Meniscal ramp lesions are disruptions of the posterior meniscotibial attachment of the medial meniscus and are commonly associated with anterior cruciate ligament injuries. However, they can be frequently missed when reviewing standard magnetic resonance imaging and difficult to treat. In this presentation, we describe our approach to repair a meniscal ramp lesion using a minimally invasive all-inside technique. We use this technique for the following surgical indications: meniscal tears involving the peripheral and meniscocapsular attachment of the posterior horn resulting in increased menisc

Medial meniscus posterior root tears in the varus knee present a challenge of balancing biological repair with biomechanical correction. Recent studies show that combining high tibial osteotomy with root repair improves arthroscopic healing rates compared with osteotomy alone; however, this does not consistently reduce meniscal extrusion or prevent osteoarthritis progression. There is a clinical disconnect between structural healing and functional outcomes, as patient-reported improvement may occur despite ongoing degeneration. We emphasize the importance of patient selection and prompt recons

Meniscal tears are a very common clinical condition, often treated with surgery. While indications for meniscal repair are expanding, most meniscal tears are still treated with arthroscopic resection. Meniscal allograft transplantation (MAT) is an evolving treatment option for young, active patients with symptomatic meniscal-deficient knees. There is a growing body of evidence for good long-term outcomes from MAT, although the long-term chondroprotective potential is still unproven. This article reviews the latest evidence regarding the indications, surgical techniques, complications, rehabili

Meniscal root tears are increasingly being recognized as important entities by the orthopaedic community. This is probably due to the catastrophic consequences of not identifying or addressing a root tear in a timely fashion, leading to the need for a total meniscectomy. The majority of the studies on root tears have been focused on natural history, diagnosis, biomechanical consequences, and fixation techniques. Conversely, rehabilitation concepts have been extrapolated from those applied after other meniscal tears/repairs, even though root tears probably constitute a completely different path

Current options for meniscal root repair include repair into trans-osseous bone tunnels, trans-osseous suture passage for surface fixation, and suture anchor fixation. Suture anchor repair techniques have been developed since it eliminates the issue of the suture abrasion, tunnel drilling, and distal fixation inherent to trans-osseous tunnel. We present a description of a new variation in the more vertical suture anchor repair technique for meniscal root tear using a novel medial quadriceptal portal. Level of evidence Therapeutic, Level V.

The importance of the menisci for stability of the knee joint is pointed out. Because of its mechanical importance in the knee a more conservative behaviour for treatment is recommended. Even the degenerated meniscus can play his role sufficiently. For each type of lesion individual surgery is applied, especially if there is locking impingement linked with mechanically induced arthritis. Out of the clinical results follows that arthroscopic partial meniscectomy is superior to conventional open meniscectomy, especially in the early postoperative period.

PURPOSE: Meniscal injuries are a common cause of knee dysfunction and healthcare utilization, with magnetic resonance imaging (MRI) being the diagnostic gold standard. However, MRI's cost, limited accessibility, and contraindications of MRI have prompted interest in ultrasound (US) as a more affordable, portable, and radiation-free alternative. This review aimed to synthesize the current evidence on the diagnostic accuracy of US for meniscal tears and to define its role alongside MRI and arthroscopy in clinical practice.
METHODS: We conducted a systematic review following the PRISMA guideline

PURPOSE: The prevalence and appreciation of meniscal tears in children have increased in both number and complexity. There is currently a paucity of high-quality evidence that can guide surgeons in treating skeletally immature patients with meniscal injuries. The aim of this study was to develop comprehensive recommendations for the management of isolated meniscal tears in skeletally immature children.
METHODS: An international, two-round, modified Delphi consensus was completed. Included 'experts' were identified as having an established adult knee practice, including children and either: (1

INTRODUCTION: The proximity of the posterior horn of the lateral meniscus to the popliteal artery determines a risk of vascular injury in its suture. The aim of this study is to determine the location of the popliteal artery, and to establish the minimal distance from the posterior wall of the lateral meniscus to the artery, the common peroneal nerve (CPN), and its correlation to other variables.
MATERIAL AND METHODS: A total of 102 magnetic resonance studies were retrospectively reviewed from patients undergoing surgery at our institution. The axial section where the lateral meniscus could b

The 20-year progression of osteoarthritis (OA) after arthroscopic partial meniscectomy (APM) in patients aged between 50 and 70 bears a long-term risk of conversion to total knee arthroplasty of 15.7%. Negative predictors at the time of surgery are the degree of knee OA, lateral meniscectomy, age at surgery, and malalignment. This confirms the evolution of the natural history of knee OA, but most importantly, it provides arguments to further restrain indications of APM in degenerative meniscus lesions (DMLs). An improved understanding of the consequences of APM for DMLs allows to increasingly

The simultaneous posterolateral tibial plateau fracture (PLTPF) with anterior cruciate ligament (ACL) injury has posed a great challenge to both orthopedic trauma and sports medicine surgeons. This study investigated the biomechanical mechanism of simultaneous PLTPF with ACL injury and demonstrated the consistency with anterolateral rotatory instability (ALRI) of the knee. A healthy male volunteer's right knee CT and MRI images were imported into Mimics software to reconstruct a three-dimensional geometric model of bone, ligament, meniscus and cartilage. The PLTPF were simulated at posterior h

Meniscal root tears remain a common problem, with devastating biomechanical and clinical consequences. Thankfully, numerous techniques have been developed to repair the symptomatic meniscal root tear. However, rates of conversion to arthroplasty are reported to be 21% to 33% at 10 years, and persistent extrusion of the meniscus at follow-up is a known limitation of current root repair techniques. There is also growing evidence that some medial meniscal root tears may be an effect of meniscal extrusion, rather than the cause of it. In that vein, failure to correct extrusion may be a key mechani

BACKGROUND: Meniscal tears affect 222 per 100,000 of the population and can be managed non-operatively or operatively with an arthroscopic partial meniscectomy (APM), meniscal repair or meniscal transplantation. The purpose of this review is to summarise the outcomes following treatment with a meniscal tear and explore correlations between outcomes.
METHOD: A systematic review was performed of MEDLINE, EMBASE, AMED and the Cochrane Central Register of Controlled Trials to identify prospective studies describing the outcomes of patients with a meniscal tear. Comparisons were made of outcomes b

The present work further contributes to building the base of the research pyramid by noting the importance of repairing knee meniscal radial root tears. Because of the extensive biomechanical studies that have now been published on the restoration of joint mechanics and with what I believe results in a higher likelihood of an improved healing environment with securing the meniscus root tear down to bone, I would strongly recommend that when one does see a meniscus root tear that the transtibial pullout root repair technique be considered over a side-to-side repair. The biggest challenge going

A root attachment injury (root tear) of the meniscus can abolish the ability of the meniscus to bear hoop stress and predispose to increase articular contact stress which contribute to femorotibial degenerative changes. A pull out suture technique to repair the root tear has been described, but the procedure making the tibial tunnel may be difficult and troublesome. This article describes a repair technique using a suture anchor and posterior trans-septal portal.

Arthroscopic resection of irreparable bucket-handle tears of the medial meniscus is a commonly performed procedure. Adequate visualization of the posterior horn of the medial meniscus can be a challenging task with the conventional use of the anterior portal. An attempt to resect the posterior horn in a blind fashion may result in iatrogenic damage of the articular cartilage in the posterior compartment, over-resection of a remnant meniscus, or an insufficient resection of the torn fragment. We describe the use of the posteromedial portal for an accurate visualization and resection of the post

Analyzing meniscal abnormalities on MR images depends on a thorough understanding of the normal appearance of meniscal structures and on strict adherence to the MR imaging criteria for meniscal tears. When proper MR imaging technique is followed, meniscal tears can be diagnosed more acutely than with clinical examination alone, even in the presence of associated ligament injuries. The value of MR imaging lies in its ability to reveal details of meniscal tears, including location, morphology, length, depth, and possible stability. If this MR imaging information can be synthesized, it will drama
