Improving pandemic preparedness through better, faster influenza vaccines.

Introduction. Timely availability of effective influenza vaccine will be critical to mitigate the next influenza pandemic. The mission of Biomedical Advanced Research and Development Authority (BARDA) is to develop medical countermeasures against pandemics, including influenza and other health security threats.Areas covered. Despite considerable gains in pandemic vaccine preparedness since 2009, old and new challenges threaten the pandemic influenza response capabilities of the U.S. Government: insufficient U.S.-based vaccine production, two-dose vaccination regimen, logistically complex adjuv

Cellular immune states in SARS-CoV-2-induced disease.

The general immune state plays important roles against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Cells of the immune system are encountering rapid changes during the acute phase of SARS-CoV-2-induced disease. Reduced fraction of functional CD8+ T cells, disrupted cross-talking between CD8+ T cells with dendritic cells (DCs), and impaired immunological T-cell memory, along with the higher presence of hyperactive neutrophils, high expansion of myeloid-derived suppressor cells (MDSCs) and non-classical monocytes, and attenuated cytotoxic capacity of natural killer (N

Multidisciplinary management of ventilator weaning and tracheostomies for COVID-19 patients at a major NYC public hospital: A blueprint for other institutions.

At the height of the coronavirus pandemic in New York City, at our hospital (NYC Health/Hospitals-Elmhurst) 95% of inpatients tested positive for COVID-19 and it operated at 500% surge ICU capacity-one of the greatest impacted centers in the nation. In the face of this we established a systematic multidisciplinary approach to manage ventilated ICU patients and select those appropriate for tracheostomy. Members from Pulmonary Critical Care, Anesthesiology, Surgery, Ethics, and Otolaryngology, created a protocolized way to assess all ICU patients in our hospital and, if deemed appropriate, help

Measuring hospital care resilience: a systematic literature review.

The recent Covid-19 pandemic has shown how even high-performing healthcare systems are often unprepared to cope with sudden and unforeseen surges in demand for healthcare services, drawing further attention on crucial factors ensuring their resilience in the face of extreme disruptive events. Despite extensive efforts to define health system resilience, a lack of consensus persists, making it difficult to operationalize the existing conceptual frameworks and to guide policy makers in developing adequate response strategies. Grounded on this, the present paper aims to systematically review how

Pooling Nasopharyngeal Swab Specimens to Increase Testing Capacity for SARS-CoV-2.

The recent emergence of SARS-CoV-2 has led to a global pandemic of unprecedented proportions. Current diagnosis of COVID-19 relies on the detection of SARS-CoV-2 RNA by reverse transcription polymerase chain reaction (RT-PCR) in upper and lower respiratory specimens. While sensitive and specific, these RT-PCR assays require considerable supplies and reagents, which are often limited during global pandemics and surge testing. Here, we show that a nasopharyngeal swab pooling strategy can detect a single positive sample in pools of up to 10 samples without sacrificing RT-PCR sensitivity and speci

Emergency response to an anthrax attack.

We developed a mathematical model to compare various emergency responses in the event of an airborne anthrax attack. The system consists of an atmospheric dispersion model, an age-dependent dose-response model, a disease progression model, and a set of spatially distributed two-stage queueing systems consisting of antibiotic distribution and hospital care. Our results underscore the need for the extremely aggressive and timely use of oral antibiotics by all asymptomatics in the exposure region, distributed either preattack or by nonprofessionals postattack, and the creation of surge capacity f

Ontogenesis of androgen receptors in the mouse submandibular gland: correlation with the developmental profiles of circulating thyroid and testicular hormones.

Specific binding of the synthetic androgen, [17 alpha-methyl-3H]methyltrienolone, to the cytosol fraction of the submandibular salivary gland (SMG) of male mice was studied in relation to the developmental profiles of testosterone and thyroid hormones in blood. The peak rise of serum triiodothyronine (T3) at prepubertal age was closely related to both the increase of maximal androgen-binding capacity in SMG and the conspicuous surge of proliferative activity as indicated by increased rate of glandular DNA content. Also, 2-month thyroidectomized mice had an age-related, strong reduction in the

Macro- and meso- shear mechanical properties of rock-grout composite structures under different stress level and initial hydration damage.

This study investigates the shear mechanical behavior of soft rock-grout coupled structures through triaxial shear tests and particle flow simulations on sandstone-resin composite specimens under varying normal stresses and immersion times. Key findings include: (1) Intensified hydration damage compromises shear resistance in both rock and interfaces, amplifies deformation, and degrades bearing capacity. (2) Elevated normal stress constrains microcrack coalescence, enhances bearing capacity. Failure modes of specimens-classified as interfacial shear sliding, mixed shear failure, or shear failu

Vaccination as cost-stabilizing health infrastructure: A Canadian perspective.

As Canada contends with rising healthcare costs and health system strain, vaccination programs stand out as stabilizing health infrastructure. Vaccination can reduce the burden of avoidable illness and dampen volatility arising from seasonal disease surges and outbreaks. Together, these effects can preserve clinical and public health capacity, improve fiscal predictability, and support health system resilience. These system-level benefits are generally not fully captured by conventional cost-utility analyses. Drawing on Canadian evidence, we outline how vaccination programs stabilize healthcar

Implementation of a paraprofessional role across the public health sector in Manitoba: Impacts on system capacity.

SETTING: Manitoba experiences sexually transmitted and blood-borne infection (STBBI) incidence rates above the national average and fundamental public health sector responses face human resource constraints. INTERVENTION: Manitoba implemented a paraprofessional role, the Communicable Disease Technician (CD Tech), to complete low-complexity STBBI investigations and support STBBI testing, care engagement initiatives, provider reporting, harm reduction supply distribution, and community engagement. Secondary roles included tuberculosis directly observed treatment (DOT) and virtual DOT, and flu a

Mitigating ED boarding during surge conditions: A multidisciplinary resilience model at a tertiary center epicenter.

BACKGROUND: Emergency department (ED) overcrowding and boarding intensified during the COVID-19 pandemic. We evaluated the association of a multidisciplinary rapid response team (RRT) with ED boarding during the 2021 surge in New Taipei City, Taiwan. METHODS: A retrospective quality improvement study was conducted at a 1058-bed, newly established tertiary referral hospital situated at the epicenter of the pandemic in Taiwan. In May 2021, an RRT implemented three strategies: (1) diversion of mild COVID-19 cases to quarantine centers via telemedicine, (2) real-time business intelligence (BI) da

Quantifying Utilitarian Outcomes to Inform Triage Ethics: Simulated Performance of a Ventilator Triage Protocol under Sars-CoV-2 Pandemic Surge Conditions.

BACKGROUND: Equitable protocols to triage life-saving resources must be specified prior to shortages in order to promote transparency, trust and consistency. How well proposed utilitarian protocols perform to maximize lives saved is unknown. We aimed to estimate the survival rates that would be associated with implementation of the New York State 2015 guidelines for ventilator triage, and to compare them to a first-come-first-served triage method. METHODS: We constructed a simulation model based on a modified version of the New York State 2015 guidelines compared to a first-come-first-served

Estrous cycle characterisation and artificial insemination using frozen-thawed spermatozoa in the bottlenose dolphin (Tursiops truncatus).

The reproductive endocrinology of the bottlenose dolphin, Tursiops truncatus, was characterized to facilitate the development of artificial insemination using cryopreserved spermatozoa. Specific objectives were: (i) to determine the excretory dynamics of urinary luteinizing hormone (LH) and ovarian steroid metabolites during the estrous cycle; (ii) to evaluate the effect of an exogenously administered synthetic progesterone analog (altrenogest) on reproductive hormone excretion; (iii) to correlate follicular growth and ovulation (as determined by transabdominal ultrasound) to urinary LH and ov

Mitigation of malondialdehyde and eight α,β-unsaturated aldehydes in flaxseed oil systems by gallates: Matrix-dependent formation patterns and inhibitory mechanisms.

The accumulation of toxic α,β-unsaturated aldehydes in lipid-rich foods poses significant safety risks. This study elucidated the formation patterns of nine aldehydes in bulk flaxseed oil versus oil-in-water (O/W) emulsions, while evaluating the inhibitory mechanisms of gallates. In simulated systems, acrolein-trapping capacity declined with increasing alkyl chain length due to steric hindrance. Crucially, in simulated lipid models, gallates failed to scavenge acrolein, proving their inhibitory effect in bulk oil relied solely on upstream antioxidant activity, with octyl gallate being most eff

The myth of pandemic preparedness: persistent gaps in global readiness for Disease X.

Recent outbreaks of H5N1 avian influenza have exposed persistent weaknesses in global pandemic preparedness. Despite scientific advances, surveillance remains fragmented, hospital surge capacity is declining, and frontline workers continue to face inadequate protection. In low-income settings, delayed outbreak detection - exemplified by cholera in Sudan and mpox in central Africa - reflects ongoing gaps in diagnostic access and communication infrastructure. These challenges are compounded by geopolitical instability, fragile supply chains, and a lack of transparent, sustained investment in pre

Integrating science-based and local ecological knowledge: a case study of mangrove restoration and rehabilitation projects in the Philippines.

Mangrove forest is an ecosystem-based solution for disaster risk reduction in the Philippines, but its historical deforestation has hampered its capacity to protect coastal communities. With the increasing occurrence of storm surge in the Philippines, mangrove reforestation projects have received renewed attention, but many have failed. Community participation was deemed to be essential in those projects that did well. Hence, this paper examines successful mangrove restoration and rehabilitation projects in the Philippines to find out how community participation contributed to the accomplishme

From Concerts to COVID: Transforming the RI Convention Center into an Alternate Hospital Site in under a Month.

Field hospitals have long been used to extend health care capabilities in times of crisis. In response to the pandemic and an anticipated surge in patients, Rhode Island Gov. Gina Raimondo announced a plan to create three field hospitals, or "alternate hospital sites" (AHS), totaling 1,000 beds, in order to expand the state's hospital capacity. Following China's Fangcang shelter hospital model, the Lifespan AHS (LAHS) planning group attempted to identify existing public venues that could support rapid conversion to a site for large numbers of patients at a reasonable cost. After discussions wi

Modeling Tool for Decision Support during Early Days of an Anthrax Event.

Health officials lack field-implementable tools for forecasting the effects that a large-scale release of Bacillus anthracis spores would have on public health and hospitals. We created a modeling tool (combining inhalational anthrax caseload projections based on initial case reports, effects of variable postexposure prophylaxis campaigns, and healthcare facility surge capacity requirements) to project hospitalizations and casualties from a newly detected inhalation anthrax event, and we examined the consequences of intervention choices. With only 3 days of case counts, the model can predict f

HEPA/vaccine plan for indoor anthrax remediation.

We developed a mathematical model to compare 2 indoor remediation strategies in the aftermath of an outdoor release of 1.5 kg of anthrax spores in lower Manhattan. The 2 strategies are the fumigation approach used after the 2001 postal anthrax attack and a HEPA/vaccine plan, which relies on HEPA vacuuming, HEPA air cleaners, and vaccination of reoccupants. The HEPA/vaccine approach leads to few anthrax cases among reoccupants if applied to all but the most heavily contaminated buildings, and recovery is much faster than under the decades-long fumigation plan. Only modest environmental sampling
