, and "wearing off" phenomenon. Awareness of this syndrome is most important for its early detection and the prompt commencement of treatment. MS should be suspected whenever the bod

Anton's syndrome accompanying withdrawal hallucinosis in a blind alcoholic.

During alcohol withdrawal, a man totally blind following ocular trauma experienced formed visual hallucinations ("alcoholic hallucinosis"), the unreality of which he recognized. During these same periods, however, he believed that his vision had been restored, and confabulated descriptions of his surroundings (Anton's syndrome). He was otherwise mentally normal. Anosognosia for blindness can occur episodically or chronically, and with either peripheral or central visual loss; dementia nearly always coexists. The basis of the syndrome is obscure, and its occurrence during alcohol withdrawal, in

Diagnosis and management of emergency department patients with alcohol withdrawal syndrome.

Alcohol withdrawal syndrome is a constellation of symptoms and signs resulting from the abrupt decrease or cessation of heavy alcohol use. Complications of alcohol withdrawal syndrome present significant dangers for patient morbidity and mortality, as well as burdens on emergency department resources. This review presents an overview of the pathophysiology of alcohol withdrawal syndrome and a systematic approach for management in the emergency department. Current evidence on treatment regimens and adjunctive therapies is reviewed, and recommendations for management of patients with alcohol use

Building Multidisciplinary Consensus on Inpatient Xylazine Management through Clinical Protocols.

The evolving unregulated drug supply in the United States has led to an unprecedented rise in xylazine-adulterated synthetic opioid use-related morbidity and mortality, of which Pennsylvania shoulders a disproportionate burden. People experiencing these xylazine harms who seek acute medical care require complex clinical management, multidisciplinary coordination, and appropriate linkage to outpatient care. We describe our experience leading a hospital-wide multidisciplinary xylazine workgroup from February to June 2024 to collaboratively develop time-sensitive clinical protocols on inpatient m

Post-acute withdrawal syndrome (PAWS) after stopping antidepressants: a systematic review with meta-narrative synthesis.

AIMS: The literature on persistent antidepressant withdrawal symptoms is sparse. This systematic review is the first to examine the prevalence, duration, severity, risk/protective factors and treatment strategies for post-acute withdrawal syndrome (PAWS) following the discontinuation of antidepressant medications. METHODS: We searched PubMed, Web of Science and PsycInfo, focusing on newer-generation antidepressants. The electronic database search was complemented with handsearching reference lists of pivotal studies. We included original studies in adults reporting on PAWS and providing data
