A novel AXIN2 gene mutation in sagittal synostosis.

The bones of the skull are held together by fibrous joints called sutures. Premature fusion of these sutures leads to a pathologic condition called as craniosynostosis. Although at least 50 nuclear genes including FGFR2, TWIST1, TCF12, and SMAD6 were identified as causative of craniosynostosis; only 25% of the patients can be genetically diagnosed. Here, we report a 3-year-old Turkish Caucasian boy with sagittal craniosynostosis with a de novo loss-of-function mutation in exon 4 of the AXIN2 gene for the first time. The patient has frontal bossing, high anterior hair line, depressed nasal brid

What are the effects of metopic synostosis on visual function?

Metopic synostosis is a premature fusion of the metopic cranial suture. Small case studies into the effects on vision have suggested that there is a raised incidence of astigmatic refractive error with increased risk of failure to develop normal vision if reconstructive surgery is delayed beyond 7 months of age. The aim of this study was to look at a much larger group of patients to give more statistical significance on the incidence of significant refractive error and strabismus in cases of metopic synostosis and compare this with that known for the general population of children at a similar

The increase of metopic synostosis: a pan-European observation.

Metopic synostosis is thought to have an incidence of about 1 in 15,000 births. Traditionally, this makes it the third most frequent single-suture craniosynostosis, after scaphocephaly (1 in 4200-8500) and plagiocephaly (1 in 11,000). Our units have, independently from each other, noted a marked increase in the number of metopic synostosis over the recent years. This is a pan-European, retrospective epidemiological study on the number of cases with metopic synostosis born between January 1, 1997, and January 1, 2006. This number was compared to the prevalence of scaphocephaly, the most frequen

Spring-assisted correction of hypotelorism in metopic synostosis.

BACKGROUND: Metopic synostosis, apart from the pointed forehead, typically is characterized by hypotelorism with egg-shaped orbits on cephalography and the frontoorbital axis parallel or even converging superiorly. The frontoorbital axis angle is a novel parameter for analyzing and describing the orientation of the orbits. Current methods of surgery often result in undercorrection of the almost ever-present hypotelorism. The present study was performed to analyze a new technique, capable in this respect, using steel wire springs in conjunction with a cranioplasty. METHODS: A retrospective stu

Astigmatism in unilateral coronal synostosis: incidence and laterality.

PURPOSE: Unilateral coronal synostosis alters orbital development and can be associated with strabismus, oblique muscle dysfunction, astigmatism, and amblyopia. While the ocular effects are most dramatic on the side of the fusion, the shape of the contralateral orbit is also abnormal and the effect of these changes on bilateral visual development has not been fully described. The purpose of this study was to analyze the incidence and laterality of astigmatism in children with unilateral coronal synostosis. METHODS: We retrospectively reviewed the charts of 39 consecutive patients with unilate

Molding helmet therapy in the management of sagittal synostosis.

The current surgical treatment of sagittal craniosynostosis often fails to produce a normal cephalic index; however, recent reports of early endoscopic suture release and postoperative helmet therapy promise improved cranial symmetry. The role of helmet therapy is critical in this approach, but there are few reports on the use of helmet therapy after craniosynostosis surgery. The authors present a retrospective review comparing the results of surgery alone versus surgery and postoperative banding in treating children diagnosed with sagittal synostosis. Cephalic index (CI) measurements and the

Observations on a recent increase in plagiocephaly without synostosis.

OBJECTIVE: To verify and determine the cause of an increase in the referral of infants with plagiocephaly without synostosis (PWS) to a single tertiary craniofacial center. DESIGN: A chart review was performed for 269 infants with a diagnosis of PWS who presented to a single tertiary craniofacial center between 1979 and 1994. The pattern of referral for PWS was analyzed using both simple linear regression and time series regression analyses. In addition, the referral pattern for PWS was compared with that for infants seen at the same center who received a diagnosis of synostotic plagiocephaly

Treatment of coronal and metopic synostosis: 107 cases.

This paper presents a personal series of 107 children with synostosis, 53 unicoronal, 27 bicoronal, and 28 metopic cases (1 child had both metopic and bicoronal synostosis). In 16 patients, other vault sutures later closed partially, some requiring operation. All of the patients underwent surgical treatment without mortality or notable morbidity. Nineteen patients showed mild or moderate mental retardation; other defects included cleft palate, epicanthic folds, strabismus, and syndactyly. Three patients with unicoronal closure had a second operation about 5 years after the initial craniectomy,

Secondary Coronal Synostosis After Posterior Vault Distraction Osteogenesis.

Secondary synostosis of initially patent cranial sutures is a rare but recognized phenomenon following cranial surgery, particularly in syndromic cases. However, its incidence and risk factors in nonsyndromic patients remain unclear. This study aimed to investigate the occurrence and causes of secondary synostosis following surgical correction of lambdoid synostosis, with a focus on posterior vault distraction. Medical records of patients who underwent surgical treatment for unilateral or bilateral lambdoid synostosis between 2015 and 2024 at Keio University Hospital were retrospectively revie

Surgical Treatment of Post-Traumatic Radio-Ulnar Synostosis.

Radio-ulnar synostosis is a rare complication which develops following forearm trauma, the main manifestation being stiffness and leading to the loss of pronation and supination. For the patient, it is a very frustrating experience due to the impairment of the normal function of the forearm, whereas for the surgeon the treatment is difficult as, unfortunately, there is no consensus regarding the best way to approach it. Many surgical techniques and other kinds of adjuvant therapies have been developed in an effort to solve this disability. This paper presents an overview of the principal facto

Persistent Cranial Defects After Endoscopic Sagittal Synostosis Surgery.

INTRODUCTION: Incomplete cranial ossification is a rare complication of calvarial-vault remodeling for sagittal synostosis often requiring reoperation. Studies show an incidence ranging from 0.5% to 18%. METHODS: Infants with sagittal synostosis who underwent endoscopic sagittal synostectomy and barrel stave osteotomies with postoperative orthotic helmeting between 2003 and 2021 were included with minimum follow-up until the completion of helmeting. RESULTS: Of 90 patients, 86 met inclusion; 3 had defects (3.5%). Patients with and without cranial defects had no difference in age of surgery (

Cranioorbital Morphology Caused by Coronal Ring Suture Synostosis.

BACKGROUND: Minor cranial sutural synostosis is currently regarded as a rare diagnosis. As clinical awareness grows, a greater number of cases are being documented. This study aims to describe the variants of unicoronal synostosis with regard to major and minor sutural involvement and secondary effects on cranial and orbital morphology. The information is aimed to improve clinical diagnosis and management. METHODS: A retrospective study was conducted collecting preoperative computed tomographic scans of patients diagnosed with unicoronal synostosis and listed for surgical interventions, ident

Secondary bicoronal synostosis after metopic craniosynostosis surgical reconstruction.

Re-synostosis after standard surgical procedures for nonsyndromic craniosynostosis is a rare event, which can occur at the same suture or rarely in adjacent sutures. Here, we report 2 patients with primary metopic craniosynostosis who developed bicoronal synostosis in previously opened sutures several months after surgery.

Posttraumatic pediatric distal tibiofibular synostosis: a case report.

INTRODUCTION: Posttraumatic pediatric distal tibiofibular synostosis is a rare complication of fractures at this level, unreported at this time. Implications include limb length discrepancy, angular malalignment, and gait and biomechanical abnormalities. MATERIALS AND METHODS: The authors present a case report of this complication in a 7-year-old child and review the relevant English literature. The patient was treated for a closed distal tibia-fibula fracture and developed a synostosis at that level, with valgus malalignment of the ankle joint. The synostosis was resected and latest follow-u

Three-dimensional morphological analysis of isolated metopic synostosis.

Morphological differences were quantified in three-dimensions among individuals with untreated isolated metopic synostosis and between those individuals and similar aged-matched normal dry skulls to test two hypotheses: first, that the dysmorphology is a self-correcting condition; and second, that a lack of vertical growth of the skull produces this dysmorphology. Three-dimensional (3D) coordinates were recorded for 22 craniofacial landmarks from CT scans of 15 metopic patients, ranging from 5- to 32-months-old, and of four normal dry skulls, ranging in age from 6- to 36-months-old. Unilateral coronal synostosis treated by internal forehead distraction.

A 1-year-old infant with left hemicoronal synostosis was treated by distraction osteogenesis of the craniofacial skeleton using an internal distraction device. Surgery was performed through a coronal incision. The frontal bone and upper half of both orbits were first osteotomized en bloc after minimal epidural dissection of the supraorbital area and no epidural dissection around the coronal osteotomy site. The lateral one fourth of the frontal bone, including the right lateral half of the orbit, was left intact. The internal distraction device was fixed in the left temporal area. A 0.5-mm per
