Short-term habituation of eye-movement responses induced by galvanic vestibular stimulation (GVS) in the alert guinea pig.

In a recent study, we showed that primary afferent neurons innervating all vestibular end organs were sensitive to galvanic vestibular stimulation (GVS) in guinea pigs. In order to determine the three-dimensional character of eye movements induced by GVS, changes in eye position were recorded using digital video oculography during delivery of bilateral GVS ranging in intensity between 20 and 80 microA. Pulses were delivered in repetitive trains in order to also ascertain the involvement of vestibular habituation. At low intensities of GVS (up to 40 microA), maintained changes in eye position w

Middle ear myoclonus: pathophysiology and management.

PURPOSE OF REVIEW: This review aims to provide a comprehensive analysis of the pathophysiology and treatment of middle ear myoclonus (MEM), a rare and under-recognized cause of objective and subjective tinnitus. RECENT FINDINGS: MEM is increasingly recognized as a distinct subset in tinnitus patients, with symptoms arising from involuntary contractions of the stapedius and/or tensor tympani muscles. Pharmacological management currently centres around agents such as clonazepam, carbamazepine, and piracetam. Surgical resection of middle ear tendons has yielded highly encouraging results, albeit

Muscles in and around the ear as the source of "physiological noise" during auditory selective attention: A review and novel synthesis.

The sensitivity of the auditory system is regulated via two major efferent pathways: the medial olivocochlear system that connects to the outer hair cells, and by the middle ear muscles-the tensor tympani and stapedius. The role of the former system in suppressing otoacoustic emissions has been extensively studied, but that of the complementary network has not. In studies of selective attention, decreases in otoacoustic emissions from contralateral stimulation have been ascribed to the medial olivocochlear system, but the acknowledged problem is that the results can be confounded by parallel m

[The original method for bypass surgery of the tympanic cavity to treat chronic dysfunction of the Eustachian tube].

The objective of the present study was to improve the effectiveness of the treatment of intractable dysfunctionof the Eustachian tube responsible for the relapses of catarrhal and exudative otitis media. The authors developed the diaphragmatic slot valve integrated into the silicone tympanic shunt tube. The proposed diaphragmatic slot valve was shown to counterbalance extra- and intratympanic pressure at the experimentally found compression threshold equaling 123.06 daPa. This original functional shunt ensured the continuous functioning of tensor tympani muscle and maintained tympano-tubal inn

Toluene effect on the olivocochlear reflex.

Animal studies have shown that toluene can cause hearing loss and can exacerbate the effects of noise by inhibiting the middle ear acoustic reflex. In this investigation, carried out in Long-Evans rats, the tensor tympani tendon was cutoff and the stapedius muscle was electrocoagulated in one or both middle ears. Rat hearing was evaluated by measuring cubic distortion otoacoustic emissions (2f1-f2; f1 = 8000 Hz; f2 = 9600 Hz; f1/f2 = 1.2) prior to, during, and after activation of the olivocochlear (OC) reflex. A band noise centered at 4 kHz was used as suppressor noise. It was delivered contra

Tenotomy of the middle ear muscles causes a dramatic reduction in vertigo attacks and improves audiological function in definite Meniere's disease.

CONCLUSIONS: Because the presented data reveal an immediate and persistent reduction of vertigo and a clear improvement in hearing function and functional scales, we conclude tenotomy to be effective in unilateral, definite Meniere's disease - laying the foundation for future prospective, randomized controlled trials. OBJECTIVES: This study compares the unique long-term results of tenotomy of the stapedius and tensor tympani muscles in definite Meniere's disease refractory to medical treatment and presents a hypothesis on why tenotomy seems effective. METHODS: This was an interventional coho

Effect of the acoustic reflex on inner ear damage induced by industrial noise.

Shipyard noise is a variable noise which induces permanent threshold shift (PTS) in exposed workers. In humans, the stapedius reflex has been found to be very stable in this type of exposure. Temporary threshold shift (TTS) in the absence of stapedius reflex has been found to extend downwards through the speech-frequencies instead of showing a high frequency dip as when the stapedius reflex is normal. The features of PTS produced by the same type of noise was investigated in rabbits with and without functioning middle ear muscles. The auditory sensitivity was measured by auditory brain stem re

Effects of prolonged noise exposure in chinchillas with severed middle ear muscles.

Using the round window-recorded cochlear microphonic as the index of acoustic reflex activity, we noted a decay in the strength of middle ear muscle contraction in the chinchilla following an eight hour exposure to octave band noise (500 Hz. center frequency, 95 dB. sound pressure level). On the basis of this observation it was concluded that the prolonged exposure reduced the effectiveness of the acoustic reflex in protecting the cochlea. This reduction, however, may have been underestimated if the exposure was also sufficient to reduce cochlear output. The present investigation examined this

Do middle ear muscles trigger attacks of Menière's disease?

OBJECTIVE: Current surgical methods for treating Ménière's disease (MD), e.g. endolymphatic sac surgery, mastoidectomy or insertion of middle ear ventilation tubes, only improve vertigo and fail to influence the hearing threshold. In this retrospective study we examined the long-term effects of tenotomy on the symptoms of MD. MATERIAL AND METHODS: A total of 45 patients suffering from definite MD underwent middle ear muscle tenotomy, a treatment that is very rarely considered nowadays. Of these 45 patients, 20 were observed postoperatively for a period of > 2 years. The disease was scored pre

Evidence of a medial olivocochlear involvement in contralateral suppression of otoacoustic emissions in humans.

Otoacoustic emissions (OAEs) evoked by click stimuli were recorded in both ears of 20 normal human subjects, in the presence and absence of a contralateral masking broad band noise. No difference in the amplitude of OAE suppression was noted between the first tested ear and the second one. In addition, 20 pathological subjects were tested according to the same protocol. Ten of them belonged to a group of patients whose vestibular nerve was sectioned on one side to relieve incapacitating vertigo and thus represented a group in whom olivocochlear efferents were severed. A great reduction of supp

Wideband detection of middle ear muscle activation using swept-tone distortion product otoacoustic emissions.

The measurement of efferent-induced suppression of otoacoustic emissions (OAEs) using contralateral acoustic stimulation (CAS) is complicated by potential contamination by the middle ear muscle reflex (MEMR), particularly at moderate to high CAS levels. When logarithmically sweeping primaries are used to measure distortion product otoacoustic emissions, the level and phase of the primaries at the entrance of the ear canal may be monitored simultaneously along with the OAEs elicited by the swept-tones. A method of detecting MEMR activation using swept-tones is presented in which the differences

Functional evaluation of paratubal muscles using electromyography in patients with chronic unilateral tubal dysfunction.

Eustachian tube dysfunction is closely related to the development of otitis media and result from several factors including inflammation within the nasal cavity and nasopharynx, adenoid hypertrophy, cleft palate and nasopharyngeal carcinoma. To some extent, eustachian tube dysfunction may be related to weakness of the paratubal muscles, such as the tensor veli palatini and levator veli palatini muscles. The aim of the study is to find out myogenic factors in eustachian tube dysfunction using electromyography (EMG), and to evaluate the clinical feasibility of EMG. Ten patients with unilateral e

Tensor veli palatini electromyography for monitoring Eustachian tube rehabilitation in otitis media.

BACKGROUND: The pathogenesis of otitis media is related to Eustachian tube dysfunction. The tensor veli palatini muscle actively opens the Eustachian tube and promotes middle-ear ventilation. This study describes a technique for paratubal electromyography that uses a surface, non-invasive electrode able to record tensor veli palatini muscle activity during swallowing. METHODS: Twenty otitis media patients and 10 healthy patients underwent tensor veli palatini electromyography. Activity of this muscle before and after Eustachian tube rehabilitation was also assessed. RESULTS: In 78.5 per cent

Contralateral-noise effects on cochlear responses in anesthetized mice are dominated by feedback from an unknown pathway.

Suppression of ipsilateral distortion product otoacoustic emissions (DPOAEs) by contralateral noise is used in humans and animals to assay the strength of sound-evoked negative feedback from the medial olivocochlear (MOC) efferent pathway. However, depending on species and anesthesia, contributions of other feedback systems to the middle or inner ear can cloud the interpretation. Here, contributions of MOC and middle-ear muscle reflexes, as well as autonomic feedback, to contra-noise suppression in anesthetized mice are dissected by selectively eliminating each pathway by surgical transection,
