Force distribution is more important than its intensity!

A common question about root resorption is raised in orthodontic practice: What is more important, the intensity of force or its distribution along the root, periodontal and alveolar structures? Diffuse distribution of forces applied to periodontal tissues during tooth movement tends not to promote neither extensive areas of cell matrix hyalinization nor significant death of cementoblasts that lead to root resorption. However, focal distribution or concentration of forces within a restricted area--as it occurs in tipping movements, even with forces of lower intensity--tend to induce extensive

Crown-root reattachment of a severe subgingival tooth fracture: a 15-month periodontal evaluation.

Traumatic tooth fractures represent an intriguing challenge for clinicians, particularly when they involve the anterior region of the maxilla. In such situations, esthetic, psychosocial, functional, and therapeutic problems may have a negative effect on the patient's quality of life. In addition, fractures that involve the biologic width of the periodontium are even more difficult to treat. An interdisciplinary approach based on crown lengthening with or without orthodontic extrusion and the definitive prosthetic rehabilitation represent the standard modality of treatment for these conditions

Method to evaluate inflammatory root resorption by using cone beam computed tomography.

INTRODUCTION: The aim of this study was to evaluate a method to measure inflammatory root resorption (IRR) by using cone beam computed tomography (CBCT) scans. METHODS: IRR sites were classified according to root third and root surface, and IRR extension was measured on the axial, transverse, and tangent views of 3-dimensional CBCT scans by using the Planimp software. A 5-point (0-4) scoring system was used to measure the largest extension of root resorption. A total of 48 periapical radiographs and CBCT scans originally taken from 40 patients were evaluated. The kappa coefficient was used to

Challenges in differential diagnosis and treatment of cervical root resorption vs. root caries.

External cervical root resorption may be occasionally mistaken for root caries and vice versa. Radiographical and clinical differential diagnoses of cervical root resorption and root caries are essential for correct treatment planning and a successful treatment outcome. This article reviews the contemporary literature and summarises the prevailing professional consensus pertaining to external cervical root resorption. Clinical diagnostics which help distinguish cervical root resorption from root caries are outlined and treatment approaches of external cervical root resorption are discussed.

Radiation therapy for head and neck cancer leads to gingival recession associated with dental caries.

OBJECTIVE: The aim of this study was to examine effects of radiation therapy (RT) for head and neck cancer (HNC) on periodontal disease and relationships to caries. STUDY DESIGN: A multicenter prospective observational cohort study (OraRad) was conducted in patients undergoing RT for HNC. Assessments were conducted by calibrated examiners at the pre-RT (baseline) visit (n = 533), the 12-month visit (n = 414), and the 24-month visit (n = 365). RESULTS: The average whole mouth mean (standard error (SE)) distance from the cementoenamel junction to the gingival margin (CEJ-GM) decreased signific

External cervical resorption: a three-dimensional classification.

This article describes a novel three-dimensional classification for external cervical resorption (ECR). The European Society of Endodontology and American Association of Endodontists & American Academy of Oral & Maxillofacial Radiology position statements advise that Cone beam computed tomography should be considered for the assessment and/or management of root resorption if it appears to be clinically amenable to treatment following clinical and conventional radiographic examination. The new classification takes into account the ECR lesion height (1: at CEJ level or coronal to the bone crest

Full-mouth treatment of gingival recessions and noncarious cervical lesions with coronally advanced flap and xenogeneic collagen matrix:
a 2-year case report.

BACKGROUND: In clinical practice it is common to observe adjacent multiple gingival recessions (MGRs) associated with noncarious cervical lesions (NCCLs). The aim of this 2-year case report was to describe the full-mouth treatment of a patient affected by MGRs and NCCLs, with a combined restorative-surgical approach using a coronally advanced flap (CAF) and a xenogeneic collagen matrix (CM). METHOD: Before surgery, a composite restoration filled the deepest portion of the NCCL defects and was finished at the level of the maximum root coverage (MRC). The surgical technique adopted for the root

Efficacy of combined periodontal-restorative treatment versus periodontal treatment alone for gingival recession associated with non-carious cervical lesions: a systematic review and meta-analysis.

OBJECTIVE: Gingival recession (GR) is frequently associated with non-carious cervical lesions (NCCLs), necessitating a combined periodontal and restorative approach for optimal clinical management. This systematic review and meta-analysis compared integrated periodontal-restorative interventions with isolated periodontal treatment for managing GRs linked to NCCLs. METHODS: A comprehensive search of electronic databases, including PubMed, Cochrane Library, SciELO, Scopus, Web of Science, Embase, OpenAIRE, ProQuest, and Google Scholar, was performed up to December 2025 to identify randomized cl

Is External Cervical Resorption an Established Manifestation of Systemic Sclerosis? A Case Report.

Scleroderma, or systemic sclerosis, is a multisystem autoimmune disorder characterized by hardening and fibrosis of the skin. To date, only a small number of case reports have established a relationship between scleroderma and external cervical resorption (ECR). The aim of this case report is to document the case of a patient with multiple external cervical resorption lesions, who was referred to our unit. A 54-year-old female patient, with a 10-year history of systemic sclerosis diagnosed by her rheumatologist, was referred to our unit regarding extensive ECR. A total of 14 maxillary and mand

The diagnosis and management of invasive cervical resorption.

UNLABELLED: A 23-year-old female patient presented complaining of sensitivity relating to LR4. She had good oral health and no history of trauma. Cone beam computed tomography, however, revealed advanced invasive cervical resorption (ICR) in LR4, as well as an incidental finding of ICR in LR6. CLINICAL RELEVANCE: CBCT proved essential in the diagnosis and treatment planning of this unusual case. Owing to the destructive nature of ICR, it is imperative to remove the entire lesion to avoid any chance of recurrence.

Invasive cervical root resorption: treatment rationale with an interdisciplinary approach.

Invasive cervical resorption (ICR) is a significant and often aggressive pathologic process that, unfortunately, might lead to tooth loss. The presence of such a lesion in the cervical area is always a clinical challenge. This article presents an ICR case in which successful treatment was achieved by combining 4 disciplines: endodontics, orthodontics, periodontics, and prosthetics. Forced eruption combined with fiberotomy was used in this case for pulling the root rapidly from within the alveolar socket, thus exposing sound and healthy tooth material beyond the affected zone suitable for crown

Comparative Clinical Evaluation of Semilunar Coronally Positioned Flap Alone and Semilunar Coronally Positioned Flap in Conjunction with Free Gingival Graft for Root Coverage.

Gingival recession accounts for apical migration of the gingival margin, resulting in exposure of the cementoenamel junction and root surface, with exposure of the root surface linked to deteriorated esthetic appearance and increased dentinal hypersensitivity. Various surgical techniques have been used to correct labial gingival recession defects. The present study evaluated and compared the results of semilunar coronally positioned flap (SCPF) alone and in conjunction with free gingival graft (FGG) for the treatment of Miller Class I and II gingival recession defects in maxillary anterior tee

[External cervical root resorption].

External cervical root resorption begins at the root surface as result of odontoclastic activity in the cervical area and is progressive in character. The pulp is only affected at a later stage of the process. The aetiology and pathogenesis of external cervical root resorption is not fully understood. Possible predisposing factors are, among others, orthodontic treatment, trauma, internal bleaching, damaged cemento-enamel junction, bruxism and hypoxia. A 'pink spot' often is the first clinical sign and a two-dimensional radiograph usually presents an amorphous radiolucency with undefined borde

A classification system for loss of papillary height.

A classification system for loss of papillary height is proposed. It uses readily identifiable anatomical landmarks for reference, and sorts the degree of loss into 3 classes. The 3 broad categories allow for a quick descriptive assessment. In addition to the basic classification, it is suggested that additional and incremental description may be included to further define the defects.

A case report of gingival enlargement associated with invasive cervical resorption.

Invasive cervical resorption (ICR) is a rare external dental resorption with unknown etiology; it progresses asymptomatically in the cervical area of the permanent teeth. Lesions are mostly misdiagnosed as internal resorption or caries, which leads to erroneous treatments. This case report presents the clinical and radiological diagnosis, as well as the results of treatment and 3-year follow-up in a 50-year-old female patient with gingival enlargement associated with ICR in tooth No. 25. Granulation tissue was removed by accessing the cervical resorption area through a flap operation. Followin

Multiple enamel pearls in two siblings detected by volumetric computed tomography.

A rare case of multiple enamel pearl formation is presented involving the maxillary molars in two siblings incidentally recognized during volumetric CT examination. Although the pathogenesis of ectopic enamel formation is not known, possible mechanisms to account for this phenomenon are discussed in the context of current knowledge regarding root genesis. The radiographic presentation of enamel pearls and its clinical significance is also discussed. The observation of multiple enamel pearls in two siblings raises the possibility of a hereditary association in the formation of enamel pearls.

Examining the prevalence and characteristics of abfractionlike cervical lesions in a population of U.S. veterans.

BACKGROUND: Abfraction is believed to be caused by biomechanical loading forces. It may be due to flexure and ultimate fatigue of tooth tissues that occur away from the point of occlusal loading. Other possible causes of cervical lesions include toothbrush abrasion and erosion. The purpose of this study was to investigate the characteristics and prevalence of abfraction-like lesions in a population of U.S. veterans. METHODS: The authors evaluated 103 teeth with noncarious cervical lesions in 32 subjects and characterized them based on the surface on which the lesion was located, history of to
