Management of flexor tendon injuries in the digits: an update.

Currently used techniques of flexor tendon repair and methods of postoperative rehabilitation differ significantly from those used still 10-15 years ago. Techniques of the repair evolved from 2-strand sutures (the Kessler suture) in the direction of much stronger 4-strand and 6-strand sutures (the Adelaide and Savage sutures), what reduced the risk of failure of the repair and made possible more intensive rehabilitation. Also rehabilitation regimes changed on more comfortable for patients than older protocols and allowed to achieve better functional outcomes of the treatment. This study presen

[Secondary reconstruction in zones V-VII in extensor tendon injuries].

BACKGROUND: Due to various functional impairments after primary extensor tendon repair or lack of treatment, secondary tendon reconstruction is often required. Anatomical considerations, the outcome of the injury and its treatment and the patients' individual demands on the function of the hand affect the choice of the procedure. OBJECTIVE: Description of techniques for secondary reconstruction after extensor tendon injury in zones V-VII. MATERIAL AND METHODS: Overview of surgical treatment concepts for secondary extensor tendon repair in zones V-VII of the extensor tendons of the fingers an

Management of Tendon and Bony Injuries of the Distal Phalanx.

Acute tendon and bony injuries of the distal phalanx are challenging injuries because they may result in chronic pain, hypersensitivity, stiffness, and deformity if they are not adequately treated. Flexor tendon avulsions require early surgical repair. Conversely, most extensor tendon injuries and fractures heal well with nonoperative treatment. However, surgery is indicated in selected patients, and meticulous technique is required to achieve good postoperative outcomes. In this article, we outline the pertinent clinical anatomy of the distal phalanx, review the current literature regarding t

Central slip extensor tendon injuries: a systematic review of treatments.

This systematic review describes and compares outcomes of operative and non-operative management of central slip extensor tendon injuries. A PRISMA-compliant methodology identified 3785 studies. Of these, 29 underwent full text review. No randomized controlled trials were identified. Nine studies evaluated treatment modalities specific to cohorts with acute central slip injuries in adults. A range of operative and non-operative elements of management was identified although no studies directly compared the two. Where aspects of rehabilitation were studied, this was always after surgery. The ev

Unravelling the genetic susceptibility to develop ligament and tendon injuries.

PURPOSE: In the last decades, new evidences supported the relationship polymorphisms and the susceptibility to develop ligament and tendon injuries. We performed a review of the genetic factors involved in tendon and ligament injuries. METHODS: A review of the literature has been performed in a systematic fashion by using the terms "sports", "ligaments", "injuries", "tendon" and "genetics". PubMed, Embase, CINAHL, Cochrane, Medline and Google Scholar databases were screened over the years 1984-2014. RESULTS: The genes currently associated with tendon and ligament injuries include gene encodi

Re-rupture rate of primarily repaired distal biceps tendon injuries.

BACKGROUND: Distal biceps tendon rupture is a common injury, and primary repair results in excellent return of function and strength. Complications resulting from distal biceps tendon repairs are well reported, but the incidence of re-ruptures has never been investigated. METHODS: A search of the Mayo Clinic's Medical/Surgical Index was performed, and all distal biceps tendon repairs from January 1981 through May 2009 were identified. All patients who completed 12 months or more of follow-up were included. All charts were reviewed and patients contacted as necessary to identify a re-rupture.

Suture anchor tenodesis in repair of distal Achilles tendon injuries.

BACKGROUND: Distal Achilles tendon avulsions are in the form of either bony and nonbony avulsion of Achilles tendon from its calcaneal insertion. METHODS: Four patients with distal Achilles tendon avulsions or ruptures which were treated with tendon to bone repair using suture anchors are presented here. Operated leg was immobilized in above-knee cast for 4 weeks while the patient walked non-weight-bearing. Then, cast was changed to below knee, and full weight-bearing was allowed. Patients underwent gait analysis minimum at first postoperative year. RESULTS: Mean American Orthopedics Foot An

Functional outcome in patients with zone V flexor tendon injuries.

INTRODUCTION: Zone V flexor tendon injuries may involve major nerves and arteries as well as the wrist and finger flexors. Although these injuries are not infrequent, few studies have reported functional outcomes. The purpose of this study was to evaluate the functional outcome in patients with flexor tendon repairs in zone V. MATERIALS AND METHODS: Eighteen patients with repaired zone V flexor tendon injuries were followed up for an average of 20 months. The postoperative rehabilitation program consisted of a combined regime of modified Kleinert and modified Duran techniques. Outcome paramet

Rehabilitation after surgery for flexor tendon injuries in the hand.

BACKGROUND: Postoperative rehabilitation of the flexor tendons in the hand consists of a short period of immobilisation while pain and swelling diminish, followed by progressive mobilisation to maximize the range of motion of the affected fingers. By altering the time of immobilisation and the manner of subsequent mobilisation different rehabilitation regimes are created. OBJECTIVES: To determine, with evidence from randomised controlled trials, the optimal rehabilitation strategy after surgery for flexor tendon injuries in the hand. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal

MR imaging of ligament and tendon injuries of the fingers.

Magnetic resonance (MR) imaging can provide important information for diagnosis and evaluation of soft-tissue trauma in the fingers. An optimal imaging technique should include proper positioning, dedicated surface coils, and specific protocols for the suspected abnormalities. Familiarity with the fine anatomy of the normal finger is crucial for identifying pathologic entities. MR imaging is a powerful method for evaluating acute and chronic lesions of the stabilizing articular elements (volar plate and collateral ligaments) of the fingers and thumbs, particularly in the frequently affected pr

Tendon transfers for peripheral nerve injuries of the lower extremity.

1. Nerve injuries in the lower extremity not infrequently result in paralysis of the muscles innervated by the injured nerve. 2. Disability caused by residual paralysis can be reduced by appropriate muscle transfers. 3. Deformity must be corrected before muscle transfer. 4. The most useful transfers in the lower extremities are the transfer of the tibialis posterior tendon anteriorly for correction of the equinovarus deformity of peroneal nerve injury; the transfer of the tibialis anterior with or without the peroneus longus to the calcaneus to prevent calcaneus deformity caused by loss of the

Load-induced regulation of tendon homeostasis by SPARC, a genetic predisposition factor for tendon and ligament injuries.

Tendons and tendon interfaces have a very limited regenerative capacity, rendering their injuries clinically challenging to resolve. Tendons sense muscle-mediated load; however, our knowledge on how loading affects tendon structure and functional adaption remains fragmentary. Here, we provide evidence that the matricellular protein secreted protein acidic and rich in cysteine (SPARC) is critically involved in the mechanobiology of tendons and is required for tissue maturation, homeostasis, and enthesis development. We show that tendon loading at the early postnatal stage leads to tissue hypotr

Pes planus in childhood due to tibialis posterior tendon injuries. Treatment by flexor hallucis longus tendon transfer.

Flat foot due to rupture of the tibialis posterior tendon has not previously been described in children. We present three young patients who developed unilateral pes planus after old undiagnosed lacerations of the tendon. Transfer of the flexor hallucis longus to the distal stump of the tibialis posterior tendon achieved good results in all three cases.

Extensor Tendon Injuries: A New Classification, Strong Repairs, and Easier Therapy.

A new 5-zone classification of extensor tendons in the hand and forearm is proposed to regroup surgical methods and discuss outcomes. Similar to flexor tendon zones, the new classification unifies the zones of fingers and thumbs. Based on this classification, strong surgical repairs (6-strand repairs) are always used for zone 2. In zone 1c and 3, 6-strand, 4-strand, or other repair methods are used. The finger is protected with short splinting with early active motion or hand use. Many principles of flexor tendon repairs are applicable to these extensor tendon areas. A solid strong repair abol
