Very Late Iliac Venous Stent Migration Into the Right Ventricle With Tricuspid Valve Injury Five Years After Implantation: A Case Report.

Stent migration into the right heart chambers is an uncommon but potentially life-threatening complication of venous stenting. Most reported cases occur shortly after implantation. A 72-year-old man with prior left iliac vein stenting presented five years later with dyspnea and hemoptysis. Imaging revealed migration of the iliac stent into the right ventricle, in direct contact with the tricuspid valve apparatus. The patient underwent successful surgical removal on a beating heart, including neocordae implantation and annuloplasty ring placement. Postoperative recovery was uneventful. This cas

Virtually Wall-Less versus Standard Thin-Wall Venous Cannula in the Minimally Invasive Mitral Valve Surgery: Single-Center Experience.

Background and Objectives: Minimally invasive cardiac surgery (MICS) has been developing since 1996. Peripheral cannulation is required to perform MICS, and good venous drainage and a bloodless field are crucial for the success of this procedure. We assessed the benefits of using a virtually wall-less cannula in comparison with the standard thin-wall cannula in clinical practice. Materials and Methods: Between January 2021 and December 2022, we evaluated 65 elective patients, who underwent isolated minimally invasive mitral valve surgery. Both the virtually wall-less and the thin-wall cannulas

Usefulness of Intraprocedural Pulmonary Venous Flow for Predicting Recurrent Mitral Regurgitation and Clinical Outcomes After Percutaneous Mitral Valve Repair With the MitraClip.

OBJECTIVES: The aim of this study was to determine the prognostic value of pulmonary venous (PV) flow during MitraClip implantation. BACKGROUND: The clinical significance of PV flow information during MitraClip implantation is unknown. METHODS: A total of 300 patients who underwent MitraClip implantation and in whom the measurement of PV flow was completed using intraprocedural transesophageal echocardiography were retrospectively reviewed. The optimal threshold of the ratio of systolic velocity-time integral (Svti) to diastolic velocity-time integral (Dvti) ratio after MitraClip placement f

Emergent veno-venous extracorporeal membrane oxygenation during aortic valve replacement following severe re-expansion pulmonary edema: A case report.

Re-expansion pulmonary edema is defined as pulmonary edema that occurs when a chronically collapsed lung rapidly re-expands, most commonly following chest tube placement for pneumothorax, re-expansion of severe atelectasis, and evacuation of pleural effusion. Though it is very rare, the sudden onset and clinical features of re-expansion pulmonary edema make it a lethal complication that requires urgent treatment. We present a 60-year-old patient who underwent an aortic valve replacement with pre-existing large bilateral pleural effusions. Intraoperatively, upon evacuation of the pleural effusi

Long term results of vein valve transplants placed in the popliteal vein for intractable post-phlebitic venous ulcers and pre-ulcer skin changes.

Most patients with chronic venous ulceration or severe pre-ulcer damaged skin have incompetent popliteal valves and a high ambulant venous pressure (AVP). Competency can be restored by a vein valve transplant taken from the arm and placed in the popliteal fossa, with subjective and objective improvement. In this study 23 patients with post-phlebitic syndromes received 25 vein valve grafts as part of management. Seventeen patients had large recurrent ulcers after unsuccessful venous surgery: 6 patients had extensive pre-ulcer skin damage. Fifteen of 17 patients healed their ulcers, and all 6 pa

Pulmonary valve replacement. Report of an operation performed for calcific pulmonic stenosis associated with total anomalous pulmonary venous drainage and atrial septal defect.

Calcification of the pulmonic valve is an uncommon lesion that is usually associated with other congenital cardiac defects. Prior attempts at valvotomy or partial excision of the puolmonic valve have carried a high mortality rate. Pulmonary valve replacement itself is an equally uncommon surgical procedure and, in the past, has been performed only during reconstruction of the right ventricular outflow tract, when the pulmonic valve itself had to be utilized as an aortic prosthesis, and in one patient in whom pulmonary regurgitation following valvotomy was not well tolerated. A case of calcific

[Anesthetic management using the arterial pressure-based cardiac output monitor and a central venous oximetry catheter for tricuspid valve replacement in a patient receiving hemodialysis].

A 64-year-old man receiving hemodialysis underwent a tricuspid valve replacement. We used an arterial pressure-based cardiac output (APCO) monitor and a central venous oximetry catheter instead of pulmonary artery catheter because the enlargement of the right ventricle caused by tricuspid valve regurgitation might make it difficult to insert the pulmonary artery catheter. Aortic calcification was so severe that an arterial cannula was sewn into the right subclavian artery. Therefore, we had to insert an arterial catheter into the shunt placed on his left upper limb, and connected it to the APC

Three-Point Bedside Congestion Score (IJV Collapsibility + IVC Collapsibility + Central Venous Pressure) in Elective Mitral Valve Surgery: A Prospective Observational Study.

BACKGROUND: Systemic venous congestion and right-ventricular (RV) dysfunction are key drivers of postoperative hemodynamic instability, prolonged ventilation, and acute kidney injury (AKI) after mitral valve surgery. Comprehensive echocardiography is informative but may be impractical in high-throughput pre-anesthesia workflows. We evaluated a pragmatic three-point bedside congestion score combining internal jugular vein (IJV) collapsibility, inferior vena cava (IVC) collapsibility, and invasively measured central venous pressure (CVP) for preoperative risk stratification. METHODS: In this pr

[Drop in oxygen saturation and blood pressure as well as increase in central venous pressure during mitral valve clipping in an 81-year-old female patient].

BACKGROUND: Atrial septal defects (ASD) following endovascular mitral valve clipping are potentially hemodynamically relevant complications. Immediate closure with an occluder can represent a safe and effective treatment. An 81-year-old female patient suffering from severe dyspnea due to previously known severe mitral valve regurgitation was scheduled for elective mitral valve clipping. The clip was successfully implanted. Removal of the transseptal cannula resulted in a sudden drop in oxygen saturation and systolic blood pressure as well as an immediate increase in central venous pressure. An

Next Generation Experimental Models of Venous Physiology and Pathology: Examining Human Relevance in Recapitulating Lower Extremity Flow and Valve Function in New Approach Methods (NAMs).

Venous return depends on compliant vessels, competent valves, and auxiliary pumps such as the calf and thoracic muscles. Disruption of these mechanisms, through immobility, valve dysfunction, or inflammation, creates disturbed flow and heterogeneous shear that drive venous dysfunction and disease progression. Traditional animal models, though informative, fail to replicate human venous geometry and hemodynamics. This review critically evaluates current in vitro and in silico venous models, highlighting their capabilities and limitations in reproducing physiological flow, endothelial responses,

Severe left-to-right shunting from combined traumatic tricuspid valve rupture and atrial septal defect: bridge to surgical repair using veno-venous ECMO.

Our patient presented to the emergency room following a motor vehicle accident. The traumatic tricuspid valve rupture was diagnosed by transthoracic echocardiogram, and his respiratory status declined rapidly. He was placed on veno-venous extracorporeal membrane oxygenation (VV ECMO) to bridge him to surgical repair. Transoesophageal echocardiography revealed a large atrial septal defect (ASD) with the combination of both injuries leading to severe left-to-right shunting that was underappreciated in the initial management of the patient. The tricuspid valve and the ASD were repaired. In this p

Combined veno-venous ECMO and direct aortic impella 5.5 support enabling emergency mitral valve replacement in acute severe mitral regurgitation with fulminant pulmonary edema.

Acute severe mitral regurgitation (MR) can cause cardiogenic shock and fulminant pulmonary edema. Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) may worsen pulmonary congestion by increasing left ventricular afterload. We report a 61-year-old man with acute severe MR and extreme respiratory failure (FiO₂ 1.0: pH 7.144, PaO₂ 48 mmHg, PaCO₂ 101 mmHg) who underwent emergency mitral valve replacement. Because severe pulmonary edema and post-cardiotomy shock compromised separation from cardiopulmonary bypass, an Impella 5.5 was inserted via a direct aortic graft, followed by veno-venou

Endoscopic Minimally Invasive Beating-Heart Tricuspid Valve Surgery Without Jugular Vein Cannulation or Caval Snaring: Clinical Outcomes Using Novel Percutaneous Venous Cannulation.

Background and Objectives: Endoscopic beating-heart tricuspid valve (TV) surgery is increasingly adopted in cardiac surgery due to its association with improved postoperative recovery. Conventional approaches require jugular vein cannulation to establish adequate bicaval venous drainage, and caval snaring is considered mandatory during beating-heart TV procedures to prevent intraoperative air lock. We report our single-center experience with a novel percutaneously placed venous cannula (Smart Cannula) that enables minimally invasive TV surgery without jugular cannulation or caval snaring. Mate
