Treatment of Trichiasis Following Modified Hughes Flap Surgery Using a Tarsoconjunctival Graft.

Reconstruction of full-thickness lower eyelid defects remains challenging. The modified Hughes flap is a reliable technique with favorable outcomes, although complications such as entropion, ectropion, and trichiasis may occur. Trichiasis is particularly problematic due to its potential to cause ocular irritation and corneal damage, yet its management after Hughes flap surgery is scarcely addressed in the literature. The authors report a 75-year-old woman who developed trichiasis of the right lower eyelid after modified Hughes flap reconstruction for basal cell carcinoma. Six weeks after flap

Rates and risk factors for unfavorable outcomes 6 weeks after trichiasis surgery.

PURPOSE: Several studies of trichiasis recurrence suggest an association between surgical factors and long-term recurrence, yet data on short-term risk factors are limited. This study was conducted to evaluate risk factors for early trichiasis recurrence and other unfavorable short-term outcomes. METHODS: Trichiasis patients presenting for surgery were evaluated for presence of active trachoma and signs of cicatricial outcomes of trachoma, including number of trichiatic lashes, epilation, and entropion. Surgical factors recorded included incision length, surgery duration, and the surgeon perf

Conjunctival expression of matrix metalloproteinase and proinflammatory cytokine genes after trichiasis surgery.

PURPOSE. Trachoma, the leading infectious cause of blindness, is a chronic inflammatory scarring condition. Blindness follows the development of trichiasis, which is treated surgically. Unfortunately, it frequently recurs, compromising the treatment. In this study, gene expression analysis was used to examine factors that may be involved in the inflammation and tissue remodeling after surgery. METHODS. Subjects were examined before and at 1 and 4 years after surgery. Conjunctival swab samples were collected for bacterial culture, Chlamydia trachomatis PCR, and RNA isolation at 1 year. Quantita

Uptake of trichiasis surgical services in Tanzania through two village-based approaches.

AIM: To determine the effectiveness of village-based strategies (using school teachers and village leaders) to increase the use of surgical services. METHODS: A cohort study was conducted in Tanzania using two village strategies (village leader and school teachers); trichiasis surgical uptake and the factors associated with uptake were measured after 1 year. RESULTS: The trichiasis surgical coverage at baseline was 16.9%; 200 patients who needed surgery were identified. One year later, we were able to re-examine and interview 163 of these patients. The surgical uptake among these patients wa

Outcomes of posterior lamellar tarsal rotation vs bilamellar tarsal rotation for trachomatous trichiasis.

BACKGROUND: Trachomatous trichiasis (TT) surgery is a key strategy for avoiding blindness and visual impairment from trachoma. We compared alternative WHO-endorsed TT surgery techniques, hypothesizing that in a "real world" study posterior lamellar tarsal rotation (PLTR) would be associated with less postoperative TT (PTT) than bilamellar tarsal rotation (BLTR). METHODS: In an ongoing TT control program in Jimma Zone, Ethiopia, TT surgeons used their preferred procedure (PLTR or BLTR) for upper eyelids with TT. Piloting a trachomatous trichiasis patient case-searching approach in two localities of Sudan.

BACKGROUND: Approximately 1.9 million people have become blind or visually impaired from trachoma, the leading cause of infectious blindness. Trachoma prevalence surveys conducted in Sudan have shown that thousands of Sudanese suffer from the advanced stages of the disease, trachomatous trichiasis (TT), and warrant sight-saving surgery. Sudan's National Trachoma Control Program (NTCP) provides free TT surgery; however, given that many TT patients live in remote areas with limited access to health services, identifying patients and providing eye care services has proved challenging. For this re

Podoconiosis, trachomatous trichiasis and cataract in northern Ethiopia: A comparative cross sectional study.

BACKGROUND: Rural populations in low-income countries commonly suffer from the co-morbidity of neglected tropical diseases (NTDs). Podoconiosis, trachomatous trichiasis (both NTDs) and cataract are common causes of morbidity among subsistence farmers in the highlands of northern Ethiopia. We explored whether podoconiosis was associated with cataract or trachomatous trichiasis (TT) among this population. METHODS: A comparative cross-sectional study was conducted in East Gojam region, Amhara, Ethiopia in May 2016. Data were collected from patients previously identified as having podoconiosis an

Optimizing Trichiasis Case Finding to Attain the Elimination of Trachoma as a Public Health Problem.

BACKGROUND: As national trachoma programmes increase efforts to reduce the burden of trachomatous trichiasis (TT), TT case finding and referral are critical public health programme components. Our research aimed to explore the most effective and efficient approaches to finding, referring, and managing TT cases. METHODS: This was a prospective descriptive study, utilizing both routine programme data and primary data collection. This study compared four different approaches to finding TT cases across three different local government areas (LGAs) in Kano State, Nigeria. Each of the study LGAs wa

Impact of oral azithromycin on recurrence of trachomatous trichiasis in Nepal over 1 year.

BACKGROUND: Recently, a significant association between Chlamydia trachomatis infection and postoperative trachomatous trichiasis (TT) recurrence was shown. The current study evaluated whether azithromycin treatment at the time of surgery could reduce recurrence. METHODS: As part of Nepal's national trachoma control programme, patients received azithromycin (53 patients) or placebo (56 patients) at surgery. Conjunctivae were graded for trachoma and swabbed to detect chlamydiae preoperatively and postoperatively up to 12 months. Logistic regression was performed for associations of treatment o

[Trachomatous entropion trichiasis at the ophthalmologic clinic of Dantec CHU (apropos of 199 cases)].

In Senegal as well as others developing countries trachoma is a real public health problem. Entropion trichiasis is its most common and its most blinding complication. The authors made a retrospective study on 137 records of patients who underwent 199 surgical cures on a two years period. Epidemiologically, the average age is 49 years with a significative higher percentage of woman. Clinically, 51.5% of the patients had already complications, mostly on the cornea. The Trabut technique was the most used and its results was good in 82.4% cases. The authors underlined the duality of entropion tri

The use of mucous membrane from the hard palate in the treatment of trichiasis and cicatricial entropion.

I have discussed a technique for grafting hard palate mucous membrane to the lid margin as a treatment for trichiasis. Because of the resistance of this material to shrinkage and recurrence of trichiasis, it forms an excellent barrier to recurrence of trichiasis.

Automated Detection and Assessment of Post-Operative Eyelid Outcome in Trachomatous Trichiasis Surgery.

Trachomatous trichiasis (TT) is an ocular condition in which the eyelid turns inward causing the eyelashes to scratch the eye, leading to blindness and affecting millions of people worldwide. The standard treatment for TT is surgery, where an incision is made to rotate the eyelid margin outward, repositioning the eyelashes to their original position. However, outcomes after surgery are often suboptimal, with a high risk of post-operative trichiasis (PTT). Studies have shown that the appearance of the immediate post-operative eyelid strongly correlates with the success of the procedure after si

Epilation for minor trachomatous trichiasis: four-year results of a randomised controlled trial.

BACKGROUND: Trachomatous trichiasis (TT) needs to be managed to reduce the risk of vision loss. The long-term impact of epilation (a common traditional practice of repeated plucking of lashes touching the eye) in preventing visual impairment and corneal opacity from TT is unknown. We conducted a randomized controlled trial of epilation versus surgery for the management of minor TT (fewer than six lashes touching the eye) in Ethiopia. Here we report the four-year outcome and the effect on vision and corneal opacity. METHODOLOGY/ PRINCIPAL FINDINGS: 1300 individuals with minor TT were recruited

Pathway-focused arrays reveal increased matrix metalloproteinase-7 (matrilysin) transcription in trachomatous trichiasis.

PURPOSE: Several genes that are associated with protection from or susceptibility to trachomatous trichiasis (TT) have been identified through genetic association studies. Yet there have been few studies in which gene expression profiles were assessed in TT cases and disease-free controls. The purpose was to identify genes that are differentially expressed in the upper tarsal conjunctiva of subjects with TT. METHOD: Pathway-focused gene arrays were used to screen conjunctival RNA expression of 226 gene transcripts of interest. The screening was followed by validation of differentially express
