Craniectomy gap patency and neosuture formation following endoscopic suturectomy for unilateral coronal craniosynostosis.

BACKGROUND: The combination of endoscope-assisted suturectomy and postoperative helmet therapy has been advocated to treat unilateral coronal synostosis. However, surgical outcomes can vary. One possible explanation for this inconsistency is early closure of the craniectomy gap. The authors examined short-term postoperative patency of the craniectomy gap and its relationship to phenotypic improvement. METHODS: A retrospective review was performed that included patients who (1) underwent endoscope-assisted suturectomy and postoperative helmet therapy for isolated unilateral coronal synostosis

Identifying offline muscle strength profiles sufficient for short-duration FES-LCE exercise: a PAC learning model approach.

UNLABELLED: Functional electrical stimulation-induced leg cycle ergometry (FES-LCE) provides therapeutic exercise for persons with spinal cord injury (SCI). However, there exists no systematic approach to predict whether an individual has sufficient thigh muscle strength necessary for FES-LCE exercise. OBJECTIVE: To develop and test a Probably Approximately Correct (PAC) learning model as a predictor of thigh muscle strengths sufficient for short-duration FES-LCE exercise and compare the model's performance with other well-known statistical methods. METHODS: Six healthy male individuals with

Analysis of distributed-collaborative activity during augmented reality exposure therapy for cockroach phobia.

Recent research presents Augmented Reality Exposure Therapy (ARET) for treatment of phobia of cockroaches as a potentially effective technique. However, to the authors' knowledge no studies have been published concerning the Human-Computer-Interaction issues of such a system. The aim of this paper is to report some preliminary data on how patients, therapists and an Augmented Reality system collaborate and interact during the therapeutic process. The results show that the therapeutic process is distributed between individuals (patient and therapist) and artifacts (e.g. AR cockroaches, a comput

The Vienna functional electrical stimulation system for restoration of walking functions in spastic paraplegia.

An eight-channel stimulation system, currently intended for stimulation of lower extremities, was developed and is introduced. The major development goals were easy handling, modularity to make the system easily adaptable for other functional electrical stimulation (FES) applications, and a wide stimulation parameter range for application-specific parameter optimization. For paraplegic stepping, the system worn by the patient consists of 2 four-channel stimulation modules, a central unit holding the battery and circuitry for power management and communication control, a wireless remote control

[Results and prognosis after highly selective vagotomy in the light of the mathematical theory of approximate sets].

Information system for 122 patients with peptic ulcer, who underwent highly selective vagotomy within 5 and 13 years earlier has been analysed with the use of technique based on the approximate sets theory. Cause-effect relationship between data describing the analysed patients before surgery and remote result of therapy (expressed in a 4-score Visick's scale) have been sought. Using a technique of the approximate sets, information system has been described from 11 to 5 significant and necessary preoperative data most closely related with the result of therapy. Models of the typical representa

Glucose consumption and methionine uptake in low-grade gliomas after iodine-125 brachytherapy.

We investigated whether 2-[18F]-fluoro-2-deoxy-d-glucose (FDG) and carbon-11 methionine are suitable tracers to monitor the effects of therapy for low-grade gliomas. Ten patients with low-grade glioma without previous treatment were studied with FDG positron emission tomography. Additionally, l-[methyl-11C]methionine uptake was measured in five subjects before and 1 year after computerized tomography (CT)-guided stereotactic and computer-assisted implantation of iodine-125 seeds. All scans were 3D-matched to CT, isodose volumes were determined, and changes in glucose metabolism and methionine

Is computer-assisted aminoglycoside dosing managed by a pharmacist a safety tool of pharmacotherapy?

This pilot prospective study verified the hypothesis that use of computer-assisted therapeutic drug monitoring of aminoglycosides by pharmacists leads to better safety therapeutic outcomes and cost avoidance than only concentration measurement and dose adjustments based on a physician's experience. Two groups of patients were enrolled according to the technique of monitoring. Patients (Group 1, n=52) underwent monitoring by a pharmacist using pharmacokinetic software. In a control group (Group 2, n=11), plasma levels were measured but not interpreted by the pharmacist, only by physicians. No s

Electropalatography for articulation disorders associated with cleft palate.

BACKGROUND: Cleft palate is the most common congenital deformity of the face. It could affect speech acquisition, resulting in articulation errors that could persist into adulthood. Electropalatography (EPG) has been used in speech therapy with individuals who have articulation problems that are unresponsive to "standard treatment" procedures. OBJECTIVES: To determine the effectiveness of speech intervention using electropalatography (EPG) for treating articulation errors in individuals with repaired cleft palate. SEARCH STRATEGY: The following databases were searched: CENTRAL 2008 (Issue1),

Patient-interactive, computer-controlled neurological stimulation system: clinical efficacy in spinal cord stimulator adjustment.

Over the past 20 years, continuing technical advances have rendered spinal cord stimulation an easily implemented low-morbidity technique for the management of chronic intractable pain in properly selected patients. Percutaneous methods for the insertion of arrays of multiple epidural electrodes, which are driven by noninvasively programmable "multichannel" implanted devices, have been among the most important of these technical improvements. The same implanted electronics may be used with peripheral nerve or intracerebral electrodes. If the capabilities of this new hardware are to be used to

Review of attentional bias modification: a brain-directed treatment for eating disorders.

OBJECTIVE: Psychological treatments for eating disorders (ED) rely on mastery of effortful attentional control to divert attention from anxiety provoking thoughts. This paper assesses the potential suitability of attentional bias modification treatment (ABMT) for EDs as a way to target early automatic attentional processes and implicitly retune threat perception that happens outside of conscious control. METHOD: We review data on anxiety in EDs, the neurobiological and behavioural relationship between anxiety disorders and EDs, attentional biases (AB) in EDs and the use of ABMT. RESULTS: Co-

Applications of virtual reality for pain management in burn-injured patients.

The pain associated with burn injuries is intense, unremitting and often exacerbated by anxiety, depression and other complicating patient factors. On top of this, modern burn care involves the repetitive performance - often on a daily basis for weeks to months - of painful and anxiety-provoking procedures that create additional treatment-related pain, such as wound care, dressing changes and rehabilitation activities. Pain management in burn patients is primarily achieved by potent pharmacologic analgesics (e.g., opioids), but is necessarily complemented by nonpharmacologic techniques, includ

Neuroscience, virtual reality and neurorehabilitation: brain repair as a validation of brain theory.

This paper argues that basing cybertherapy approaches on a theoretical understanding of the brain has advantages. On one hand it provides for a rational approach towards therapy design while on the other allowing for a direct validation of brain theory in the clinic. As an example this paper discusses how the Distributed Adaptive Control architecture, a theory of mind, brain and action, has given rise to a new paradigm in neurorehabilitation called the Rehabilitation Gaming System (RGS) and to novel neuroprosthetic systems. The neuroprosthetic system considered is developed to replace the func

A novel, end-to-end framework for avoiding collisions between the patient's body and gantry in proton therapy.

BACKGROUND: Administration of external radiation therapy via proton therapy systems carries a risk of occasional collisions between the patient's body and gantry, which is increased by the snout placed near the patient for better dose distribution. Although treatment planning software (TPS) can simulate controlled collisions, the computed tomography (CT) data used for treatment planning are insufficient given that collisions can occur outside the CT imaging region. Thus, imaging the three-dimensional (3D) surface outside the CT range and combining the data with those obtained by CT are essenti

Internet-based telehealth system for the treatment of agoraphobia.

In this work that is being validated within the VEPSY project, we present a system that allows the patient to continue a psychological virtual reality treatment from his or her home PC as complementary therapy. In the consulting room, we have been using virtual therapy for panic disorder and agoraphobia treatment to expose the patient to several situations. For the complementary therapy, a structured treatment via the Internet has been prepared, which consists of several parts: an assessment protocol; a structured treatment protocol organized in several blocks (such as psychoeducation and expo

Facial asymmetry correction with moulded helmet therapy in infants with deformational skull base plagiocephaly.

PURPOSE: The recommendation issued by the American Academy of Pediatrics in the early 1990s to position infants on their back during sleep to prevent sudden infant death syndrome (SIDS) has dramatically reduced the number of deaths due to SIDS but has also markedly increased the prevalence of positional skull deformation in infants. Deformation of the base of the skull occurs predominantly in very severe deformational plagiocephaly and is accompanied by facial asymmetry, as well as an altered ear position, called ear shift. Moulded helmet therapy has become an accepted treatment strategy for i
