Urinary diversion in bladder exstrophy and incontinent epispadias: 25 years of experience.

PURPOSE: To determine the optimal surgical approach in achieving complete urinary continence with preservation of the upper urinary tract in the exstrophy-epispadias complex we reviewed the records of patients treated at our institution. MATERIALS AND METHODS: From 1968 to July 1994, 115 patients with bladder exstrophy/incontinent epispadias were treated of whom followup was available for 104 and 2 died of causes unrelated to urinary diversion. Mean followup is 16.7 years. In 43 of the 102 patients surgery was primarily performed at our institution (urinary diversion in 39, a modified Young-D

Ileourethral fistula following cystectomy: a rare complication.

Fistulae between the urinary and intestinal tracts of adult male patients usually result from trauma, inflammatory diseases, necrosis and infiltration of a neoplasm of either the bladder, prostate or colon, or radiation therapy. We report 2 patients who developed an ileourethral fistula following cystectomy which had been performed for carcinoma of the bladder in 1 patient and leukoplakia of the bladder in the other. The diagnosis of an ileourethral fistula was established by retrograde urethrography and a barium follow-through study. Computed tomography performed in 1 patient showed a tumor m

Successful Treatment of Coexisting Paraganglioma of the Retroperitoneum and Urinary Bladder by Intermediate-Dose 131I-MIBG Therapy: A Case Report.

Paragangliomas (PGLs) are rare neuroendocrine neoplasms of the autonomic nervous system dispersed from the skull base to the pelvic floor. We reported an extremely rare case with coexisting PGL of the retroperitoneum and urinary bladder. The patient complained of episodes of severe headache and palpitation during micturition, with a 1-year history of uncontrolled hypertension. The (131)I-metaiodobenzylguanidine ((131)I-MIBG) scintigraphy revealed high uptake in both of the retroperitoneum and urinary bladder mass accompanied with increasing 24 hour urinary norepinephrine and epinephrine. The p

A Tumor of Intrigue: Undifferentiated Carcinoma With Osteoclastic Giant Cells of the Urinary Bladder.

Undifferentiated carcinoma with osteoclastic giant cells (UCOGC) of the urinary bladder is a recently described entity classified as a subtype of poorly differentiated urothelial carcinoma, which is a form of invasive urothelial carcinoma. It is an extremely rare and aggressive malignancy that occurs predominantly in elderly men. The rarity of this neoplasm and its morphological similarity to other giant cell-rich lesions may present diagnostic challenges. Here, we present a case of urinary bladder carcinoma with a biphasic appearance. Histomorphological examination and immunohistochemical stu

The association of bladder myeloid sarcoma and unclassified myelodysplastic/myeloproliferative disease.

Myeloid sarcoma of urinary bladder is a rare disorder. We report a 71-year old man with hematuria. He was also under follow up for unclassified myeloproliferative/ myelodysplastic disorder, diagnosed 2 months ago. Abdominal ultrasonography and computed tomography findings were normal. Diagnostic cystoscopy revealed patchy areas of mucosal swelling with hyperemia. Histopathological examination of biopsies demonstrated a neoplasm composed of blasts showing myeloperoxidease positivity by immunohistochemistry. To our knowledge, current case is the first case of myeloid sarcoma without evidence of

[Sigmoid-vesical fistula as a consequence of sigmoid tumor].

Because of unusual rarity of neoplastic sigmoid-vesical fistula in young patients the case of 43 years old woman, in whom the fistula was the consequence of sigmoid adenocarcinoma infiltration of urinary bladder, was presented. The patient was treated radically by resection of the sigmoid and partial resection of the urinary bladder within healthy tissue. After operation the patient was exposed to radiotherapy, after which chemotherapy followed (6 courses of 5-day chemotherapy with fluorouracil and calcium folinate). 18 months after operation, 16 months after radiotherapy and 14 after chemothe

Robot-assisted transvesical partial cystectomy for leiomyoma of bladder trigone.

INTRODUCTION AND OBJECTIVE: Leiomyomas of the urinary bladder are very rare neoplasms and are the most common benign mesenchymal tumors of the bladder, accounting for 35% of these. Treatment of leiomyomas is mainly surgical and approaches range from transurethral resection to open segmental resection or laparoscopic partial cystectomy. We sought to present the surgical technique of robot-assisted transvesical partial cystectomy for bladder leiomyoma. MATERIALS AND METHODS: A 25-year-old man presented to urology department with urinary frequency and urgency. Ultrasound and MRI examinations rev

Intestinal-Type Villous Adenoma of the Urinary Bladder Presenting as an Asymptomatic Diffuse Plaque-Like Trigonal Lesion: A Rare Case With Multimodal Clinicopathologic Correlation.

Villous adenoma of the urinary bladder is a rare, benign glandular neoplasm with intestinal differentiation. This neoplasm is frequently associated with chronic urinary irritation and intestinal metaplasia of the urothelium. Fewer than 50 cases have been documented, and limited information is available regarding its clinicoradiologic spectrum. A 51-year-old man with severe anemia was found to have multiple broad-based exophytic lesions of the bladder on ultrasound. There was no hematuria, mucosuria, or other lower urinary tract symptoms. A minimally enhancing, plaque-like lesion measuring appr

Non-Functional Paraganglioma of the Urinary Bladder: A Rare Cause of Bladder Tumor.

INTRODUCTION: Paragangliomas of the urinary bladder (PUBs) are rare neuroendocrine tumors, representing less than 0.06% of all bladder neoplasms. While most are functional and associated with catecholamine excess, approximately 15% are non-functional, often presenting asymptomatically and discovered incidentally. Due to their rarity, standardized management guidelines are lacking. CASE PRESENTATION: We report a 57-year-old female evaluated for ureterolithiasis who was incidentally found to have a 1.5 cm posterior bladder lesion. Biopsy confirmed paraganglioma. Functional studies, including pl

Alpha-methylacyl-coenzyme a racemase-expressing urachal adenocarcinoma of the abdominal wall.

Urachal adenocarcinomas are very rare and about one third of these neoplasms arise in urachal remnants. To demonstrate the origin of the urachal adenocarcinoma is not easy, but it is very important for managing patient care. We report on a 35-year-old man who complained of a palpable mass in the periumbilical area. The mass was incidentally identified 10 days earlier. Computed tomography revealed a well-defined enhancing mass with internal calcification and septation abutting on the dome of the urinary bladder. The clinical diagnosis was urachal cancer, which seemed to invade the urinary bladd

Unrecognised adrenergic symptoms and the delayed diagnosis of urinary bladder paraganglioma.

INTRODUCTION: Paraganglioma is a rare neuroendocrine neoplasm that may arise from the extra-adrenal autonomic paraganglia. Urinary bladder paraganglioma is typically presented as repeated episodes of palpitations, headache or blood pressure rise immediately after micturition. Management of these tumors includes radical surgical treatment with preoperative antihypertensive preparation, and a life-long follow-up. CASE REPORT: We presented a middle-age female patient with functional urinary bladder paraganglioma, with a 3-year history of repeated episodes of abdominal pain, dysuria and hematuria

Mature ovarian dermoid cyst invading the urinary bladder.

Mature ovarian dermoid cysts are common lesions, accounting for up to 10-25% of all ovarian neoplasms. Uncomplicated dermoid cysts are often asymptomatic and are relatively easy to diagnose on imaging and to treat. Symptoms develop once complications set in and these may cause diagnostic dilemmas. Torsion (16%) is the most common complication, while rupture, suppuration and malignant transformation are relatively uncommon. Of all these complications, spontaneous rupture into the urinary bladder is least common. The diagnosis of this condition has been through the use of cystoscopy or laparotom

The laryngological ward of the district hospital in Piła has been carrying out the diagnosis of neoplastic cases. There were 34 cases of carcinoma of the larynx and or with the lower part of the pharynx were discovered. 4 out of there were confirmed to have second primary neoplastic region. Two patients the neoplasm synchronously developed and in two other patients the neoplasm metachronously developed. In there cases the region of second primary neoplasm was found in the area of the head and neck (thyroid gland, lower lip and the side of the nose) and in one case the second region was found i

Is urethral sparing at cystectomy a safe procedure?

Urethrectomy is performed with cystectomy when the neoplasm is multifocal, at the bladder neck, or associated with carcinoma in situ (CIS). The records of the most recent 20 patients treated by radical cystectomy at our institution were reviewed. In 50 percent of the cases, the extent of disease within the bladder was found to have been underestimated to the point that a urethral sparing operation (i.e., to maintain urinary continence with an intestinal segment and an intact urethra) might have been dangerous.

Correlation between induction of DNA fragmentation in urinary bladder cells from rats and humans and tissue-specific carcinogenic activity.

Seven chemicals, six of which are known to induce epithelial neoplasms of the urinary bladder in rats, were assayed for their ability to induce DNA damage in primary cultures of rat and human cells from urinary bladder mucosa, and in urinary bladder, liver and kidney of intact rats. Significant dose-dependent increases of DNA fragmentation, as measured by the Comet assay, were obtained in cells from both rats and humans with the following concentrations of five test compounds: 2-naphthylamine and N-nitrosodi-n-butylamine 0.5 and 1 mM, phenacetin 2 and 4 mM, cyclophosphamide from 2 to 8 mM, and

Serous adenocarcinoma of the uterus metastatic to the urinary bladder mimicking primary bladder neoplasia. A report of a case.

A 72-year-old woman with a history of transitional cell carcinoma of the urinary bladder was found to have a vesical neoplasm at cystoscopy. Microscopic examination of a biopsy specimen showed papillary adenocarcinoma. A subsequent endometrial biopsy, performed because of vaginal spotting, disclosed a serous papillary adenocarcinoma. A hysterectomy was performed. Pathologic examination showed that the tumor was deeply invasive of the myometrium. The uterine and bladder tumors were similar histologically. This case suggests that when examining a pure adenocarcinoma of the bladder, the pathologi

Carcinoma in situ of the urinary bladder. Clues to host involvement in human carcinogenesis.

BACKGROUND: Carcinoma in situ (CIS) of the urinary bladder is a neoplasm of uncertain biologic behavior. It rarely occurs as the primary disease and exists most often in association with high grade, invasive tumors. The unfavorable prognosis ascribed to CIS may not be due to tumor-related factors but to low host resistance or to host-tumor relationships established for previous or co-incident invasive cancers. The scant available evidence indicates that a large proportion of patients with primary CIS have a low frequency of muscle invasion and death from disease. METHODS: Of 2000 patients wit

Morphological keys in the differential diagnosis of bladder inverted papilloma. Study of two types, trabecular and glandular.

OBJECTIVE: Inverted papilloma of the urinary bladder is an uncommon urothelial neoplasm that may be specially difficult to distinguish from urothelial carcinoma. METHODS: Two patients with obstructive symptoms and hematuria have been studied. In the transurethral resection, accidentally, one showed a papillary lesion in the context of nodular hyperplasia of the prostate, where as the other showed a polypoid tumor of the urinary bladder RESULTS: Histologically, in both cases, a bladder inverted papilloma was demonstrated, originating from the surface transitional epithelium. Neuroendocrine tumours of the urinary bladder: recent advances.

Small cell carcinoma is the most frequently encountered neuroendocrine tumour (NET) of the urinary bladder, and it may present as either pure or in combination with urothelial carcinoma or other histological subtypes. Large cell neuroendocrine carcinoma is increasingly recognized in this location, but it is not yet fully characterized. Well-differentiated NET and paraganglioma of the bladder are rare neuroendocrine neoplasms. Advances in the molecular characterization of these tumours have enhanced our understanding of their biology and can provide better classification and more accurate risk
