[Diagnostic steps in disk dislocations and other frequent illnesses of the temporomandibular joint].

Modern diagnosis of the temporomandibular joint (TMJ) consists of multiple steps, based on each other and amplifying one another. The first step is the clinical functional analysis, respecting general medical as well as specific dental aspects. If need arises electronic axiography, recording the movement of TMJ hingeaxis, is being added as well as occlusal analysis on mounted plaster casts. In case of open questions at this point of analysis, magnetresonance- and computer-tomographic imaging is brought in. The present paper is an overview describing the current step by step diagnosis of TMJ di

Arthrocentesis procedure: using this therapeutic maneuver for TMJ closed lock management.

Temporomandibular joint (TMJ) disorder is a term that encompasses a number of overlapping conditions, such as closed lock. Closed lock of the TMJ is considered a consequence of a nonreducing deformed disc acting as an obstacle to the sliding condylar head that usually causes a decrease in the maximum mouth opening and acute pain. The management of the TMJ is still controversial. Thus, arthrocentesis of the TMJ is a valuable modification of the traditional method of arthroscopic lavage, which consists of washing the joint in order to remove chemical inflammatory mediators and intra-articular ad

Recent developments in understanding temporomandibular joint disorders. Part 1: Bone marrow abnormalities of the mandibular condyle.

Improvements in diagnostic imaging of the temporomandibular joint in the past 20 years have shown that disk displacement is the most frequent abnormality in patients with joint pain and dysfunction. The aetiology of the pain is poorly understood. Recent studies with magnetic resonance imaging have demonstrated a number of other changes in the TMJ. In this paper I review the possible significance of alterations in the condylar bone marrow and its relationship to osteonecrosis.

[Magnetic resonance imaging of the TMJ disc in asymptomatic volunteers].

To determine whether disc displacement of temporomandibular joint exists in asymptomatic subjects in China, we examined fifty temporomandibular joints (TMJs) of 39 asymptomatic volunteers by magnetic resonance imaging (MRI). All subjects had no history of TMJ pain, joint noise, deviation, and opening limitation, Oblique sagittal and coronal MR images were obtained by scanning perpendicular and paralell to the longitudinal axis of the condyle respectively. MR imaging depicted anterior, medial and rotational disc displacement in 32% of the asymptomatic joints. Disc displacement in asymptomatic s

Craniomandibular stiffness in myogenous and arthrogenous CMD patients, and control subjects: a clinical and experimental investigation.

The purpose of this study was to compare active maximum mouth opening, endfeel distance, and craniomandibular stiffness values of three craniomandibular disorder (CMD) patient subgroups and a control group. The CMD patient subgroups consisted of myogenous pain patients and arthrogenous pain patients with a 'closed lock' and arthrogenous pain patients without a 'closed lock'. Both myogenous pain patients and the 'closed lock' patients showed great differences on all parameters (P < 0.05-P < 0.001). However, the arthrogenous pain patients without a 'closed lock' revealed no statistical differenc

The mechanism of beneficial effect of radiofrequency therapy on masticatory muscles in temporomandibular disorders - a literature review.

Temporomandibular disorders (TMD) is one of the most common problem in contemporary dentistry. It is a term covering dysfunction of the masticatory muscles and the temporomandibular joints. Patients are suffering from severe pain, followed by limited mandibular opening and sounds in the temporomandibular joints during jaw movement. TMD influences the quality of life because the symptoms can become chronic and difficult to manage. Radiofrequency waves (RF) are electromagnetic waves with low energy and high frequency. They provide pain relief without causing significant damage to the nervous tis

[The CT scanner and the temporomandibular joint. Anatomo-radiologic comparisons. Apropos of 14 cases of algodysfunctional of the manducator system].

Among 500 CT scan of temporomandibular joint (TMJ), examined since 1982 by bilateral direct sagittal method (Department of Radiology, Pr. A. TREHEUX, CHU Nancy-Brabois), the authors have retained 14 cases of patients with symptoms related to TMJ's dysfunction cured by surgery (Department of Maxillo Facial Surgery, Pr STRICKER, CHU Nancy). These cases were chosen among hundred patients annually examined by CT scan, for various diseases (TMJ's dysfunctions, traumatisms, infections, inflammatory diseases...). These correlations between radiology and surgery about 26 TMJ (2 patients underwent surg

Temporomandibular joint dysfunction and facial pain in children: an approach to diagnosis and treatment.

A model for the diagnosis and treatment of temporomandibular joint dysfunction and facial pain in children is presented. Emphasis is placed on systematic assessment of physical, psychologic, and behavioral factors when conservative medical therapy is inadequate for symptom relief. The model represents a multidisciplinary approach to patient care which is described through case presentations. The results of research on the incidence of primary psychopathology in 53 children and 322 adults evaluated during a 3-year period for temporomandibular joint dysfunction and facial pain are also presented

High-resolution computed tomography of the osseous temporomandibular joint. Some normal and abnormal appearances.

A standardized CT procedure for examination of the temporomandibular joint (TMJ) with axial and coronal scanning as well as reformatted coronal and sagittal sections, was performed on 32 adults. These included subjects with normal TMJ and patients suffering from muscular dysfunction/disc displacement, arthrosis or rheumatoid arthritis. Some normal CT appearances simulating disease were presented. Axial CT scanning appeared to be the most useful method for demonstrating osseous abnormalities of the TMJ. The diagnostic information was occasionally supplemented by the coronal scanning, which may

Benign chronic orofacial pain. Clinical criteria and therapeutic approaches.

Trigeminal neuralgia and temporomandibular joint (TMJ) pain-dysfunction syndrome are the most common benign orofacial pain disorders. Because orofacial pain can arise from many sources and can be exacerbated by emotional stress, diagnosis is complex. Primary trigeminal neuralgia (tic douloureux) is characterized by severe paroxysms of pain often initiated by stimuli applied to trigger zones. Atypical facial neuralgia is a more loosely defined collection of orofacial neuralgias with variable distribution of pain and age of onset. Treatment (ie, pharmacologic and surgical procedures, nerve block

The red ear syndrome.

The complaint of a painful, burning, red ear may be associated with irritation of the third cervical root, temporomandibular joint dysfunction, or thalamic syndrome. It may also occur without obvious structural cause in response to touch or heat. The condition may be an example of the ABC (Angry Back-firing C-nociceptor) syndrome with the increase in ear temperature being caused by the antidromic release of vasodilator peptides.
