Bilateral medial rectus-levator inhibitory synkinesis: a case report and review of literature.

Congenital oculopalpebral synkinesis is a rare phenomenon in which specific ocular movements induce paradoxical eyelid responses. Most reported cases involve excitatory forms, whereas inhibitory variants are distinctly uncommon. We present a 15-year-old boy with congenital right ptosis who demonstrated bilateral gaze-evoked synkinetic eyelid movements. In addition, paradoxical right upper lid retraction was observed on downgaze, and vertical lid twitching was evident in upgaze and lateral gazes. Ocular motility and pupillary reactions were normal, and no jaw-winking was present.

Botulinum Toxin Type A Timing Efficacy in Patients With Synkinesis After Bell's Palsy.

OBJECTIVE: This study examines whether the timing of botulinum toxin type A (BoNTA) treatments affects outcomes in patients with synkinesis, a sequela of Bell's Palsy characterized by disconjugate facial muscle movement. STUDY DESIGN: A retrospective chart review. SETTING: An urban academic medical center. METHODS: We reviewed the charts of synkinesis patients treated from 2016 to 2022. Data included procedural notes, intervention dates, and scores from the House-Brackmann (HB), Facial Clinimetric Evaluation (FaCE), Synkinesis Assessment Questionnaire (SAQ), and Facial Grading System (FGS).

Bilateral Sprengel Deformities, Mirror Movements Synkinesis, and Arthrogryposis Multiplex Congenita: A Novel Combination.

INTRODUCTION: Introduction: Pellegrini-Stieda syndrome, characterized by medial collateral ligament (MCL) calcification or ossification, often follows a history of trauma. While rare, its distinct radiographic features pose diagnostic challenges. Conservative treatments are effective for many, but surgical intervention is necessary when they fail. CASE REPORT: A 31-year-old male with knee pain and stiffness, an inability to extend his knee, and a fixed flexion deformity. Radiological examinations confirmed heterotopic ossification along the MCL, indicating post-traumatic Pellegrini-Stieda syn

Rare Case of Post-Traumatic Abducens-Oculomotor Nerve Synkinesis Exhibiting Unusual Ocular Motility.

BACKGROUND Acquired synkinesis is a well-known phenomenon following oculomotor nerve injury. The abnormal movement appears within the distribution of 1 nerve, or other cranial nerves may be involved. The common misdirection of aberrant regeneration of oculomotor nerve involves the lid, extraocular muscles, or pupil. This report presents a case of aberrant connection between abducens and oculomotor nerve, which is quite rare. CASE REPORT A 21-year-old man with history of left-sided frontal, lateral orbital wall, and zygomatic fracture from head trauma in a motor vehicle accident presented for e

Masseteric-zygomatic nerve transfer for the management of eye closure-smile excursion synkinesis.

OBJECTIVE: The purpose of this study is to illustrate the efficacy of masseteric-to-zygomatic nerve transfer to address eye closure-smile excursion synkinesis after facial nerve paralysis. BACKGROUND: Synkinesis after facial nerve paralysis represents a wide range of facial movement disability. One manifestation is involuntary smiling with eye closure and a concomitant reduction of oral commissure movement with attempted smile ("frozen smile") - arising as a result of aberrant fibers populating the zygomatic branch-muscle complex. This is a particularly difficult area to treat with conservati

Spatial patterns of intrinsic brain activity and functional connectivity in facial synkinesis patients.

OBJECTIVES: As one of the most objectionable sequelae of facial paralysis, patients with facial synkinesis are more likely to be depressed and have lower quality of life than other facial paralysis patients. However, there is no research on the spatial patterns of intrinsic brain activity and functional connectivity in these patients. The objective of this study was to investigate the spatial patterns and cerebral plasticity of facial synkinesis patients. METHODS: A total of 20 facial synkinesis patients (18 men and 2 women; mean age: 33.35 ± 6.97 years old) and 19 healthy controls (17 men an

Repair of ocular-oral synkinesis of postfacial paralysis using cross-facial nerve grafting.

We present the surgical techniques and results of cross-facial nerve grafting that have been developed in the repair of ocular-oral synkinesis after facial paralysis. Eleven patients with ocular-oral synkinesis after facial paralysis underwent the cross-facial nerve grafting with facial nerve transposition at a tertiary academic hospital between 2003 and 2009. The patient selection for the study was based on the degree of disfigurement and facial function parameter rating using the Toronto Facial Grading System. The procedures used were surgeries done in two stages. All cases were followed up

Objective Assessment of the Repeated Botox Treatment to the Synkinesis of Facial Paralysis by the Integrated Electromyography.

BACKGROUND: The widely used botox type A (BTX-A) is effective against synkinesis in facial palsy sequelae. Repeated injections are necessary and permanent improvements have been reported. We objectively evaluated the changes in synkinesis at >6 months after BTX-A injection, including changes over time with the number of administrations. METHODS: In 48 patients who received multiple BTX-A injections, evaluation by the Sunnybrook Facial Grading System (FGS) and integrated electromyography (iEMG) was performed before treatment and at least 6 months after the first, second, and third BTX-A inject

A case with a rare type of trigemino-oculomotor synkinesis: clinical and blink reflex study.

Several abnormal synkinetic eye movements during jaw movements may often be seen after trauma or congenitally in the Marcus Gunn jaw winking phenomenon. The most frequent type consists of unilateral ptosis and retraction of the ptotic lid upon moving the jaw. The authors describe a case with isolated simultaneous adduction of the left eye upon jaw movement. This paper presents a rare case of Marcus Gunn jaw winking with trigemino-oculomotor synkinesis.

Selective mid-facial neurectomy using intraoperative nerve monitoring system for post-facial paralysis synkinesis.

Selective neurectomy of facial nerves is a treatment for post-facial paralysis synkinesis (PFS), which is a sequela of facial paralysis. Each branch of the facial nerve is stimulated during surgery, and nerve resection selection is determined based on the macroscopic findings. However, accurate assessment of the extent to which each nerve branch innervates a mimetic muscle is difficult. This may result in PFS recurrence or worsening of paralysis after surgery. Herein, we report a case of selective mid-facial neurectomy for PFS using an intraoperative nerve monitoring system. A 60-year-old man

Nerve Switch for Reducing Synkinesis After Gracilis Muscle Transfer Innervated Via the Masseter Nerve.

BACKGROUND: In surgical treatment for longstanding facial paralysis, muscle transplantation is considered a useful and important method. To obtain a spontaneous smile, the use of the facial nerve of the healthy side as a motor source is better, but use of the masseter nerve allows prompt reinnervation and powerful movement. However, in some patients in whom the masseter nerve is used, separating masticatory movement and commissure contraction is difficult. Solutions for such patients have not been determined. CASE HISTORY AND DISCUSSION: A 46-year-old female patient presented with longstandin
