Risk of ankylosis of 400 avulsed and replanted human teeth in relation to length of dry storage: A re-evaluation of a long-term clinical study.

BACKGROUND/AIMS: Ankylosis and subsequent tooth loss is a serious complication following tooth avulsion and replantation. The aim of the study was to evaluate the risk of ankylosis of human teeth replanted after various periods of dry storage prior to replantation. The material originates from a previously reported clinical study of 400 replanted teeth. METHODS: Initial treatment, clinical and radiological examination and follow-up were conducted according to a standardized protocol. PDL was not removed. Follow-up periods ranged from 0.2 to 20 years (mean 5.1 years). The risk of ankylosis was

Pulpal healing after luxation injuries and root fracture in the permanent dentition.

Pulpal healing patterns were studied in a clinical material of 637 luxated and 95 root-fractured permanent incisors followed routinely for up to 11 years. It was found that pulpal healing patterns could generally be divided into 3 groups according to the degree of injury sustained by the pulp: little, moderate or severe. Thus after luxation injuries, pulp survival could be without radiographic change (PS), with pulp canal obliteration (PCO) or nonhealing (pulp necrosis (PN]. After root fracture, similar healing patterns could be observed: healing by hard tissue union of fragments (HT), by conn

Revascularization in an immature necrotic permanent incisor after severe intrusive luxation injury: a case report.

BACKGROUND: Pulp necrosis as a result of trauma is common in almost all intruded teeth, and the ideal treatment method for intruded immature and necrotic teeth has not yet been determined. Therefore, the aim of this report was to present a case of pulp revascularisation in a severely intruded immature maxillary right incisor. CASE REPORT: After clinical and radiographic examination, the incisor was defined as necrotic. Revascularisation therapy was performed over multiple appointments. After 21 months of recall appointments, healing of the periapical area and apical closure were seen to be co

Potential for danger: case series of traumatic dental injuries associated with e-bike or e-scooter use.

This paper presents three adult cases referred to a Scottish Dental Hospital and Research School for management of traumatic dental injuries (TDIs), associated with either electric bike (e-bike) or electric scooter (e-scooter) use. E-bikes and e-scooters are widely available for purchase within the UK. This article discusses the factors relevant to e-bikes and scooters which may affect resulting injuries. These include the mechanism of trauma, alcohol intake and helmet-use. Patients sustaining such injuries are likely to present to the dental team in primary care in the first instance, often w

[Fixation of injured tooth].

In cases of an avulsed or luxated tooth, splinting of the injured tooth is recommended. Splint is defined as a fixation of the injured tooth to adjacent teeth in the dental arch. The preferred splint type is semi-rigid, which should be placed for 10 to 14 days and enable proper oral hygiene and will not cause any kind of impingement to the gingiva. It is effective in reducing the replacement resorption process, which affects the periodontal tissue surrounding roots exposed to dental trauma.

Management of Ankylotic Root Resorption Following Dental Trauma: A Short Review and Proposal of a Treatment Protocol.

Ankylotic root resorption is a serious complication following traumatic dental injuries. The aetiology of root resorption includes acute injury to the cementum and periodontal ligament, and subsequent biological processes that propagate the harm. The aim of the present paper is to present a structured treatment protocol for teeth that have experienced trauma and are at risk of developing ankylotic root resorption, followed by a decoronation protocol for situations in which ankylotic root resorption developed. This protocol provides a structured road map from the primary dental trauma, through

Trauma to the primary dentition.

The guiding principle in managing trauma to the primary dentition is potential sequelae to the permanent tooth. If the risk to the permanent tooth is great, the primary tooth should be removed. If esthetics are a concern to the parent, a number of appliances can be fabricated to temporarily manage this issue. It is important that parents realize that the desire to maintain optimum esthetics by maintaining a significantly injured primary tooth can, in many cases, cause more damage to the permanent tooth. Where avulsions have occurred, the old adage "when in doubt, leave it out" is still perhaps

Esthetic rehabilitation of avulsed-replanted anterior teeth: a case report.

Avulsion is a complex injury affecting the pulp, periodontal ligament and the alveolar bone. Avulsed permanent teeth can survive following replantation. However, post-traumatic external root resorption eventually resulting in loss of the traumatized tooth is a frequent finding. After replantation of the avulsed teeth, esthetic requirements can be needed. Treatment options include porcelain laminate veneers, metal-ceramic restorations and all-ceramic crowns as well as minimally invasive procedures such as direct resin composite bonding. This article describes the restoration of avulsed and repl

Displacement of deciduous tooth into hypopharynx due to endotracheal intubation.

Intubation may lead to several dental complications. Furthermore, a tooth damaged during intubation may be subsequently dislocated. In the present case, the upper primary incisor was avulsed during intubation and, unbeknownst to the anesthesiologist, displaced to the larynx. We report here on the findings and indicate appropriate treatment. Intubation for general anesthesia in children can result in tooth damage and/or dislocation of primary teeth with subsequent root resorption. Prevention is key, and thus it is critical to evaluate the patient's dental status before and after intubation. Fur

Esophageal perforation by a tooth in child abuse.

A unique case of child abuse in a six-year-old Hispanic girl is reported. In this case, a tooth avulsed and swallowed during an episode of physical abuse and/or sexual abuse, caused an esophageal perforation resulting in a retropharyngeal abscess and mediastinal abscess containing the tooth. Although traumatic esophageal injuries in child abuse have occurred, the presence of a tooth within the mediastinal abscess is an unique manifestation of child abuse.

Dental tissues involved in exarticulation, root resorption and factors influencing prognosis in relation to replanted teeth. A review.

Prognosis in relation to an exarticulated, replanted tooth is poor. Late complications are frequent and often lead to tooth loss. However, the root of a replanted, avulsed tooth may be slowly resorbed preserving alveolar width and postpones replacement of the tooth to a more convenient time. During healing following transplantation or replantation of teeth, various dental and oral structures are involved. Late complications relate to various factors. Tissues and factors involved are reviewed. Knowledge of this subject derives mostly from results of transplantation studies in animals.

Dental injuries in paediatric mandibular fracture patients.

PURPOSE: Dental injuries (DIs) are associated with facial fractures, particularly mandibular fractures. As paediatric mandibular fractures have special features, we sought to clarify the occurrence and types of DIs among this patient group. We assessed how age, injury type, and fracture location affects the occurrence of DIs and thereby defined which patients are most susceptible. METHODS: This retrospective study included patients < 18 years with a recent mandibular fracture. Predictor variables were gender, age group, mechanism of injury, type of mandibular fracture, and other associated fa

Clinical practice: dental trauma.

Approximately 50% of children under the age of 15 are victims of various kinds of injuries in the orofacial region. Post-traumatic complications may occur, including crown discolouration, cervical root fracture, ankylosis, root resorption and tooth loss. The most severe complication after dental injury in primary dentition can affect the developing permanent tooth germ, and various consequences may be seen several years later when the permanent tooth erupts. In the permanent dentition, the most severe dental injury affects the surrounding alveolar bone structure and will lead to loss of the to
