Vaccine development for emerging virulent infectious diseases.

The recent outbreak of Zaire Ebola virus in West Africa altered the classical paradigm of vaccine development and that for emerging infectious diseases (EIDs) in general. In this paper, the precepts of vaccine discovery and advancement through pre-clinical and clinical assessment are discussed in the context of the recent Ebola virus, Middle East Respiratory Syndrome coronavirus (MERS-CoV), and Zika virus outbreaks. Clinical trial design for diseases with high mortality rates and/or high morbidity in the face of a global perception of immediate need and the factors that drive design in the fac

Development of a Clinically Viable Heroin Vaccine.

Heroin is a highly abused opioid and incurs a significant detriment to society worldwide. In an effort to expand the limited pharmacotherapy options for opioid use disorders, a heroin conjugate vaccine was developed through comprehensive evaluation of hapten structure, carrier protein, adjuvant and dosing. Immunization of mice with an optimized heroin-tetanus toxoid (TT) conjugate formulated with adjuvants alum and CpG oligodeoxynucleotide (ODN) generated heroin "immunoantagonism", reducing heroin potency by >15-fold. Moreover, the vaccine effects proved to be durable, persisting for over eigh

Exploiting viral natural history for vaccine development.

The partial successes of the Phase 2 gB-based vaccine trials for HCMV highlight the very real likelihood that vaccine-mediated induction of antibodies that neutralize the fusion pathway of fibroblast infection is not sufficient as a singular strategy to confer protective efficacy against primary HCMV infection. Alternative strategies that serve as adjuncts to gB-based vaccines are likely required to target different aspects of the complex lifecycle of HCMV infection. There has been considerable recent interest in targeting the gH/gL/UL128/UL130/UL131 pentamer complex (gH/gL-PC) to neutralize t

The mucosal immune system for vaccine development.

Mucosal surfaces are continuously exposed to the external environment and therefore represent the largest lymphoid organ of the body. In the mucosal immune system, gut-associated lymphoid tissues (GALTs), including Peyer's patches and isolated lymphoid follicles, play an important role in the induction of antigen-specific immune responses in the gut. GALTs have unique organogenesis characteristics and interact with the network of dendritic cells and T cells for the simultaneous induction and regulation of IgA responses and oral tolerance. In these lymphoid tissues, antigens are up taken by M c

Critical research concepts in tuberculosis vaccine development.

A new and improved vaccine against tuberculosis (TB) would provide a powerful tool to conquer one of the most insidious infectious diseases of mankind. Protection afforded by bacillus Calmette-Guérin (BCG) has been shown to be limited and inconsistent, especially in adults that are known to transmit TB disease. In the last two decades, several new vaccines have been developed and tested with the aim to elicit robust and long-lived T-cell responses against Mycobacterium tuberculosis antigens. Although much progress has been made in the TB vaccine field, there is an urgent need to address critic

Issues related to recent dengue vaccine development.

Dengue fever (DF) and dengue hemorrhagic fever (DHF) are mosquito-transmitted diseases of global importance. Despite significant research efforts, no approved vaccines or antiviral drugs against these diseases are currently available. This brief article reviews the status of dengue vaccine development, with particular emphasis on the vaccine strategies in more advanced stages of evaluation; these include traditional attenuation, chimerization and engineered attenuation. Several aspects of these vaccine design strategies, including concerns about vaccine candidates inducing infection-enhancing

Shigellosis: the current status of vaccine development.

PURPOSE OF REVIEW: Shigellosis, a major form of bacillary dysentery, is caused by infection with Shigella organisms. In poor countries, Shigella-caused dysentery is endemic and causes an estimated 163 million illness episodes annually and more than one million deaths. Although several strategies have been used to develop vaccines targeting shigellosis, none has been licensed for use outside China. Owing to the wide range of Shigella serotypes and subtypes, there is a need for a multivalent vaccine representing prevalent species and serotypes. RECENT FINDINGS: Vaccine development has been limi

Safety monitoring in vaccine development and immunization.

The development of vaccines has been one of the most important achievement in preventive medicine. As the incidence of vaccine-preventable diseases is reduced by immunization, general public becomes increasingly concerned about the safety associated with vaccine. Vaccine safety is extensively evaluated through animal safety studies, clinical trials, during manufacturing processes, and postlicensure surveillance. Safety monitoring in postlicensure surveillance has relied on passive reporting system and epidemiological studies, including Vaccine Adverse Event Reporting System (VARES), Vaccine Sa

[Perspectives for development of polyvalent meningococcal vaccine].

Experimental whole-culture oral polyvalent meningococcal vaccines against serogroups A, B and C consisting of three monovalent components in different proportions have been developed and evaluated. Kinetics of IgG response to meningococcal antigens (outer membrane proteins, polysaccharide, lypooligosaccharide of these serogroups) in sera of rabbits immunized orally with these preparations was measured. Sharp rise of IgG levels (on 2 - 3 orders) compared to baseline has been detected as well as persistence of high titers during the observation period (322 days).

Vaccine development for capsulate bacteria causing pneumonia.

Pneumonia strikes the extremes of the age spectrum, causing maximal death and disability in children and the elderly. Despite its worldwide impact, there is a paucity of epidemiologic data regarding its incidence and the causative organisms. The two leading causes of bacterial pneumonia in childhood are Streptococcus pneumoniae (SP) and Haemophilus influenzae type b (Hib). SP is the major cause of pneumonia beyond the newborn period. In neonates, Group B Streptococcus (GBS) remains a major cause of sepsis and pneumonia despite recent reductions due to targeted perinatal antibiotic prophylaxis.

Vaccine development against infection with Helicobacter pylori.

Infection with Helicobacter pylori, is one of the most prevalent infections world-wide, where approximately 50% of adults in the developed world and over 90% of inhabitants in the developing world are infected. Chronic infection with H. pylori is the cause of gastritis, peptic ulcer disease and is a risk factor for gastric adenocarcinoma. Recent studies have demonstrated the suitability of an immunization strategy in the prevention and treatment of H. pylori infection, and the potential for management of disease. Mucosal administration of purified recombinant sub-unit proteins of H. pylori, to

Development of an antipathology vaccine for schistosomiasis.

The data presented here clearly demonstrate that IL-12 can act as an adjuvant, suppressing both granuloma formation and fibrosis induced after natural schistosome infection. Recently, we showed that IL-12 can increase protective immunity provided by an attenuated larval schistosome vaccine as well. In both cases the vaccines appear to suppress in large part the parasite-induced Th2 responses. Thus, the use of cytokines as adjuvants offers a rational approach for immunomodulation when the effector mechanism of a particular vaccine is known. Clearly, IL-12 has enormous potential for modulating t

The case for model-informed vaccine development.

This perspective paper illustrates how model-informed vaccine development (MIVD) impacts vaccine discovery and development decisions, saving time and resources. It discusses the benefits of mechanistic and other mathematical models within MIVD. We outline key applications, opportunities, challenges, and future steps for MIVD as a call for the continued integration of mathematical modelling during vaccine development.

The History of Measles and Vaccine Development.

Measles is one of the oldest human diseases, but the fact that it was separate from smallpox, scarlatina and rubella was only disclosed a century ago. It is extremely contagious, as the infection may spread for up to 4 days before the first symptoms appear. One patient may infect 12-18 secondary cases, who then spread the infection further. Before the vaccine era, measles caused 2-3 million deaths a year, with remote populations facing chaotic consequences from epidemics. Safe and inexpensive live-virus vaccines, often combined with mumps and rubella components, have been available for 50 year

A systematic approach to tuberculosis vaccine development.

M. tuberculosis is once again the leading infectious cause of death worldwide despite the existence of a licensed vaccine (BCG). In this issue of Cell, Vidal et al. systematically evaluate 42 candidate antigens and develop a trivalent mRNA vaccine that demonstrates effective and durable protection from tuberculosis in several mouse models. This vaccine also enhances protection conferred by BCG.

Immunoproteomics: Approach to Diagnostic and Vaccine Development.

The study of large protein sets (proteomics) involved in the immunological reaction is known as immunoproteomics. The methodology of immunoproteomics plays a major role in identifying possible vaccine candidates that could protect against pathogenic infection. The study of immunogenic proteins that are expressed during the outset of infection is the focus of the crosstalk between proteomics and immune protection antigens utilizing serum. Peptide presentation by MHC provides the new 'window' into changes that occur in the cell. Thus, there is strong, intense pressure on the pathogen that has be

Advances in vaccine development for Chlamydia trachomatis.

Chlamydia trachomatis is the most prevalent bacterial sexually transmitted infection globally. Antibiotic treatment is highly effective, but infection is often asymptomatic resulting in most individuals going undetected and untreated. This untreated infection can ascend to the upper female genital tract to cause pelvic inflammatory disease, tubal factor infertility, and ectopic pregnancy. Chlamydia screening and treatment programs have failed to control this epidemic and demonstrate the need for an efficacious vaccine to prevent transmission and disease. Animal models and human epidemiological
