METHODS: Eight adult patients with mild bronchial asthma and normal liver function were infused aminophylline intravenously (6.6 mumol.kg-1). Intraperitoneal (IP) injection of high doses of theophylline decreased memory acquisition dose dependently. Low but not high doses of the dopamine receptor D2 antagonist sulpiride and the dopamine D1 receptor antagonist SCH 23390 decreased the theophylline response. We reviewed the clinical manifestations of 53 episodes of theophylline intoxication in 50 patients over a one year study period, in order to identify the specific features as they presented to the Emergency Department (ED). Intestinal exsorption of salicylic acid, urea and quinidine was measured during the perfusion of the rat intestinal lumen with Tyrode solution. The intestinal clearance (CLi) of the three compounds was measured by dividing the rate of appearance in the intestinal luminal perfusate by the plasma concentration of the compound. Substantial in vitro and animal data suggest that methylxanthines, such as caffeine and theophylline, act as adenosine receptor antagonists. Toxic side effects of elevated theophylline levels range from minor symptoms to life-threatening complications. Theophylline has been shown to be a potent cardiovascular teratogen in animals, but there has been no evidence linking theophylline with congenital anomalies in infants. The cardiovascular anomalies in our three patients were aortic anomalies, double-outlet right ventricle, transposition of the great arteries, total anomalous pulmonary venous connection, and hypoplasia of the left ventricle. Some of these rare defects were also induced frequently in animal experiments with theophylline, which suggests that this drug can be a cardiovascular teratogen in a susceptible human fetus.

Room temperature phosphorimetry (RTP) of a mixture of two methylxanthines is described. The similar spectral characteristics of caffeine and theophylline require use of a separation procedure prior to RTP analysis.
