Primary thrombosis in the upper extremity.

Deep vein thrombosis may occur in the axillary or subclavian veins in the upper extremities, sometimes in young patients without the typical risk factors for thrombus development. This unusual clinical situation usually follows strenuous activity and is often referred to as "effort" thrombosis. Since some cases have occurred with mild exertion and there is a lack of obvious predisposing vessel trauma, a more correct term might be "primary" thrombosis. Early recognition of this condition is necessary to prevent potentially fatal acute effects and minimize chronic sequelae.

Extravasation injuries in the upper extremity.

Fourteen patients with intravenous extravasations of chemotherapeutic medications, radiographic contrast agents, vasopressors, fluorescein, or albumin/potassium phosphate were evaluated with regard to function and cosmesis. The average follow-up period was 8.4 months (range, one-24 months). Five patients initially were managed conservatively and, after demarcation of the necrotic area, had surgical procedures for skin coverage. Four of these patients had poor functional and cosmetic results. The remaining patient had an excellent final result. Because of the generally poor results observed in

Noninvasive evaluation of the upper extremity.

Vascular problems of the arm and hand can be assessed by a number of noninvasive modalities that are chosen on the basis of the history and physical examination. For suspected upper-extremity ischemia, we begin with SLPs and velocity-waveform analysis. The former test will define the extent and approximate location of the disease process, and subjective assessment of the waveform will further determine the degree and location of occlusive disease. When digital ischemia is suspected, the Doppler examination combined with intermittent compression of the radial and ulnar arteries is valuable for

Diagnostic imaging of upper extremity trauma.

The radiologic evaluation of upper extremity trauma relies primarily on standard as well as optional radiographic projections. Certain injuries are more fully evaluated with fluoroscopy, arthrography, ultrasound, computed tomography, or magnetic resonance imaging. The imaging approach to upper extremity trauma is presented, with emphasis on subtle or commonly overlooked lesions. The indications for evaluation by fluoroscopy, arthrography, and advanced imaging modalities are discussed.

"Think nerve" in upper extremity reconstruction.

Upper extremity reconstruction may be greatly facilitated by "thinking nerve" in the initial evaluation. Frequently, the obvious soft tissue deformity or bone or joint pathology distracts the reconstructive surgeon from an underlying nerve problem. When pain is a portion of the presenting complaints, thinking nerve becomes more obvious. This article attempts to direct the reconstructive surgeon approaching the upper extremity to look for underlying nerve injury in the region of a damaged wrist capsule, neuromas of the cutaneous nerve, and sites of nerve entrapment.

External fixation of the upper extremity.

External fixation of forearm and arm fractures is an excellent treatment option, particularly in compound, comminuted fractures that result from high-energy trauma. Limb salvage and good function have been achieved in 75% of cases by applying the principles outlined. Problems with delayed bone union have been recognized and emphasize the importance of supplemental bone grafting. A number of external fixation systems are applicable to these upper limb fractures, and the authors have not found one system superior to the others. Open pin insertion is preferred, and pins (three proximal and three

Thermoregulatory control after upper extremity replantation.

Four patients with complete forearm amputations between the wrist and elbow were analyzed prospectively to determine the interrelationships of thermoregulation to pain, cold intolerance, arterial integrity, venous competence, motor nerve recovery, sensory nerve recovery, and time. Patients were examined at 6 weeks, 3 months, 6 months, 1 year, and 2 years after replantation. Vasomotor regulatory capacity was assessed by isolated cold-stress testing. Pain had diminished in all patients by 6 months to 1 year, regardless of other factors. Cold tolerance and normal thermoregulatory response, as ind

Varicose ulcer of the upper extremity.

Serious complications of local venous hypertension occur rarely in the upper extremity. These are most frequently the result of a dysfunctional arteriovenous fistula and may produce changes indistinguishable from those of chronic lower-extremity venous insufficiency. A combination of arterial and venous contrast imaging is essential for identifying major venous outflow obstruction and for planning appropriate surgical therapy.

Venous gangrene of the upper extremity.

Gangrene of the hand associated with acute upper extremity venous insufficiency has been seen in four limbs in three patients treated at Vanderbilt University Medical Center. All three patients had life-threatening illnesses associated with diminished tissue perfusion, hypercoagulability, and venous injury. One patient progressed to above-elbow amputation, but venous thrombectomy in one limb and thrombolytic therapy in two others were successful in preventing major tissue loss. All three patients eventually died from their underlying illness. Thirteen previously reported patients with "venous

Compartment syndrome in the upper extremity.

It has been the purpose of this article to assist us in our approach to compartment problems, for, although a full-blown compartment syndrome following forearm injury is statistically uncommon, the results of this ischemic process can be devastating. A compartment pressure of greater than 30 mmHg for eight hours or more can cause cell death by interference with the microcirculation. Clinical diagnosis is made on the basis of a number of modalities, especially pain and paresthesias. Peripheral pulses or capillary flow are not specific indicators of compartment well-being. Direct compartment pre

Upper extremity thrombosis: etiology and prognosis.

Upper extremity thrombosis is responsible for 1-2% of all cases of deep venous thrombophlebitis (DVT). Little has changed concerning the clinical presentation, diagnostic investigation, or treatment of the disorder over the last forty years. However, a changing pattern in the natural history of the disease seems to be emerging with the advent of more invasive procedures and prolonged medical care of critically-ill individuals. Case histories of sixteen (16) patients with seventeen (17) episodes of upper extremity thrombosis are reviewed. Fewer cases of "primary" thrombosis, and fewer acute or

Rollover injuries of the upper extremity.

Eleven patients were treated for rollover injuries of the upper extremities from 1971 to 1976 resulting from the rollover accidents of off-road vehicles in rough terrain. Injuries to the volar surfaces of the extremity were most common, but injury to the extensor surfaces seemed to be more severe. Soft-tissue injuries predominated as did injury to the nondominant hand. The majority of patients returned to their original jobs after sometimes prolonged recovery and rehabilitation. A redesign of the rollover cage with interior hand holds could reduce the frequency and severity of these novel inju

Upper extremity complications of axillofemoral grafts.

Four patients sustained five upper extremity complications after axillobifemoral grafting including brachial plexus injuries, axillary artery thrombosis, and arterial steal. To avoid these problems, the following precautions are recommended: (1) proper positioning of the patient to prevent hyperabduction of the shoulder; (2) adequate exposure of the axillary artery during dissection and gentle handling of the nerve trunks; (3) meticulous technic during construction of the proximal anastomosis and avoidance of undue tension on the graft; and (4) careful preoperative assessment of the axillary-s
