A high-resolution nested PCR assay for genotyping Orientia tsutsugamushi based on a defined 700 bp segment of the TSA56 gene.

OBJECTIVES: To define a reproducibly specified TSA56 fragment that preserves genotype assignment relative to the full-length gene and to establish a standardized sequence-based genotyping workflow using nested PCR and phylogenetic assignment. METHODS: We screened 355 complete TSA56 sequences across the gene and designed nested primers to generate an ∼800-bp amplicon covering the selected segment. Analytical performance was assessed across 17 genotypes, clinical applicability was evaluated in 216 acute-phase blood samples, and interlaboratory agreement was assessed across three laboratories.

A rare case of scrub typhus complicated by severe hemolytic anemia, septic shock, and multi-organ dysfunction in a patient with thalassemia.

Scrub typhus, a re-emerging zoonosis caused by Orientia tsutsugamushi, can present with severe, life-threatening complications, including multi-organ dysfunction and hemolytic anemia. This report details the case of a 40-year-old female with underlying thalassemia who presented with symptoms initially suggestive of pyelonephritis but rapidly progressed to septic shock, multi-organ dysfunction syndrome, and a severe hemolytic crisis. Concurrent Escherichia coli and Enterococcus gallinarum pyelonephritis further complicated the clinical picture. Diagnosis was confirmed by the identification of a

Comparison of Clinical Outcomes and Fever Clearance Time with Azithromycin versus Doxycycline in Paediatrics Scrub Typhus.

BACKGROUND: Scrub Typhus is an important cause of acute febrile illness in children in endemic regions, often presenting with nonspecific clinical features that delay diagnosis. Early initiation of appropriate antibiotic therapy is critical in reducing morbidity and preventing complications. Doxycycline has traditionally been the drug of choice; however, Azithromycin is increasingly used in Paediatric populations due to safety considerations, especially in younger children. Comparative evidence evaluating clinical outcomes and fever clearance time between these two agents in children remains l

Epidemiological Trends and Patterns of Scrub Typhus in India: A Retrospective Analysis Based on Integrated Disease Surveillance Programme (IDSP) Data (2009-2018).

BACKGROUND: Scrub typhus, a vector-borne febrile illness caused by Orientia tsutsugamushi, has shown significant global resurgence, affecting an estimated one billion people at risk and causing up to one million cases annually. In India, it is a major cause of acute undifferentiated fever but remains under-recognized and a clear research gap regarding long-term national trends, and the heterogeneity of case fatality across states exists. This study aimed to quantify the national burden, geographical distribution, temporal trends, and mortality associated with scrub typhus using Integrated Dise

Predictors of Delayed Defervescence in Scrub Typhus: A Retrospective Cohort Analysis.

Scrub typhus, caused by Orientia tsutsugamushi, is a mite-borne febrile illness endemic to the Asia-Pacific region. Although most patients respond promptly to appropriate antibiotic therapy, a subset exhibits delayed defervescence, the clinical significance and predictors of which remain insufficiently understood. In the present study, the authors aimed to identify clinical, laboratory, and radiological factors associated with delayed defervescence in patients with scrub typhus. In this retrospective cohort study, scrub typhus patients who were hospitalized at Chosun University Hospital, Gwang

Eschar in scrub typhus: clinical signature, immunopathogenesis, and diagnostic implications.

SUMMARYScrub typhus, caused by Orientia tsutsugamushi, remains a major cause of acute undifferentiated febrile illness with an expanding global footprint. Early diagnosis is frequently delayed because of nonspecific systemic manifestations and geographic heterogeneity. The eschar, traditionally regarded as a characteristic but unreliable clinical sign, is better understood as a dynamic and biologically active lesion rather than a static cutaneous finding. Accumulating clinical, epidemiologic, and pathologic evidence demonstrates that eschar morphology and detectability vary widely with timing,

Maternal and Fetal Outcomes Associated With Scrub Typhus in Pregnancy: A Systematic Review.

Scrub typhus, caused by Orientia tsutsugamushi and transmitted by larval trombiculid mites, is endemic across much of the Asia-Pacific region. It is frequently missed or misdiagnosed in pregnant individuals, where its consequences may be severe. Despite the severe maternal and fetal risks associated with scrub typhus, a comprehensive synthesis of the infection's total burden during pregnancy remains a critical gap in the literature. We searched five bibliographic databases (PubMed, CINAHL, MEDLINE Ultimate, ClinicalKey, and Ovid Embase), four trial registries (World Health Organization Interna

Clinical Profile, Severity Markers, and Outcomes of Hospitalized Patients With Scrub Typhus in North India: A Prospective Observational Study.

Scrub typhus is an important cause of acute febrile illness in India, with often devastating complications in hospitalized patients. These complications contribute to morbidity and mortality if the condition is not diagnosed early and treated. A prospective observational study was conducted in the Department of General Medicine at a tertiary care teaching hospital in North India from June 2024 to May 2025 and included 35 patients with confirmed scrub typhus. Laboratory parameters, complications, and outcomes were analyzed. Data were analyzed statistically using IBM SPSS Statistics version 25.0

Forgotten but Not Gone: Pediatric Scrub Typhus in an Urbanizing Tropical Region.

Scrub typhus is an acute febrile illness caused by Orientia tsutsugamushi, transmitted to humans through the bite of infected larval trombiculid mites. Its nonspecific clinical presentation often leads to delayed diagnosis and treatment, increasing the risk of severe complications. As urbanization progresses and awareness declines, clinicians may overlook this diagnosis, particularly when characteristic findings such as an eschar are absent or have been resolved. We report the case of an 11-year-old boy from Vietnam who was admitted with recurrent fever persisting for two weeks, occasionally a

Detection of Scrub Typhus Antibodies by Serological Tests and Their Comparative Evaluation for Validation of Serological Testing.

BACKGROUND: Scrub typhus has emerged as a significant cause of acute febrile illness in India, with increased morbidity when diagnosis is delayed. Its nonspecific clinical presentation and limited availability of advanced diagnostic facilities pose challenges in timely identification. Although the indirect immunofluorescence assay is the reference standard, it is not widely accessible in resource-limited settings, necessitating the use of rapid and reliable diagnostic tests. OBJECTIVE: The objective of this study was to evaluate the diagnostic performance of a rapid immunochromatographic test

Rickettsial seropositivity in Lao PDR smallholder livestock farms: Implications for animal and human health.

Rickettsioses are emerging zoonotic diseases in Southeast Asia caused by vector-borne, intracellular Rickettsia bacteria that threaten public health, animal welfare, and food security. Despite their importance to rural livelihoods, the epidemiology of rickettsial exposure in livestock remains poorly understood. This study used abattoir-based serology to evaluate livestock as sentinels of environmental exposure to Rickettsia in Lao PDR and to identify spatial hotspots and risk factors for seropositivity. Abattoir-based serological data were generated from cattle, pigs, and water buffalo samples

Targeting surface cell antigen 2 increases sensitivity of Rickettsia typhi detection.

BACKGROUND: Murine typhus is a flea-borne disease caused by Rickettsia typhi that typically presents as an acute febrile illness. The diagnosis is often missed, leading to delays in appropriate treatment. A qPCR targeting a single gene (ompB) for R. typhi is widely used for diagnosis; however, it has low sensitivity for detecting bacterial DNA in patients' blood. We aimed to increase sensitivity of detection of R. typhi using qPCR by targeting a gene containing repetitive sequences, sca2 (surface cell antigen 2). METHODOLOGY: We compared diagnostic accuracy with the standard assay targeting s
