An overview in tendon's physiology, pathomorphology, and treatment options.

Tendon degeneration and tendinopathy are common musculoskeletal disorders characterized by chronic pain, impaired function, and structural changes within the tendon. A cascade of pathological changes appears due to several risk factors such as ageing, overuse, obesity, or specific medications. Among these changes are collagen disorganization, neovascularization, altered extracellular matrix composition, and changes in cellular components. Recent studies have highlighted the role of tenocyte apoptosis, matrix metalloproteinase activity, and mechanical stress in the progression of tendinopathy.

Engineered Nestin+ TSPC-Derived Exosomes Promote Tendon Repair Via Metabolic Reprogramming.

Tendon repair remains a substantial clinical problem with limited treatment options. While Nestin+ tendon stem/progenitor cells (TSPCs) are known to regulate tendon healing, their clinical translation faces challenges. Here, we show that exosomes derived from engineered Nestin-overexpressing TSPCs (Nes-EVs) significantly ameliorate tendinopathy in multiple mouse models. We isolated Nes-EVs following lentiviral Nestin overexpression and demonstrated their capacity to reduce cellular senescence and promote tenogenic differentiation in vitro. In collagenase-induced (WT and Nes-CreERT2;iDTR) and n

Editorial Commentary: Shoulder Long Head of the Biceps Tendon Transposition Without Tenotomy May Be a Beneficial Augment for Massive Rotator Cuff Tear Repair.

Although many surgeons discard the long head of the biceps (LHB) tendon during arthroscopic rotator cuff repairs, several authors have argued that the LHB tendon may serve as a biologic and structural augment for massive repairs. There are multiple benefits of incorporating the LHB tendon autograft in difficult-to-repair massive rotator cuff tears. First, by maintaining the glenoid origin, the tendon likely retains a significant native blood supply, as well as tenocytes, which may support the healing process. In addition, there are several biomechanical benefits. The LHB may act as a dynamic h

Lipid nanoparticle-assisted miR29a delivery based on core-shell nanofibers improves tendon healing by cross-regulation of the immune response and matrix remodeling.

Inferior healing and peritendinous adhesions are the major clinical problems following Achilles tendon injury, leading to impaired motor function and an increased risk of re-rupture. These complications are presumed to be inextricably linked to inflammation and fibroscar formation. Here, microRNA29a is identified as a promising therapeutic target for tendon injury through the cross-regulation of the immune response and matrix remodeling. MiR29a-LNPs were successfully prepared by microfluidic technology. They are then loaded into the core-shell nanofibers to achieve local delivery in the injure

Calcifying tendinitis of the long head of the biceps brachii and superior labrum: a report of one case and literature review.

Calcifying tendinitis of the shoulder is characterized with the formation of the calcium deposits in tendon, which mostly involves supraspinatus. The formation of the calcium deposits may be related to the excessive mechanical stimulation and non-tenocytes generation from tendon stem cell. The disease is usually associated with pain, especially when activating the shoulder. We describe one case of calcifying tendinitis involving the long head of the biceps brachii and superior labrum. The related literature and researches are also reviewed. After reviewing the past articles, there are no simil

Thymoquinone inhibited the chondrogenic differentiation of tendon-derived stem cells caused by tendon injury.

BACKGROUND: The micro-injury of collagen fibers occurs as the tendon is stretched repeatedly between the strains of 4% and 8%, which results in the cumulative micro-damage in tendon. In prior studies, we have shown that micro-injured tendon slices with 6.4% strain promoted the chondrogenic differentiation of tendon-derived stem cells (TDSCs) through the activation of endoplasmic reticulum (ER) stress. AIM: To investigate the potential of thymoquinone (TQ) to alleviate ER stress, and, consequently, to suppress the chondrogenic differentiation of TDSCs. METHODS: Decellularized tendon slices, s

Cyclic strain alters the expression and release of angiogenic factors by human tendon cells.

Angiogenesis is associated with the tissue changes underlying chronic overuse tendinopathy. We hypothesized that repetitive, cyclic loading of human tendon cells would lead to increased expression and activity of angiogenic factors. We subjected isolated human tendon cells to overuse tensile loading using an in vitro model (1 Hz, 10% equibiaxial strain). We found that mechanically stimulated human tendon cells released factors that promoted in vitro proliferation and tube formation by human umbilical vein endothelial cells (HUVEC). In response to cyclic strain, there was a transient increase i

The role of recombinant human platelet-derived growth factor-BB (rhPDGF-BB) in orthopaedic bone repair and regeneration.

Recombinant human PDGF BB homodimer (rhPDGF-BB) is a potent recruiter of, and strong mitogenic factor for, cells crucial to musculoskeletal tissue repair, including mesenchymal stem cells (MSCs), osteogenic cells and tenocytes. rhPDGF-BB also upregulates angiogenesis. These properties allow rhPDGF-BB to trigger the cascade of bone and adjoining soft tissue repair and regeneration. This mechanism of action has been established in numerous preclinical and clinical studies. Demonstration of the safety and efficacy of rhPDGF-BB in the healing of chronic foot ulcers in diabetic patients and regener

Scleraxis is required for maturation of tissue domains for proper integration of the musculoskeletal system.

Scleraxis (Scx) is a basic helix-loop-helix transcription factor that is expressed persistently in tendons/ligaments, but transiently in entheseal cartilage. In this study, we generated a novel ScxCre knock-in (KI) allele, by in-frame replacement of most of Scx exon 1 with Cre recombinase (Cre), to drive Cre expression using Scx promoter and to inactivate the endogenous Scx. Reflecting the intensity and duration of endogenous expression, Cre-mediated excision occurs in tendinous and ligamentous tissues persistently expressing Scx. Expression of tenomodulin, a marker of mature tenocytes and lig

Transcription factor Meis1b regulates craniofacial morphogenesis in zebrafish.

BACKGROUND: Meis family of transcription factors operates in Pbx-Meis-Hox regulatory network controlling development of various tissues including eye, limbs, heart, hindbrain or craniofacial skeletal elements originating from the neural crest. Although studies in mouse provide abundant information about Meis factors function in embryogenesis, little is known about their role in zebrafish. RESULTS: We generated zebrafish lines carrying null mutations in meis1a, meis1b, meis2a, and meis2b genes. Only meis1b mutants are lethal at larval stage around 13 dpf whereas the other mutant lines are viab

Editorial Commentary: Long Head Biceps Tendon Can Be Used Like a Split Skin Graft: Mesh It and Augment Rotator Cuff Repairs.

Full-thickness tears of the rotator cuff are common, and surgical treatment is indicated in most cases. Arthroscopic repair has become the standard technique, but despite improved surgical techniques, instrumentations, and implants, failure rates continue to be high and can reach 40% and more. Augmented repairs with allograft patches or autograft tissue, such as long head biceps tendon or fascia lata, have been used with mixed results. However, autograft augmentation seems to be resulting in superior outcomes and higher healing rates. Treating skin defects with split-skin grafts is the gold st

The Holy Grail of Orthopedic Surgery: Mesenchymal Stem Cells-Their Current Uses and Potential Applications.

Only select tissues and organs are able to spontaneously regenerate after disease or trauma, and this regenerative capacity diminishes over time. Human stem cell research explores therapeutic regenerative approaches to treat various conditions. Mesenchymal stem cells (MSCs) are derived from adult stem cells; they are multipotent and exert anti-inflammatory and immunomodulatory effects. They can differentiate into multiple cell types of the mesenchyme, for example, endothelial cells, osteoblasts, chondrocytes, fibroblasts, tenocytes, vascular smooth muscle cells, and sarcomere muscular cells. Forelimb contractures and abnormal tendon collagen fibrillogenesis in fibulin-4 null mice.

Fibulin-4 is an extracellular matrix glycoprotein essential for elastic fiber formation. Mice deficient in fibulin-4 die perinatally because of severe pulmonary and vascular defects associated with the lack of intact elastic fibers. Patients with fibulin-4 mutations demonstrate similar defects, and a significant number die shortly after birth or in early childhood from cardiopulmonary failure. The patients also demonstrate skeletal and other systemic connective tissue abnormalities, including joint laxity and flexion contractures of the wrist. A fibulin-4 null mouse strain was generated and us

Rotator cuff degeneration and lateral epicondylitis: a comparative histological study.

OBJECTIVES: Rotator cuff tendinitis and lateral epicondylitis are common in clinical practice but the underlying pathology is poorly understood. The study examined both normal and biopsy tendon specimens histologically, to determine the mechanisms involved in tendon degeneration. METHODS: Rotator cuff tendons from 83 cadavers aged 11-94 and tendon biopsy specimens from 20 patients with lateral epicondylitis aged 27-56 years were examined histologically. RESULTS: The microscopic changes found in the tendon biopsies from the elbow were similar to those found in the cadaveric rotator cuff tendo

Editorial Commentary: Allogenic Dermal Fibroblasts in Collagen Matrix Scaffold Enhance Rotator Cuff Repair in an Animal Model.

There has been a recent surge of interest on the use of biologic supplements to facilitate rotator cuff repair healing. Experimental evidence appears to support use of allogenic dermal fibroblasts (ADFs), either in the form of local injection or tenocytes embedded in collagen matrix scaffold, to enhance healing of a repaired rotator cuff tendon tear in an animal model. When compared with the ADFs, the platelet-rich plasma (PRP)-induced response seems to be limited in terms of the specific increases in local collagen 1 concentration, thus resulting in a bone-tendon healing response that is infe

Tendon homeostasis and regeneration: Interplay of cells, immune response, and bioactive compounds.

Tendons are specialized connective tissues that transmit mechanical forces from muscle to bone, ensuring joint stability and efficient locomotion. Their homeostasis and regenerative capacity depend on the interplay between extracellular matrix (ECM), resident and recruited cell populations, and immune-mediated signaling. This review provides an overview of tendon structure and composition, emphasizing the collagen-based organization and the functional role of non-collagenous matrix components in mechanotransduction and cell signaling. The heterogeneity of tendon-resident cells, including tenoc

SIRT3-dependent mitochondrial redox homeostasis protects against age-related tendon degeneration.

AIMS: Mitochondrial redox homeostasis is closely linked to the ageing processes of tendons, whether in normal or pathological conditions. This study employed an innovative approach to examine the role of sirtuin 3 (SIRT3) in mitochondrial dysfunction in the aged murine supraspinatus tendon (ST). METHODS: Pathological changes and collagen organization in tendons, along with variations in SIRT3 levels and oxidative stress, were initially assessed in mice of different ages. In vitro experiments were then performed to evaluate the effects of oxidative senescence on tendon cells, including cellula

Statin treatment increases the clinical risk of tendinopathy through matrix metalloproteinase release - a cohort study design combined with an experimental study.

Recent experimental evidence indicates potential adverse effects of statin treatment on tendons but previous clinical studies are few and inconclusive. The aims of our study were, first, to determine whether statin use in a cohort design is associated with tendinopathy disorders, and second, to experimentally understand the pathogenesis of statin induced tendinopathy. We studied association between statin use and different tendon injuries in two population-based Swedish cohorts by time-dependent Cox regression analysis. Additionally, we tested simvastatin in a 3D cell culture model with human

Enhanced Achilles tendon healing by fibromodulin gene transfer.

UNLABELLED: Tendon injury is a major musculoskeletal disorder with a high public health impact. We propose a non-viral based strategy of gene therapy for the treatment of tendon injuries using histidylated vectors. Gene delivery of fibromodulin, a proteoglycan involved in collagen assembly was found to promote rat Achilles tendon repair in vivo and in vitro. In vivo liposome-based transfection of fibromodulin led to a better healing after surgical injury, biomechanical properties were better restored compared to untransfected control. These measures were confirmed by histological observations

Effects of fluoroquinolone antibiotics on extracellular matrix-related phenotypes in tendon cells.

Fluoroquinolones (FQ) are amongst the most widely prescribed antibiotics used to treat a wide range of infections because of their excellent gastrointestinal absorption, superior tissue penetration and broad-spectrum activity. However, FQs can cause rare, though serious, collagen-associated adverse reactions, including tendinopathy, and there is still no clear framework for understanding the phenotypes relating to how tenocyte-produced extracellular matrix (ECM) and related molecules change in response to FQs. Here, we show carrier-mediated intracellular uptake of the FQ ciprofloxacin by mouse
