Attentional set-shifting in Parkinson's disease patients with freezing of gait-acquisition and discrimination set learning deficits at the background?

Cognitive loading aggravates the freezing of gait (FoG), which is observed in approximately 50% of patients with Parkinson's disease (PD) in the advanced stages. To investigate whether a specific pattern of executive deficits, that is, attentional set-shifting and/or inhibitory control, are associated with FoG in PD, 30 PD patients with FoG (PD-FoG+) and 36 PD patients without FoG (PD-FoG-) and 22 control healthy subjects were examined with a comprehensive neuropsychological battery. Intra-Extra Dimensional Set shifting Test (IED) and Stop Signal Task (SST), selected from the Cambridge Automat

The neuropsychology of starvation: set-shifting and central coherence in a fasted nonclinical sample.

OBJECTIVES: Recent research suggests certain neuropsychological deficits occur in anorexia nervosa (AN). The role of starvation in these deficits remains unclear. Studies of individuals without AN can elucidate our understanding of the effect of short-term starvation on neuropsychological performance. METHODS: Using a within-subjects repeated measures design, 60 healthy female participants were tested once after fasting for 18 hours, and once when satiated. Measures included two tasks to measure central coherence and a set-shifting task. RESULTS: Fasting exacerbated set-shifting difficulties

Gait initiation in children with cerebral palsy.

The task of gait initiation (GI), or taking a first step from a static standing position requires the development of forward momentum while maintaining dynamic balance as the body's center of mass (COM) moves forward and outside the base of support. The dynamics of GI in children (aged 7-12 years) with hemiplegic cerebral palsy (CP), diplegic CP and children with typical development (TD) were compared to characterize the mechanics and control of this task. Ground reaction forces (GRFs) and muscle activity were collected during GI at three different self-selected speeds (slow, moderate and fast

Neural substrates of cognitive flexibility in cocaine and gambling addictions.

BACKGROUND: Individuals with cocaine and gambling addictions exhibit cognitive flexibility deficits that may underlie persistence of harmful behaviours. AIMS: We investigated the neural substrates of cognitive inflexibility in cocaine users v. pathological gamblers, aiming to disambiguate common mechanisms v. cocaine effects. METHOD: Eighteen cocaine users, 18 pathological gamblers and 18 controls performed a probabilistic reversal learning task during functional magnetic resonance imaging, and were genotyped for the DRD2/ANKK Taq1A polymorphism. RESULTS: Cocaine users and pathological gamb

Dyslexia and attentional shifting.

Dyslexia is a neurocognitive deficit primarily expressed in reading difficulties, but also affecting non-linguistic performance. Several studies report that dyslexics perform differently in the attentional blink paradigm, which indicates an impaired capacity to rapidly shift visual attention. However, attentional shifting can occur at different levels of cognitive processing, and it is unclear whether dyslexic attentional shifting is impaired at all levels, or only at the peripheral levels. We studied performance on a task-switching paradigm by dyslexics and normal readers to test whether the

Inequalities in tuberculosis control in Ethiopia: A district-level distributional modelling analysis.

BACKGROUND: Implementation studies indicate that the addition of tuberculosis diagnosis and treatment services into the community health extension workers' tasks-that is 'task-shifting'-improved case detection and treatment outcomes in Ethiopia. Given resource and operational constraints, only a limited number of areas can be targeted by an expanded task-shifting program. Therefore, we mapped the distributional disparities in tuberculosis services across regions and districts and modelled the equity pathways towards optimising national scale-up of this task-shifting intervention in Ethiopia.

Substitution of nurses for physicians in the hospital setting for patient, process of care, and economic outcomes.

RATIONALE: The demand for health services to deliver hospital-based care has increased due to an ageing population, more complex healthcare needs, comorbidities, and increasing healthcare costs. Nurse-physician substitution can improve access to care for patients who may otherwise have a significant wait for review by a physician. OBJECTIVES: The main objective of this review was to examine the impact of substituting nurses for physicians in the hospital setting (hospital inpatient units and outpatient clinics) on patient outcomes, process of care outcomes, and economic outcomes. The secondar

Impacting lives of people with diabetes: lessons learnt in India may be applicable to other low- and middle-income countries.

Delivering optimal care to people with diabetes in low- and middle-income countries (LMICs) such as India presents several challenges. The magnitude of the population affected by diabetes is considerable. Providing quality diabetes care to all people with diabetes is therefore not easy given the resource constraints, including inadequate numbers of trained specialists, diabetes educators and nurses, and lack of specialist clinics. Moreover, a large percentage of people with diabetes have to pay 'out of pocket' for their treatment, as the majority of healthcare in LMICs, especially in urban are

Organisational Impact of Remote Patient Monitoring for Heart Failure Management: A Survey of 28 Cardiology Departments and Community Practices in France.

BACKGROUND: Remote patient monitoring (RPM) for chronic heart failure (CHF) management has demonstrated clinical and economic benefits. However, data on its organisational impact remain limited, particularly in the French healthcare context following the integration of RPM into standard care pathways. The objective of this study was to describe the organisational impact of RPM for CHF management from the perspective of healthcare professionals across diverse practice settings in France. METHODS: A cross-sectional survey was conducted among 28 French cardiology departments and community practi

Operant procedures for assessing behavioral flexibility in rats.

Executive functions consist of multiple high-level cognitive processes that drive rule generation and behavioral selection. An emergent property of these processes is the ability to adjust behavior in response to changes in one's environment (i.e., behavioral flexibility). These processes are essential to normal human behavior, and may be disrupted in diverse neuropsychiatric conditions, including schizophrenia, alcoholism, depression, stroke, and Alzheimer's disease. Understanding of the neurobiology of executive functions has been greatly advanced by the availability of animal tasks for asse

Can Stimulus Valence Modulate Task-Switching Ability? A Pilot Study on Primary School Children.

Executive functions and emotional processes follow a time-dependent development that reflects the brain's anatomo-functional maturation. Though the assessment of these cognitive functions is largely examined, in children the role of emotions in the mental set-shifting is still rarely investigated. The aim of this study was to assess how attention shifting can be modulated by the valence of emotional stimuli. To this end, sixty-two primary school children were tested with a new emotional task-switching paradigm obtained by manipulating the emotional valence and physical features of the stimulus

Increased volumes of lobule VI in a valproic acid model of autism are associated with worse set-shifting performance in male Long-Evan rats.

Autism spectrum disorder (ASD) is a neurodevelopmental disorder with a skewed sex-based diagnostic ratio. While males are at a higher risk for ASD, it is critical to understand the neurobiology of the disorder to develop better treatments for both males and females. Our prior work has demonstrated that VPA (valproic acid) treated offspring had impaired performance on an attentional set-shifting task. The current study used MRI and regions of interest analyses to measure the volumes of cerebellar subregions in VPA and controls rats that had participated in the attentional set-shifting task. VPA

Profiles of vagal withdrawal to challenging interactions: Links with preschoolers' conceptual shifting ability.

The current study investigated profiles of vagal withdrawal in response to a challenging task in preschoolers. Also, the association between those profiles and conceptual shifting ability was assessed. Electrocardiogram of 43 four-year-olds was registered during a sequence of games including a win phase and a lose phase, while conceptual shifting ability was assessed via a standardized test. Cluster analyses revealed three profiles of cardiac vagal response to the task. Children in the first cluster displayed significant vagal withdrawal, children in the second cluster showed nonsignificant va

The factorial structure of executive functions in kindergarten children: an explorative study.

Executive functions (EF) are higher cognitive processes involved in the management of new, complex tasks with changing rules and/or the necessity to inhibit interferences. EF are central for academic success; they are necessary for planning, monitoring, and error detection. For adults, a three-factorial structure is often assumed, with the interrelated factors updating, shifting, and inhibition. Results regarding kindergarten EF are heterogeneous. Studies often do not include a direct comparison between different models. Some results support a unitary EF factor, while there is also evidence fo

Age of exposure-dependent effects of amphetamine on behavioral flexibility.

Drug use typically begins during adolescence, which is a period of ongoing neurobiological development that may confer heightened vulnerability to develop drug dependence. Previously, our lab has shown that amphetamine (AMPH)-induced deficits in a medial prefrontal cortex (mPFC)-sensitive working memory task are greater in rats exposed to the drug during adolescence compared to adulthood. Here, we examine potential age-dependent effects of AMPH exposure on behavioral flexibility tasks that are sensitive to disruptions in mPFC and orbitofrontal cortex (OFC) function. Male Sprague-Dawley rats we

Enhanced neural mechanisms of set shifting in musically trained adolescents and young adults: converging fMRI, EEG, and behavioral evidence.

Musically trained individuals have been found to outperform untrained peers in various tasks for executive functions. Here, we present longitudinal behavioral results and cross-sectional, event-related potential (ERP), and fMRI results on the maturation of executive functions in musically trained and untrained children and adolescents. The results indicate that in school-age, the musically trained children performed faster in a test for set shifting, but by late adolescence, these group differences had virtually disappeared. However, in the fMRI experiment, the musically trained adolescents sh

The Cognitive Cost of Repetitive Thinking: A Study on the Effects of Shifting and Updating on Rumination of Emotional Experiences.

The present study aimed to investigate the consequence of resource competition between post-emotional processing and concurrent cognitive tasks. Previous studies have shown that such a resource competition engenders both short-term (e.g., defeats in the execution of the working memory task) and long-term effects (e.g., procrastination or rumination following an emotional experience). We expected these effects to vary as a function of the different WM components involved (shifting, Study 1; updating, Study 2). In two studies, participants (Study 1: N = 48; Study 2: N = 42) were administered one

Increasing the use of long-acting injectable behavioral health medications in the community - A pharmacist-psychiatrist collaborative model.

BACKGROUND: Persons with severe mental illness or substance use disorder (SUD) benefit from long-acting injectable behavioral health medications (LAMs), which improve treatment adherence, reduce hospitalizations and emergency visits, and mitigate long-term biopsychosocial decline. LAMs remain underutilized due to multiple barriers across patients, pharmacies, providers, and payors. OBJECTIVE: The primary objective of this study was to conduct a time study and workflow analysis to inform a scalable and equitable payment model for community pharmacy-based administration of LAMs. METHODS: Six c

Strengthening primary care with the world health organization's basic package of interventions for rehabilitation: developing a list of interventions and a clinical resource.

BACKGROUND: Access to rehabilitation, a critical component of universal health coverage (UHC) remains limited, particularly in primary care (PC) and low-resource settings. To address this gap, the WHO Rehabilitation Programme developed the Basic Package of Interventions for Rehabilitation (Basic PIR). This toolkit is designed to empower PC non-rehabilitation workers to deliver a prioritised set of easy, safe and effective rehabilitation interventions through task-sharing. This article describes the participatory process used to develop the WHO's Basic PIR, specifically its list of intervention
