OBJECTIVE: Thyroid disorders including hyperthyroidism are common during pregnancy. Untreated hyperthyroidism can result in adverse outcomes for pregnancy.
METHODS: Iodine, propylthiouracil (PTU), carbimazole (CMZ), and methimazole (MMI) are common medications for hyperthyroidism treatment. The literature regarding antithyroid medication use in pregnancy and breastfeeding is reviewed.
RESULTS: Animal studies for PTU have suggested congenital anomalies while animal studies for MMI have only indicated adverse outcomes at higher doses than used in humans. Epidemiological studies have noted an i

Modification of the terminal tails of histones is considered one of the documented mechanisms for epigenetic control of gene expression. Histone deacetylase inhibitors (HDACi) lead to a state of hyperacetylation of histone, a condition that can affect normal gene transcription. Furthermore, HDACi have many other protein targets involved in regulation of gene expression, cell proliferation and cell death. For these properties some HDACi are nowadays used as anticancer drugs with promising results. Several molecules with HDACi properties (valproic acid, trichostatin A, apicidin, MS-275, sodium b

Thalidomide was originally developed in 1954 as a sedative that was commonly used to ameliorate morning sickness. However, thalidomide exposure during the first trimester of pregnancy caused multiple birth defects (e.g. phocomelia and amelia), affecting ≈ 10,000 children worldwide in the late 1950s and early 1960s. Thalidomide is now recognized as a clinically effective, albeit strictly restricted, drug for the treatment of leprosy and multiple myeloma. Investigators have studied thalidomide teratogenicity for half a century, proposing over 30 hypotheses to account for its actions. Among these

A review is presented on the effects of lithium in therapeutic doses on the outcome of human pregnancy. The results of various studies including cohort, prospective, retrospective and small number case reports indicate that lithium is a "weak" teratogen in humans. The main effects attributable to lithium are, cardiac malformations and babies with increased birth weight. There is a possibility that, in particular, lithium may be associated with the Ebstein anomaly but present evidence cannot definitely affirm or deny this association. Animal studies with lithium using doses comparable to human

This study was performed to evaluate the effects of prenatal development of rats were exposed to Polnoks R and its monomer 2,2,4-trimethyl-1,2-dihydroquinoline (TMDHQ) by gavage every day on days 6-15 of gestation at doses equivalent 6%, 13% and 25% of LD50. Polnoks R and TMDHQ administered per os associated with significant maternal toxicity, embryonal lethality, retarded fetal development and congenital defects. Polnoks R induced skeletal malformations, internal hydrocephalus, and hydronephrosis. 2,2,4-trimethyl-1,2-dihydroquinoline produced internal malformations (exencephale, hydrocephalus

Limb abnormalities are one of the most common and visible phenotypic effects of several human teratogens. The specific effects are different for most teratogens and include effects on limb morphogenesis (thalidomide, warfarin, phenytoin, valproic acid) and the effect of vascular disruption on a limb that had formed normally (misoprostol, chorionic villus sampling, and phenytoin). Either duplication (preaxial polydactyly of hands and feet) or deficiency (absence of thumb) is a common effect of thalidomide; no other human teratogen identified to date has this effect on the developing limb. Proce

C-14 formaldehyde crosses the placenta and enters fetal tissues. The incorporated radioactivity is higher in fetal organs (i.e., brain and liver) than in maternal tissues. The incorporation mechanism has not been studied fully, but formaldehyde enters the single-carbon cycle and is incorporated as a methyl group into nucleic acids and proteins. Also, formaldehyde reacts chemically with organic compounds (e.g., deoxyribonucleic acid, nucleosides, nucleotides, proteins, amino acids) by addition and condensation reactions, thus forming adducts and deoxyribonucleic acid-protein crosslinks. The fol

Beta-thujaplicin (TP) was studied by in vitro and in vivo tests for teratogenicity using ICR mice. In the in vitro study, TP (0, 3.125, 6.25, 12.5 microg/ml medium) dissolved in dimethyl sulfoxide (DMSO) was administered to cultured embryos on 9 day of gestation. After 24 hr of exposure to TP, the embryos were examined for developmental parameters and external anomalies. Growth retardation and embryos with facial dysplasia or hydrocyst of the tail tip were observed among the embryos given 12.5 microg/ml. In the in vivo study, TP (0, 420, 560, 750 or 1000 mg/kg) dissolved in olive oil was admin

The maternal phenylketonuria (PKU) syndrome refers to the teratogenic effects of PKU during pregnancy. These effects include mental retardation, microcephaly, congenital heart disease, and intrauterine growth retardation. In untreated pregnancies wherein the mother has classic PKU with a blood phenylalanine level > or = 1,200 microM (20 mg/dl), the frequencies of these abnormalities in offspring are exceedingly high, approaching 75-90% for microcephaly and mental retardation and 15% for congenital heart disease. There is a dose response relationship with progressively lower frequencies of thes

Ergotamine is a purified ergot alkaloid that has been suspected of being teratogenic. Animal data have shown effects mainly at doses toxic to the mother. Clinical reports in humans have been anecdotal, but in many the malformations are consistent with vascular injury. However, epidemiologic studies have not shown any clear increase in malformations among exposed infants. This may reflect the limited exposure and toxicity when used episodically. Ergot toxicity which can result in severe vasospasm does not spare the fetus or mother and ergotamine should not be considered without risk. Ergotamine

The embryotoxic and fetotoxic potential of Spirulina was investigated in mice. The algae was given to pregnant animals at the levels of 0, 10, 20 and 30 g/100 in the diet through days 7-13, 1-13 and 1-19 of gestation. On day 19, the rats were sacrificed, and the uterine horns were examined for live, dead and resorbed fetuses. The live fetuses were weighed and examined for external malformations and either skeletal or visceral abnormalities. With the litter as the unit of analysis and when the total number of fetuses affected was considered, no significant difference was found in the number of

Valproic acid use during pregnancy results in an absolute risk for spina bifida of 1-2%. This increased risk is comparable to the recurrence risk for neural tube defects and warrants informed counselling and access to prenatal diagnosis. There is no substantial evidence that valproic acid use increases the risk for other specific major malformations above the increased risk due to maternal epilepsy. Valproic acid may cause a characteristic pattern of minor facial malformations. Further definition and confirmation are required, and the magnitude of the risk needs to be determined. There are ina

The possibility that antiemetics used during pregnancy are teratogenic has been hotly debated; the effect of vomiting itself, however, has been largely ignored. The relation between vomiting and congenital malformations was examined in a prospective study of 16398 women who registered for prenatal care at or before 20 weeks' gestation. The odds ratios for malformations among women who vomited compared with women who did not were 1.14 for major malformations (p = 0.13), 0.88 for deformations (p = 0.39), 1.03 for hernias or undescended testes (p = 0.82), 1.06 for any of these three conditions (p

A case of holoprosencephaly is reported in which the mother was prescribed high doses of oestroprogestins during the first 5 months of the pregnancy. Investigation of the family failed to reveal any sign of physical abnormality. A normal karyotype was detected in the proband. The authors suggest that this case may shed some light on the normal and abnormal way in which embryonic fields develop.

Thiabendazole (TBZ) was tested for teratogenicity using Jcl:ICR mice. TBZ suspended in olive oil was given orally to pregnant mice at different stages of organogenesis. All foetuses were removed from the uterus on day 18 of gestation, and were examined for external and skeletal anomalies. In mice given 700, 1300 or 2400 mg TBZ/kg body weight/day on days 7-15 of gestation, dose-dependent external and skeletal anomalies, especially cleft palate and fusion of vertebrae, were observed. In mice given a single dose of TBZ (2400 mg/kg) on any one of days 6-13, an increased number of malformations was

The use of pharmacokinetics can improve the extrapolation of animal teratology data for human risk evaluation. Before one can extrapolate between species, however, the pharmacokinetic model must be predictable within the species for which it was developed. This article summarizes an approach being used for correlating pharmacokinetics and teratology endpoints in the same animal and predicting the teratogenic outcome for other animals of the same species. With the aid of micro-sampling procedures, and sensitive and rapid analytical techniques, blood, urine and feces samples are obtained from in

Acetaldehyde, the proximal metabolite of ethanol, was administered intraperitoneally in single and triple doses (50, 75, 100 mg/kg) to pregnant CF rats on days 10, 11 and 12 of gestation and fetuses were collected on day 21. Besides significant fetal resorptions, malformations were found which included edema, microcephaly, micrognathia, micromelia, hydrocephaly, exencephaly, hemorrhages etc. Somatometric measurements of fetuses (i.e. crown rump length, transumbilical distance and tail length) revealed significant growth retardation. Alizarin red-stained skeletal preparations showed considerabl

An analysis of available epidemiologic data leads the present reviewers to conclude that the use of exogenous hormones during human pregnancy has not been proved to cause developmental abnormality in nongenital organs and tissues. This conclusion is further supported by the animal laboratory data. The preponderance of evidence at this writing indicates a lack of causal association between hormonal use during pregnancy and nongenital malformation of the offspring. The quality of the epidemiologic data does not, at this time, permit a definitive conclusion that sex hormones during pregnancy may
