A Consensus on Stomal, Parastomal, and Peristomal Complications.

OBJECTIVE: To establish a consensus on terminology used to define stomal, parastomal, and peristomal complications in Australia. METHODS: A list of stomal, parastomal, and peristomal complications was generated through group dialogue, which was informed by clinical and academic knowledge of the researchers. An extensive literature review was undertaken to identify any additional terms and to create a database of definitions/descriptions. A library of images related to the identified conditions was generated. An online Delphi process was conducted among a representative, purposive sample of Au

Appendicostomy irrigation for facilitating colonic evacuation in colostomy patients. Preliminary report.

A method for bowel irrigation through an appendicostomy (antegrade colonic enema) for patients with a left colostomy is described. The appendicostomy is easily constructed without morbidity. Irrigation through the appendicostomy is performed with minimum equipment, uses a small volume of irrigation water, and takes a relatively short time. This may improve colonic evacuation in patients with left colostomy.

Risk factors for the development of a parastomal hernia in patients with enterostomy: a systematic review and meta-analysis.

PURPOSE: Parastomal hernia (PSH) is a common and serious complication in patients with enterostomy, but there is no current consensus for the risk factors for PSH from previous studies. Therefore, this study systematically analyzed the risk factors for PSH to provide a reference for prevention and treatment of this condition. METHODS: Seven databases and 3 registers were systematically searched from database inception to January, 2021. Study quality was assessed by Newcastle-Ottawa Scale. Review Manager 5.3 software was used for statistical analysis. The data that could not be combined quanti

Prevention of parastomal hernia by the placement of a mesh at the primary operation.

INTRODUCTION: Parastomal hernia is a well known clinical problem, and up to 50% of all patients having a stoma will eventually develop a parastomal hernia. There are many surgical options available for the repair of a parastomal hernia, but the prevention of hernia development has only recently received scientific attention. The most encouraging results have included the use of a mesh inserted at the primary operation. We have, therefore, chosen to review the literature regarding the results of operative techniques, including mesh placement, for the prevention of a parastomal hernia. MATERIAL

Segmental versus extended colectomy for colonic Crohn's disease in advanced therapy era: a systematic review and Bayesian meta-analysis.

BACKGROUND: The optimal extent of resection for colonic Crohn's disease (CD) remains controversial. Segmental colectomy (SC) may preserve bowel and avoid permanent stoma but has been historically associated with higher recurrence than extended colectomy (EC). In the advanced therapy era, contemporary data and Bayesian methods may refine this risk-benefit balance. METHODS: PubMed, Embase, and Cochrane Library were searched from inception to December 1, 2025. The primary outcome was surgical recurrence; secondary outcomes were reoperation, permanent stoma, overall postoperative complications, a

Management of Crohn's Disease and Complications in Patients With Ostomies.

Fecal diversion with ostomy construction can be a temporary or definitive surgical measure for the treatment of refractory inflammatory bowel disease (IBD). However, the fecal diversion surgery is associated with various stoma, peristomal complications, and recurrence or occurrence of de novo small bowel Crohn's disease (CD). Stoma complications often need enterostomal therapy or surgical revision. Peristomal cutaneous lesions, such as pyoderma gangrenosum, usually require immunomodulator or biological therapy. Routine monitoring for occurrence or recurrence of CD with endoscopy or imaging sho

Long-term parietal complications following surgery for inflammatory colitis: An underestimated issue.

BACKGROUND: Surgical management of inflammatory colitis often requires a staged approach with multiple procedures. Data on incidence, risk factors, and management of parietal complications remain limited. METHODS: All adult patients who underwent surgery for inflammatory colitis between March 2010 and May 2024 were included. The primary endpoint was the incidence of incisional hernia, parastomal hernia, or stoma prolapse after complete surgical treatment. Risk factors were assessed, with age, body mass index ≥25 kg/m2, and permanent stoma retained for multivariate analysis. RESULTS: One hund

Sutureless enterostomy for extremely low birth weight infants.

OBJECTIVE: In recent years, improved survival rates of extremely low birth weight infants (ELBWIs) have led to an increasing number of enterostomy performed for those with meconium obstruction of prematurity (MOP)1,2, spontaneous intestinal perforation (SIP)3,4. To prevent serious stoma-related complications such as stoma side perforation, prolapse, fall and surgical site infection, we introduce our new "sutureless enterostomy" technique. METHODS: We present the procedures in detail. We reviewed the medical records of twelve patients who underwent "sutureless enterostomy" in our neonatal inte

The use of stomas in the early management of Hirschsprung disease: Findings of a national, prospective cohort study.

BACKGROUND/PURPOSE: Primary pull-through without a stoma has become preferred practice in managing Hirschsprung disease (HD). The aims of this study were to establish stoma rate and identify factors associated with stoma formation in a population-based cohort in the UK and Ireland. METHODS: Live-born infants with HD were prospectively identified in all 28 specialist pediatric surgical units in the UK and Ireland between October 2010 to September 2012. Method of colonic decompression was recorded and multivariable logistic regression was used to identify factors associated with stoma formation

Early Laparoscopic Colostomy in Advanced Cancer Patients with Rectovaginal Fistula: Results of Seven Patients.

A rectovaginal fistula (RVF) is an abnormal tract between the rectum and vagina, which requires surgical intervention in many cases. Although there are many different therapeutic approaches for RVF depending on the patients' condition, there are no established guidelines for the care of RVF. This study aimed to evaluate the results of laparoscopic colostomy in advanced cancer patients with RVF, and the safety and efficacy of this surgery. In this study, seven female advanced cancer patients with RVF were hospitalized and successfully treated with laparoscopic colostomy from 2015 to 2018 at our

Gallbladder in the Wrong Neighbourhood: Tackling Para-Urostomal Herniated Cholecystitis With Open Cholecystostomy.

Acute cholecystitis is usually managed with a cholecystectomy. Percutaneous cholecystostomy serves as a viable alternative when cholecystectomy is not appropriate. While parastomal hernias are a frequent complication of stomas, we present an unusual case of an elderly man with acute cholecystitis developing within a para-urostomal hernia. Given his significant comorbidities and personal preference, an open/surgical cholecystostomy under local anaesthesia was successfully performed, demonstrating a tailored approach to the management of a complex clinical scenario.

Parastomal Variceal Bleeding Attributed to Obstructive Pathology Successfully Treated by Percutaneous Variceal Embolization.

Parastomal variceal bleeding is a rare cause of gastrointestinal bleeding, but it should be considered as a potential etiology in patients with portal hypertension and surgical stomas. Though standard endovascular management has not yet been established, transjugular intrahepatic portosystemic shunt (TIPS) is a recommended treatment option leading to decompression of underlying portal hypertension. We present a case of parastomal bleeding treated with percutaneous parastomal variceal embolization, which is an emerging alternative therapy when TIPS cannot be performed.

Different results with early single-stage and staged repair in infants with ileal atresia: a two-case report on the effect of timing and treatment approach.

INTRODUCTION AND IMPORTANCE: Ileal atresia represents a major cause of newborn intestinal obstruction, requiring prompt surgical correction. Although a preferred procedure is the primary anastomosis in stable patients, the timing and surgical procedure can play a pivotal role in affecting morbidity, nutritional state, and the course of hospitalization. This article highlights the importance of these factors. CASE PRESENTATION: We describe two neonates diagnosed with isolated ileal atresia and treated using two different techniques. In case 1, early primary anastomosis was performed on day 5 o

[Median incisional hernias and coexisting parastomal hernias : new surgical strategies and an algorithm for simultaneous repair].

The co-occurrence of incisional and parastomal hernias (PSH) remains a surgical challenge. Standardized treatment guidelines are missing, and the patients concerned require an individualized surgical approach. The laparoscopic techniques can be performed with incised and/or stoma-lateralizing flat meshes with intraperitoneal onlay placement. The purely laparoscopic and laparoscopic-assisted approaches with 3-D meshes offer advantages regarding the complete coverage of the edges of the stomal areas and the option of equilateral or contralateral stoma relocation in cases of PSH, which are diffic

Outcomes of damage control surgery in perforated sigmoid diverticulitis: a comparison before and after implementing a new treatment algorithm.

BACKGROUND: Non-restorative resection (NRR) is the most common procedure for perforated diverticulitis, but frequently leads to permanent stoma and morbidity related to stoma reversal. For selected patients, primary anastomosis (PRA) is another option. Damage control surgery (DCS) may be an alternative, potentially offering a greater likelihood of stoma-free discharge. We compared DCS to conventional surgical treatment in terms of stoma-free discharge and postoperative complications. METHODS: This single center trial compared patients treated before and after the introduction of a new treatmen
