STUDY DESIGN: Decision analysis from prior randomized controlled trial (RCT) data.
OBJECTIVE: To describe the importance of developing baseline utility indices while identifying effective treatment options for cervical spine disease.
SUMMARY OF BACKGROUND DATA: Cervical total disc replacement (CTDR) was developed to treat cervical spondylosis while preserving motion. Although anterior cervical discectomy and fusion (ACDF) has been the standard of care, a recent RCT suggested similar outcomes for 2-level disease. The quality-of-life benefit afforded by both CTDR and ACDF has never been fully

PURPOSE: Cervical total disc replacement (TDR) aims to maintain normal cervical kinematics after surgery. This study investigates the relation between shifted location of centre of rotation (COR) and subsequent surgical outcomes after multilevel cervical TDR (MCTDR) and identifies radiological parameter that corresponded to this change of COR after MCTDR.
METHODS: The study included a consecutive 24 patients treated with MCTDR following the diagnosis of multilevel cervical disc herniation or stenosis. Numeric Rating Scale (NRS), range of motion (ROM) at both C2-7 segment and TDR implanted lev

The proposed advantages of total disc replacement (TDR) over fusion in the lumbar spine are the preservation of motion and the avoidance of adjacent-level disease. One of the complications inherent in TDR is the possibility of vertebral body fracture due to trauma or a malpositioned implant. The resulting dilemma is that posterior decompression of the displaced bone fragment could then have a destabilizing effect and possibly require fusion, thus obviating the benefit of an arthroplasty procedure. In this study, the authors describe the technical considerations and feasibility of the treatment

STUDY DESIGN: Radiographic evaluation of lumbar total disc replacement (TDR).
OBJECTIVES: To assess radiographically segmental angulation and mobility after lumbar TDR, to determine the rate of posterior component impingement, and to investigate the influence of implantation level and mono- versus bi-segmental implantations.
SUMMARY OF BACKGROUND DATA: Polyethylene (PE)-wear can lead to inferior outcome after lumbar TDR due to aseptic loosening. One contributing factor might be increased segmental lordosis with component impingement.
METHODS: Fifty-six consecutive patients with 66 ProDisc-L

BACKGROUND: According to published meta-analyses, cervical total disc replacement (CTDR) seems to be superior to anterior cervical decompression and fusion (ACDF) in most clinical parameters. Despite short-term clinical success of CTDR, there are concerns regarding long-term durability of these prostheses.
METHODS: This prospective study involved 382 patients who received standalone CTDR or a hybrid procedure (ACDF/CTDR). A retrospective comparison between different CTDR devices was conducted regarding patient-reported outcome measures (PROMs), failure scenarios, and revision surgeries. The M

Introduction The traditional treatment for patients with radiculopathy and myelopathy caused by degenerative disc disease was anterior cervical discectomy and fusion (ACDF). However, a documented complication of ACDF is adjacent segment degeneration (ASD). An alternative that was developed was total disc replacement (TDR). The aim of this study was to determine and compare the short- and medium-to-long-term outcomes after a TDR or ACDF. Methods A retrospective review of 154 patients who had single and two-level ACDFs and 90 TDRs performed by a single surgeon between 2011 and 2017 was conducted

BACKGROUND CONTEXT: Total lumbar disc replacement (TDR) intends to avoid fusion-related negative side effects by means of motion preservation. Despite their widespread use, the adequate quality and quantity of motion, as well as the correlation between radiographic data with the patient's clinical symptomatology, remains to be established. Long-term data are lacking in particular.
PURPOSE: This study aimed to perform a clinical and radiographic long-term investigation following TDR with special emphasis on motion preservation assessment and to establish any potential correlation with patient-

OBJECTIVE The purpose of this study was to evaluate the 7-year cost-effectiveness of cervical total disc replacement (CTDR) versus anterior cervical discectomy and fusion (ACDF) for the treatment of patients with single-level symptomatic degenerative disc disease. A change in the spending trajectory for spine care is to be achieved, in part, through the selection of interventions that have been proven effective yet cost less than other options. This analysis complements and builds upon findings from other cost-effectiveness evaluations of CTDR through the use of long-term, patient-level data f

STUDY DESIGN: Prospective, randomized trial.
PURPOSE: The treatment of degenerative disc disease (DDD) with two-level fusion has been associated with a reasonable rate of complications. The aim of the present study was to compare (Hybrid) stand-alone anterior lumbar interbody fusion (ALIF) at L5/S1 with total disc replacement at L4/5 (TDR) as an alternative surgical strategy to (Fusion) 2-level circumferential fusion employing transforaminal lumbar interbody fusion (TLIF) with transpedicular stabilization at L4-S1.

BACKGROUND CONTEXT: Although several publications in the last decade have proved equality in safety and efficacy of the total disc replacement (TDR) to the anterior fusion procedure in cervical spine, the claim that TDR may reduce the incidence of adjacent segment disease (ASD) has not been corroborated by clinical evidence.
PURPOSE: We attempt to predict the true incidence of symptomatic ASD after TDR surgery in the cervical spine at one or two levels at a median follow-up period of 4 years.

BACKGROUND: The biomechanical behavior of total disc replacement (TDR) and anterior cervical discectomy and fusion (ACDF) incomplex multiplanar motion is incompletely understood. The purpose of this study was to determine whether ACDF or TDR significantly affects in vitro kinematics through a range of complex, multiplanar motions.
METHODS: Seven human cervical spines from C4-7 were used for this study. Intact cervical motion segments with and without implanted TDR and ACDF were tested by use of unconstrained pure bending moment testing fixtures in 7 mechanical modes: axial rotation (AR); flex

BACKGROUND: Difficulties in performing randomized controlled trials (RCTs) to evaluate new treatment options are increasing. Higher costs and patient unwillingness are the main obstacles. A spinal surgery register has been in use in Sweden for 11 years. Our aim was to determine whether this register can provide the same information as an RCT and whether register data compare favorably with RCT data, making RCTs unnecessary. If not the case, was patient selection or follow-up frequency the cause of any differences?
MATERIALS AND METHODS: We compared baseline data and outcome, retrieved from ou

Background/Objectives: The optimal surgical approach for patients with lumbar degenerative disc disease (DDD) remains a subject of ongoing debate, particularly when comparing anterior lumbar interbody fusion (ALIF) and total disc replacement (TDR). The primary objective of this study was to compare short-term outcomes between these two surgical approaches in terms of clinical efficacy, patient satisfaction, and safety profile. The secondary objective was to evaluate patient-specific variables associated with postoperative improvement in order to identify potential factors that may guide a pers

INTRODUCTION: Military aviators experience earlier spine degeneration and neck pain than civilians. Concerns exist about the performance of cervical total disc replacement (CTDR) in tactical military flight, including during aircrew ejection. Aviators in ejection-seat aircraft may obtain a medial waiver and return to flight after anterior cervical discectomy and fusion (ACDF); however, ACDF limits index/surgical level motion, increases motion at adjacent segments, accelerates adjacent spinal degeneration, and increases the risk for future cervical spine surgery. CTDR reduces the adverse effect
