Endothelin 1 in migraine and tension-type headache.

We determined the plasma levels of ET1, both interictally and ictally, in 50 migraine patients, 20 with aura (MPA) and 30 without aura (MPWA), comparing them with the levels of 40 age-matched tension-type headache patients (20 episodic and 20 chronic) (ETTHP and CTTHP) and the levels of a group of 20 healthy control subjects (CS). No statistically significant difference was evident between the mean ET1 plasma levels of MPA and those of MPWA, assessed in headache-free periods. The mean ET1 plasma levels of MPA and MPWA, assessed interictally, were significantly higher than those of CS. However,

Tension-type headache: psychosomatic clinical assessment and treatment.

Tension-type headache (TTH) is an ill-defined nosographic entity. The classification of headaches according to the Headache Classification Committee of the International Headache Society is closer to clinical reality with respect to the past classification. From an aetiopathological standpoint, hypotheses trying to explain primary headache disorders in a unified way are interesting. The treatment of this disorder requires a thorough diagnostic framing, which should take into account its psychosomatic aspects, focusing on the individual patient and his history. In fact, the many studies which a

Tension type headache: a neuropsychological and neurophysiological study.

Characteristic personality profiles of patients suffering from either migraine headache (MH) or tension type headache (TTH) have been described in the light of Minnesota Multiphasic Personality Inventory (MMPI). In the present study we evaluated personality profile changes following modifications of the painful symptoms after treatment, through the administration of the MMPI to MH and TTH patients. In TTH patients with no clinical improvement the depression scale scores were higher after 6 months of treatment, whereas in MH patients similar scores were observed before and after therapy, thus s

[Tension-type headache. Narrative review of physiotherapy treatment].

INTRODUCTION: Headache is a very common phenomenon with a high economic and psychosocial impact. Tension-type headache (TTH) is the most prevalent (40%), especially amongst adult western women. The aim was to evaluate the existing evidence on the effectiveness of physiotherapy techniques in treating TTH. METHODS: Literature review of randomised clinical trials (RCT) and systematic reviews, published over the last five years, on the physiotherapy techniques most used in treating TTH: therapeutic exercise, suboccipital inhibition, cervical manip-ulation, massage, joint mobilisation and puncture

Migraine and Tension-Type Headache: Diagnosis and Treatment.

Migraine and tension-type headache are highly prevalent. Migraine is associated with significant work- and family-related disability. Migraine is underdiagnosed; it reasonable to err on the side of migraine when choosing between primary headaches. Barriers to appropriate treatment of migraine include lack of access to providers, misdiagnosis, and acute and preventive therapies not being prescribed. Acute, rescue, and preventive treatment options are extensive, and new classes of treatments are either available or in development. This review addresses diagnostic challenges including recognizing

Sleep disturbances in tension-type headache and migraine.

Current research into the pathogenesis of tension-type headache (TTH) and migraine is focused on altered nociceptive pain processing. Among the potential factors that influence sensitization mechanisms, emotional stress, depression, or sleep disorders all have an essential role: they increase the excitability of nociceptive firing and trigger hyperalgesic responses. Sleep disturbances and headache disorders share common brain structures and pathogenic mechanisms and TTH, migraine, and sleep disturbances often occur together; for example, 50% of individuals who have either TTH or migraine have

Ketoprofen for episodic tension-type headache in adults.

BACKGROUND: Tension-type headache (TTH) affects about 1 person in 5 worldwide. It is divided into infrequent episodic TTH (fewer than one headache day per month), frequent episodic TTH (2 to 14 headache days per month), and chronic TTH (15 headache days a month or more). Ketoprofen is one of a number of analgesics suggested for acute treatment of headaches in frequent episodic TTH. OBJECTIVES: To assess the efficacy and safety of ketoprofen for treatment of episodic TTH in adults compared with placebo or any active comparator. SEARCH METHODS: We searched the Cochrane Central Register of Cont

[Tension-type headache and mental disorders in children].

Based on the analysis of current literature, the authors reviewed diagnostic aspects and the relationship between tension-type headache (TH) and mental disorders in 78 children, aged from 7 to 15 years, admitted to a neurological unit in 2004-2007 and in 45 children admitted to a psychiatric hospital. The latter group included 22 patients with depressive episodes, 11 with anxiety-phobic disorder and 12 with schizotypal personality disorder. The comparison of clinical features of headache in these groups revealed that TH was associated with mental disorders and chronic TH in most cases was a sy

Epidemiology and cultural differences in tension-type headache.

Tension-type headache (TTH) is the most prevalent headache worldwide Stovner et al. (Cephalalgia 27:193-210, 2007••); Robbins and Lipton (Semin Neurol 30:107-19, 2010••); Jensen and Stovner (Lancet Neurol 7:354-361, 2008). Despite being the most prevalent primary headache type, there is still a relative lack of epidemiologic data when compared to that for migraine Robbins and Lipton (Semin Neurol 30:107-19, 2010••). Out of the available data, it is apparent that there are wide variations and differences in tension-type headache epidemiology across different cultures Stovner et al. (Cephalalgia

Acute calcific prevertebral tendonitis mimicking tension-type headache.

A 74-year-old woman was admitted to our hospital due to severe nuchal pain and occipitalgia. Neurological examination found neck stiffness but no throat pain or dysphagia. Blood examination showed slight elevation of white blood cell count, but C-reactive protein level was normal. Cerebrospinal fluid examination found no abnormalities. Computed tomography (CT) and magnetic resonance (MR) imaging demonstrated no abnormalities in the brain. Cervical CT showed a small calcification in front of the C1 body. Cervical T(2)-weighted MR imaging showed a high intensity area in front of the upper cervic

Tension-type headache and migraine in multiple sclerosis.

We review the evidence for a link between multiple sclerosis (MS) and two of the most common primary headache disorders: tension-type headache and migraine. We argue that the association between migraine and MS is biologically plausible and is confirmed by most studies. We discuss possible explanations for the association. First, we consider the possibility that the association is spurious. Next, we consider unidirectional causal models in which one of the conditions increases the risk of the other. A bidirectional model would suggest that each disease predisposes to the other. Alternatively,

Tension-type headache: current research and clinical management.

Tension-type headache (TTH) is the most common form of headache, and chronic tension-type headache (CTTH) is one of the most neglected and difficult types of headache to treat. The pathogenesis of TTH is multifactorial and varies between forms and individuals. Peripheral mechanisms (myofascial nociception) and central mechanisms (sensitisation and inadequate endogenous pain control) are intermingled: the former predominate in infrequent and frequent TTH, whereas the latter predominate in CTTH. Acute therapy is effective for episodes of TTH, whereas preventive treatment--which is indicated for

The long-term prognosis of tension-type headache.

Tension-type headache (TTH) is a very common problem that usually causes only minimal to moderate discomfort and little disability. If episodic TTH evolves to chronic TTH, then the morbidity in terms of discomfort, disability, and use of medication escalates dramatically. There are no long-term studies, but inferences can be made from population surveys. These suggest that episodic TTH occurs in 15% to 75% of the population, but 30% to 40% is the most common estimate. There is a modest increase in prevalence between the ages of 30 and 50 years and a decrease to 25% to 35% prevalence after the

Diagnosis, epidemiology, and impact of tension-type headache.

Although tension-type headache is the most prevalent headache and affects 78% of the general population, the substantial societal and individual burden associated with this primary headache has been overlooked. In contrast to migraine headache, there has been limited focus on tension-type headache. Most patients with the chronic form of tension-type headache, which affects 3% of the population, are left virtually without any specific treatment. Chronic tension-type headache differs from the episodic form in frequency, lack of effect to most treatment strategies, more medication overuse, and mo

Central and peripheral sensitization in tension-type headache.

Recent studies on tension-type headache indicate that the nociceptive input to the central nervous system may be increased as a result of activation or sensitization of peripheral sensory afferents. Moreover, pain perception studies and pharmacologic studies strongly suggest that the central nervous system is sensitized in patients with chronic tension-type headache. The barrage of nociceptive impulses from the periphery may be responsible for the sensitization of second- and third-order neurons in the central nervous system. In this way, sensitization may play a role in the initiation and mai

Controlled trial of ketorolac in tension-type headache.

Intramuscular ketorolac 60 mg, meperidine 50 mg plus promethazine 25 mg, and normal saline were compared in acute exacerbations of tension-type headache. Forty-one subjects (30 females and 11 males) were randomized into three groups and evaluated by the McGill Short-Form Pain Questionnaire before treatment, and 0.5, 1, 2, 3, 4, 5, and 6 hours after treatment. All three groups showed a significant treatment effect that persisted for the 6 hours of evaluation. Ketorolac treatment was significantly better than placebo at 0.5 and 1 hour by the Visual Analog Scale (VAS) and Pain Rating Index, and b
