Retrievable Inferior vena cava filters in pregnancy: Risk versus benefit?

OBJECTIVE: Venous thromboembolism remains one of the leading causes of maternal mortality in the developed world. Retrievable inferior vena cava (IVC) filters have a role in the prevention of lethal pulmonary emboli when anticoagulation is contraindicated or has failed [1]. It is unclear whether or not the physiological changes in pregnancy influence efficacy and complications of these devices. The decision to place an IVC filter in pregnancy is complex and there is limited information in terms of benefit and risk to the mother. The objective of this study was to determine the efficacy and saf

Providing Context: Medical Device Litigation and Inferior Vena Cava Filters.

Over the last few years, an increasing number of lawsuits have been filed involving inferior vena cava filters. This has prompted the U.S. Judicial Panel on Multidistrict Litigation to centralize these lawsuits into two multidistrict litigations: one for Cook's filters and one for Bard's. Both sets of cases share similar questions of facts, in particular whether these filters' design and manufacturing practices made them unreasonably prone to serious complications. The resolution of these cases will add to a larger legal debate concerning how much legal protection the 1976 Medical Device Amend

Duplicate inferior vena cava filters: more is not always better.

Duplication of the inferior vena cava (IVC) has been reported in literature. This achieves clinical significance in the setting of lower extremity venous thromboembolism with a contraindication for anticoagulation. We describe a case of lower extremity deep vein thrombosis with duplicate IVC. Anticoagulation was contraindicated in this case leading to successful treatment with double IVC filters. We conducted a PubMed search for all current English language published literature, where filters were placed in the presence of duplicate IVC. We suggest that patients with deep vein thrombosis shoul

Vascular disease patient information page. Inferior vena cava (IVC) filters.

IVC filters are medical devices that are placed in the large vein of the abdomen to prevent blood clots from traveling from the veins of the legs to the lungs. They are most commonly used when a patient cannot receive blood thinners to treat leg blood clots (DVT). IVC filter insertion can be done as an outpatient under local anesthesia, with only a puncture to a vein in the leg or neck. Most IVC filters are designed to be removed when no longer necessary. IVC filter complications are uncommon.

Inferior vena cava filters in patients with advanced-stage cancer.

BACKGROUND: Cancer and its treatment are recognized risk factors for venous thromboembolism (VTE). Inferior vena cava (IVC) filters are utilized to provide mechanical thromboprophylaxis to prevent pulmonary embolism (PE) or to avoid bleeding from systemic anticoagulation. PATIENTS AND METHODS: A retrospective analysis of 107 cancer patients who had IVC filters inserted and followed up at our institution was performed. All patients had active cancer; a majority (> 90%) had advanced-stage disease, whereas only five patients (5.8%) had stages I or II disease. RESULTS: Eighty six patients (80.3%

Reporting the impact of inferior vena cava perforation by filters.

BACKGROUND: Perforation of the inferior vena cava by filters struts is a known complication. The goal of our review is to assess the impact of inferior vena cava perforation by filters based on an open, voluntary national database. METHODS: We reviewed 3311 adverse events of inferior vena cava filters reported in Manufacturer and User Facility Device Experience database from January 2000 to June 2011. Outcomes of interest were incidence of inferior vena cava perforation, type of filter, clinical presentation, and management of the perforation, including retrievability rates. RESULTS: Three h

Advanced techniques for removal of retrievable inferior vena cava filters.

Inferior vena cava (IVC) filters have proven valuable for the prevention of primary or recurrent pulmonary embolism in selected patients with or at high risk for venous thromboembolic disease. Their use has become commonplace, and the numbers implanted increase annually. During the last 3 years, in the United States, the percentage of annually placed optional filters, i.e., filters than can remain as permanent filters or potentially be retrieved, has consistently exceeded that of permanent filters. In parallel, the complications of long- or short-term filtration have become increasingly eviden

Venous thromboembolism after removal of retrievable inferior vena cava filters.

The purpose of this study was to examine the incidence of new or recurrent venous thromboembolism (VTE) after retrieval of inferior vena cava (IVC) filters and risk factors associated with such recurrence. Between March 2001 and September 2008, at our institution, implanted retrievable vena cava filters were retrieved in 76 patients. The incidence of new or recurrent VTE after retrieval was reviewed and numerous variables were analyzed to assess risk factors for redevelopment of VTE after filter retrieval. In 5 (6.6%) of the 76 patients, redevelopment or worsening of VTE was seen after retriev

[Suprarenal inferior vena cava filters. Retrospective review of 30 cases].

BACKGROUND: Inferior vena cava (IVC) filters are used to prevent massive pulmonary embolism in cases where anticoagulation is contraindicated or has failed. It is usually implanted below the renal veins. In a few cases it is necessary to deploy the filter above them, with theoretical risk of secondary renal failure. AIM: To report the experience with filters located above the renal veins. PATIENTS AND METHODS: Medical records of all patients with percutaneous suprarenal filters are reviewed. RESULTS: Between May 1993 and May 2007, 361 percutaneous IVC filter procedures were performed. In th

The use of inferior vena cava filters in young children.

The present report describes the authors' experience with the placement and retrieval of inferior vena cava (IVC) filters in children with a maximum IVC diameter of 1 cm. Over a 14-month period, three filters were placed in three children. Two filters were placed via the right internal jugular vein, with one placed via the right femoral vein. The filters were deployed successfully in all three children and retrieved in two. In one case, the child was receiving palliative care and removal was not attempted. No complications were encountered during placement, the period of implantation, or retri

Inferior vena cava filters in the management of venous thromboembolism.

Inferior vena cava (IVC) filters, both retrievable and permanent, are indicated for the prevention of pulmonary embolism (PE) in patients contraindicated for anticoagulant therapy, in those with anticoagulant therapy complications, and perhaps for those with recurrent PE despite therapeutic anticoagulation. Because of the lack of randomized controlled trials (only 1 has been published), clinicians have little evidence-based information to assist them in determining proper use of IVC filters. The introduction of retrievable filters and the ease of insertion have stimulated increased use of thes

Medical literature and vena cava filters: so far so weak.

STUDY OBJECTIVE: With the development of percutaneous inferior vena cava (IVC) filters, IVC interruption has become a widely used procedure in patients with or at risk for venous thromboembolism. In an attempt at clarifying the indications for filter placement, a systematic literature review was undertaken. DESIGN: Bibliographic search and analysis. MEASUREMENTS AND RESULTS: A systematic MEDLINE search about vena cava filters produced a total of 568 references with abstracts between 1975 and 2000 inclusively. Each reference was analyzed according to predetermined criteria. Nearly two thirds

Indications, management, and complications of temporary inferior vena cava filters.

PURPOSE: We describe the results of a preliminary prospective study using different recently developed temporary and retrievable inferior vena cava (IVC) filters. METHODS: Fifty temporary IVC filters (Günther, Günther Tulip, Anthéor) were inserted in 47 patients when the required period of protection against pulmonary embolism (PE) was estimated to be less than 2 weeks. The indications were documented deep vein thrombosis (DVT) and temporary contraindications for anticoagulation, a high risk for PE, and PE despite DVT prophylaxis. RESULTS: Filters were removed 1-12 days after placement and n

[Inferior vena cava filters. Indications and results in 111 patients].

BACKGROUND: Anticoagulation is the treatment of choice for deep vein thrombosis and pulmonary embolism. Occasionally this treatment is contraindicated or fails to prevent pulmonary embolism. In these patients, inferior cava vein interruption is indicated and filter insertion is the procedure commonly performed. AIM: To report the results of inferior cava vein filter insertions in 111 patients. PATIENTS AND METHODS: A retrospective review of patients in whom inferior cava vein filters were inserted was performed. RESULTS: During the period 1983-1997, filters were inserted in 111 patients (56

Venous interruption as prophylaxis of pulmonary embolism: vena cava filters.

Interruption of vena cava for prevention of pulmonary embolism (PE) was achieved in the past with surgical ligation or placement of clips outside the infrarenal vena cava. At present, this procedure is performed with percutaneous insertion of vena cava filters. Vena cava filters can be permanent or temporary, catheter-retrievable. Main indications for placement of a vena cava filter are: contraindication for anticoagulant therapy in patients with severe PE in whom a further embolic episode would be fatal or patients with PE (or its recurrence) undergoing adequate anticoagulant therapy. Tempora

Inferior vena cava "birds nest" filters--2 year follow-up.

This study examines long term efficacy and saftey of "birds nest" filters (Cook), and the use of doppler ultrasound to assess patency. Of the seventeen "birds nest" filters inserted twelve were available for study. All patients were reviewed for evidence of recurrent pulmonary emboli or filter complication. Mean length of follow up was 10 months, (7 had follow up of > 12 months). Three have had further episodes of deep venous thrombosis, with probable recurrent pulmonary embolism in one. No patient had evidence of thrombosis of the inferior vena cava. No fracture or migration of the "birds nes

Placement of two Greenfield filters in a duplicated vena cava.

Congenital anomalies of the inferior vena cava (IVC) are not unusual and may complicate the placement of caval interruption devices. A duplicated IVC was demonstrated by venography in a patient with left iliofemoral venous thrombosis. A Greenfield filter was placed in each cavum from a right internal jugular vein approach. Because anomalies of the IVC are common, ascending venograms should be obtained in all patients who undergo caval interruption to document the number and position of the IVC, the location of the caval bifurcation, and the diameter of the IVC, as well as to identify the prese
