[Potential risks of the Valsalva maneuver during reciprocating tachycardia in patients with WPW: a case report].

A 42-year-old man with cardiac preexcitation syndrome due to a left postero-septal anomalous pathway, showed an atrio-ventricular reciprocating tachycardia. During a Valsalva manouvre the arrhythmia changed into atrial fibrillation with ventricular rate and consequent hemodynamic worsening. The authors discuss the possible untoward effect of this manouvre in patients with anomalous a-v conduction pathways. They suggest that Valsalva manouvre should not be used outside of the hospital in patients with accessory pathways defined ¿at risk¿ on the basis of the electrophysiologic study.

Transcatheter ablative techniques for treatment of the permanent form of junctional reciprocating tachycardia in young patients.

Five patients with medically refractory incessant supraventricular tachycardia due to a posterior septal, slowly conducting accessory connection underwent transcatheter closed chest ablative treatment. The tachycardia characteristics were consistent with the permanent form of junctional reciprocating tachycardia. In each patient the ablative attempts resulted in independent interruption of either the anterograde limb (atrioventricular node-His bundle conduction) or the retrograde limb (accessory connection) of the tachycardia circuit. Permanent retrograde pathway ablation was achieved in only

[Association of nodoventricular and atrioventricular fibers with the origin of reciprocating tachycardia. Electrophysiological and anatomopathological aspect].

A 25 year old patient presented with several types of paroxysmal tachycardia. Analysis of the recordings and the results of endocavitary electrophysiological investigation suggest the presence of nodoventricular and atrio-ventricular fibres. These two accessory pathways were latent in sinus rhythm but formed part of reentry loops. The reciprocating rhythms used the Kent bundle in the retrograde direction, and either the normal nodo-hisian or the Mahaim fibres in the anterograde direction. Simple echos ascending by the nodo-hisian pathway and descending by the nodo-ventricular pathway were also

Comparison of adenosine effects on atrioventricular node reentry and atrioventricular reciprocating tachycardias.

BACKGROUND: Adenosine is an established first line therapy for the treatment of narrow complex tachycardias. The two most common etiologies of paroxysmal supraventricular tachycardia (SVT) are atrioventricular node reentry tachycardia (AVNRT) and atrioventricular reciprocating tachycardia (AVRT). HYPOTHESIS: We postulated that adenosine might have different effects on the termination of AVNRT vs. AVRT, and that these differences might assist in the noninvasive differentiation between these diagnoses. METHODS: Fifty-nine patients referred for the diagnosis and treatment of SVT were included i

[Reproducibility of reciprocal atrioventricular tachycardia during intracardiac and transesophageal electric stimulation].

A feasibility to provoke reciprocal atrioventricular tachycardias was examined in 23 patients with atrioventricular nodal tachycardia and 17 with orthodromal tachycardia in the presence of the Wolff-Parkinson-White syndrome with endocardiac and transesophageal diagnostic pacing. Atrioventricular nodal tachycardia could be induced in all 23 (100%) patients both by endocardiac and transesophageal pacing. Orthodromal tachycardia was provoked only in 9 (53%) of 17 patients by transesophageal pacing. It was generally noted that tachycardia induction required more "aggressive" regimens of transesoph

[Pseudoventricular tachycardia linked to a reciprocal antidromic rhythm from Mahaim's fibers].

A 24 year old female presented with paroxysmal pseudo-ventricular tachycardia with wide QRS complexes and complete atrioventricular dissociation. Electrophysiological studies showed a junctional tachycardia with an antidromic reciprocating rhythm. The activation involved Mahaïm nodo-ventricular fibres in the anterograde direction, the nodo-hisian pathway being the retrograde limb of the reentry pathway. The main point of interest of this case was the presence of complete atrioventricular dissociation during tachycardia, the atria remaining in sinus rhythm with the possibility of sinus capture

[Protective electrophysiological effects of somatostatin in patients with paroxysmal atrioventricular reciprocal tachycardias].

Within 30 seconds after intravenous injection of somatostatin (sandostatin, 0.1 mg) we observed restoration of sinus rhythm in patients with atrioventricular (AV) junctional or orthodromic reciprocal tachycardias. This action was associated with interruption of re-entry on anterograde direction of AV conductivity. Somatostatin elicited pronounced cardiac electrophysiological effects which were due to activation of parasympathetic nervous system. Besides, it evoked transient elevation of systolic (+12.6%) and diastolic (+13.5%) blood pressures. High efficacy and good tolerance of somatostatin a

[Criteria for a pathogenetic approach to antiarrhythmic therapy in paroxysmal supraventricular reciprocal tachycardia].

An electrophysiological study using programmed transesophageal atrial pacing was performed in 106 patients with abnormalities in the myocardial conduction system, of whom 102 suffered from paroxysmal supraventricular reciprocal tachycardia and/or paroxysmal atrial fibrillation. The electrophysiological properties of the myocardial conduction system were demonstrated to be labile. The factors determining the frequency and length of palpitations were outlined. The conditions for an onset of supraventricular reciprocal tachycardiac paroxysm and for existence of a reentrant chain were found to be

A new electrocardiographic algorithm using retrograde P waves for differentiating atrioventricular node reentrant tachycardia from atrioventricular reciprocating tachycardia mediated by concealed accessory pathway.

OBJECTIVES: The purpose of this study was to use an electrocardiographic (ECG) algorithm, derived from the results of radiofrequency ablation, to discriminate atrioventricular node reentrant tachycardia (AVNRT) from atrioventricular reciprocating tachycardia (AVRT) and to localize a concealed accessory pathway, prospectively. BACKGROUND: Information about ECG criteria for differentiating AVNRT from AVRT is limited and has not been confirmed by surgical or catheter ablation. METHODS: Four hundred six ECGs (obtained from 406 different patients) that demonstrated narrow QRS complex (< 0.12 s) s

Discrepant results of the total pacing prematurity in orthodromic reciprocating tachycardia with right bundle branch block.

The total pacing prematurity (TPP) is useful for distinguishing orthodromic reciprocating tachycardia (ORT) from atrioventricular nodal re-entrant tachycardia, but it may not be effective in patients with right bundle branch block (RBBB). We faced this challenge in an elderly woman, as RBBB and a prolonged transseptal conduction made it difficult to diagnose the tachycardia using the TPP. It is important to consider the presence or absence of RBBB when evaluating the results of the TPP.

Radiofrequency catheter ablation of patients with permanent junctional reciprocating tachycardia and long-term follow-up results.

BACKGROUND: Permanent junctional reciprocating tachycardia (PJRT) is an infrequent form of atrioventricular re-entrant tachycardia. We report the clinical and electrophysiological properties of PJRT and outcomes of radiofrequency catheter ablation (RCA) in a large group of patients. METHODS: We included 62 patients with the diagnosis of PJRT. Radiofrequency catheter ablation was performed in all. RESULTS: Location of accessory pathway was right posteroseptal in 37 (59,7%) cases, right midseptal in 3 (4,8%), left posterior in 7 (11,3%), left lateral in 5 (8,1%), left posterolateral in 3 (4,8%

Long-term efficacy of atenolol for atrioventricular reciprocating tachycardia in children less than 5 years old.

Atrioventricular reciprocating tachycardia (AVRT) using an accessory pathway is the most common supraventricular tachycardia observed in infancy and childhood. There is a general agreement to treat children older than 5 years who are on a potentially long-term antiarrhythmic agent with radiofrequency catheter ablation. Atenolol, a relatively long-acting and cardioselective beta-adrenoceptor blocker, has been used to control the various types of supraventricular tachycardia in children and adults. There are few reports on the use of atenolol in children <5 years old with AVRT. This retrospectiv

Effects of atrioventricular intervals on right atrial monophasic action potential duration during pacing or reciprocating tachycardia.

We evaluated the relationship between changes in the atrioventricular (AV) intervals and right atrial monophasic action potential duration at 90% repolarization (RA-MAPD90) in humans. In 4 patients, RA-MAPD90 was measured during AV reciprocating tachycardia (AVRT, n = 2) or AV node reentrant tachycardia (AVNRT, n = 2) and atrial or ventricular pacing at similar cycle lengths. In 10 patients, RA-MAPD90 was measured during continuous atrioventricular sequential pacing at AV intervals of 0, 50, 100, 150, 200, 250, 300, 350 msec for 90 sec at a cycle length of 333-400 msec. In patients with AVRT,

Enhanced atrioventricular conduction in patients without preexcitation syndrome: relation to heart rate in paroxysmal reciprocating tachycardia.

We studied the electrophysiologic characteristics of atrioventricular (AV) nodal conduction in patients with reciprocating tachycardia (RT) without ventricular preexcitation, and the relation of these characteristics to RT cycle length (CL). Thirty-five symptomatic patients who had a normal PR interval (0.13-0.20 second) during sinus rhythm underwent detailed intracardiac electrophysiologic study during which ventricular preexcitation was excluded, and the RT mechanism was determined. RT was due to reentry using an accessory AV pathway capable of conduction only in the retrograde direction (co

Retrograde atrial preexcitation following premature ventricular beats during reciprocating tachycardia in the Wolff-Parkinson-White syndrome.

In 34 successive patients with Wolff-Parkinson-White syndrome premature beats were induced from the right ventricular apex during reciprocating tachycardia (RT) at progressively shorter coupling intervals. The presence of an accessory pathway was confirmed by a reduction in the atrial cycle length (A-A interval) during which the premature ventricular beat was introduced. This retrograde preexcitation occurred at a time when the His-AV node pathway was refractory; i.e. there was premature activation of the atria over a pathway other than the His-AV node. 3 patients were excluded because of unsa
