[The International Standards for Tuberculosis Care (ISTC): what is the importance for Japan?].

In 2005, the World Health Assembly resolved that all Member States should ensure that all persons with tuberculosis (TB) "have access to the universal standard of care based on proper diagnosis, treatment and reporting consistent with the DOTS strategy..." The purpose of the International Standards for Tuberculosis Care (ISTC) is to define the widely accepted level of care of persons either suspected of, or diagnosed with, TB by all health practitioners, especially those in the private sector, who often lack guidance and systematic evaluation of outcomes provided by government programs. Since

Implementing tuberculosis control guidelines in a hospital environment.

AIM: This review assessed the effectiveness and results of a programme implementing the 1992 Tuberculosis Control Guidelines to hospital staff at Green Lane and National Women's hospitals. METHODS: All staff tuberculosis surveillance and patient contact tracing contacts carried out by the occupational health unit and the resultant outcomes were assessed. RESULTS: A higher risk of Mantoux conversion does exist in hospital staff. These conversions occur in nonhigh risk occupational groups as well as the more recognized at risk groups. CONCLUSIONS: The 1992 guidelines proved impracticable to a

Positive skin and serologic test results of diagnostic assays for bovine tuberculosis and subsequent isolation of Mycobacterium interjectum in a pygmy hippopotamus (Hexaprotodon liberiensis).

A 20-yr-old male pygmy hippopotamus (Hexaprotodon liberiensis), weighing 250 kg, arrived at Zoological Society London Whipsnade Zoo (United Kingdom) from a captive collection in Portugal. A quarantine health check was performed including a comparative intradermal tuberculosis (IDTB) test. Assessment of the comparative IDTB test at 72 hr revealed a strong positive reaction at the bovine site. Serum was tested with a rapid immunochromatographic assay (TB STAT-PAK) and was positive for tuberculosis antibodies. The tuberculosis tests were repeated 6 wk later with the same positive test outcome. In

The crossroads for patients: idealised nostalgia vs scientific evidence of past health.

Patients sometimes recall a supposedly healthier past, imagined as a time when life was "natural" and free of disease. Such views, often amplified by anti-science movements, collapse when confronted with demographic and palaeopathological evidence. Pre-modern societies were marked by high infant and maternal mortality, recurrent epidemics, chronic malnutrition, and a life expectancy at birth rarely exceeding 35 years. Skeletal and mummified remains reveal tuberculosis, leprosy, syphilis, anaemia, malnutrition, trauma, and degenerative joint disease, underscoring the relentless burden of illnes

Recommendations Concerning the First-Line Treatment of Children with Tuberculosis.

This document describes the recommendations of a group of scientific societies concerning the first-line therapeutic approach to paediatric tuberculosis (TB). The treatment of pulmonary TB should be based on the existence of parenchymal involvement and the risk of antibiotic resistance. The treatment of extra-pulmonary TB is based on the regimens used for severe pulmonary TB. The administration of corticosteroids is recommended only in cases of miliary TB, tuberculous meningitis and tuberculous pericarditis. Vitamin B6 may be indicated in the case of isoniazid-treated TB in breastfeeding infan

[Guidelines for the diagnosis and treatment of latent tuberculosis infection and active tuberculosis in patients with inflammatory joint diseases proposed for treatment with tumour necrosis factor alpha antagonist drugs].

The Portuguese Society of Rheumatology (SPR) and the Portuguese Society of Pulmonology (SPP) have developed guidelines for the diagnosis and treatment of latent tuberculosis infection (LTBI) and active tuberculosis (AT) in patients with inflammatory joint diseases (IJD), namely rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis, treated with tumour necrosis factor alpha (TNF-a) antagonists. Due to the high risk of tuberculosis (TB) in patients with IJD, LTBI and AT screening should be performed as soon as possible, ideally at the moment of IJD diagnosis. Even if TB screening

Early and efficient detection of Mycobacterium tuberculosis in sputum by microscopic observation of broth cultures.

Early, efficient and inexpensive methods for the detection of pulmonary tuberculosis are urgently needed for effective patient management as well as to interrupt transmission. These methods to detect M. tuberculosis in a timely and affordable way are not yet widely available in resource-limited settings. In a developing-country setting, we prospectively evaluated two methods for culturing and detecting M. tuberculosis in sputum. Sputum samples were cultured in liquid assay (micro broth culture) in microplate wells and growth was detected by microscopic observation, or in Löwenstein-Jensen (LJ)

Impact of human immunodeficiency virus infection on the epidemiology, clinical features, management, and control of tuberculosis.

After years of decreasing prevalence and increasing hope that tuberculosis, like smallpox, could be eliminated, the disease has resurfaced as a major public health problem in the United States. Particularly ominous are the appearance of multiple-drug-resistant strains and their impact on patients and health care workers who are infected with the human immunodeficiency virus, among whom mortality rates reach 80% 2-3 months postdiagnosis. To respond effectively to this new threat, it is critical that we reorient our thinking about tuberculosis and redirect health care resources to programs for t

[Social diseases in Poland during the in-between wars period].

In the paper the methods of prevention and control of social diseases (tuberculosis, trachoma, increased infantile and maternal mortality, alcoholism) in Poland of interwar period were presented. A considerable prevalence of these diseases was connected with social and economic conditions, in which broad classes of society had to live. The most effective methods in reducing the incidence of the diseases were prophylaxis and organization of free health care.

[The importance of Robert Koch's discovery for the development of bacteriology].

On March 24th, 1882 Robert Koch reported to the Physiological Society of Berlin the discovery of microbes causing tuberculosis. Simultaneously he expressed his views on the pathogenesis of this nation-wide disease. With his work he contributed decisively to the development of a theory on infectious process. The results of his investigations on cholera and typhoid fever greatly stimulated the development of the epidemiology of contagious diseases.

[Amendment of tuberculosis prevention law and prospect of tuberculosis control program].

Tuberculosis Control Law, which provides a legal basis for national tuberculosis control, was amended in 2004 and entered into force on April 1, 2005. As it is more than half a century since its initial enactment, the law has been drastically amended based on some of the relatively new important ideas such as up-to-date scientific evidence, recent epidemiological conditions of tuberculosis, decentralization and respect for human rights. Japan has once seen a time when considerable part of producing population were affected with tuberculosis which caused severe infliction on the whole Japanese

[Use of molecular techniques in the diagnosis of pulmonary and extrapulmonary tuberculosis].

Tuberculosis, is a life-threatening infectious disease and one of the leading causes of mortality worldwide. It is a multisystemic disease, and involvement of lungs and pleura is seen in majority of patients. The global control of tuberculosis is impeded by the relatively low sensitive conventional diagnostic assays, especially for drug resistant strains. The current gold standard for tuberculosis diagnosis is the combination of culture and clinical diagnosis. Since conventional diagnostic assays require long time, are labor intensive and insufficient for species-level identification, new solu

[Experience of rapid drug desensitization therapy in the treatment of mycobacterial disease].

BACKGROUND: Drugs for tuberculosis and non-tuberculosis mycobacterial diseases are limited. In particular, no new drugs for non-tuberculosis mycobacterial disease have been developed in recent years. Antimycobacterial drugs have many adverse reactions, for which drug desensitization therapy has been used. PURPOSE: Rapid drug desensitization (RDD) therapy, including antituberculosis drugs and clarithromycin, has been implemented in many regions in Europe and the United States. We investigated the validity of RDD therapy in Japan. PATIENTS AND METHOD: We report our experience with RDD therapy

[Primary prevention and treatment of latent tuberculosis infection with isoniazid: efficacy of a control program, 1997-2002].

OBJECTIVE: To analyze the efficacy of a specific program for the study and follow up of tuberculosis contacts. To study factors related to low adherence to treatment and to the development of liver toxicity caused by isoniazid. PATIENTS AND METHODS: Between December 1996 and December 2002, we found 458 contacts of 79 cases of pulmonary tuberculosis in patients uninfected by human immunodeficiency virus. The contacts were screened for tuberculosis infection and chemoprophylaxis was prescribed according to the recommendations of the Spanish Society of Pulmonology and Thoracic Surgery (SEPAR).

A White Death Among the Ranks. Tuberculosis in the Austro-Hungarian Army, 1882-1914.

This article is devoted to the incidence of tuberculosis in the Austro-Hungarian Imperial-Royal Army between the years 1882 and 1914, with a special focus on the region of Cisleithania. The first part of the article discusses the organisation of the Austro-Hungarian armed forces, drawing attention to those features of the organisation that may prove informative when researching the health status of the wider population as a whole. The question of the extent of the threat of tuberculosis in Austria-Hungary is then addressed, which shall allow us to move on to the incidence of the disease in the

Lack of consensus in the diagnosis and treatment for ocular tuberculosis among uveitis specialists.

PURPOSE: To assess the approach of specialists to ocular tuberculosis (TB). METHODS: The American Uveitis Society (AUS) Listserv was surveyed using two clinical cases and general questions. RESULTS: Of 196 members, 87 responded (44.4%), of whom 64 were affiliated with practices in North America, while 23 were outside of North America. The survey provided normative data on how physicians evaluate patients with uveitis as well as opinions about ocular TB. Responses varied widely on such issues as (1) the pretest probability that a patient with granulomatous panuveitis had TB uveitis (range 1-7

[Clinical analysis of pulmonary tuberculosis found by mass screening medical examination].

Five hundred twenty-one patients with pulmonary tuberculosis were diagnosed in our three affiliated hospitals during past 10 years. Among them, 43 cases (22 men and 21 women; mean age 54.8 years) who were detected by active case-finding were clinically evaluated. Most of them were detected in an annual mass screening examination, but seven cases were found by contacts examination which was performed on subjects who were contacted with newly diagnosed patients with pulmonary tuberculosis. Fifteen of them were socially jobless on admission. Eighteen cases (41.9%) had underlying diseases, and gas

Latent tuberculosis testing through the ages: the search for a sleeping killer.

Tuberculosis has been present in the world's population for as long as there has been written language. It is a disease known to the ancient Egyptians, Greeks, Romans, and Hebrews, but its etiology eluded the world for thousands of years. Even after the germ theory was accepted and early scientists hypothesized a pathogen as the cause, the identity of the sleeping killer in society remained a mystery. That is until Robert Koch was able to grow and visualize Mycobacterium tuberculosis. Koch introduced his Old Tuberculin solution as a diagnostic therapy of tuberculosis (TB), with the intent to r
