[In situ dissolution of uric acid obstructive calculi].

The dissolution of uric acid calculi can be obtained by per os or parenteral alkalinization, but this method cannot be used in case of obstructive calculi. With 3 patients, whose urinary tract was obstructed by a radio-transparent calculi at the pyelo-ureteral junction, the ureteral probe setting by endoscopic way permitted to realize the in situ alkalinization. After 5 to 11 days of treatment, the calculi have been completely dissolved. A patient also presented a slightly radio-opaque lithiasis, but it remained still. The in situ alkalinization is an effective and non invasive treatment in ca

Serum and urinary uric acid in primary hyperparathyroidism.

Serum and 24-h urinary uric acid were measured in 29 patients with primary hyperparathyroidism pre- and post-operatively. No significant difference was observed. A positive correlation was found between the ratios of urinary calcium/creatinine and urinary uric acid/creatinine pre-operatively but not after surgery. The serum uric acid level showed an increase from the first to the third day post-operatively and returned to normal within a week, but in patients undergoing other urological operations there was no post-operative increase. On a low calcium diet the level of serum uric acid was foun

Dissolution of uric acid calculi with systemic alkalization.

We treated 14 patients with uric acid calculi in the renal pelvis and calices (10) or ureter (4) with constant intravenous infusion of a one-sixth molar lactate solution. Dissolution was successful in all but 2 cases. The success of therapy is believed to be related to the sustained urinary alkalization obtained with intravenous infusion compared to the intermittent alkalization that occurs when oral agents are used.

Idiopathic uric acid lithiasis: epidemiologic and metabolic aspects.

Of 264 patients with urolithiasis those with pure uric acid or urate stones were compared to those with other types of calculi for differences in epidemiologic factors and uric acid, calcium and phosphate metabolism. Patients with uric acid stones were predominantly older men. These patients had comparatively lower incomes and spent less money on food but consumed more alcohol. The urinary pH was lower than in the other groups. The absence of abnormally elevated serum uric acid levels and elevated 24-hour urinary uric acid excretion suggests this variety of uric acid lithiasis to be idiopathic

Renal excretion of uric acid during prolonged fasting.

Serum and urine uric acid were evaluated during prolonged therapeutic fasting in 15 obese patients. With increasing ketonemia the serum uric acid rose from a control value of 5.9 +/- 0.4 to 12.5 +/- 1.0 mg/100 ml at 7 days and then decreased progressively to 7.7 +/- 1.3 mg/100 ml by 28 days despite sustained ketonemia. The uric acid clearance were 5.5 +/- 0.9, 1.8 +/- 0.2, and 4.4 +/- 1.5 ml/min at days 0, 7, and 25 of fasting. At the same times the creatinine clearances were 114 +/- 11, 80 +/- 6, and 64 +/- 6.3 ml/min. There was no evidence of a renal tubular abnormality as assessed by glycos

Association of the uric acid-to-high-density lipoprotein cholesterol ratio with uric acid stones: a single-center retrospective cohort study.

BACKGROUND: Uric acid stones (UAS) are a common metabolically associated subtype of urolithiasis. Their formation is strongly driven by acidic urine and frequently coexists with dysglycemia and other cardiometabolic abnormalities. The serum uric acid-to-high-density lipoprotein cholesterol ratio (UHR) is a readily available composite index reflecting uric acid burden and high-density lipoprotein (HDL)-related metabolic status. However, evidence linking UHR to composition-confirmed UAS in clinical practice remains limited. This study aimed to investigate the association between UHR and UAS. ME

Serum Uric Acid/Serum Creatinine Ratio and Cardiovascular Mortality in Diabetic Individuals-The Uric Acid Right for Heart Health (URRAH) Project.

Several studies have detected a direct association between serum uric acid (SUA) and cardiovascular (CV) risk. In consideration that SUA largely depends on kidney function, some studies explored the role of the serum creatinine (sCr)-normalized SUA (SUA/sCr) ratio in different settings. Previously, the URRAH (URic acid Right for heArt Health) Study has identified a cut-off value of this index to predict CV mortality at 5.35 Units. Therefore, given that no SUA/sCr ratio threshold for CV risk has been identified for patients with diabetes, we aimed to assess the relationship between this index a

Elevated Blood Glucose Can Promote Uric Acid Excretion: A Cross-Sectional Study Involving Urinary Glucose and Urinary Uric Acid in China.

PURPOSE: Uric acid and blood glucose are important indicators of metabolic disorders. Numerous studies have elucidated the association between them, but the focus on their relationship through examination of urinary glucose and uric acid excretion has been limited. In this study, we conducted a comprehensive analysis on these indicators to explain the relationship. PATIENTS AND METHODS: This study involved the recruitment of 2498 patients who underwent fractional excretion of uric acid (FEUA) testing during their hospitalization at the Health Department of Qilu Hospital (Qingdao), Shandong Un

The role of uric acid and other crystals in osteoarthritis.

Clinicians have long assumed that an association exists between crystal arthropathies and the presence of osteoarthritis (OA). However, studies establishing an independent association between calcium pyrophosphate or uric acid crystal disease and OA are sparse. Even less is known about a possible pathogenic relationship. Whereas some studies suggest that the relationships between crystals and OA may not be incidental and that crystal deposition may contribute to the onset and/or acceleration of OA joint damage, other authors have challenged this assertion. In this review, we provide an overvie

[Uric acid, a neglected molecule--the value of imaging].

Imaging plays a key role in two longterm effects of hyperuricemia: those on the urinary tract and on the skeletal system. This overview concentrates on the diagnosis of urinary stones and of musculoskeletal gout. Urinary stones are negative on radiography, i.e., they cannot be differentiated from the surrounding tissues, and even at intravenous urography only non specific filling defects are shown that may be hard to differentiate from blood clots and papillary necrosis whereas computed tomography without a contrast agent is highly sensitive in detecting all types of stones whether negative or

Separation and determination of liver uric acid and allantoin.

We previously described the only satisfactory procedure yet achieved for separating uric acid and allantoin from rat liver. The procedure was based on trichloroacetic acid (TCA) extraction, acid hydrolysis, treatment with Hg-acetate, and cation- and anion-exchange chromatography. After separation, allantoin was quantified by a colorimetric method, and uric acid enzymatically using uricase. Since this procedure is too time-consuming, we propose an improved version which avoids the need for anion-exchange chromatography and the complex assay of catabolic compounds. The new method consists of a v

Conservation of vitamin C by uric acid in blood.

Urate at physiological concentrations increased the stability of ascorbate in human serum approximately fivefold. These measurements were made by depleting human serum of urate with uricase. In model experiments (using phosphate buffer instead of serum), urate protected against iron-induced ascorbate oxidation and, to a lesser extent, pH-induced ascorbate oxidation. In both human serum and in phosphate buffer, urate exerted its protective effect without itself undergoing measurable oxidation as determined by spectrophotometric and high pressure liquid chromatography (HPLC) techniques. [Which uric acid value is in need of treatment?].

Asymptomatic hyperuricemia should be treated only if the plasma uric acid levels are around 10 mg/100 ml or more on several determinations. In addition, patients on a purine-free diet who excrete more than 600 mg uric acid per 24 h should be treated. In both cases, treatment is intended to be prophylactic against gouty nephropathy. At present there is no evidence that primary hyperuricemia alone is a risk factor for early atherosclerosis and especially coronary artery disease. However, more attention should be paid to the accompanying risk factors such as obesity, hyperlipoproteinemia, diabete

Bempedoic acid and increased serum uric acid levels: a matter of concern?

AIMS: Several observational and genetic studies have found an association between high urate serum levels and hypertension, heart failure, and cardiovascular mortality. Bempedoic acid is a recently approved hypocholesterolemic agent acting by inhibiting the ATP citrate lyase and thus the hepatic cholesterol biosynthesis. Despite its optimal safety profile, a minor and reversible increase of serum uric acid levels has been observed. This effect is due to the inhibition by bempedoic acid of organic anion transporters 2 and 3 (OAT2 and OAT3) which mediate the renal excretion of urate. DATA SYNTH

Tear analysis of ascorbic acid, uric acid and malondialdehyde with capillary electrophoresis.

Tears have a significant role in antioxidant defense in ocular tissues and since their collection is quick and noninvasive, their analysis would facilitate monitoring of pathophysiological changes. However, their low volume and low content of antioxidants makes analysis difficult; methods of high sensitivity are needed. In this paper, we present a method for tear analysis of two antioxidant molecules (ascorbic and uric acid) and of a lipid peroxidation indicator (malondialdehyde) with capillary electrophoresis. Tears were collected with Schirmer strips, extracted with a low-pH phosphate buffer

Uric acid as a potential biomarker for cardiomyopathy in dystrophinopathy.

BACKGROUND: Cardiomyopathy is a significant cause of mortality in dystrophinopathy, and early detection and intervention are critical to reduce the disease burden. Currently, cardiomyopathy detection primarily relies on echocardiography and cardiac magnetic resonance imaging (CMR), which are inconvenient for paediatric patients and those in remote areas. Uric acid (UA) is associated with various heart diseases and serves as a biomarker of injury severity, but its level changes and connection with myocardial injury in dystrophic cardiomyopathy remain unclear. Therefore, we investigated the rela

The role of uric acid in the development of cardiovascular disease.

The main purpose of the present supplement, which is based on a symposium held in Bologna, 6-8 November 2014, is twofold: first of all, to reinforce the role of uric acid in the pathogenesis of gout and gout-related non-rheumatic diseases including renal involvement; second, to provide an updated review of the evidence supporting the relevant role of elevated uric acid as a risk factor for cardiovascular diseases.
