Chest Wall Deformities and Congenital Lung Lesions: What the General/Thoracic Surgeon Should Know.

Pectus excavatum, carinatum, and arcuatum are 3 developmental chest wall deformities that may evolve during childhood and cause cardiac and/or pulmonary compression. Evaluation may include nonsurgical subspecialty consultations and imaging studies. Treatment may be nonoperative or surgical. Long-term follow-up studies have identified rare complications of traditional open repair. Routine in utero ultrasonography has led to increasing identification of congenital lung anomalies, including congenital cystic adenomatoid malformations, pulmonary sequestrations, and bronchogenic cysts. Short-term f

Association of Thoracic Epidural Pain Management with Urinary Retention after Complex Abdominal Wall Reconstruction.

The association of thoracic epidural analgesia and urinary retention after complex abdominal wall reconstruction (CAWR) is unknown. The purpose of this study was to investigate the association between the presence of a thoracic epidural, timing of Foley catheter removal, and the rates of urinary retention and catheter-associated urinary tract infections (CAUTIs) in patients undergoing CAWR. All patients undergoing CAWR, who had an epidural catheter for postoperative pain management at our institution from September 2015 through April 2016, were prospectively followed. Patients were divided int

Wall stress on ascending thoracic aortic aneurysms with bicuspid compared with tricuspid aortic valve.

OBJECTIVE: Guidelines for repair of bicuspid aortic valve-associated ascending thoracic aortic aneurysms have been changing, most recently to the same criteria as tricuspid aortic valve-ascending thoracic aortic aneurysms. Rupture/dissection occurs when wall stress exceeds wall strength. Recent studies suggest similar strength of bicuspid aortic valve versus tricuspid aortic valve-ascending thoracic aortic aneurysms; thus, comparative wall stress may better predict dissection in bicuspid aortic valve versus tricuspid aortic valve-ascending thoracic aortic aneurysms. Our aim was to determine wh

[Vascular Graft Perforation caused by Calcified Aortic Wall Late after a Thoracic Aortic Replacement].

A 61-year-old female on hemodialysis with chronic renal failure presented with hemoptysis 10 years after replacement of the distal arch and the descending aorta. Computed tomography showed hematoma around the prosthetic graft and intrapulmonary hemorrhage adjacent to the graft. At emergency open surgery, perforation was revealed in the distal portion of the graft. It was considered to be caused by the friction between the surrounding calcified aortic wall and the graft. The ruptured graft was successfully replaced.

Aortic wall stress in hypertension and ascending thoracic aortic aneurysms: implications for antihypertensive therapy.

The objective of this study was the evaluation of aortic wall stress in patients with ascending thoracic aortic aneurysms (TAA) because of the paucity of data to guide medical therapy for blood pressure (BP) management in TAA. Twelve men, age 67.4 ± 3.3 years (SEM) with hypertension and ascending TAA without other etiology, previous aortic surgery or associated significant aortic valve disease, had maximum dimensions of the ascending aorta measured from CT angiogram (CTa) and transthoracic echocardiogram (TTE) with aortic wall thickness measured on TTE. Wall stress (WS(σ)(P)) at peak systolic

Obesity increases the risk of chest wall pain from thoracic stereotactic body radiation therapy.

PURPOSE: Stereotactic body radiation therapy (SBRT) is increasingly being used to treat thoracic tumors. We attempted here to identify dose-volume parameters that predict chest wall toxicity (pain and skin reactions) in patients receiving thoracic SBRT. PATIENTS AND METHODS: We screened a database of patients treated with SBRT between August 2004 and August 2008 to find patients with pulmonary tumors within 2.5 cm of the chest wall. All patients received a total dose of 50 Gy in four daily 12.5-Gy fractions. Toxicity was scored according to the NCI-CTCAE V3.0. RESULTS: Of 360 patients in the

Incidence of chest wall paresthesia after needlescopic video-assisted thoracic surgery for palmar hyperhidrosis.

OBJECTIVE: Chest wall paresthesia is a reported sequela of thoracotomy and Video-Assisted Thoracic Surgery (VATS) which is distinct from wound pain. Although needlescopic VATS confers less post-operative pain and better cosmesis, the incidence of paresthesia after needlescopic VATS has not been quantified. METHODS: For homogeneity of the patient cohort, we studied 50 patients who received bilateral needlescopic VATS sympathectomy (T2-T4 excision) for palmar hyperhidrosis using 2 or 3 mm instruments during a 36-month period at a single institute. A standard questionnaire was administered by te

Thoracic wall deformity in Bengal cats - Morphological findings and results from a breeder questionnaire.

Thoracic wall deformity (TWD) is reported in several cat breeds. The Burmese and Bengal cats appear predisposed, suggesting a genetic background. Thoracic changes in Bengal kittens were investigated using whole-body computed tomography, macro- and microscopic examination of the rib cage. By using a questionnaire, breeding performance and kitten mortality among Bengal cats in Sweden during a two-year period was analyzed. TWD comprised a spectrum of symmetrical and asymmetrical rib cage changes. Macroscopically, lordosis, flat chest and pectus excavatum were frequent findings. When asymmetric, t

Chest radiographic thoracic areas and respiratory outcomes in infants with anterior abdominal wall defects.

OBJECTIVES: Infants with anterior abdominal wall defects (AWD) can suffer from pulmonary complications. Our aims were to determine if the chest radiographic thoracic areas (CRTAs) on day one differed between infants with exomphalos or gastroschisis, whether this related to differing severity of outcomes and if they were lower than those of controls indicating abnormal antenatal lung growth. METHODS: A review of infants with exomphalos or gastroschisis born between January 2004 and January 2023 was conducted. The control group was term, newborn infants ventilated for poor respiratory drive at

Chest Wall Reconstruction Using Titanium Mesh in a Dog with Huge Thoracic Extraskeletal Osteosarcoma.

A 6-year-old castrated male mixed dog presented with a rapidly growing mass at the right chest wall two weeks after initial detection. A mesenchymal origin of the malignancy was suspected based on fine-needle aspiration. Computed tomography (CT) revealed that the mass originated from the right chest wall and protruded externally (6.74 × 5.51 × 4.13 cm3) and internally (1.82 × 1.69 × 1.50 cm3). The patient revisited the hospital because of breathing difficulties. Radiography confirmed pleural effusion, and ultrasonography-guided thoracocentesis was performed. The effusion was hemorrhagic, and m

Effectiveness of Thoracic Wall Blocks in Video-Assisted Thoracoscopic Surgery, a Network Meta-Analysis.

INTRODUCTION: Thoracic epidural analgesia (TEA) and thoracic paravertebral blocks (PVB) are well-established techniques for pain management in thoracotomy. Here, we examine the efficacy of various thoracic fascial plane blocks vs TEA and PVB for intraoperative and postoperative analgesia for video assisted thoracoscopy surgery (VATS) with network meta-analysis. METHODS: A search for prospective randomized control studies using adult patients undergoing VATS with general anesthesia. The interventions of interest were any regional anesthesia techniques used for postoperative pain control after

Pneumothorax Due to a Nontraumatic Thoracic Wall Rupture Due to Steroid-Induced Muscle Wasting.

Spontaneous pneumothorax can be classified into primary and secondary variants. A 58-year-old patient presented with a 7-week history of severe coughing and chest pain. He noticed progressive swelling of the face and the upper part of the body. His medical history revealed osteoporosis and severe rheumatoid arthritis treated with steroids and disease-modifying antirheumatic drugs. Computed tomography of the thorax revealed complete rupture of the thoracic wall through costae 9 and 10 with lung herniation. The defect was closed using dual mesh and the pneumothorax was treated. Two weeks after s

Mesothelial cyst derived from chest wall pleura growing after thoracic surgery: a case report.

BACKGROUND: Intrathoracic mesothelial cysts almost always arise in the mediastinum, and extramediastinal mesothelial cysts are extremely rare. Here we describe a case of mesothelial cyst derived from the chest wall pleura growing after thoracic surgery. CASE PRESENTATION: A 63-year-old Japanese woman was referred to our department. She had undergone total hysterectomy for cervical carcinoma and two lung wedge resections for metastatic lung cancer on the upper and lower lobes of her right lung and lower lobe of her left lung. After the thoracic surgery, an intrathoracic chest wall mass was fou

Large posterior chest wall defect reconstructed with a de-epithelised trans-thoracic TRAM flap.

Extensive posterior chest wall defects, especially those closer to the midline of the trunk, pose surgical challenges in plastic surgery. In these cases, prior to tissue coverage, the status of the pleural cavity and skeletal support need to be addressed to obtain a functional and anatomical reconstruction. We recently operated upon a patient presenting with an aggressive ossifying fibromyxoid tumour located in the lower dorsal paraspinal region on the right side. After ablative surgery and intra-operative radiotherapy, a broad defect in the chest wall of 15 x 10 cm, including the proximal par

Hyperkyphotic measures using distance from the wall: validity, reliability, and distance from the wall to indicate the risk for thoracic hyperkyphosis and vertebral fracture.

UNLABELLED: C7WD is a practical, valid, and reliable measure that could clearly indicate a risk of hyperkyphosis and vertebral fracture in the elderly. The findings might be particularly of use in regions difficult to access radiology or for the determination of those who need further invasive radiologic examination and therapy. PURPOSE: To investigate psychometric properties of the 7th cervical vertebra wall distance (C7WD) to determine the risk of thoracic hyperkyphosis and spinal fracture, as compared to a standard radiologic Cobb's method. METHODS: Community-dwelling elderly (n = 104), a

In Vivo Quantification of Ascending Thoracic Aortic Aneurysm Wall Stretch Using MRI: Relationship to Repair Threshold Diameter and Ex Vivo Wall Failure Behavior.

Ascending thoracic aortic aneurysms (aTAAs) can lead to life-threatening dissection and rupture. Recent studies have highlighted aTAA mechanical properties as relevant factors associated with progression. The aim of this study was to quantify in vivo aortic wall stretch in healthy participants and aTAA patients using displacement encoding with stimulated echoes (DENSE) magnetic resonance imaging. Moreover, aTAA wall stretch between surgical and nonsurgical patients was investigated. Finally, DENSE measurements were compared to reference-standard mechanical testing on aTAA specimens from surgic

Accidental compression of the thoracic wall. Mechanical asphyxia rather than trauma is the main culprit.

We present three cases of traumatic asphyxia after thoracic compression. All victims were Caucasian males aged 22-50 years. One man was crushed by a truck trailer, another was crushed by an overturned vehicle, and the last was crushed by a large heavy stone slab. None of the patients survived the accident. There was no evidence of trauma or only minor trauma from the bones or vital organs of the thoracic cavity and abdomen.

The widths of the medial and lateral pedicle walls in adolescent idiopathic scoliosis with major thoracic curves.

BACKGROUND CONTEXT: The widths of medial and lateral pedicle walls in the normal spine of middle-aged and elderly adults have been investigated and these studies found that the medial pedicle wall was thicker than the lateral pedicle wall. However, none had evaluated the widths of medial and lateral pedicle walls on adolescent or young adult scoliotic spines. PURPOSE: This study aims to identify the distribution and variation of medial and lateral pedicle wall widths throughout the different vertebral levels of the scoliotic spine and its differences according to age, gender, body mass index
