Developments and Controversies in Invasive Diagnosis of Coronary Microvascular Dysfunction in Angina With Nonobstructive Coronary Arteries.

Approximately half of all coronary angiograms performed for angina do not show obstructive coronary artery disease, and many of these patients have coronary microvascular dysfunction (CMD). Invasive testing for CMD has increased with the advent and wider availability of thermodilution systems. We review CMD pathophysiology and invasive diagnostic testing using the Doppler and thermodilution systems. We report the results of a PubMed search of invasive microvascular testing and discuss limitations of current diagnostic algorithms in the diagnosis of CMD, including controversies regarding the op

Haemodynamic monitoring after paediatric cardiac surgery using echocardiography and PiCCO.

BACKGROUND: Haemodynamic instability is common after surgical repair of CHDs in infants and children. Monitoring cardiac output in addition to traditional circulation parameters could improve the postoperative care of these patients. Echocardiography and transpulmonary thermodilution are the two most common methods for measuring cardiac output in infants. OBJECTIVES: To compare the results of cardiac output measurements using echocardiography and a transpulmonary thermodilution setup after paediatric cardiac surgery. METHODS: Forty children, scheduled for elective repair of a ventricular sep

The influence of extravascular lung water on cardiac output measurements using thoracic impedance cardiography.

The purpose of this study was to investigate the influence of pulmonary oedema as measured with the double indicator dilution technique on the accuracy of cardiac output (CO) measurement using thoracic impedance cardiography (TIC) compared with thermodilution in thirteen sepsis patients. Differences in the Kubicek and Sramek-Bernstein equation with respect to pulmonary oedema were explored theoretically and experimentally. From a parallel two cylinder model a hypothesis can be derived that CO determined with the Kubicek equation is oedema independent, whereas CO determined using the Sramek-Ber

Coronary Flow Reserve and Myocardial Resistance Reserve Changes After Transcatheter Aortic Valve Implantation in Aortic Stenosis.

Aortic valve stenosis (AS) induces an alteration in hemodynamic conditions that are responsible for coronary microvasculature impairment. Relief of AS by transcatheter aortic valve implantation (TAVI) is expected to improve the coronary artery hemodynamic. We aimed to assess the midterm effects of TAVI in coronary flow reserve (CFR) and myocardial resistance reserve (MRR) by a continuous intracoronary thermodilution technique. At-rest and hyperemic coronary flow was measured by a continuous thermodilution technique in 23 patients with AS and compared with that in 17 matched controls, and repea

Noninvasive monitoring of cardiac output in human neonates and juvenile piglets by inductance cardiography (thoracocardiography).

PURPOSE: Thoracocardiography has been used in adult patients to noninvasively estimate changes in cardiac output (CO) by analysis of ventricular volume curves recorded by an inductive plethysmographic transducer encircling the chest at the level of the heart. The purpose of this study was to investigate the potential of thoracocardiography to monitor cardiac output in human neonates and in a small animal model, the juvenile piglet. MATERIALS AND METHODS: In 6 human premature neonates weighing 1.0 to 1.6 kg the effect of periodic respiration on CO was studied with thoracocardiography. In 9 pig

Diagnostic Yield and Testing Characteristics of an Invasive Coronary Function Testing Program.

BACKGROUND: Angina, ischemia, or myocardial infarction without non-obstructive coronary arteries (ANOCA, INOCA, or MINOCA) are common conditions yet often underdiagnosed. Invasive coronary function testing (CFT), which includes coronary thermodilution and coronary reactivity testing, can provide accurate diagnoses and improve patient outcomes. AIMS: This study describes the diagnostic yield of an invasive CFT program at a single tertiary care center and presents the findings of coronary thermodilution and coronary reactivity testing in the first 104 patients from 2021 to 2025. METHODS: We co

Echocardiographic measure of dynamic arterial elastance predict pressure response during norepinephrine weaning: an observational study.

The purpose of this study was to determine whether dynamic elastance EAdyn derived from echocardiographic measurements of stroke volume variations can predict the success of a one-step decrease of norepinephrine dose. In this prospective single-center study, 39 patients with vasoplegic syndrome treated with norepinephrine and for whom the attending physician had decided to decrease norepinephrine dose and monitored by thermodilution were analyzed. EAdyn is the ratio of pulse pressure variation to stroke volume variation and was calculated from echocardiography stroke volume variations and from

Predicting fluid responsiveness with transthoracic echocardiography is not yet evidence based.

An essential part of intensive care is to accurately identify fluid responders among patients with circulatory failure. Over the past few years, new techniques have been assessed for rapid and non-invasive prediction of fluid responsiveness. As transthoracic echocardiography (TTE) is becoming an integrated tool in the intensive care unit, this systematic review examined studies evaluating the predictive value of TTE for fluid responsiveness. In October 2012, we searched Pubmed, EMBASE and Web of Science for studies evaluating the predictive value of TTE-derived variables for fluid responsivene

Value of a modified continuity equation method to quantify mitral valve area in patients with mitral stenosis and sinus rhythm.

To quantify valve area in mitral stenosis, a modified continuity equation method using continuous wave Doppler and thermodilution measurements was applied. In 14 patients with mitral stenosis and sinus rhythm (age: 49 +/- 11 years), transmitral flow velocity was recorded by continuous wave Doppler during right and left heart catheterization. Mitral valve area was calculated by three different methods: 1. According to the continuity equation, stroke volume (thermodilution technique) was divided by the registered time velocity integral of the mitral stenotic jet (continuous wave Doppler). 2. Mit

Quantification of left ventricular ejection fraction and cardiac output using a novel semi-automated echocardiographic method: a prospective observational study in coronary artery bypass patients.

BACKGROUND: Echocardiographic quantification of ejection fraction (EF) by manual endocardial tracing requires training, is time-consuming and potentially user-dependent, whereas determination of cardiac output by pulmonary artery catheterization (PAC) is invasive and carries a risk of complications. Recently, a novel software for semi-automated EF and CO assessment (AutoEF) using transthoracic echocardiography (TTE) has been introduced. We hypothesized that AutoEF would provide EF values different from those obtained by the modified Simpson's method in transoesophageal echocardiography (TOE) a

Assessing Microvascular Dysfunction in Angina With Unobstructed Coronary Arteries: JACC Review Topic of the Week.

Coronary microvascular dysfunction is a highly prevalent condition of both structural and functional coronary disorders in patients with angina and nonobstructive coronary artery disease (ANOCA). Current diagnostic modalities to assess microvascular function are related to prognosis, but these modalities have several technical shortcomings and lack the opportunity to determine true coronary blood flow and microvascular resistance. Intracoronary continuous thermodilution assessment of absolute coronary flow (Q) and microvascular resistance (R) was recently shown to be safe and feasible in ANOCA

Left Ventricular Unloading in Nonischemic Dilated Cardiomyopathy Improves Coronary Haemodynamic Reserve.

BACKGROUND: The Impella CP is a catheter-based ventricular assist device used in the management of cardiogenic shock and to support high-risk percutaneous coronary interventions (PCI). Despite its growing use, the effects of Impella CP on coronary flow dynamics as measured by intracoronary continuous thermodilution have not been fully quantified. AIMS: This study aimed to evaluate the impact of percutaneous ventricular unloading (PVU) with Impella CP on coronary flow reserve (CFR) and microvascular resistance reserve (MRR) in patients with severe nonischemic dilated cardiomyopathy undergoing

Effects of Acute GLP-1 Infusion on Pulmonary and Systemic Hemodynamics in Patients With Heart Failure: A Pilot Study.

PURPOSE: Cardiovascular-safety studies assessing glucagon-like peptide (GLP)-1 receptor agonists and dipeptidyl peptidase 4 inhibitors have provided inconsistent data on the risk for developing heart failure. Animal studies have shown that GLP-1 is a vasodilator; if confirmed in humans, this may ameliorate heart failure symptoms. METHODS: In a single-center, observational pilot study, we recruited 10 patients with advanced heart failure undergoing right heart catheterization, and we recorded pulmonary hemodynamic measures, including cardiac output calculated by thermodilution and the indirect

Clinical non-invasive measurement of effective pulmonary capillary blood flow.

Since traditional pulmonary function testing is centered on measurements of air flow and lung volume, a method to assess the pulmonary circulation might improve our ability to evaluate diseases that impact upon pulmonary hemodynamics. We have developed a PC based application that rapidly calculates pulmonary blood flow. Subjects rebreath a mixture of 10% argon and 3.5% freon for 20 seconds. Gas concentrations at the mouth are monitored by a clinical mass spectrometer and signals are acquired and processed with off-the-shelf hardware. To test the accuracy and reproducibility of this technique,

[A new catheter for hemodynamic studies].

It is reported on a new catheter technique for the determination of the cardiac output by means of the method of thermodilution. It consists of a thin sensorcatheter (3 Ch), which contains a microthermistor as temperature sensor and additionally two electrodes. The insertion of the catheter is carried out via puncture of the subclavian vein by means of a special introducer set, the outer sheath of which can used for injecting the cold solution. The positioning of the sensor catheter into the pulmonary artery may be done by intracardiac ECG monitoring with the help of the two electrodes. By mea

Comparison of a Fully Angiography-Derived Versus a Hybrid of Angiography and Pressure-Wire-Derived Approach to Assess Coronary Microvascular Resistance: The Oxford Acute Myocardial Infarction Hybrid (OxAMI-HYBRID) Study.

BACKGROUND: Despite growing evidence of its clinical implications, assessment of coronary microvascular dysfunction (CMD) remains limited in routine clinical practice. Hence, there is an increasing interest in angiography-derived indices to encourage assessment of CMD in the cardiac catheterisation laboratory. AIMS: We investigate here an alternative approach, using a combination of pressure-wire-based assessment of distal coronary pressure (Pd) and an angiography-derived surrogate of coronary flow, to derive a hybrid index of microcirculatory resistance (IMRhybrid). Evaluation of Intravenous Versus Intracoronary Adenosine in Coronary Reactivity Testing.

Coronary flow reserve (CFR) and index of microcirculatory resistance (IMR) obtained through coronary bolus thermodilution are used to assess and treat patients with angina and no obstructive coronary artery disease. Previous studies demonstrate comparable results assessing epicardial ischemia by fractional flow reserve using intravenous (IV) or intracoronary (IC) adenosine. It is unknown if there is a similarity between IC and IV hyperemia with adenosine when performing coronary reactivity testing (CRT). We reviewed CRT data and baseline demographics in a cohort of patients who underwent CRT f
