Partial replacement of the dentin-pulp complex by periodontal supporting tissues in a traumatically intruded primary maxillary incisor.

This paper describes a case of a 9-year-old patient who presented a completely intruded primary maxillary incisor because of a traumatic injury sustained at the age of 3 years. After tooth extraction, histological analysis revealed that the dentin-pulp complex was partially replaced by cementum, periodontal ligament and alveolar bone. No signs of ankylosis were noticed. It is suggested that the lack of spontaneous re-eruption of the traumatized primary tooth after 6 years could be due to the development of functional periodontal supporting tissues in the pulp chamber secondary to the traumatis

Endodontic management of the avulsed tooth and tooth transplantation.

The major causes of post-transplantation tooth loss following tooth replacement after avulsion (exarticulation) and surgical replacement of impacted maxillary canines (auto-transplantation) are inflammatory root resorption and replacement resorption (ankylosis). This paper attempts to outline the treatment and management of replanted teeth using the current philosophies of splinting, endodontic therapy and the use of calcium hydroxide in the re-attachment of the periodontal ligament (PDL). Clinical research during the past 20 years has provided sufficient information for the development and es

Long-term Stability of Autotransplanted Premolars as a Substitute for Molars in Adults.

Autotransplantation is a viable treatment option for tooth replacement when a suitable donor tooth is available. This case report presents 2 cases that describe successful treatment outcome and prognosis after autotransplantation of a mature premolar as a missing molar. The first premolar was extracted for orthodontic purposes and transplanted to the missing first molar site in the mandible. The transplanted tooth was monitored up to 7 years. The transplant was functional and maintained a normal bone level throughout the follow-up period. Clinical examination showed normal physiologic movement

Treatment of an avulsed maxillary permanent central incisor replaced by autotransplantation of a mandibular premolar: 14-year follow-up.

AIM: To present the 14 year follow-up of a case in which an avulsed permanent maxillary central incisor was replaced by autotransplantation of a mandibular premolar. SUMMARY: A mandibular premolar (Moorrees' stage 3) was transplanted into the space left by an avulsed permanent maxillary central incisor after a delay of 3 months. Recall examination 14 years after transplantation revealed a normal periodontal architecture with absence of infection, ankylosis or progressive resorption. The transplantation of a premolar is seen as a promising method to replace a lost permanent tooth and to restor

Treatment of a vertical root fracture using dual-curing resin cement: a case report.

Introduction. Vertical root fracture (VRF) is one of the most frustrating complications of root canal treatment. The prognosis of the root with VRF is poor therefore tooth extraction and root amputation are usually the only treatment options. However, bonding of the fracture line with adhesive resin cement during the intentional replantation procedure was recently suggested as an alternative to tooth extraction. Methods. A vertically fractured left maxillary incisor was carefully extracted, fracture line was treated with adhesive resin cement, a retrograde cavity was produced and filled with c

The unerupted maxillary canine--a post-surgical review.

The orthodontic records of 685 patients referred for surgical exposure of an unerupted impacted maxillary canine tooth were examined. The condition was more common among females than males, slightly less than 2:1. The impacted teeth had a palatal-labial ratio of 3:1. All of the teeth were exposed using the open surgical technique and in 98% of cases the tooth erupted and was orthodontically aligned. In 2% of cases ankylosis occurred and the teeth were subsequently extracted. The presence of peg-shaped lateral incisors associated with the impacted maxillary canine tooth was 3.4% of the total nu

Impairment of molar tooth eruption caused by x-radiation.

The effect of x-radiation on erupting molars is presented. New born, 5 day old Wistar rats were locally irradiated in the molar area with doses of 20 Gy. They were killed in two groups, 30 and 60 days postirradiation respectively. Two other groups of non irradiated, age matched rats were killed at the given times. In addition a control group of 5 day old animals was also studied. Radiographic and histologic studies were performed. Odontoblastic atrophy, odontodysplasia, rootless formation, and ankylosis of tooth to bone by osteodentin formation with the resulting lack of tooth eruption were ob

Enamel Veneer Rehabilitation of a Premolar to an Incisor After Autotransplantation.

Tooth autotransplantation is one of the methods used for tooth loss rehabilitation in children. Premolars are usually used as autotransplants requiring esthetic alterations. The purpose of this paper is to present an innovative, alternative and inexpensive way to restore auto-transplanted teeth using the crown of the tooth of the recipient site. A seven-year-old male with a non-contributory medical history presented with an intrusion of his permanent maxillary right central incisor. The tooth underwent orthodontic extrusion using fixed appliances but eventually showed signs of ankylosis. At 10

Eight-year follow-up of autogenous tooth transplantation involving multidisciplinary treatment.

Although autogenous tooth transplantation is a widely reported procedure, its success is dependent on a number of factors. Here we describe the surgical technique, endodontic treatment and rehabilitation employed for a patient in whom a lower right third molar was transplanted to substitute an adjacent second molar with extensive caries. During an 8-year follow-up period, normal periodontal healing was observed and no infection, ankylosis or progressive resorption occurred. It may be concluded that transplantation of a third molar is a practicable approach for replacement of a lost permanent t

Autogenous tooth transplantation: a reevaluation.

This paper discusses whether autogenous tooth transplantation can survive as an option in the future dental armamentaria. Clinically, successful transplants must show a radiolucent space between the roots and surrounding bone. There must be no evidence of ankylosis, no permanent root resorption, and no inflammation. Histologically, the criterion of success is a normal and functional periodontal ligament between the root surfaces of transplants and the bone or gingival connective tissues. Case reports are presented to illustrate replantation or transplantation of teeth. Possible donor teeth are

Risk of ankylosis of avulsed teeth immediately replanted or stored under favorable storage conditions before replantation: A long-term clinical study.

BACKGROUND/AIM: There are few long-term clinical follow-up studies on human teeth replanted immediately or after storage in a suitable storage medium prior to replantation. This study aimed to assess the risk of ankylosis in avulsed human teeth replanted immediately or after storage in physiological media for a short time. MATERIAL: Data from 116 patients with 145 replanted avulsed permanent teeth were selected from a comprehensive dental trauma database in Copenhagen University Hospital. The following teeth were selected: Group 1 comprised 36 teeth replanted immediately (dry time <6 min; wet

Early Treatment of Failure of Eruption of a Permanent Molar.

Failure of eruption of a permanent first molar is a relatively infrequent clinical condition that affects the normal development of dentition and may cause malocclusion. There are two conditions that might result in failure of eruption: (1) mechanical failure (ankylosis) and (2) primary failure of eruption, with different clinical features and therapeutic approaches for each. It is often challenging for orthodontists and pediatric dentists to establish differential diagnoses of these conditions to ensure a successful treatment outcome. The purpose of this case report is to discuss the diagnosi

Changing the Angulation of the Tooth Germ in the Bony Crypt: A Case Report.

An ankylosed primary molar may cause rotation or ectopic impaction of succedaneous premolar. When conventional treatment modalities such as observation, surgical exposure with or without orthodontic traction, and autotransplantation are not possible, the simple surgical relocation method could be an alternative treatment option for a lingually rotated premolar during the tooth germ stage before opting to extraction. In the case reported herein, the lingually rotated permanent mandibular second premolar tooth germ was surgically relocated within its bony crypt. Continued root development and sp

Intentional surgical repositioning of an ankylosed permanent maxillary incisor.

Replacement resorption (ankylosis) may be a significant complication after replantation of avulsed permanent incisor teeth. This report explains the aetiology, diagnosis, management and current treatment options in ankylosis and then describes an alternative surgical technique, intentional luxation and repositioning. This technique, in the presence of an acceptable root length, may be a realistic treatment option in adolescence until osseointegrated implants can be considered at the age of 18-20 years.

The management of a persistent periapical lesion caused by an apicomarginal defect, associated with a root end fracture in an endodontically treated tooth: a clinical report.

This is a case report of the presentation and the management of an apicomarginal defect which was associated with a root end fracture in an endodontically treated tooth. In spite of a successful Root Canal Treatment (RCT), if the fractured root end is left intact, considering inadverdent ankylosis or resorption, it may progress to the development of a bony dehiscence.

Autotransplantation combined with orthodontic treatment: a case involving the maxillary central incisors with root resorption after traumatic injury.

Traumatic dental injury can result in avulsion of anterior teeth. In young patients, it is a challenge to the dental professional because after replantation, late complications such as ankylosis require tooth extraction. Although prosthetic and orthodontic treatment, and implant placement have been described as the options for intervention, autogenous tooth transplantation could be an effective procedure in growing patients if there is a suitable donor tooth available. This case presents the treatment of a patient who suffered a traumatic injury at 9 years old with avulsion of tooth 21, which

Autotransplantation of a maxillary third molar to replace a maxillary premolar with vertical root fracture.

AIM: To report the successful autotransplantation of a fully developed third molar that required nonsurgical and surgical interventions for tooth adaptation. SUMMARY: This case report describes the autotransplantation of a third molar with complete root development after the loss of a fractured premolar in a 47-year-old male. To allow better adaptation of the donor tooth, the buccal roots of the third molar were removed using a diamond bur and the canal entrances were filled. Recall examination 6 years after completion of root-canal treatment showed normal periodontal healing with absence of

Surgical and orthodontic treatment after incidental radiographic discovery of a severely infraoccluded primary molar.

Dentoalveolar ankylosis, the fusion of a tooth with the alveolar bone, is often associated with primary molars and is the main cause of infraocclusion. This report describes an uncommon case of a severely infraoccluded primary first molar associated with loss of space in the maxilla. After anamnesis and a clinical examination, absence of the primary maxillary left first molar was observed. Periapical radiographs revealed a primary molar completely submerged under the gingiva. Cone beam computed tomography was performed to determine the proximity of the ankylosed tooth to the germ of the perman

[Fixation of injured tooth].

In cases of an avulsed or luxated tooth, splinting of the injured tooth is recommended. Splint is defined as a fixation of the injured tooth to adjacent teeth in the dental arch. The preferred splint type is semi-rigid, which should be placed for 10 to 14 days and enable proper oral hygiene and will not cause any kind of impingement to the gingiva. It is effective in reducing the replacement resorption process, which affects the periodontal tissue surrounding roots exposed to dental trauma.
