Subtalar Joint Statistical Shape Modeling Differentiates Cavus-to-Planus Foot Types From Weightbearing CT.

BACKGROUND: Foot type significantly impacts the development and progression of foot and ankle pathologies by influencing biomechanics and force distribution. However, it is typically assessed qualitatively and with a 2D radiographic measurement called Meary's angle. This study seeks to determine the minimum number of bones required in a statistical shape model (SSM) to accurately represent the full cavus through planus spectrum, enabling future machine learning applications in clinical practice. METHODS: Our study included weightbearing computed tomography (WBCT) data from 151 patients groupe

Subtalar joint nonunions following isolated ipsilateral ankle arthrodesis a systematic review and meta analysis.

Open arthrodesis remains the gold standard for management of end stage subtalar joint arthritis. Rate of isolated subtalar fusion complicated by nonunion has been documented between 4-22.8 %. However, little is known about success of subtalar joint fusion when performed underneath a previously constructed ankle arthrodesis. The aim of this systematic review was to assess the cases of nonunion in patients who underwent a subtalar joint fusion with documented history of previous ipsilateral ankle fusion. A broad literature search was performed through PubMed, Google Scholar, and Cochrane Databas

Centre of Rotation of the Human Subtalar Joint Using Weight-Bearing Clinical Computed Tomography.

Accurate in vivo quantification of subtalar joint kinematics can provide important information for the clinical evaluation of subtalar joint function; the analysis of outcome of surgical procedures of the hindfoot; and the design of a replacement subtalar joint prosthesis. The objective of the current study was to explore the potential of full weight-bearing clinical computed tomography (CT) to evaluate the helical axis and centre of rotation of the subtalar joint during inversion and eversion motion. A subject specific methodology was proposed for the definition of the subtalar joint motion c

Conservative management of posterior tibial tendon dysfunction, subtalar joint complex, and pes planus deformity.

Conservative management of PTTD can present from fairly simple to quite complex based on the wide range of clinical presentation that is inherent to this pathology. Treatment for PTTD ranges from the use of orthopedic footwear to the use of PTB AFOs, which certainly substantiates the prevalence of this disorder and the high rate of conservative management. With the increase in the population number and the fitness and health awareness that pervades our society, PTTD will more than likely continue to be a large part of the conservative footcare practitioner's practice in the future. The footcar

Increased Lateral Calcaneal Body and Subtalar Joint Visualization Utilizing a Modified Sinus Tarsi Approach.

BACKGROUND: The traditional sinus tarsi approach (TST) for open reduction and internal fixation (ORIF) of calcaneus fractures has gained popularity because of adequate fracture exposure with lower wound complication rates. Visualization, as compared with more extensile approaches, can be limited, however. This study introduces a modified sinus tarsi approach (MST) that extends the TST incision approximately 2 cm posterodorsally as well as 2 cm anteroplantarly, to enhance exposure. The aim of this study is to quantify the amount of exposure obtained with the traditional sinus tarsi approach and

Biomechanical conditions of subtalar joint arthrodesis with calcaneal locking nail: A probabilistic numerical study.

INTRODUCTION: Subtalar joint arthrodesis is primarily indicated for advanced osteoarthritis, hindfoot deformity, and/or instability. During the first 6-10 weeks after surgery, there is an intermediary structurally weaker state before complete bony fusion of the calcaneus and talus occurs. Loading of the foot can lead to mechanical stresses and relative movements in the former joint gap, which can impede the fusion process. The objective of this study was to examine the mechanical healing conditions for a subtalar arthrodesis with a calcaneal locking nail. METHODS: A probabilistic finite eleme

Posterior approach for subtalar joint distraction arthrodesis by compact external fixation: a technique guide.

Subtalar joint arthrodesis is a commonly used surgical procedure for the management of rearfoot pathologic features. We present a technique guide for a posterior incisional approach to subtalar joint arthrodesis for correction of a calcaneal deformity secondary to calcaneal fracture malunion. This technique uses a monolateral external fixation device for controlled distraction and intercallary allograft placement. In contrast to a standard lateral approach, this incision provides better visualization of the joint space, and the use of a distractor enhances access to the joint surfaces for fusi

Effects of added trunk load on the in vivo kinematics of talocrural and subtalar joints during landing.

BACKGROUND: Landing from heights is a common movement for active-duty military personnel during training. And the additional load they carry while performing these tasks can affect the kinetics and ankle kinematic of the landing. Traditional motion capture techniques are limited in accurately capturing the in vivo kinematics of the talus. This study aims to investigate the effect of additional trunk load on the kinematics of the talocrural and subtalar joints during landing, using a dual fluoroscopic imaging system (DFIS). METHODS: Fourteen healthy male participants were recruited. Magnetic r

Calcaneal Apophyseal Fracture Followed by Subtalar Joint Instability in a Young Female: A Case Report.

Calcaneal apophyseal fractures are usually considered sport-related injuries. Previous repetitive microtrauma has been proposed as a predisposing factor for such injuries. However, unlike previously reported cases, in our patient, the fracture resulted from acute trauma after stepping on uneven ground. Although the first treatment option for such cases is closed reduction, most cases will require surgical treatment. Furthermore, lesions accompanying this injury have not been sufficiently considered. We report the case of a 9-year-old female with a calcaneal apophyseal fracture treated by fixat

Distraction arthrodesis of the subtalar joint using allogeneic bone graft: a review of 15 cases.

Distraction arthrodesis of the subtalar joint is often used for the correction of neglected calcaneal fractures. Although different techniques have been advocated, there remains some debate as to the optimal type of bone graft for this purpose. This study retrospectively reviewed one surgeon's results for distraction arthrodesis of the talocalcaneal joint for 15 consecutive feet in 15 patients using 12 frozen femoral head and 3 freeze-dried iliac crest allografts. Indications for distraction arthrodesis in this series included neglected calcaneal fracture (n = 10), failed open reduction with i

The geometric architecture of the subtalar and midtarsal joints in rheumatoid arthritis based on magnetic resonance imaging.

OBJECTIVE: To compare in vivo the 3-dimensional (3-D) geometric architecture of the subtalar and midtarsal joints in normal and rheumatoid arthritic (RA) feet, using magnetic resonance imaging (MRI) analysis. METHODS: MRI was performed on 23 patients with RA, all of whom had disease activity in the subtalar and/or midtarsal joints. Image processing techniques were used to create 3-D reconstructions of the calcaneus (C), cuboid (c), navicular (N), and talus (T) bones. Twenty-four standard architectural parameters were measured from the reconstructions and were compared with data from 10 normal

Displaced fractures of the os calcis involving the subtalar joint: the key role of the superomedial fragment.

Fourteen displaced intra-articular fractures of the os calcis are reviewed following open reduction and internal fixation using a lateral approach with an average follow-up of 22 months (range, 12 to 44 months). Postoperative management consisted of early subtalar motion with delayed weightbearing. Twelve of 14 fractures were considered good results on the basis of no pain, 50% normal subtalar motion, and near-normal anatomy. Pain correlated with incomplete reduction of the superomedial fragment and, thus, incongruent reduction of the posterior facet in two cases. The importance of effecting a

Repair of the Nonunion of the Lateral Process of the Talus Will Improve Osteoarthritis of the Subtalar Joint: A Case Report.

Fracture of the lateral process of the talus (FLPT) is a rare injury. FLPT is frequently missed or diagnosed late, often leading to nonunion, malunion, or subtalar osteoarthritis. We present a case of a 44-year-old male with schizophrenia who sustained an FLPT after falling downstairs. Initial radiographs missed the injury, and conservative treatment was pursued despite the presence of bone fragments. After four months, persistent pain led to further imaging, revealing subtalar joint osteoarthritis. Surgical intervention was required due to nonunion and large, displaced bone fragments. A 4 cm

Clinical Outcomes and Safety Profile of Open and Arthroscopic Subtalar Joint Arthrodesis: A Systematic Review.

BACKGROUND: Open and arthroscopic subtalar arthrodesis are safe and effective surgeries for patients with notable arthritis; however, not an established superiority among them has been found. This systematic review aims to present all of the available literature on studies directly comparing both surgeries. METHODS: Two independent authors completed a systematic literature search using the following databases: PubMed, Embase, and the Cochrane library. The Preferred Reporting Items for Systematic Reviews and Meta-Analysis protocol and the Cochrane Handbook guidelines were followed. In addition

Surgical Treatment of Subtalar Joint Instability: Safety and Accuracy of a New Technique in a Cadaver Model.

New surgical strategies to treat symptomatic subtalar joint (STJ) instability are evolving. We modified a previously described reconstruction strategy and then refined our new surgical technique through simulated surgery and subsequent cadaver dissections. Our purpose was to show that a tunnel intended to facilitate STJ stabilization surgery could safely be drilled across the footprints of the interosseous talocalcaneal ligament (ITCL). A percutaneous fluoroscopically guided tunnel for the purpose of ITCL reconstruction was created in 10 cadaveric below-knee specimens. Accuracy of the tunnel w

Cystic Adventitial Disease of the Tibial Vein Arising From the Subtalar Joint: A Case Report.

Soft tissue ganglion cysts are a well-known cause of tibial nerve compression in the tarsal tunnel. We describe a patient who presented with tibial nerve symptoms and was found to have an adventitial cyst of the tibial vein arising from the subtalar joint, with the joint connection confirmed both on imaging and at surgery. Surgical decompression of the cyst with transection of the vascular pedicle arising from the subtalar joint improved her symptoms at 6 months, and postoperative magnetic resonance imaging showed resolution of the cyst. Cystic adventitial disease is a rare, poorly understood
