Migraine: where and how does the pain originate?

Migraine is a complex neurological disease with a genetic background. Headache is the most prominent and clinically important symptom of migraine but its origin is still enigmatic. Numerous clinical, histochemical, electrophysiological, molecular and genetical approaches form a puzzle of findings that slowly takes shape. The generation of primary headaches like migraine pain seems to be the consequence of multiple pathophysiological changes in meningeal tissues, the trigeminal ganglion, trigeminal brainstem nuclei and descending inhibitory systems, based on specific characteristics of the trig

Distribution of CGRP and TRPV2 in Human Paranasal Sinuses.

Immunohistochemistry for protein gene product 9.5 (PGP 9.5), calcitonin gene-related peptide (CGRP) and the transient receptor potential cation channel subfamily V member 2 (TRPV2) was performed on human paranasal sinuses. It was found that in the paranasal sinuses, mucous membranes contain PGP 9.5-immunoreactive (PGP 9.5-IR) nerve fibers. Such nerve fibers terminated around large blood vessels as fine varicosities. Isolated PGP 9.5-IR nerve fibers were scattered beneath the epithelium. Glandular tissues were also innervated by PGP 9.5-IR nerve fibers. These fibers were numerous in the maxilla

Facial pain as an initial manifestation of intramedullary cervical spinal cord tumor: A case report and literature review.

BACKGROUND: Facial pain resembling trigeminal neuralgia is not a common clinical feature of cervical spinal cord tumor. Depending on nature of the facial pain, differential diagnosis tends to include neurovascular conflict, multiple sclerosis, cerebellopontine angle tumors, herpes zoster, facial injuries, and other conditions involving trigeminal nerve, ganglion, and root. Here, we present a unique case of pain in trigeminal distribution due to an intramedullary tumor in the upper cervical spinal cord. CASE DESCRIPTION: A 27-year-old male was admitted with complaints of intense facial pain on

Central nervous system distribution of the transcription factor Phox2b in the adult rat.

Phox2b is required for development of the peripheral autonomic nervous system and a subset of cranial nerves and lower brainstem nuclei. Phox2b mutations in man cause diffuse autonomic dysfunction and deficits in the automatic control of breathing. Here we study the distribution of Phox2b in the adult rat hindbrain to determine whether this protein is selectively expressed by neurons involved in respiratory and autonomic control. In the medulla oblongata, Phox2b-immunoreactive nuclei were present in the dorsal vagal complex, intermediate reticular nucleus, dorsomedial spinal trigeminal nucleus

Differential regulation of alpha- and beta-CGRP mRNAs within oculomotor, trochlear, abducens, and trigeminal motoneurons in response to axotomy.

Spinal and cranial motoneurons express alpha- and beta-calcitonin gene-related peptide (CGRP) mRNAs constitutively at variable ratios, and these two mRNAs are differentially regulated following axotomy in spinal, facial, and hypoglossal motoneurons. The purpose of this study was to investigate the change in CGRP mRNA expression following nerve injury in oculomotor, trochlear, abducens, and trigeminal motor nuclei in which beta-CGRP mRNA is predominantly expressed under normal conditions. Using male Sprague-Dawley rats, either the left eyeball and the orbital contents including the bulbar muscl

Topography and cytoarchitecture of the motor nuclei in the brainstem of salamanders.

The organization of the motor nuclei of cranial nerves V (including mesencephalic nucleus), VI, VII, IX, and X is described from HRP-stained material (whole mounts and sections) for 25 species representing five families of salamanders, and the general topology of the brainstem is considered. Location and organization of the motor nuclei, cytoarchitecture of each nucleus, and target organs for nuclei and subnuclei are described. The trigeminal nucleus is separated distinctly from the facial and abducens nuclei and consists of two subnuclei. The abducens nucleus consists of two distinct subnucle

Pontine influences on respiratory control in ectothermic and heterothermic vertebrates.

Respiratory rhythm generators appear both evolutionarily and developmentally as paired segmental rhythm generators in the reticular formation, associated with the motor nuclei of cranial nerves V, VII, IX, X, and XII. Those associated with the Vth and VIIth motor nuclei are "pontine" in origin and in fishes that employ a buccal suction/force pump for breathing the primary pair of respiratory rhythm generators are associated with the trigeminal nuclei. In amphibians, while the basic respiratory pump remains the same, the dominant site of respiratory rhythm generation has been assumed by the fac

Hypothalamic activation discriminates painful and non-painful initiation of the trigeminal autonomic reflex - an fMRI study.

BACKGROUND: The role of the trigeminal autonomic reflex in headache syndromes, such as cluster headache, is undisputed but sparsely investigated. The aim of the present study was therefore, to identify neural correlates that play a role in the initiation of the trigeminal autonomic reflex. We further aimed to discriminate between components of the reflex that are involved in nociceptive compared to non-nociceptive processing. METHODS: Kinetic Oscillation Stimulation (KOS) in the left nostril was applied in order to provoke autonomic symptoms (e.g. lacrimation) via the trigeminal autonomic ref

Integration of nociceptive activity from orofacial, cranial and cervical regions in the trigeminocervical nucleus: a scoping review with clinical implications.

A connection between orofacial, cranial and cervical regions has been documented through the trigeminocervical nucleus (TCN), but the predominant direction (orofacial/cranial-to-cervical or cervical-to-orofacial/cranio) of this connection still remains unclear. This scoping review was designed to determine the dominant direction of the connection between these two areas. Searches were conducted by combining these concepts: TCN, cervical and face/jaw/head region. Eighty-three studies were included. The most predominant direction was reported to be from the orofacial/cranial to the cervical regi

Early trigeminal nerve involvement in Listeria monocytogenes rhombencephalitis: case series and systematic review.

Listeria monocytogenes is associated with rhombencephalitis. However, the exact mechanisms of brainstem invasion remains poorly understood. Here, we demonstrate clinical and radiological data suggesting that Listeria may invade the brainstem via the trigeminal nerve. Three females (41, 64 and 70 years) with culture proven L. monocytogenes bacteremia and rhombencephalitis were investigated in the period of 2014-16. T2-weighted and contrast-enhanced T1-weighted MRI revealed a cerebellopontine abscess in all three patients, including the involvement of the trigeminal nerve root. In two patients,

The representation of experimental tooth pain from upper and lower jaws in the human trigeminal pathway.

FMRI was used to study the differences of cerebral processing of nociceptive input from the 2nd and the 3rd branches of the trigeminal nerve by electrical stimulation of the tooth pulps of the upper and lower canines. The focus of the study was an investigation of the different levels of the trigeminal system in brainstem, thalamus and in cortical regions which are known to be involved in pain processing. Increased blood oxygen level dependency (BOLD) signals were found ipsilaterally in the trigeminal ganglion and the spinal nucleus (SpV) of the trigeminal nerve. SpV-related activations showed

Presence of S-100 beta in cholinergic neurones of the rat hindbrain.

The double staining of S-100 beta and choline acetyltransferase (ChAT) revealed that S-100 beta immunoreactivity was localized in most, but not in all, cholinergic neurones in the somatomotor nuclei of the cranial nerves and in the ambiguus nucleus. S-100 beta was present in almost all cholinergic neurones in the brain stem reticular, red, vestibular (excluding medial), mesencephalic trigeminal and cerebellar nuclei. However, S-100 beta immunoreactivity was lacking in cholinergic neurones in the parabrachial complex, the dorsal motor nucleus of the vagal nerve and most sensory nuclei. No S-100

Neurinoma of the trigeminal root presenting as atypical trigeminal neuralgia: diagnostic values of orbicularis oculi reflex and magnetic resonance imaging. A case report.

A case of trigeminal root neurinoma presenting as atypical trigeminal neuralgia was reported. The orbicularis oculi reflex was absent on the affected side, but reappeared after operation. The demonstration of the tumor was much clearer on magnetic resonance imaging. The usefulness of these electrophysiological and radiological studies in differentiating varied entities of trigeminal neuralgia is stressed.

Effects of brainstem lesions on the masseter inhibitory reflex. Functional mechanisms of reflex pathways.

The masseter inhibitory reflex (MIR) was investigated in 16 patients with localized brainstem lesions involving the trigeminal system. The MIR consists of two phases of EMG silence (S1 and S2) evoked by stimulation of the mental nerve during maximal clenching of the teeth. The extent of the lesions was assessed by neurological examination, nuclear magnetic resonance imaging (MRI), and recording of trigeminal reflexes with known paths (jaw jerk and blink reflex). The MIR appeared to be abnormal in lesions involving the pontine tegmentum, from the pontomedullary junction to midpons, and some les

Mating call phonotaxis in female American toads: lesions of central auditory system.

Female American toads were tested for mating call phonotaxis (prostaglandin-induced) after brain lesions. The pretrigeminal nucleus, all of the telencephalon (except the preoptic area and immediately adjacent septal area), the dorsal thalamus (including all of the anterior nuclei and most of the central nuclei), the optic tectum, and the dorsal and medial parts of the torus were removed without interferring with the hormonal sensitization, acoustic triggering, or production of phonotaxis. These lesions removed most of the forebrain and much of the midbrain auditory areas. These data are compar

Astrocytes promote peripheral nerve injury-induced reactive synaptogenesis in the neonatal CNS.

Neonatal damage to the trigeminal nerve leads to "reactive synaptogenesis" in the brain stem sensory trigeminal nuclei. In vitro models of brain injury-induced synaptogenesis have implicated an important role for astrocytes. In this study we tested the role of astrocyte function in reactive synaptogenesis in the trigeminal principal nucleus (PrV) of neonatal rats following unilateral transection of the infraorbital (IO) branch of the trigeminal nerve. We used electrophysiological multiple input index analysis (MII) to estimate the number of central trigeminal afferent fibers that converge onto

Age-related changes in androgen receptor levels in cranial nerve nuclei of male rats.

Sensory and motor events are important for mating, and several cranial nerve nuclei mediating these events contain androgen receptors (AR). Since mating behavior declines with age, we tested whether AR binding is decreased in cranial nerve nuclei of old male rats. Cytosol and cell nuclear androgen receptor (AR) binding was assessed in the cochlear, hypoglossal and facial nuclei in young (4 months) and old (20 months) male Fischer 344 rats. For comparison with other neural and non-neural tissue, AR binding in combined hypothalamus, preoptic area and amygdala (HPA), and muscle tissue from tongue

Trigeminal neuropathic pain alters responses in CNS circuits to mechanical (brush) and thermal (cold and heat) stimuli.

Functional magnetic resonance imaging was used to study patients with chronic neuropathic pain involving the maxillary region (V2) of the trigeminal nerve in patients with spontaneous pain and evoked pain to brush (allodynia). Patients underwent two functional scans (2-3 months apart) with mechanical and thermal stimuli applied to the affected region of V2 and to the mirror site in the unaffected contralateral V2 region, as well as bilaterally to the mandibular (V3) division. Patients were stimulated with brush, noxious cold, and noxious heat. Significant changes were observed in regions withi

Trigeminal neuropathy: evaluation with MR imaging.

Neuropathy of the trigeminal nerve can involve its full course, from its nuclei in the brain stem to its peripheral branches. The nerve can be divided into four segments--brain stem, cistern, the Meckel cave and cavernous sinus, and extracranial--and consideration of the pathologic entities by these locations simplifies the differential diagnosis. Multiple sclerosis, infarct, and glioma are the most common abnormalities in the brain stem leading to trigeminal neuropathy. The most common cisternal cause is neurovascular compression, followed by acoustic and trigeminal schwannomas, meningiomas,
