Staged Repair of Truncus Arteriosus Associated with Complete Atrioventricular Septal Defect.

We report a case of successful repair of truncus arteriosus (TA) associated with complete atrioventricular septal defect (c-AVSD) using a staged approach. TA associated with c-AVSD is a very rare congenital cardiac anomaly. No report of successful staged repair in South Korea has yet been published. We performed bilateral pulmonary artery banding when the patient was 33 days old, and total correction using an extracardiac conduit was performed at the age of 18 months. The patient recovered uneventfully and is doing well.

Prenatal diagnosis of truncus arteriosus using multiplanar display in 4D ultrasonography.

Prenatal diagnosis of truncus arteriosus with two-dimensional sonography requires expertise in fetal echocardiography. Indeed, truncus arteriosus shares with tetralogy of Fallot and pulmonary atresia with a ventricular septal defect (VSD) the sonographic finding of a single arterial trunk overriding a VSD. The diagnosis of truncus arteriosus can be confirmed when either the main pulmonary artery or its branches are visualized arising from the truncus itself. This requires sequential examination of multiple scanning planes and a process of mental reconstruction of their spatial relationships. T

Successful staged Fontan completion for truncus arteriosus with hypoplastic left ventricle.

We report a case of truncus arteriosus type II with a large outlet ventricular septal defect and a hypoplastic left ventricle. The first-stage palliation was the removal of both branch pulmonary arteries from the ascending aorta, patch augmentation of the pulmonary artery confluence, and modified Blalock-Taussig shunt at age 1 month. The second stage palliation was a bidirectional Glenn at 6 months. The final stage was an extracardiac Fontan at age 3 years. Eight years later, the patient is doing well, with an unobstructed Fontan pathway and mild-to-moderate truncal valve insufficiency.

Eisenmenger syndrome in adults: ventricular septal defect, truncus arteriosus, univentricular heart.

OBJECTIVES: Morbidity and mortality patterns were characterized in adults with the Eisenmenger syndrome when two ventricles with a ventricular septal defect (VSD) joined two great arteries or one great artery, or when one ventricle joined two great arteries. BACKGROUND: Although afterload in these disorders differs, clinical differences have not been defined. METHODS: Seventy-seven patients were studied. Group A comprised 47 patients with VSD, aged 23 to 69 years (mean 39.5+/-10.2), follow-up 5 to 18 years (mean 7.2+/-4.9); group B, 14 patients with truncus arteriosus, aged 27 to 50 years (m

Persistent truncus arteriosus in monozygotic twins: case report and literature review.

We report on a pair of monozygotic twins with persistent truncus arteriosus. They had no evident clinical signs of DiGeorge syndrome. Pathologic examination of the placenta and DNA analysis in chromosomes 7, 8, and 15 was consistent with monozygosity. Fluorescence in situ hybridization test was negative for chromosome 22q11 microdeletion. Family history revealed a female cousin with tetralogy of Fallot. The isolated presence of this conotruncal abnormality in monozygotic twins is extremely rare. The genetic considerations are discussed.

Late development of functional subvalvar stenosis after repair of truncus arteriosus.

The late development of subvalvar stenosis following truncus repair is described. It may occur in patients with an overriding truncal valve and a relatively small ventricular septal defect (VSD) closed with an inappropriately sized patch. Management consisted in enlarging the VSD and closing it with a large, tailored patch. Consideration should be given to enlarging the VSD at the time of original repair if the truncal valve is overriding.

Fate of small homograft conduits after early repair of truncus arteriosus.

Neonatal repair of truncus arteriosus is being performed in a number of centers, often with the use of small homograft conduits. The fate of the homograft and the risk of replacement were the subjects of this study. Between January 1987 and October 1991, 43 infants aged less than 3 months (range, 3 to 90 days) underwent primary repair of truncus arteriosus including implantation of a valved homograft conduit (diameter, 7 to 12 mm). Twenty-nine had follow-up of more than 6 months (range, 6 to 65 months; mean, 21.9 months). After a mean period of 31 months (range, 8 to 65 months), 7 patients sho

Two-dimensional echocardiographic prospective diagnosis of common truncus arteriosus in infants.

Two-dimensional echocardiographic prospective diagnosis of truncus arteriosus was made in 7 infants. Two infants had truncus arteriosus type I, 3 patients had truncus arteriosus type II, 1 infant had truncal valve stenosis with an interrupted aortic arch, and 1 had type IV truncus arteriosus with pulmonary hypertension. Multiple imaging views were utilized to confirm the diagnosis. The parasternal long-axis view demonstrated the great vessel-ventricular septal override and the origin of the pulmonary arteries from the posterior aspect of the ascending truncus. The suprasternal notch image faci

Truncus arteriosus and previous pulmonary arterial banding: clinical and hemodynamic assessment.

Twenty-seven patients with truncus arteriosus and previous pulmonary arterial banding were evaluated 1 1/2 to 14 years (mean 7 years) after banding. Ages at the time of cardiac catheterization ranged from 3 to 18 years (mean 9 years). Current symptoms were severe in five patients and were related to truncal valve incompetence or decreased pulmonary blood flow (or both) rather than to age, duration of palliation or band location. Twenty-one of 22 patients with two pulmonary arteries were considered to be in a hemodynamically operable state at the time of study. The condition of three of five pa

Myocardial calcifications in a neonate with surgically corrected truncus arteriosus: Case report.

BackgroundMyocardial calcifications in neonates with truncus arteriosus are rare and poorly understood. These calcifications, resulting from dystrophic processes in injured tissue, may have significant clinical implications. This case highlights a neonate with truncus arteriosus and interrupted aortic arch who developed extensive myocardial calcifications following surgical correction and extracorporeal membrane oxygenation (ECMO).MethodsA neonate with truncus arteriosus underwent surgical repair at 11 days of life, requiring ECMO support. Clinical data, imaging findings, and therapeutic inter

Multicenter Analysis of Truncal Valve Management and Outcomes in Children with Truncus Arteriosus.

Truncal valve management in patients with truncus arteriosus is a clinical challenge, and indications for truncal valve intervention have not been defined. We sought to evaluate truncal valve dysfunction and primary valve intervention in patients with truncus arteriosus and determine risk factors for later truncal valve intervention. We conducted a retrospective cohort study of children who underwent truncus arteriosus repair at 15 centers between 2009 and 2016. Multivariable competing risk analysis was performed to determine risk factors for later truncal valve intervention. We reviewed 252 p

The Initial Glimpse at Long-term Outcomes Following the Repair of Truncus Arteriosus.

The article by Naimo and colleagues (outcomes of truncus arteriosus repair in children: 35 years of experience from a single center) provides the initial glimpse at long-term outcomes following truncus arteriosus repair. Although survival has improved over the past decade, the morbidity resulting from subsequent re-operations now comprises a significant area for improvement in the ensuing decade.

Survival and reintervention after neonatal repair of truncus arteriosus with valved conduit.

OBJECTIVE: Neonatal primary repair has progressively become the treatment of choice for truncus arteriosus with encouraging survival. However, use of valved conduits to reconstruct the right ventricular outflow tract (RVOT) inevitably induces reintervention. This study estimates survival and rate of catheter-interventional and surgical reinterventions. METHODS: Thirty-five consecutive neonates who underwent truncus repair with 27 homografts and 8 Contegras from 1987 to 2007 were studied. Interrupted aortic arch (IAA) was associated in nine patients. Actuarial survival and freedom from reinter

[Total correction of truncus arteriosus with severe truncal valve insufficiency in neonate].

The prognosis of infants with truncus arteriosus associated with severe truncal valve insufficiency is quite poor. Total correction was successfully performed in a neonate with such a complicated anomaly. The patient was 21 days old female with anuria due to severe congestive heart failure preoperatively in spite of medical treatment. She underwent Rastelli operation and pulmonary artery was reconstructed using autologous pericardial 3 valved conduit. Truncal valve was 4 cusps with the malformed nodular margins and one cusp had cleft. This cleft was closed suturing the cleft cusp and adjacent

Pathogenesis of persistent truncus arteriosus induced by nimustine hydrochloride in chick embryos.

Nimustine hydrochloride (ACNU) is a nitrosourea derivative anticancer agent which has been shown to cause persistent truncus arteriosus in chick embryos. The objective of this study was to confirm the teratogenic effects of ACNU on the cardiovascular system of chick embryos and to determine whether ACNU induces persistent truncus arteriosus by interfering with neural crest cells. Various doses of ACNU ranging from 10 to 200 micrograms were injected under the chorioallantoic membrane of chick embryos on the third day of incubation. Saline solution was used as the control. After 10 to 11 days of

Cardiac malformations in relatives of children with truncus arteriosus or interruption of the aortic arch.

Familial recurrence risks for congenital cardiac malformations have been estimated at 1 to 4%, but little is known of recurrence risks for uncommon congenital cardiac malformations. This study determined the incidence of congenital cardiac malformations in relatives of patients with either truncus arteriosus (TA) or interruption of the aortic arch (IAA) seen between 1955 and 1985. Prior to this study, only 2 cases of IAA in siblings had been reported and the recurrence risk of TA had been estimated at 1.2%. In the families of 36 index cases of IAA, there were 98 siblings, 2 with congenital car
