Functional projection of regenerated rat sural nerve axons to the hindpaw skin after sciatic nerve lesions.

The functional regeneration of polymodal C-fibers and low-threshold mechanoreceptive axons in the rat sural nerve was evaluated 3 months after a sciatic nerve crush lesion or neurotomy and suture. The distribution of polymodal C-fibers was tested through stimulation-induced extravasation of Evans blue albumin in the foot skin. As judged from the area of stained skin, the polymodal sural C-fibers regenerated efficiently after both lesion types. In regenerates, though, the borders of the stained domain were less clearly defined than in controls, and there were minor territorial abnormalities. Th

Relationships of the superficial fibular nerve and sural nerve with respect to the lateral malleolus: implications for ankle surgeons.

PURPOSE: Superficial fibular nerve (SFN) and sural nerve are at risk during osteosynthesis of the lateral malleolus. The aim of this anatomical study was to describe the relationships of the superficial fibular and sural nerves with respect to the lateral malleolus. METHODS: Nine corpses (18 ankles) were dissected, using a direct lateral approach. Measurements were recorded between the fibula and the nerves, and the pattern variations of the SFN were recorded for both right and left side to assess intra-individual variability. RESULTS: Distance between the tip of the lateral malleolus and th

MRI of peripheral nerves and pathology of sural nerves in hereditary motor and sensory neuropathy type III.

We investigated two patients with hereditary motor and sensory neuropathy type III, one with Déjérine-Sottas disease and the other with congenital hypomyelination neuropathy based on nerve pathology and MRI of the sciatic nerve. On biopsy of the sural nerve of the patient with Déjérine-Sottas disease, myelin debris, indicating demyelination, was observed in an onion-bulb pattern surrounding myelinated fibres. In the patient with congenital hypomyelination neuropathy, onion bulbs were formed of two parallel layers of basement membrane. There was no evidence of myelin breakdown. On axial T2-weig

Percutaneous Zadek Osteotomy and Sural Nerve Proximity: A Cadaveric Anatomical Mapping Study.

BACKGROUND: The Zadek osteotomy, a dorsal closing-wedge procedure of the calcaneus, is commonly used in the surgical treatment of insertional Achilles tendinopathy, particularly in patients unresponsive to conservative measures. Although minimally invasive surgical (MIS) techniques offer potential benefits such as reduced soft tissue trauma and faster recovery, concerns persist regarding the risk of iatrogenic injury to the sural nerve because of limited visualization. This cadaveric study aims to provide a detailed anatomical assessment of the sural nerve and its branches in relation to the e

Normobaric hypoxia does not influence the sural nerve cutaneous reflex during standing.

Hypoxia increases postural sway compared to normoxia, but the underlying sensorimotor factors remain unclear. An important contributor to balance control is cutaneous feedback arising from the feet, which can be partially characterized by electrically evoking a reflex from a purely cutaneous nerve (i.e., sural) and sampling the subsequent motor activity of a muscle. The purpose of the present study was to determine how normobaric hypoxia influences sural nerve reflex parameters during a standing posture. It was hypothesized that normobaric hypoxia would reduce cutaneous reflex area compared to

Anatomy of the sural nerve complex: Unaccounted anatomic variations and morphometric data.

BACKGROUND: The sural nerve (SN) is a cutaneous sensory nerve innervating the posterolateral leg. The SN is formed from a highly variable set of contributing nerves called the sural nerve complex (SNC). The SNC is made up of the lateral sural cutaneous nerve, medial sural cutaneous nerve, sural communicating branch, and SN. The SN is frequently cited as the most common donor nerve graft and is commonly injured in procedures of the lower extremity. Recent meta-analysis standardized six morphologies of the SNC and established a required reporting criterion for the group of nerves forming the SN.

Evaluation of the sensory deficit after sural nerve harvesting in pediatric patients.

BACKGROUND: The sural nerve is a sensory nerve that innervates the proximal part of the lateral aspect of the foot. The sural nerve is often harvested for nerve grafting. Sensory loss in the area supplied by the sural nerve could be expected, causing a lack of protective sensation and a potential risk of injury. The sensory outcome of sural nerve harvesting has not been documented in children. The aim of this study was to evaluate the sensory deficit following sural nerve harvest in infants. METHODS: The authors conducted a controlled study. Evaluation and mapping of the sensory thresholds in

Targeted Nerve Repair With Reverse Vascularized Nerve Flap Combined With Sural Nerve Grafts for a Long Sciatic Nerve Gap: A Case Report.

Nerve grafting for large sciatic nerve gap has been associated with low functional recovery rates and unsatisfactory outcomes. The authors present a case of an 18-year-old male with successful surgical management for a 9-cm sciatic nerve gap presenting with complete loss of right ankle and toe movements (Medical Research Council M0) as well as right lower limb sensory deficits (S0). Nerve reconstruction was performed using a novel surgical technique, the reverse vascularized nerve flaps. The medial sural cutaneous and superficial peroneal nerves were selected as donor nerves due to their limit

Culture and gene expression profiling of Schwann cells from human sural nerve.

Polyneuropathies (PNP) are diseases of the peripheral nervous system (PNS) whose pathophysiology remains poorly understood. Schwann cells (SC) are the principle glial cells in the PNS, and their dysfunction can lead to demyelination and secondary axonal degeneration, causing or exacerbating PNP. The study of SC can thus shed light on the molecular and cellular mechanisms involved in PNP. Protocols have been developed to isolate and culture human and animal SC from large amounts of starting material. The isolation of human SC from patients, where only small biosamples are available, remains a c

Extracellular Matrix Remodelling in the Human Sural Nerve in Peripheral Vascular Disease.

Background and Objectives: Peripheral nerve adaptation to different pathological conditions is accompanied by the remodelling of the nerve's extracellular matrix (ECM). Ischemic conditions caused by peripheral vascular disease are known to affect the function of peripheral nerves; however, the morphological changes to their ECM remain insufficiently examined and understood. Bearing in mind that alterations in collagen I, collagen IV, and laminin content may compromise peri- and endoneurial integrity, the aim of our study was to analyse whether peripheral vascular disease (PVD) induces distinct

Synovial Lipomatosis of Ankle with Sural Nerve Neuropathy: A Rare Case Report.

Synovial lipomatosis, also known as lipoma arborescens, is a rare benign condition characterized by villous fatty proliferation of the synovium with replacement of the subsynovial tissue by mature adipocytes. Although it predominantly affects the knee joint, involvement of the ankle is distinctly uncommon. We report the case of a 52-year-old woman who presented with progressive left ankle pain and swelling of 1 year duration, associated with mechanical symptoms and restriction of motion. Physical examination revealed a posterolateral ankle swelling with mild limitation of range of motion, whil

Sural nerve injury risk during endoscopic gastrocnemius recession: Comparison of two approaches.

BACKGROUNDS: The gastrocnemius recession was originally introduced as a method of treating isolated gastrocnemius contractures among pediatric population, however currently this procedure allows to treat various foot pathologies. Different modifications of open technique has been developed over the years and recently endoscopic approach become a widespread technique among many surgeons. It provides smaller incision and shorter operative time, however might be associated with the higher risk of sural nerve damage as a result of poor visualization. The aim of this study was to evaluate two diffe

Unusual presentation of lateral sural cutaneous nerve schwannoma: An exceptional case report.

INTRODUCTION: Schwannoma is the most common benign nerve sheath tumor. Peripheral nerves of the lower extremity are rarely involved and usually asymptomatic. CASE PRESENTATION: We report the case of a misleading clinical presentation of lateral sural cutaneous nerve schwannoma. DISCUSSION: To the best of our knowledge, no case has been reported about the location of schwannoma in the lateral sural cutaneous nerve. MRI and anatomopathologic assessment, after microscopic enucleation, are required to confirm diagnosis. CONCLUSION: Care must be taken to not miss a schwannoma of lateral sural cu

Sural nerve pathology in TFG-associated motor neuron disease with sensory neuropathy.

The tropomyosin-receptor kinase fused gene (TFG) functions in vesicles formation and egress at the endoplasmic reticulum (ER). A heterozygous missense mutation c.854C > T (p.Pro285Leu) within TFG has been reported as causative for hereditary motor and sensory neuropathy with proximal predominance. Here, we describe two unrelated Chinese pedigrees with 13 affected members harboring the same variant. The clinical, electrophysiological and pathological findings are consistent with motor neuron disease with sensory neuropathy. The main symptoms were painful muscle cramps, slowly progressive proxim
