Early ischemic lesions following subarachnoid hemorrhage: common cold remedy as precipitating factor?

A 46-year-old woman presented with tetraplegia contrasting with a relatively preserved consciousness following aneurysmal subarachnoid hemorrhage (SAH). Multiple ischemic lesions were detected by magnetic resonance imaging (MRI), in the absence of vasospasm or signs of increased intracranial pressure. During the weeks before SAH, the patient had repeatedly used a nasal decongestant containing phenylephrine. After coiling of the aneurysm harboured by the right posterior cerebral artery, symptomatic vasospasm developed in the territory of the right middle cerebral artery and required aggressive

Treatment-Related Reversible Cerebral Vasoconstriction Syndrome.

Reversible cerebral vasoconstriction syndrome (RCVS) is a rare but significant cause of intracranial arteriopathy and stroke in young adults. The syndrome encompasses a spectrum of disorders radiologically characterized by reversible narrowing and dilation of intracranial arteries, often triggered by vasoactive drugs or the postpartum period. The hallmark clinical feature of RCVS is thunderclap headache with or without other neurological signs. Though endothelial dysfunction and sympathetic hyperactivation are hypothesized to be key mechanisms, the exact pathogenesis of RCVS is still unclear.

Recurrent Cerebral Vasospasm and Delayed Cerebral Ischemia Weeks Subsequent to Elective Clipping of an Unruptured Middle Cerebral Artery Aneurysm.

BACKGROUND: Cerebral vasospasm and delayed ischemic neurologic deficits are well-known clinical aftereffects of subarachnoid hemorrhage due to rupture of an intracranial aneurysm. However, vasospasm with consequential ischemia after clipping of an unruptured aneurysm is an exceedingly rare sequela encountered in the reported neurosurgical literature. CASE DESCRIPTION: A 53-year-old woman had presented for elective craniotomy with microsurgical clipping of an unruptured left middle cerebral artery bifurcation saccular aneurysm, which was successfully treated without complications. Despite an i

Elevated Flow Velocity in Transcranial Doppler: Discrepancies Between Actual Vasospasm Locations and Flow Measurements.

BACKGROUND: Transcranial Doppler (TCD) is widely used to predict cerebral vasospasm in patients with subarachnoid hemorrhage by monitoring mean flow velocity (MFV) in major cerebral arteries. However, its accuracy in identifying specific arteries affected by vasospasm remains unclear. This study investigates the correlation between elevated MFV and the actual location of vasospasm, considering the influence of adjacent artery vasospasms. METHODS: We conducted a retrospective observational study of patients with subarachnoid hemorrhage who underwent transcranial Doppler monitoring and subseque

Cerebral revascularization using radial artery grafts for the treatment of complex intracranial aneurysms: techniques and outcomes for 17 patients.

OBJECTIVE: The goal of this report is to illustrate the use of radial artery grafts as bypass conduits in the management of complex intracranial aneurysms and to describe a new "pressure distension technique" to eliminate postoperative vasospasm, which was a common problem early in our experience. METHODS: This study included a series of 17 patients who were surgically treated between 1994 and January 2001 for complex intracranial aneurysms. Five patients were surgically treated without the pressure distension technique; for 12 patients, the technique was used to reduce postoperative vasospas

Patient risk factors and outcomes of vasospasm following aneurysmal subarachnoid hemorrhage: A propensity-score matched analysis.

BACKGROUND: Vasospasm is a serious complication following subarachnoid hemorrhage (SAH) secondary to an aneurysm rupture. It has been associated with poor clinical outcomes and higher healthcare costs; however, the studies have been limited to relatively small institutional samples. We aimed to identify patient risk factors and outcomes associated with vasospasm following an aneurysmal SAH and develop a predictive model for the risk of vasospasm. METHODS: The national inpatient sample was used to identify patients admitted with SAH secondary to intracranial aneurysm rupture. The eligible pati

Improved biomechanical metrics of cerebral vasospasm identified via sensitivity analysis of a 1D cerebral circulation model.

Cerebral vasospasm (CVS) is a life-threatening condition that occurs in a large proportion of those affected by subarachnoid haemorrhage and stroke. CVS manifests itself as the progressive narrowing of intracranial arteries. It is usually diagnosed using Doppler ultrasound, which quantifies blood velocity changes in the affected vessels, but has low sensitivity when CVS affects the peripheral vasculature. The aim of this study was to identify alternative biomarkers that could be used to diagnose CVS. We used a 1D modelling approach to describe the properties of pulse waves that propagate throu

Transcranial Doppler ultrasonography in the neurologic intensive care unit.

Transcranial doppler ultrasonography (TCD) is a noninvasive monitoring tool which allows imaging of blood flow velocities in intracranial blood vessels. It is safe, portable, easy to perform and provides accurate information regarding underlying physiology which may help to guide therapy in critically ill neurologic patients. It has significantly contributed to the management of vasospasm related to subarachnoid hemorrhage in the neurologic intensive care unit. TCD is also helpful in the early diagnosis of a variety of complications that can occur in patients with head injury such as vasospasm

The incidence of cerebral arterial vasospasm following aneurysmal subarachnoid haemorrhage: a systematic review and meta-analysis.

PURPOSE: To describe a pooled estimated incidence of cerebral arterial vasospasm (aVSP) following aneurysmal subarachnoid haemorrhage (aSAH) and to describe sources of variation in the reported incidence. METHODS: We performed a systematic review and meta-analysis of randomised clinical trials (RCTs) and cohort studies. The primary outcome was the proportion of study participants diagnosed with aVSP. We assessed for heterogeneity based on mode of imaging, indication for imaging, study design and clinical characteristics at a study level. RESULTS: We identified 120 studies, including 19,171 p

[Secondary complications of acute subarachnoid hemorrhage].

Despite further improvement of diagnostic procedures and the management of patients with acute subarachnoid hemorrhage it is still a severe clinical condition often worsened by several secondary complications after the initial bleeding. The most important and most frequent are early rehemorraging, cerebral vasospasm and the development of hydrocephalus. In addition there are many other sequelae, such as disturbances of electrolytes, seizures and the general complications of intensive care medicine which can greatly influence clinical outcome. This report provides an overview of the possible me

Spontaneous disappearance of an intracranial aneurysm after subarachnoid hemorrhage.

Spontaneous disappearance of an intracranial aneurysm after subarachnoid hemorrhage is an uncommon event and usually associated with severe cerebral vasospasm, giant aneurysms or the use of antifibrinolytics. We present a young woman who suffered a grade 5 subarachnoid hemorrhage with severe vasospasm caused by a small anterior communicating artery aneurysm. The patient underwent a slow recovery and two years later requested surgery. Angiography demonstrated complete disappearance of the aneurysm. The neurosurgeon should be aware that spontaneous thrombosis of cerebral aneurysms can occur and

Hyponatremia and cerebral vasospasm in subarachnoid hemorrhage: a systematic review and meta-analysis.

BACKGROUND: Hyponatremia, defined as serum sodium < 135 mEq/L, is a common complication following aneurysmal subarachnoid hemorrhage (aSAH) and has been implicated in the development of cerebral vasospasm, a significant contributor to delayed cerebral ischemia and poor neurological outcomes. However, the strength and consistency of this association remain unclear. This study aimed to evaluate the relationship between hyponatremia and angiographically or radiologically confirmed vasospasm in patients with aSAH. METHOD: A comprehensive search of PubMed, Scopus, Embase, and Medline was conducted

The surgical management of contralateral anterior circulation intracranial aneurysms.

This study reviews the surgical management of contralateral anterior circulation aneurysms in patients with bilateral intracranial aneurysms repaired following a unilateral craniotomy. Between 1993 and 1999, 27 patients had 88 intracranial aneurysms repaired. Eleven patients presented following subarachnoid haemorrhage. Excluding midline aneurysms, 31 anterior circulation aneurysms were contralateral to the craniotomy and all were repaired at the same time that ipsilateral or midline aneurysms were repaired. Morbidity included one death and one case of loss of unilateral vision directly attrib

Intra-arterial fasudil infusion for symptomatic cerebral vasospasm following clipping of an unruptured middle cerebral artery aneurysm: A case report and literature review.

We report the first known case of symptomatic unruptured intracranial aneurysm-associated cerebral vasospasm (UIA-CVS) following surgical clipping that was successfully treated with intra-arterial fasudil infusion. UIA-CVS is a rare postoperative complication for which no standardized treatment has been established. A 67-year-old woman with a 5-mm saccular aneurysm at the bifurcation of the left middle cerebral artery (MCA) underwent microsurgical clipping via a distal transsylvian approach. On postoperative day 7, she developed motor aphasia. Magnetic resonance angiography and digital subtrac

Postoperative Symptomatic Cerebral Vasospasm: Requiring Attention Following an Uneventful Resection of an Epidermoid Cyst - A Case Report and Literature Review.

Cerebral vasospasm associated with epidermoid cyst can be caused by tumor content spillage, such as spontaneous rupture and postsurgical resection. Symptomatic cerebral vasospasm following the resection of an intracranial epidermoid cyst is a rare but serious complication that lacks a consensus on treatment. Case presentation: A 10-year-old girl underwent an uneventful complete resection of a left cerebellopontine angle epidermoid cyst. On the second postoperative day (POD 2), she exhibited reduced speech, confusion, and hyperventilation followed by hypocapnia. On POD 4, she developed right he
