Successful ultrasonically guided percutaneous ethanol injection for secondary hyperparathyroidism.

Ultrasonically guided percutaneous ethanol injection to all of four enlarged parathyroid glands, which were confirmed by fine-needle aspiration biopsy, was successfully performed in a hemodialysis patient with secondary hyperparathyroidism. In addition to marked reduction of serum parathyroid hormone, the size of parathyroid glands was significantly decreased after the procedure. As an adverse effect, the patient experienced a transient self-limiting dysphonia due to right recurrent nerve palsy after the first injection. This procedure may be an effective alternative to surgical treatment or a

[Carotid thromboendarterectomy. The authors' personal experience].

Carotid endarterectomy is the most frequently employed surgical intervention in the treatment of strokes due to atherosclerosis obstruction of the blood flow in extracranial carotid district. The presence of cerebrovascular clinical symptoms is accepted indication for surgical treatment. For the patients without symptoms there is, however, not yet a complete agreement on the treatment modalities. The authors believe that, due to the low morbidity and mortality of carotid endarterectomy, a surgical approach can be used in patients in which is present a degree of stenosis > 75% and, moreover, in

Echinococcus granulosus infection: the challenge of surgical treatment.

BACKGROUND: Cystic echinococcosis (CE) is a worldwide zoonosis caused by larval stages of the cestode Echinococcus granulosus. Surgery, chemotherapy, and interventional procedures are the therapeutic options. Surgery can cure the patient if the parasite is removed entirely. However, the technical procedures are inconsistent and comprise partial liver resection or opening of the parasitic cyst and removal of the parasite. Laparotomy is the most common approach. In selected cases laparoscopic methods are successful. Retrospective studies outweigh prospective ones by far. However, proper manageme

Safety of ultrasonic bone curette in ear surgery by measuring skull bone vibrations.

HYPOTHESIS: Mastoidectomy using an ultrasonic bone curette (UBC) is as safe for the inner ear as a mastoidectomy using a high-speed drill. BACKGROUND: Transcanal endoscopic ear surgery (TEES) is a minimally invasive, secure, and functional technique, which has been performed using high-speed drills. We have started to use a UBC instead of the high-speed drill because a UBC allows for simultaneous bone cutting, irrigation, and aspiration. These features of the UBC make it an excellent candidate for use in one-handed TEES, but the safety of the UBC still needs to be confirmed. We thus measured

[Transcranial Doppler ultrasonic examination in vertebro-basilar circulatory pathology].

Vertebrobasilar ischemic disease cannot be adequately explored by echo-Doppler ultrasonic investigating procedures, since they do not permit the study of the intracranial arteries. The transcranial pulsed Doppler (2 MHZ) velocity detection method developed by R. Aaslid now offers such possibilities, although it has not yet been validated in this field. The authors relate their experience and methodological criteria (patient's position, deepness of access, flow direction, response to compression tests). They conducted 2 studies, one bearing on the detection of 14 cases of 75% stenosis of the su

Ultrasonic total uncinectomy: a novel technique for complete anterior decompression of cervical nerve roots.

BACKGROUND: In standard anterior cervical diskectomy, complete resection of the uncinate process is not commonly attempted because of the risk of injury to the vertebral artery. This may result in incomplete decompression of the nerve root when there is severe osseous foraminal stenosis. OBJECTIVE: To demonstrate the feasibility of total uncinectomy with an ultrasonic bone dissector in patients with concomitant central and foraminal stenosis requiring anterior decompression and fusion. METHODS: A case series of patients with cervical radiculopathy or myeloradiculopathy who underwent single-l

Laparoscopic surgery in a 0.5-t interventional magnetic resonance unit.

BACKGROUND: Intraoperative imaging using magnetic resonance imaging (MRI) is now possible in interventional MR (IMR) units. Magnetic resonance imaging has potential advantages over other methods used to guide surgery. These advantages include visualization of structures deep to the two-dimensional endoscopic image and clarification of surgical anatomy. This study investigates the feasibility of laparoscopic surgery with intraoperative imaging within an IMR unit. METHODS: The procedures were performed in a 0. 5-T General Electric IMR scanner. Surgical ergonomics and intraoperative imaging were

Mitigating saliva aerosol contamination in a dental school clinic.

BACKGROUND: Transmission of COVID-19 via salivary aerosol particles generated when using handpieces or ultrasonic scalers is a major concern during the COVID-19 pandemic. The aim of this study was to assess the spread of dental aerosols on patients and dental providers during aerosol-generating dental procedures. METHODS: This pilot study was conducted with one volunteer. A dental unit used at the dental school for general dental care was the site of the experiment. Before the study, three measurement meters (DustTrak 8534, PTrak 8525 and AeroTrak 9306) were used to measure the ambient distri

Ultrasonic aspiration in neuroendoscopy: first results with a new tool.

Neuroendoscopic techniques are often considered inapplicable to lesion resection because most lesions are too large for effective endoscopic resection in an appropriate time frame. To evaluate the potential of ultrasonic aspiration in neuroendoscopic procedures, the authors developed a new handpiece for endoscopic application. The instrument was subsequently tested in 10 cadaveric pig brains and applied in 5 clinical cases. In the pig brain, a precise and accurate aspiration of ventricular ependyma and brain parenchyma was obtained. Clinically, the device was applied in 3 patients with obstruc

Focused Ultrasound: An Emerging Therapeutic Modality for Neurologic Disease.

Therapeutic ultrasound is only beginning to be applied to neurologic conditions, but the potential of this modality for a wide spectrum of brain applications is high. Engineering advances now allow sound waves to be targeted through the skull to a brain region selected with real time magnetic resonance imaging and thermography, using a commercial array of focused emitters. High intensities of sonic energy can create a coagulation lesion similar to that of older radiofrequency stereotactic methods, but without opening the skull. This has led to the recent Food and Drug Administration approval o

[Ultrastructural changes in the retina after microinvasive 25G ultrasound vitrectomy (experimental study)].

UNLABELLED: Current trends in the development of vitrectomy systems, particularly vitreous cutters, focus on reducing instrument gauge while maintaining high cutting efficiency and vitreous fragmentation capacity. Pneumatic guillotine vitreous cutters have several design limitations that constrain further improvement of their functional capabilities. The use of ultrasonic energy for vitreous fragmentation represents an alternative direction in the development of vitrectomy. The emergence of microinvasive ultrasonic 25-gauge (25G) vitreous cutters creates the need for a comprehensive assessment

Utilization of ultrasonic aspirator for combined aortic and mitral valve decalcification: a case study.

Mitral and aortic annular calcification is an age-related degenerative process that can result in severe mitral and/or aortic stenosis and/or regurgitation. Annular calcification not only increases the surgical complexity but also increases the risk of complications. In this case report, we present the innovative use of the Sonopet ultrasonic surgical aspirator for aortic and mitral annular decalcification in a patient with hypertrophic obstructive cardiomyopathy, mild aortic stenosis and moderate mitral regurgitation in the presence of mitral annular calcification (MAC) and aorto-mitral curta
