Congenital extracardiac shunts in tetralogy of Fallot may significantly alter the history and clinical findings and also have significant management implications. An unusual variant of severe tetralogy of Fallot in a 54-year-old man enabled long survival with minimal symptoms. The patient had pulmonary valvular and infundibular stenosis, a large ventricular septal defect, an overriding aorta, and large volume systemic-to-pulmonary artery shunting from his right internal mammary and accessory internal mammary arteries to his right pulmonary artery.

Pf 51 cases of Fallot's tetralogy who underwent preoperative angiography before a palliative anastomosis and then a second angiography before complete correction, 10 had developed pulmonary atresia (infundibular in 8 cases, valvular in 2 cases). The type of anastomosis (Blalock-Taussig of Waterston), and the interval between the two operations do not seem to be determinant factors. A patient presented with a a unique combination of cardiovascular abnormalities: tetralogy of Fallot, absent pulmonary valve, stenosis of peripheral pulmonary arteries, and partial anomalous pulmonary venous connection to the azygos vein. In surgical correction of tetralogy of Fallot (TOF), transannular patch (TAP) repair has been the most prevalent right ventricular outflow tract (RVOT) reconstruction technique. However, resultant pulmonary regurgitation affects postoperative mortality and morbidity, and reintervention for RVOT lesion is frequently required late after TAP repair. There is no consensus for the best technique for TAP repair even in the current era. The optimal design of the patch, patch material, and reconstruction technique for the best long-term preservation of RVOT function should be explored in the future.

Tetralogy of Fallot with pulmonary stenosis has a diverse clinical spectrum with the degree of right ventricular outflow tract obstruction (RVOTO) and size of the branch pulmonary arteries driving clinical management. Over the last years, the surgical techniques used to repair Tetralogy of Fallot as well as the cross-sectional cardiac imaging techniques have substantially improved. Now, the survival rate after surgical repair is more than 90% at 40 years old. A follow-up is needed and the imaging evaluation should be guided by the surgical techniques used. This article reviews the most common surgical procedures for a complete repair, the associated anatomic and hemodynamic complications and the role of cardiac imaging, mainly magnetic resonance imaging.

Outcomes for surgical repair of tetralogy of Fallot with pulmonary stenosis are often considered to be a useful benchmark for the assessment of congenital surgical programs. This procedure, including preoperative assessment and postoperative management, falls in the middle of the spectrum of difficulty of all the congenital cardiac procedures. Between October 1987 and November 1992, 165 consecutive patients underwent isolated transpulmonary repair for tetralogy of Fallot. Their ages ranged from 1 to 44 years and their weight ranged from 6 to 61 kg (mean 19.4). Of these 165 patients, 65% required annulus enlargement. The surgical management of tetralogy of Fallot has undergone important changes in recent years. Earlier repair of tetralogy of Fallot is now favored by many institutions. At Stanford University Medical Center, we have performed definitive repair of tetralogy of Fallot at the time of presentation, regardless of the child's age, with few exceptions. In this report, we describe our results with early repair, and we believe these support the contention that infants who undergo early repair (< 1 year of age) have postoperative results similar to those of children who undergo repair at an older age.
