[Electronic documentation of injuries of the hand with a semantic network: effective and efficient methods for the documentation of clinical and administrative processes].

BACKGROUND: An efficient medical documentation is mandatory for a trauma-oriented department in the DRG environment. Besides the continuously increasing clinical/administrative demands, the additional documentation for quality assurance, clinical studies, and research requires additional efforts. Standard solutions are only partially effective. Especially in hand surgery there is a high demand for sophisticated clinical documentation, represented by a wide variety of classifications in diagnosis and therapy. The standard documentation tools lack accuracy. The development of a software tool tha

Anglo-American vs. Franco-German emergency medical services system.

It has been stated that the Franco-German Emergency Medical Services System (FGS) has considerable drawbacks compared to the Anglo-American Emergency Medical Services System (AAS): 1. The key differences between the AAS and the FGS are that in the AAS, the patients is brought to the doctor, while in the FGS, the doctor is brought to the patient. 2. In the FGS, patients with urgent conditions usually are evaluated and treated by general practitioners in their offices or at the patient's home; initially, very few approach an emergency department. 3. Emergency patients with life-threatening traum

Performance evaluation for screening laboratories of the Asia-Pacific region.

The Centers for Disease Control and Prevention (CDC) has a long history of involvement in quality assurance (QA) activities for support of newborn screening laboratories. Since 1978, CDC's Newborn Screening Quality Assurance Program (NSQAP), has distributed dried-blood spot (DBS) materials for external QA and has maintained related projects to serve newborn screening laboratories. The first DBS materials were distributed for congenital hypothyroidism screening in 1978 and by 2001, NSQAP had expanded to over 30 disorders and performance monitoring for all filter paper production lots from appro

Responsibilities and activities of the Advisory Board on Radiation and Worker Health.

As noted in the introductory editorial by Moeller and Toohey, the Energy Employees Occupational Illness Compensation Program Act (EEOICPA) was passed by the U.S. Congress in October 2000 and became effective on 31 July 2001. The Act provides a lump-sum compensation of $150,000 to workers who contracted certain diseases due to exposures to beryllium, silica, or radiation while working for the U.S. Department of Energy (DOE), its contractors, or subcontractors, in the nuclear weapons industry. The Secretaries of DOE, the U.S. Department of Health and Human Services (HHS), and the U.S. Department

Transferring patients with a urinary catheter.

Transferring patients with indwelling urinary catheters from secondary to primary care can be problematic and the process requires careful planning. Both the patient and the healthcare professional taking over the plan of care need details on the reason for catheterisation and plans for ongoing care. Failure to plan transfers, including the provision of appropriate equipment for use at home, can lead to inappropriate care and, on occasion, results in referral back to hospital. Coral Seymour outlines new guidelines to assist nurses in making a discharge plan for these patients.

Preventing pressure ulcers in Connecticut hospitals by using the plan-do-study-act model of quality improvement.

BACKGROUND: Seventeen hospitals and the Peer Review Organization of Connecticut (Qualidigm) attempted to increase early identification of high-risk patients and utilization of pressure ulcer preventive measures. METHODS: A multihospital retrospective cohort study with medical record abstraction was used to obtain a total of 1,955 (baseline) and 891 (follow-up) patients aged 65 years and older discharged after treatment for pneumonia, cerebrovascular disease, or congestive heart failure with a length of stay > or = five days. During a nine-month period, the hospitals conducted four plan-do-stu

Design of a surveillance system of antibiotic use and bacterial resistance in German intensive care units (SARI).

BACKGROUND: Data on antibiotic consumption and bacterial resistance are important for benchmarking, ensuring quality of antibiotic treatment and helping to understand the relationship between the use of antibiotics and the emergence of resistance. METHODS: The SARI project is an ecological study that has established laboratory-based surveillance in German intensive care units (ICU). Resistance rates of 13 sentinel pathogens are reported and certain alert organisms are sent for genotyping and retesting of antimicrobial resistance. RESULTS: The project, initiated in February 2000, now includes

Elimination of bleeding associated with urinary catheterization in neonates.

Three cases of hematuria associated with urinary catheterization in infant males in this NICU over a year prompted an investigation by the health care team. The team tracked catheterizations in the unit, conducted a national survey, and did a literature review. Based on their findings, the team revised the unit policy and procedure and evaluated new products. After changes were implemented, the incidence of hematuria--originally at 20.7 percent--dropped to 5.5 percent during the first evaluation period and to zero during a second follow-up period.

Improving management of atrial fibrillation and anticoagulation in a community hospital.

BACKGROUND: Clinical trials have established the safety and efficacy of warfarin anticoagulation for stroke prevention in patients with atrial fibrillation. Other studies have documented patterns of underutilization and suboptimal warfarin therapy; physician underuse of warfarin may reflect the demands associated with monitoring the drug's effects. BASELINE STUDY: At Carney Hospital, a 230-bed acute care community teaching hospital in Boston, a retrospective chart review indicated that between July 1, 1995, and June 30, 1996, of 465 patients admitted with atrial fibrillation, 209 (45%) patient

Improvement in rheumatic fever and rheumatic heart disease management and prevention using a health centre-based continuous quality improvement approach.

BACKGROUND: Rheumatic heart disease (RHD) remains a major health concern for Aboriginal Australians. A key component of RHD control is prevention of recurrent acute rheumatic fever (ARF) using long-term secondary prophylaxis with intramuscular benzathine penicillin (BPG). This is the most important and cost-effective step in RHD control. However, there are significant challenges to effective implementation of secondary prophylaxis programs. This project aimed to increase understanding and improve quality of RHD care through development and implementation of a continuous quality improvement (CQ

Engaging the third estate: the Transplant Growth and Management Collaborative.

The Organ Donation and Transplantation Collaboratives that occurred within the United States from 2004 to 2008 helped contribute to a significant increase in organ donors and transplants across the country. Centers were needed to accommodate and maintain this increase in capacity to perform successful transplantations for candidates on the waiting list. The Transplant Growth and Management Collaborative was created to help fulfill this new performance level expectation. In 2007 the US Department of Health and Human Services, Health Resources and Services Administration published a best-practic

Maryland's first inpatient chest pain short stay unit as an alternative to emergency room-based observation unit.

INTRODUCTION: Approximately 8 million people in the United States visit emergency rooms (ERs) annually for chest pain but only about 1.2 million were ultimately diagnosed with acute myocardial infarction. Of concern, up to 4% to 5% of patients with acute myocardial infarction are those inappropriately discharged from the ER. ER-based observation units (EROU) were developed to enable safe, expedited, and effective management of these patients with negative initial workup. In the state of Maryland, the unique reimbursement system serves as a disincentive to operate EROU. The inpatient chest pain

Can district nurses and care home staff improve bowel care for older people using a clinical benchmarking tool?

A quasi-experimental study tested a clinical benchmarking tool (Essence of Care) to improve bowel-related care for older people living in six care homes. In the intervention care homes, district nurses and care home staff used the clinical benchmarking tool to discuss and plan how to improve bowel care for residents. In the control care homes, staff were provided with detailed information about the residents and continence services contact details. The intervention was acceptable to care home and district nursing staff, and possible to incorporate into existing working patterns. The study did
