BACKGROUND: In some trigger finger patients, tenderness is found in the dorsal proximal interphalangeal (PIP) joint. The etiology and prevalence of this condition are unclear. Furthermore, surgical outcomes for trigger fingers with coexisting dorsal PIP tenderness have not been reported. This study (1) determined the prevalence and risk factors for PIP joint tenderness in trigger fingers and (2) compared postoperative outcomes for trigger fingers with and without joint tenderness.
METHODS: This prospective cohort study was conducted between August 2018 and March 2020. We enrolled 190 patients

INTRODUCTION: Trigger finger is a common and functionally limiting disorder. Finger immobilization using an orthotic device is one of the conservative treatment options for treating this condition. The most common orthosis previously described for trigger finger is metacarpophalangeal joint immobilization. There are limited studies describing the effectiveness of proximal interphalangeal joint orthosis for treatment of trigger finger.
METHODS: This study was a single group pretest-posttest design. Adult patients with single digit idiopathic trigger finger were recruited and asked to wear a fu

BACKGROUND: Trigger finger is characterized by sometimes painful snapping or locking when flexing the finger. Although trigger finger is frequently seen in clinical practice, no standard treatment protocol has been established as "best practice."
OBJECTIVE: The aim of this study was to achieve consensus on a multidisciplinary treatment guideline for trigger finger.
DESIGN: A European Delphi consensus strategy was initiated. Systematic reviews reporting on the effectiveness of surgical and nonsurgical interventions were conducted and used as an evidence-based starting point for this study.

Introduction: Mucopolysaccharidosis is a rare and congenital autosomal recessive lysosomal storage disorder of glycosaminoglycans. An enzyme defect leads to cell, tissue and organ dysfunction. Carpal tunnel syndrome and trigger finger are the results of mucopolysaccharid deposition. Material and methods: We are treating 6 patients with mucopolysaccharide associated trigger fingers in an interdisciplinary setting with the department of pediatric hematology and oncology at Hannover Medical School, where each patient is examined inter alia for symptoms of trigger finger annually. Besides an inter

A locked metacarpophalangeal joint (LMCP) is a rare condition often confused with trigger finger. Its causes are numerous and typically divided into two broad categories: acquired LMCP in younger patients, and degenerative LMCP in older patients. This pathology usually affects only one MCP. Even though several external reduction techniques have been described, the main risk of non-surgical reduction treatment is recurrence. Thus, its management is most often surgical. We report the case of a 60-year-old woman with 3 simultaneous LMCP (3rd, 4th, and 5th rays of the left hand) due to degenerativ

BACKGROUND AND OBJECTIVE: Corticosteroid injection is an effective treatment for trigger digits but the pain during the injection is an ever-present side effect. Since the palmar skin has a high density of sensitive receptors, injecting through dorsal skin could be less painful. Our aim was to assess whether a dorsal technique through the dorsal web is safe for extra-sheath injection of trigger fingers and thumb.
MATERIAL AND METHOD: This is an anatomical study in sixteen cadaveric hands. An injection through the dorsal web was performed on each digit. After careful resection of the palmar sk

OBJECTIVE: The purpose of the present study was to identify length and landmark of proximal of A1 pulley in hand.
MATERIAL AND METHOD: An anatomical study of the length and landmark of A1 pulley in hand was performed in 510 fingers and thumbs from 51 preserved cadavers. There were 25 females and 26 males whose ages at the time of death ranged from 48 to 89 years.
RESULTS: The lengths of A1 pulley were 5.30 +/- 0.53 mm in average of thumbs and 6.32 +/- 0.17, 6.58 +/- 0.19, 6.32 +/- 0.20, and 5.30 +/- 0.49 for the index, middle, ring, and small finger respectively. The average of all fingers w

PURPOSE: Variations in the description of the flexor pulley system exist, particularly in whether the A1 and A2 pulleys represent discrete or confluent entities. This has potentially important clinical relevance at the time of A1 pulley release for symptomatic trigger finger, given the goal of adequate release without overrelease. The purpose of this study was to determine the relative prevalence of confluent A1 pulleys on a digit-by-digit basis employing 2.5× loupes alone, thereby simulating a clinical surgical environment.
METHODS: Cadaveric anatomic specimens underwent flexor pulley system

Persistent trigger finger distally to the A2 pulley after A1 pulley release is uncommon. We report five fingers in four patients successfully treated using a stepwise approach including A1 pulley release, ulnar flexor digitorum superficialis slip excision and flexor tenoplasty.Level of Evidence: Level IV.

BACKGROUND: Hand surgical procedures are common interventions in elective and emergency settings. The complex nature of the injuries and management by multiple specialities could be a potential source of medical errors and never events (NEs). Awareness of the common NEs could potentially help prevent their occurrence in the future.
OBJECTIVE: To analyse the NHS England database to identify the common NEs in hand surgery and present a simple, practical safety checklist for hand surgery.
METHODS: The NHS NEs database from 2012 to 2021 has been analysed to identify the common hand surgery-relat

INTRODUCTION: Recurrent trigger finger after surgery is one of the major adverse events. However, studies to identify factors associated with recurrence after open surgical release in adult trigger finger patients are still limited.
PURPOSE: To identify factors associated with recurrent trigger finger after open surgical release.
METHODS: This 12-year retrospective observational study included 723 patients with 841 trigger fingers who underwent open A1 pulley release. Patients were categorized into 2 groups: those with recurrent trigger finger after surgery and those without. Associations be

Platelet-rich plasma (PRP) has emerged as a viable therapeutic alternative for a number of clinical applications and has potential benefit for use in trigger finger. However, PRP use has not been previously reported for this condition. We report a 63-year-old woman sustaining a refractory trigger finger after a series of three PRP injections over a 2-week period. There was resolution of triggering, with no symptom recurrence at 3-month follow-up. On the basis of this report, we can assume that the treatment of trigger finger with PRP is a promising therapeutic option to be further explored wit

Trigger digit is a common condition which is largely idiopathic in etiology. Less frequently, it has been described secondary to anatomical variations or space occupying lesions. We describe a patient who developed a recurrent trigger finger after a surgical release of the first annular pulley. Intraoperatively, the trigger was noted to be caused by herniation of the flexor digitorum profundus (FDP) tendon into a noose formed by the two slips of the flexor digitorum superficialis tendon (FDS) and the proximal edge of the second annular pulley. Level of Evidence: Level V (Therapeutic).

Developmental trigger thumb, although uncommon, can be easily identifiable in the pediatric outpatient visit. Patients often present with their thumb locked in flexion and a firm nodule at the base of the thumb. The thumb is usually passively correctable and nonpainful. It is important to examine the opposite thumb as bilateral trigger thumbs occur at a rate of 25% to 30%. Nonsurgical options have been proposed in the past including watchful waiting, extension exercises, splinting, and steroid injections with mixed results. Surgical intervention is indicated when there is painful triggering or

OBJECTIVE: No studies have reported the use of ultrasound for the evaluation of trigger finger after steroid injection. We evaluated the clinical features and ultrasound appearance of trigger finger before and after steroid injection under ultrasound guidance.
MATERIALS AND METHODS: Thirty-eight digits with triggering were included. A single steroid injection into the tendon sheath was administered. Ultrasound findings and clinical symptoms, including pain, triggering, and the Quinnell score, were analyzed before injection and at 1 and 3 weeks after injection.
RESULTS: Ultrasound indicated t

Tendinopathies involving the hand and wrist are common. Many are diagnosed easily, and in many cases, the management is straightforward, provided the pathology and principles are understood. Common conditions involving the tendons of the hand and wrist include trigger finger, tenosynovitis of the first through sixth dorsal extensor compartments, and flexor carpi radialis tendonitis. Management strategies include nonsurgical treatments, such as splinting, injection, or therapy, and surgical techniques such as tendon release.

We present two cases of trigger finger caused by partial laceration of a flexor tendon. Both patients had preceding skin injury and required operative treatment with resection of the lacerated portion of the tendon and incision of the A1 pulley. We describe keys to the diagnosis of this type of lesion.

The recognized treatment of persistent congenital trigger thumb (CTT) is surgical release of the flexor pollicis longus (FPL) tendon by transection of the first annular (A1) pulley at the thumb metacarpal head. Twenty-seven children with persistent CTT were operated between 2000 and 2004. The average age at surgery was 34 months. In 16 patients, the CTT appeared unilaterally and was the sole anomaly in the hand. The others suffered from either multiple hand abnormalities or another syndrome. In only 3 of the 16 thumbs (ie, 19%) division of just the A1 pulley at the metacarpal (MC) head was suf
