Thoracic duct cannulation during left internal jugular vein cannulation: A case report.

BACKGROUND: Central venous catheter insertion is an invasive procedure that can cause complications such as infection, embolization due to air or blood clots, pneumothorax, hemothorax, and, rarely, chylothorax due to damage to the thoracic duct. Herein, we report a case of suspected thoracic duct cannulation that occurred during left central venous catheter insertion. Fortunately, the patient was discharged without any adverse events related to thoracic duct cannulation. CASE SUMMARY: A 46-year-old female patient presented at our department to undergo cytoreductive surgery and hyperthermic in

Thoracic duct lymphangioma and chylous ascites in a 24-year-old female.

Lymphangiomas are rare and correspond to 0.7% to 4.0% of mediastinal tumors, and isolated mediastinal location occurs in 1% of cases. They are benign tumors that originate from a congenital malformation of the lymphatic vessels and are diagnosed more frequently in children less than 2 years of age. Chylous ascites is a clinical manifestation of thoracic duct lymphangioma and is composed of lymph accumulation caused by dilation of this lymphatic channel. It appears milky in the peritoneal cavity, containing triglyceride levels higher than 200 mg/dl. We report the case of a young patient with ch

Respiratory Failure in Noonan Syndrome Treated by Microsurgical Thoracic Duct-Venous Anastomosis.

Noonan syndrome is a disorder characterized by central and peripheral lymphatic conducting anomalies, leading to chylothorax, chylous ascites, and metabolic derangement. Novel imaging methods, including dynamic contrast magnetic resonance lymphangiography and intranodal lymphangiography, have allowed for increased visualization of lymphatic pathology. Severe pulmonary insufficiency and chylothoraces developed in a 61-year-old man with Noonan syndrome. Dynamic contrast magnetic resonance lymphangiography and intranodal lymphangiography demonstrated central thoracic duct (TD) occlusion. The pati

Where Have All the Punctures Gone? An Analysis of Thoracic Duct Embolizations.

PURPOSE: To determine how frequently and how severely intra-abdominal structures are affected by transabdominal thoracic duct embolization (TDE). MATERIALS AND METHODS: Thirty-five TDE procedures in 35 patients (22 male; mean age, 57 y; age range, 10-79 y) with therapy-refractory chylous effusions were evaluated in which radiopaque embolization material outlined the access route on postinterventional CT. CT data were analyzed by 2 TDE-experienced radiologists. Abdominal structures and organs transgressed by the access route were recorded, and findings were correlated with clinical postinterve

Correction of complete thoracic duct obstruction with lymphovenous bypass: A case report.

Thoracic duct injury can be a devastating injury with disruption of lymphatic flow leading to potentially chylothorax and/or severe lymphedema. Standard treatment modalities include thoracic duct ligation or embolization for chylothorax, but treatment options to date are few for resultant lymphedema. In this case report, we describe lymphaticovenous bypass of the thoracic duct to the jugular venous system in a 21-year-old male with secondary lymphedema after iatrogenic thoracic duct injury. The patient experienced improvement of lymphedema symptoms including decreased weight and limb girth as

Acute pancreatitis after thoracic duct ligation for iatrogenic chylothorax. A case report.

BACKGROUND: To report the association between thoracic duct ligation and acute pancreatitis. The association between sudden stop of lymphatic flow and pancreatitis has been established in experimental models. CASE PRESENTATION: A 57-year-old woman operated for thymoma presented a iatrogenic chylothorax. After thoracic duct ligation, she presented an acute pancreatitis which resolved after conservative treatment. The chylothorax disappeared within 4 days of thoracic duct ligation. CONCLUSIONS: This is the first report of acute pancreatitis following thoracic duct ligation. The pancreas and di

Video-assisted right supradiaphragmatic thoracic duct ligation for non-traumatic recurrent chylothorax.

BACKGROUND: Chylothorax is an uncommon disorder with respiratory, nutritional and immunological manifestations. Surgical management is indicated in case of recurrence or failure after conservative treatment. We report our experience with video-assisted right-sided supradiaphragmatic thoracic duct ligation for non-traumatic, non-postoperative persistent or recurrent chylothorax. PATIENTS AND METHODS: The medical records of six patients operated at our institution between 1999 and 2004 were retrospectively reviewed. A right-sided chylothorax was found in four patients, a left-sided in one, and

Inadvertent puncture of the thoracic duct during attempted central venous catheter placement.

We report a case of puncture of the thoracic duct during left subclavian vein catheterization on the intensive care unit. Computed tomography and measurement of the triglyceride levels in the aspirated fluid proved the inadvertent penetration of the guidewire into the thoracic duct. Early recognition of central line misplacement avoided serious complications. Inadvertent central venous catheter placement into the thoracic duct may have the potential complications of infusion mediastinum and chylothorax.

Thoracic duct injury associated with left internal jugular vein catheterization: anatomic considerations.

Ultrasound (US)-guided cannulation of the internal jugular vein (IJV) has become the preferred approach for venous access as a result of its higher success rate and lower incidence of complications. This report describes a case of thoracic duct injury during US-guided left IJV catheterization. The normal and variant anatomy of the thoracic duct in the neck is illustrated.

[Spontaneous occlusion of a thoracic duct fistula following leftsided shoulder girdle amputation].

Reference is made to the anatomy and variant courses of the thoracic duct, as well as to possibilities of surgical and conservative therapy. A chylous fistula developed in a 53-year old man after his left shoulder girdle had been amputated because of a recurrent low grade chondrosarcoma. Nineteen liters of chyle were excreted over a 5-week period. It was possible to achieve a spontaneous occlusion of the fistula by means of repeated punctures, compression bandages, and a low-fat diet.

Morphology of the distal thoracic duct and the right lymphatic duct in different head and neck pathologies: an imaging based study.

BACKGROUND: The purpose of this study was to assess the influence of head and neck pathologies on the detection rate, configuration and diameter of the thoracic duct (TD) and right lymphatic duct (RLD) in computed tomography (CT) of the head and neck. METHODS: One hundred ninety-seven patients were divided into the subgroups "Работает", "benign disease" and "malignant disease". The interpretation of the images was performed at a slice thickness of 3 mm in the axial and coronal plane. In each case we looked for the distal part of the TD and RLD respectively and subsequently evaluated their conf

Effective Thoracic Duct Ligation at T9 for Postoperative Chylothorax: A Case Report.

Postoperative chylothorax after lung resection and mediastinal lymph node dissection is a rare complication that may be difficult to manage when conservative treatment is ineffective, and the thoracic duct or leak point cannot be identified intraoperatively. Chylothorax is thought to result from thoracic duct injury associated with mediastinal lymph node dissection. Although surgical thoracic duct ligation is recommended in refractory cases, identification of the leakage site or thoracic duct itself is often challenging, particularly in patients with intrathoracic adhesions or in facilities wh

Thoracic Duct Injury in a Patient Undergoing Axillary Lymphadenectomy: A Case Report.

A 56-year-old female patient, with no significant comorbidities, presented with abnormal breast exam findings. Imaging revealed a 5.4-cm irregular nodule in the left breast, diagnosed as invasive breast carcinoma (NST, Grade 2). Neoadjuvant chemotherapy was initiated, leading to a reduction in lesion size. Surgical intervention included quadrantectomy, sentinel lymph node biopsy, and axillary lymphadenectomy, which revealed residual carcinoma and positive lymph nodes. Postoperatively, chylous drainage through a Portovac drain was observed, prompting reoperation, during which the injured lympha

Four cases of unique thoracic duct anatomical characteristics identified by fluorescence thoracoscopy.

We identified one unique kind of anatomical characteristics of thoracic duct (TD) in four cases using fluorescence thoracoscopy during right lung cancer surgery. In these patients, a collateral branch was gave off from TD at the level of the azygos vein arch, which ran along the superior edge of the azygos vein arch, moving from posterior to anterior, and then turned upward in the anterior mediastinum with complex branching patterns. The branches can be easily injured during surgery, which could lead to postoperative chylothorax.

Pediatric Chylothorax Treated With Lymphangiography and Thoracic Duct Embolization: A Case Report.

A chylothorax, the accumulation of lymphatic fluid in the pleural space, may occur for a variety of reasons. It is commonly seen in adults post-thoracic surgery. We present the case of a seven-month-old girl with a right-sided chylothorax in the setting of non-accidental trauma. Treatment options for a chylothorax include surgical ligation of the thoracic duct or, as in this case, a minimally invasive procedure performed by interventional radiology known as lymphangiography with thoracic duct embolization. This case highlights interventional radiologists' ability to treat complex lymphatic pat

Looking Backward and Looking Forward: Revisiting "Clinical Usefulness of Thoracic Duct Cannulation".

Over the past decade, there has been a resurgence of interest in the thoracic duct and central lymphatic system and its disorders, stimulated by advances in interventional magnetic resonance imaging techniques and urgent challenges in the clinical management of lymphatic malformations and lymphatic complications from congenital heart disease. The following chapter reprinted here written shortly after the formal founding of the discipline of lymphology and the International Society of Lymphology, describes early efforts, and also suggests future directions now being revisited and others yet to

Novel Subdiaphragmatic Ligation of Left Thoracic Duct for Refractory Postoperative Left Chylothorax.

A postoperative chylothorax is an uncommon but problematic surgical complication in 0.5% to 4.0% of surgical cases that nevertheless still plagues every busy thoracic surgeon. Fortunately, most chylothoraces are low volume and are readily controlled by conservative measures. A high-volume chylothorax (>1 L/24 h) fortunately occurs in less than one-third of patients, usually responding to the published treatment algorithms and generally requiring invasive techniques. We report a case of a postlobectomy high-volume, left-sided chylothorax refractory to all the usual recommended interventions tha
