Correlation between diabetic lower-extremity arterial disease and diabetic neuropathy in patients with type II diabetes: an exploratory study.

The lower-extremity vascular injuries and neuropathy are the most salient complications of diabetes which could lead to the poor prognosis, especially for the type II diabetes. The lower extremity vascular injuries and neuropathy usually coexist, yet their correlation in the pathogenesis of lower extremity lesions has received little attention in previous studies. To investigate the correlation between the degree of lower-extremity arterial injuries and lower-extremity neurological functional status in patients with type II diabetes, 32 patients with type II diabetes were examined for the mean

Sonography as an aid to neurophysiological studies in diagnosing tarsal tunnel syndrome.

Diagnosing tarsal tunnel syndrome can be difficult because of varying clinical diagnostic criteria and equivocal physical signs. We present a case of tarsal tunnel syndrome where nerve conduction identified distal tibial neuropathy and high-resolution sonography was able to show nerve swelling within the tarsal tunnel.

Nerve Ultrasound for the Diagnosis of Tarsal Tunnel Syndrome: Findings in 26 Clinically and Electrophysiologically Confirmed Feet.

Background/Objectives: Posterior tarsal tunnel syndrome is a compressive neuropathy of the tibial nerve at the level of the ankle within the tarsal tunnel. However, there is no established gold standard for the diagnosis of tarsal tunnel syndrome to date. High-resolution ultrasound could add important value in this setting. But up to date, to the best of our knowledge, only six clinical studies have investigated the use of ultrasound for the diagnosis of tarsal tunnel syndrome, with partially conflicting results. Most authors identify nerve swelling at the level of anatomical compression as th

Tarsal tunnel syndrome: a compression neuropathy involving four distinct tunnels.

Tarsal tunnel syndrome is a complex and often under-diagnosed or misdiagnosed condition that affects the foot and ankle. It is a compression neuropathy of the posterior tibial nerve as it passes in the anatomic tarsal tunnel in the medial ankle under flexor retinaculum. This article reviews diagnosis, conservative treatment, and surgical outcomes, which have dramatically improved with more comprehensive release of the foot nerves in addition to the tibial nerve. Internal neurolysis facilitates a second level of nerve decompression in needed cases. Physical therapy protocols have made it possib

Common Peroneal Nerve Injury Due to an All-Inside Repair of the Posterior Horn of Lateral Meniscus: A Case Report.

CASE: An 18-year-old man sustained a peroneal nerve (PN) injury during an all-inside repair of the posterior horn of the lateral meniscus from the medial portal. Although he could dorsiflex his ankle actively after emergence from general anesthesia, he had a foot drop on the day after surgery. Exploration of the PN at 5 months postoperatively revealed that the nerve was entrapped by the suture. Fifteen months after a nerve repair using a sural nerve graft, he recovered from the foot drop. CONCLUSION: This case report highlights the risk of PN injury during an all-inside repair of the posterio

Does the tibial and sural nerve transection model represent sympathetically independent pain?

Neuropathic pain can be divided into sympathetically maintained pain (SMP) and sympathetically independent pain (SIP). Rats with tibial and sural nerve transection (TST) produce neuropathic pain behaviors, including spontaneous pain, tactile allodynia, and cold allodynia. The present study was undertaken to examine whether rats with TST would represent SMP- or SIP-dominant neuropathic pain by lumbar surgical sympathectomy. The TST model was generated by transecting the tibial and sural nerves, leaving the common peroneal nerve intact. Animals were divided into the sympathectomy group and the s

[Tarsal tunnel syndrome. Diagnosis and differential diagnosis].

The tarsal tunnel syndrome represents an entrapment neuropathy of the tibial nerve behind the internal malleolus under the retinaculum of the flexor muscles of the foot. In more than 3 out of 4 cases, it is a late consequence of twisting or fracture of the ankle. Local pain is increased by walking. In addition to local pain on pressure behind the internal malleolus, at a later stage paresis of the small foot muscles becomes evident with impaired abduction of the toes, decreased sensation and abnormally dry, smooth skin over the sole of the foot. The impairment of tibial nerve function can also

Computed tomographic analysis of pes cavus.

Patterns of muscle degeneration in patients with peripheral neuropathies exhibiting pes cavus deformity were studied by computed tomography (CT). Twenty-six patients attending the muscle disease clinic at Newington Children's Hospital with hereditary sensory motor neuropathies (HSMN) I, II, or III had clinical and radiographic assessment in addition to CT scans of the feet and legs at designated levels. The pattern of muscle degeneration was analyzed with other variables, including age, sex, tibial torsion, cavus, heel varus, and claw toes. Multiple regression/correlation analysis clearly demo

Vacor neuropathy: ultrastructural and axonal transport studies.

Distal axonal degeneration has been correlated with abnormalities of fast axonal transport in several toxic neuropathies. We have investigated axonal transport in experimental PNU (N-3-pyridylmethyl-N'-p-nitrophenylurea; Vacor) neuropathy, because of the rapid and synchronous degeneration of many terminal axons after a single dose of PNU. Almost all axon terminals at neuromuscular junctions in hindfoot muscles degenerated by 24 hours after the administration of PNU. Fewer affected axons were found in intramuscular nerve twigs, and fewer still in the posterior tibial nerves. No abnormal myelina

Combined common peroneal and tibial nerve injury after knee dislocation: one injury or two? An MRI-clinical correlation.

OBJECT Knee dislocations are often accompanied by stretch injuries to the common peroneal nerve (CPN). A small subset of these injuries also affect the tibial nerve. The mechanism of this combined pattern could be a single longitudinal stretch injury of the CPN extending to the sciatic bifurcation (and tibial division) or separate injuries of both the CPN and tibial nerve, either at the level of the tibiofemoral joint or distally at the soleal sling and fibular neck. The authors reviewed cases involving patients with knee dislocations with CPN and tibial nerve injuries to determine the localiz

Effect of various nerve decompression procedures on the functions of distal limbs in streptozotocin-induced diabetic rats: further optimism in diabetic neuropathy.

It is known that diabetic neuropathy is the result of endoneurial edema caused by various biochemical reactions triggered by hyperglycemia. This sequence of events can cause cessation of circulation at the perineurial level, or the tough layer, which is not resilient enough to spread intraneural pressure. Internal and external limiting structures create a double crush phenomenon to the nerve structure. Decompression of the nerve trunk at separate levels is one of the adjuncts to the overall treatment plan for diabetic neuropathy. In this study, the right sciatic nerves of 30 rats with streptoz

Charcot-Marie-Tooth type 2CC misdiagnosed as Chronic Inflammatory Demyelinating Polyradiculoneuropathy.

BACKGROUND AND AIMS: Charcot-Marie-Tooth (CMT) is a heterogeneous group of genetic neuropathies and is typically characterized by distal muscle weakness, sensory loss, pes cavus and areflexia. Herein we describe a case of CMT2CC presenting with proximal muscle weakness and equivocal electrophysiological features, that was misdiagnosed as chronic inflammatory demyelinating polyneuropathy (CIDP). CASE REPORT: A 30-year-old woman complained of proximal muscle weakness with difficulty climbing stairs. Neurological examination showed weakness in lower limb (LL) muscles, that was marked proximally

Sonographic Nerve Enlargement in a Patient with Sarcoidosis.

We herein report a 73-year-old woman case with sarcoid neuropathy showing nerve enlargement assessed by nerve ultrasound both before and after treatment. The site of conduction block in the left tibial nerve corresponded to the site of nerve enlargement with a hypo-echoic pattern. After treatment with prednisolone, nerve ultrasound detected the remission of the nerve enlargement, and the conduction block and clinical symptoms also improved. Nerve enlargement may reflect inflammation of the peripheral nerve. A follow-up study of sonographic nerve enlargement may be of clinical significance for

Peripheral neuropathy in patients with beta-thalassaemia.

As some patients with beta-thalassaemia manifested neurological signs, clinical and electrophysiological investigations were carried out on 53 thalassaemic patients and 29 healthy control subjects. Twenty per cent of the patients showed clinical and electrophysiological findings of a mild peripheral sensorimotor neuropathy, mainly of the lower limbs. The clinical symptoms were numbness, pins and needles sensations, muscular cramps, myalgia and muscle weakness. The electrophysiological abnormalities were manifested by decreased motor conduction velocity (MCV) and prolonged F-wave latencies of t

Central and peripheral SEP defects in neurologically symptomatic and asymptomatic subjects with low vitamin B12 levels.

Somatosensory evoked potentials (SEPs) following median nerve stimulation were abnormal in 7 patients with sensory impairment due to vitamin B12 deficiency. Extensor plantar reflexes indicated a central sensory pathway lesion in 4 cases and absent tendon jerks suggested peripheral neuropathy in 4, but median nerve SEPs indicated a predominantly central lesion without marked peripheral nerve involvement in 6 and an axonal neuropathy without CNS involvement in 1. The latter had evidence of central slowing of conduction in SEPs following posterior tibial nerve stimulation. Consequently, it is sug

Peak pressure data and pressure-time integral in the contralateral limb in patients with diabetes and a trans-tibial prosthesis.

BACKGROUND: Clinicians currently rely on observational clinical data pertaining to the biomechanics of the diabetic foot. However, advances in technology can objectively describe this. A thorough understanding of the functional and mechanical consequences following trans-tibial amputations is lacking. RESEARCH QUESTION: Does a trans-tibial prostheses significantly increase peak plantar pressures and pressure time integrals in the intact foot of patients with type-2 diabetes and neuropathy? METHODS: A prospective quantitative matched-subject design was employed. Twenty participants living wit

Endoscopic Ganglionectomy and Internal Drainage of Hourglass Ganglion of the Tarsal Tunnel.

Tarsal tunnel syndrome is a common entrapment neuropathy that is sometimes elicited by ganglia in the tarsal tunnel. The tarsal ganglion may penetrate through the flexor retinaculum and have an hourglass appearance with components deep and superficial to the retinaculum. The purpose of this Technical Note is to describe technical details of endoscopic management of hourglass ganglion of the tarsal tunnel. This comprises excision of the part of the ganglion superficial to the flexor retinaculum and internal drainage of the deep part of the ganglion to the tendon sheath of the posterior tibial t

Spatial variation in mechanical properties along the sciatic and tibial nerves: An ultrasound shear wave elastography study.

Ultrasound shear wave elastography has become a promising method in peripheral neuropathy evaluation. Shear wave velocity, a surrogate measure of stiffness, tends to increase in peripheral neuropathies regardless of etiology. However, little is known about the spatial variation in shear wave velocity of healthy peripheral nerves and how tensile loading is distributed along their course. Sixty healthy young adults were scanned using ultrasound shear wave elastography. Five regions of the sciatic (SciaticPROXIMAL, SciaticDISTAL) and tibial nerve (TibialPROXIMAL, TibialINTERMEDIATE, and TibialDIS
