Electromyographic analysis of muscle function after latissimus dorsi tendon transfer.

The latissimus dorsi transfer is an established method for irreparable tears of the rotator cuff. The original task of the muscle is to move the arm downward and to rotate it internally. After the transfer, it has to perform the completely new function of elevating the arm. We used surface electromyography to calculate the mean values of the activity pattern in relation to the Constant score in 45 patients. We assessed patients after 6 weeks and at 6, 9, and 19 months. After the operation, most patients showed a typical activity pattern of the latissimus dorsi on electromyography, which improv

[Flexor digitorum superficialis tendon transfer for intrinsic paralysis in leprosy].

A follow up study was performed in the rehabilitation centre for patients with leprosy in Hôchiminhville - Vietnam. All patients had claw-hand deformity due to ulnar and median nerve intrinsic paralysis. Thirty-two affected hands (128 long fingers) were included in the study. A Bunnel-Littler tendon transfer procedure was performed using a four-tailed graft of the flexor digitorum superficialis of the third finger. Clinical evaluation included anatomical measurements of interphalangeal and metacarpal joints in complete extension and in the intrinsic position. In the open hand assessment, 48.5%

Tendon transfer for equinovarus deformed foot caused by cerebrovascular disease.

Surgical correction was performed on 125 patients who had equinovarus deformity caused by a cerebrovascular accident and who needed an ankle foot orthosis for walking. The operative procedures involved anterior transfer of the long toe flexors (flexor hallux longus and flexor digitorum longus; long toe flexor group) or lateral transfer of the anterior tibial tendon (anterior tibial tendon group), combined with lengthening of the Achilles tendon. On evaluation more than 2 years after surgery, 83 of 110 patients of the long toe flexor group and eight of 15 patients of the anterior tibial tendon

Extensor hallucis longus tendon transfer for correction of hallux varus.

The authors describe radiographic results from nine patients in whom hallux varus was repaired by transferring the extensor hallucis longus tendon (EHLT) under the deep transverse intermetatarsal ligament into the proximal-lateral base of the proximal phalanx. X-ray films revealed clinically significant changes in biomechanical relations, specifically in the hallux abductus angle. No clinically significant changes were seen in the intermetatarsal angle. The EHLT transfer procedure described is a valuable surgical alternative for correction of iatrogenic hallux varus deformity when combined wit

Split posterior tibial tendon transfer for spastic equinovarus foot deformity.

This paper describes a simplified technique for split posterior tibial tendon transfer in the treatment of spastic equinovarus deformity of the foot. Thirteen children with spastic equinovarus foot deformities were treated at Children's Rehabilitative Services in Phoenix, Arizona, from 1983 to 1986. The technique was modified in 10 of the 13 patients by attaching the split posterior tibial tendon more proximally to the peroneus brevis, compared to a more distal attachment as described by other authors. The mean length of followup was 21 months. Eleven patients obtained a good or excellent resu

Wrist deformity in rheumatoid arthritis extensor carpi ulnaris tendon transfer.

A procedure designed to correct volar subluxation of the ulnar carpus in patients with rheumatoid wrist deformity is described. Twenty-five patients were treated with synovectomy, distal ulna resection, and transfer of the distal extensor carpi ulnaris to the extensor carpi radialis longus. Comfort and improved function were reported, and no significant complications were noted. Follow-up during the 14-month study period demonstrated improvement in volar-carpal subluxation confirmed on x-ray films, stability of metacarpal-phalangeal alignment, and no deterioration of the ability to ulnar devia

Juncturae Tendinum Transfer for Extensor Tendon Reconstruction: A Case Report.

CASE: A 9-year-old girl, with a history of crush injury to her right hand 2 years ago, presented with inability to extend her index finger. Exploration revealed segmental loss of extensor tendons to the index finger with a pre-existing scar in the route of tendon transfer. The juncturae tendinum (JT) between extensor digitorum communis of the middle and ring fingers (Von Schroeder -3y) was transferred to reconstruct the extensor tendon. At 1-year follow-up, finger extension was restored, with improved grasp and release. CONCLUSION: JT transfer could prove useful in reconstructing zone II exte

Thumb Function and Electromyography Result after Modified Camitz Tendon Transfer.

BACKGROUND: Various techniques of opponensplasty have been developed with the aim of restoring the thumb function. The modified Camitz opponensplasty is a simple technique done together with an open carpal tunnel release. It restores thumb palmar abduction soon after the procedure, during such time that the abductor pollicis brevis (APB) is still recovering. The aim of this study was to assess the recovery and level of activity of the abductor pollicis brevis and palmaris longus (PL) muscles during thumb opposition and abduction after performing the modified Camitz opponensplasty. METHODS: Th

The "double wrist flexor" tendon transfer for radial nerve palsy.

In isolated high radial nerve palsy, it is traditionally taught that one should not use both wrist flexors for tendon transfers. Over the last 17 years, the author has encountered 4 unusual cases of high radial nerve palsy with concurrent direct injury to the pronator teres, flexor digitorum superficialis, and the palmaris longus in the proximal forearm. In these cases, the author used both wrist flexors, namely, the flexor carpi radialis to restore wrist extension and the flexor carpi ulnaris to restore finger/thumb extension as well as thumb radial abduction. Despite the major loss of wrist

Split posterior tibial tendon transfer for varus deformity of hindfoot.

Sixteen patients (23 feet) who underwent split posterior tibial tendon transfers were evaluated. The patients were seen on a followup basis for a minimum of 1 year postoperatively. The causes were spastic cerebral palsy in 13 feet, spastic-athetoid cerebral palsy in 3 feet, hydrocephalus in 3 feet, and other diseases in 4 feet. The indication for surgery was varus deformity during the stance phase of gait and increased varus deformity during the swing phase of gait because of spasticity of the posterior tibial muscle. Heel cord lengthening was done on 17 feet. Preoperative and postoperative ga

Opponensplasty by extensor indicis and flexor digitorum superficialis tendon transfer.

From 1977 to 1988, 166 patients with median nerve paralysis of varied aetiology underwent opponensplasty. In 50 of these the extensor indicis was used, and in 116 the flexor digitorum superficialis of the ring finger. An analysis of these hands showed that the EI opponensplasty was best in supple hands and FDS opponensplasty was more suitable for less pliable hands. There were fewer complications seen after FDS opponensplasty if the detachment of the donor tendon was done through a volar oblique incision rather than the conventional lateral incision.

Combined anteroposterior tibial tendon transfer in post-traumatic peroneal palsy.

Posterior tibial tendon transfer to the dorsum of the foot for correction of traumatic paralytic peroneal palsy is a well-accepted treatment option. A careful review of the literature reveals that the results of treatment in this specific patient group are not always encouraging. This paper reports the results of a combined anteroposterior tibial tendon transfer in nine patients with traumatic paralytic peroneal palsy who initially used an ankle-foot orthosis to ambulate. Percutaneous tendo achillis lengthening to correct fixed ankle equinus was performed in six of the patients. At a minimal 2

[Tendon transfer of the extensor carpi radialis longus muscle in tendon rupture of the extensor pollicis muscle].

The most common ruptures of all of the tendons of the hand are those of the extensor pollicis longus tendon (EPL). As to its etiology, the rupture of EPL can spontaneously occur tendosynovitis in rheumatic patients or rarely in result of conservative treatment of distal forearm fractures. During the last 18 months, 4 cases of EPL ruptures following the conservative treatment of distal forearm fractures have been diagnosed in our clinical material. All patients underwent the transfer of extensor carpi radialis longus (ECRL) tendon due to. We present our indications of the technique, and the cli
