Ulnar/cubital tunnel syndrome is the second most common compressive neuropathy of the upper limb. Permanent location of the ulnar nerve anterior to the medial epicondyle is extremely rare, with only five cases reported in the literature. Using ultrasound elastography and diffusion tensor imaging with fiber tractography, we diagnosed a case in which ulnar nerve entrapment was associated with anterior nerve location. Surgical release confirmed the diagnosis and the patient was symptom free 3 months after surgery.

BACKGROUND: Ulnar nerve entrapment at the elbow (UNE) is the second most prevalent entrapment neuropathy after carpal tunnel syndrome. The objective of this study was to evaluate the expert opinion of different surgical disciplines regarding the need for electrodiagnostic or ultrasound confirmation of UNE and, if so, which test was preferred for confirmation.
METHODS: A questionnaire was sent to all neurosurgeons and plastic or hand surgeons in the Netherlands to evaluate the current practice in planning surgical treatment of UNE.
RESULTS: The response rate was 36.4 % (134 out of 368). 94 %

The median, radial, and ulnar nerves of the upper limbs may be affected by various peripheral neuropathies, each of which may be categorized according to its cause, as either an entrapment or a nonentrapment neuropathy. Entrapment neuropathies, also referred to as nerve compression syndromes, include the supracondylar process syndrome, pronator syndrome, anterior interosseous nerve syndrome, carpal tunnel syndrome, posterior interosseous nerve syndrome, cubital tunnel syndrome, and Guyon canal syndrome. Nonentrapment neuropathies include traumatic nerve injuries, infectious and inflammatory co

Cubital tunnel syndrome (CubTS) is one of the most common compression-traction neuropathy in the upper extremity. The gold standard is simple in situ decompression with revision of potential compression sites through skin incision as small as rationally possible. Properly conducted conservative treatment is more effective in CubTS as opposed to carpal tunnel syndrome. At the same time, optimal management of CubTS remains controversial. Nevertheless, there is a subclass of patients with symptoms of CubTS that have ulnar nerve instability (UNI) with subluxation of the nerve over the medial epico

Clinical investigation precisely determines the localization of ulnar nerve entrapment in Guyon's canal. The cause of compression can be due to internal (tumor, distal fibrous arch or proximal ligament arcade, nerve or muscle anomalies) or external influences (ulnar neuropathy of cyclists, thrombosis of ulnar artery by hypothenar hammer syndrome, fractures of pisiform or hook of hamate). Treatment involves splitting the roof of Guyon's canal, the ligaments or muscle anomalies, and excision of a tumor.

BACKGROUND: Ulnar nerve entrapment at the elbow is the second most common nerve entrapment syndrome at the upper limp, after carpal tunnel syndrome. Many surgeons feel that ulnar nerve instability contra-indicates endoscopic nerve release. Published studies, however, found no evidence that pre-operative or intra-operative ulnar nerve instability adversely affected clinical outcomes. The objective of this prospective study was to define the indications and describe the outcomes of endoscopic ulnar nerve release at the elbow.
HYPOTHESIS: Endoscopic ulnar nerve release at the elbow is a valid op

Repetitive vibratory trauma in cyclists is one of the most common causes of ulnar nerve compression at the wrist. This study describes five cases of clinically suspected compression syndrome of the deep terminal branch of the ulnar nerve (DBUN) defined by high-resolution ultrasound (HRUS) with a 18-MHz linear transducer. Reliable HRUS visualization of the DBUN was feasible in all patients. The affected nerves had focally swollen and hypo-echoic changed segments. HRUS is a good imaging tool for the evaluation of a suspicious DBUN neuropathy if no electrodiagnostic testing is available or in pat

Among neural lesions of the arms, the most common one is the medianus lesion in the carpal tunnel, followed by the ulnar nerve lesion in the elbow, while the ulnar nerve compression in the Guyon tunnel is found rarely. Besides the history and the clinical picture, the electromyoneurographic analyses make an early and accurate diagnosis possible. The characteristics of the evoked muscle potential and distal latencies in the diagnosis of the carpal tunnel syndrome are of utmost importance. In diagnosing the cubital syndrome, the multisegmental determination of the motor conduction velocity of th

This report is the first account of Guyon's syndrome after the bilateral use of forearm crutches. Crutch palsy is usually neuropraxic in nature and associated with clinical and electrophysiologic recovery of nerve function, especially if patients are instructed to not bear excessive weight on the wrist. The present case history follows this pattern. In establishing the diagnosis of a focal compression neuropathy, a combination of clinical assessment and neurophysiologic studies are typically used. This report describes the additional application of ultrasound imaging to verify the diagnosis an

Cubital tunnel syndrome is the second most common nerve compression syndrome in peripheral nerve compression disease. Although potential ulnar nerve entrapment can occur at multiple points along its course, such as the arcade of struthers, the medial intermuscular septum, the medial epicondyle, the cubital tunnel, and the deep flexor pronator aponeurosis, the most common site of entrapment is the cubital tunnel. However, cubital tunnel syndrome could also be caused by the occupying masses along the course of ulnar nerve, such as intraneural or extraneural ganglia. The cubital tunnel syndrome c

INTRODUCTION AND IMPORTANCE: The ulnar's nerve compression at the Guyon's canal is not a frequent entity add to it that vascular lesions are rarely involved as a causative agent of this syndrome.
CASE PRESENTATION: We report a case of a young male patient admitted in our department for a Guyon's canal syndrome due to an aneurysm of the ulnar artery and underwent a surgical decompression. Post-operative course was uneventful and the patient was satisfied with the result.
CLINICAL DISCUSSION: Many etiologies are involved in the Guyon's canal syndrome and these etiologies can be arranged into g

An unusual case of pain and weakness in the hand and forearm due to a ganglion cyst of the ulnar nerve at the elbow is presented. The patient was managed initially as a case of cervical disc disease and cervical spondylosis and later as a case of carpal tunnel syndrome at an another institution. Cervical radiography and cervical magnetic resonance imaging scans were inconclusive. Neurosurgical referral revealed tenderness at the right cubital tunnel, weakness of the right hand and forearm muscles, and sensory deficit along the medial border of the forearm and the hand. The diagnosis of ulnar n

Parsonage-Turner syndrome (PTS) is a rare peripheral neuropathy characterized by the sudden onset of shoulder pain followed by muscle weakness and atrophy. This condition primarily affects the brachial plexus and may mimic or coexist with other compressive neuropathies. We present a case of a 39-year-old female with a prior diagnosis of PTS who developed progressive right-sided scapular and elbow pain, accompanied by numbness in the fourth and fifth digits of the right hand. Her symptoms had been ongoing for approximately three months. Physical examination revealed atrophy of the intrinsic mus

Peripheral nerve compression is a prevalent concern for primary care physicians and hand surgeons, with carpal tunnel syndrome (CTS) and median nerve compression at the wrist being some of the most commonly diagnosed conditions. However, for less common nerve entrapment syndromes, it is crucial for healthcare providers to recognize their symptoms and consider potential underlying issues, particularly those related to the brachial plexus. This case report highlights a 57-year-old male who presented with classic symptoms of left median and ulnar nerve compression in the setting of an enlarging l
