[Clinical application of 133-Xe ventilation scintigraphy].

Xenon-133 ventilation scintigraphy provides three sets of scintigrams: wash-in, equilibrium, and washout images. Krypton-81m or 99mTc-Technegas can be used as alternatives depending on their physical characteristics and the purpose of scintigraphy. Xenon-133 scintigraphy is compatible with SPECT when a multi-detector gamma camera is used. Ventilation images are analyzed by the height/area method to calculate regional lung volume and the mean transit time of the posterior planar view. If perfusion scintigraphy is done during the same examination, a ventilation/perfusion ratio image is also obta

Comparison of observer variability and accuracy of different criteria for lung scan interpretation.

UNLABELLED: Different criteria have been advocated for the interpretation of ventilation/perfusion (V/Q) lung scans in patients with suspected pulmonary embolism (PE). Besides these predefined criteria, many physicians use an integration of the different sets of criteria and their own experience-the so-called Gestalt interpretation. The purpose of this study was to evaluate interobserver variability and accuracy of 3 sets of criteria: the Hull and PIOPED (Prospective Investigation of Pulmonary Embolism Diagnosis) criteria and the Gestalt interpretation. METHODS: Two experienced observers inte

Hepatopulmonary syndrome.

The hepatopulmonary syndrome (HPS) is a rare complication of liver cirrhosis and is characterised by the typical triad of liver cirrhosis, arterial hypoxemia, and intrapulmonary vascular dilatation. Except for pleural effusions associated with liver cirrhosis no other disease of the lungs or the heart is detectable. The structural hallmark of HPS is dilatation of pulmonary precapillary vessels which impairs diffusion-perfusion and causes unequal ventilation-perfusion. The diagnosis of HPS is based on PaO2 measurements when breathing room air and 100% oxygen. The increased intrapulmonary vascul

Strategy that includes serial noninvasive leg tests for diagnosis of thromboembolic disease in patients with suspected acute pulmonary embolism based on data from PIOPED. Prospective Investigation of Pulmonary Embolism Diagnosis.

OBJECTIVE: To estimate the percentage of patients with suspected acute pulmonary embolism in whom a noninvasive diagnosis or exclusion of thromboembolic disease might be safely made on the basis of ventilation-perfusion (VQ) lung scans, single noninvasive tests of the lower extremities, and, in patients with adequate cardiorespiratory reserve, serial noninvasive tests of the lower extremities. METHODS: Calculations were made among 662 patients who participated in the collaborative study Prospective Investigation of Pulmonary Embolism Diagnosis (PIOPED) and who had blood gas values measured wh

Pathophysiology of acute respiratory failure.

The term respiratory failure implies the inability to maintain either the normal delivery of oxygen to tissues or the normal removal of carbon dioxide from the tissues. There are actually three processes involved: the transfer of oxygen across the alveolus, the transport of tissues (by cardiac output), and the removal of carbon dioxide from the blood into the alveolus with subsequent exhalation into the environment. Failure of any step in this process can lead to respiratory failure. An overview of the normal physiology of pulmonary gas exchange and the pathophysiology of respiratory failure i

Mechanisms of hypoxemia in patients with status asthmaticus requiring mechanical ventilation.

Eight consecutive patients (mean +/- SD age, 43 +/- 11 yr) with acute severe asthma (status asthmaticus) requiring assisted ventilation were studied within the first 24 to 48 h of admission, at maintenance FIO2 and while breathing 100% O2, using the multiple inert gas elimination technique. Ventilation-perfusion (VA/Q) inequality was characterized by a marked bimodal blood flow distribution (perfusion to normal and low VA/Q populations) in all but two patients, with a mean of 27.6 +/- 12.3% of the total perfusion present in the low VA/Q ratio units (between 0.1 and 0.005). As a result, the dis

Contribution of central and reflex nervous activity to the rapid increase in pulmonary ventilation at the start of muscular exercise in man.

To investigate the relative contributions of the central and peripheral neural drive to hyperventilation at the onset of muscular exercise, five volunteers were tested during the first ten breaths while performing both voluntary (VM) and passive (PM) ankle rotations with a frequency of 1 Hz and through an angle of 10 degrees. Resulting breathing patterns for the two movements were compared. Hypocapnic hyperventilation, found in both PM and VM, indicated its neural origin. Respiratory changes were higher in VM than in PM. In both experimental conditions, increases in ventilation (VE) depended m

Gallium-67 citrate scintigraphy in pulmonary embolism.

Eight cases of pulmonary embolism were evaluated with Ga-67 citrate scintigraphy. Of the eight patients, all but one showed evidence of abnormal Ga-67 citrate localization within involved regions of the lung as demonstrated by ventilation-perfusion lung scintigraphy. The sole patient who did not demonstrate Ga-67 localization within the lung had marked breast accumulation of the radiotracer which may have obscured lung parenchymal uptake. Since Ga-67 citrate is known to accumulate within inflammatory lung processes, this radiotracer would not have a specific role in differentiating pulmonary e

Gas exchange in chronic pulmonary disease.

Patients with asthma, chronic obstructive lung disease and diffuse interstitial fibrosis were studied. These three groups of patients illustrate very different patterns of VA/Q mismatching in general, consistent with the known pathology of their disease. Some common threads run through all three groups. One is that the level of hypoxemia in an individual depends critically on the interplay between mixed venous PO2 and the degree of VA/Q mismatching. Thus patients with a high cardiac output can have little hypoxemia in the face of severe VA/Q mismatching, while patients with inadequate cardiac

Ventilatory responsiveness to hypercapnic hypoxia during dopamine infusion in humans.

The effect of dopamine on the ventilatory response to transient asphyxia was examined to test the hypothesis that dopaminergic mechanisms modulate carotid body function in humans and to determine whether a change in ventilatory responsiveness is effected by changes in tidal volume or breathing frequency. Five healthy adults were subjected to brief episodes of hypercapnic hypoxia before and at the end of a 45-min intravenous infusion of dopamine hydrochloride (5 micrograms/kg/min). Mean ventilatory response to hypercapnic hypoxia decreased from 1.71 +/- 0.33 L/min/% SaO2 (+/- SD) to 1.30 +/- 0.

Cerebral perfusion pressure and abnormal intracranial pressure wave forms: their relation to outcome in birth asphyxia.

Intracranial pressure (ICP) studies were carried out in 14 infants with severe birth asphyxia and brain damage. A markedly low cerebral perfusion pressure (CPP) was noted in infants who died and in 1 infant who survived with cerebral palsy. The long-term ICP tracing revealed negative waves and plateau waves in 2 infants. Cushing response was noted in 2 infants who had elevated ICP. The value and significance of evaluated CPP and of abnormal waveforms are discussed.

[Comparative study of venous contamination with low FiO2 and with pure oxygen in disease-induced pulmonary edema].

A comparative study of the venous contamination (Qs/Qt = (Cc'O2 - CaO2) / (Cc'O2 - C-vO2) under FIO2 = 0.45 - 0.6 and under FIO2 = 1, was realised in 9 patients with lesions of pulmonary oedema. Increasing the FIO2 lead to an increase in the venous contamination three times (average increase was 36 p. 100 of the initial value), to no change two times and a decrease four times (average decrease of 34 p. 100). The values of the venous contamination under average FIO2 are statistically lower in the group where it rises on increasing the FIO2 than in that in which it falls or remains unchanged (p

Local regulation of collateral ventilation by oxygen and carbon dioxide.

The effects of changes in CO2 and O2 on the mechanics of collateral ventilation were studied in anesthetized paralyzed dogs. A doublelumen catheter was wedged into a peripheral airway obstructing a segment of lung distal to the catheter. Through one lumen of the catheter, air, 5% CO2 in air, 10% CO2 in air, 5% O2 in N2, or 5% CO2 in N2 was infused at a constant flow (V). Pressure (Ps) was monitored through the other lumen. At functional residual capacity the resistance to collateral flor Rcoll = Ps/V. When V was interrrupted , the time for Ps to fall 63% was defined as the time constant for co

Therapeutic benefits of proning to improve pulmonary gas exchange in severe respiratory failure: focus on fundamentals of physiology.

NEW FINDINGS: What is the topic of this review? The use of proning for improving pulmonary gas exchange in critically ill patients. What advances does it highlight? Proning places the lung in its 'natural' posture, and thus optimises the ventilation-perfusion distribution, which enables lung protective ventilation and the alleviation of potentially life-threatening hypoxaemia in COVID-19 and other types of critical illness with respiratory failure. ABSTRACT: The survival benefit of proning patients with acute respiratory distress syndrome (ARDS) is well established and has recently been found

Advances in functional and structural imaging of the human lung using proton MRI.

The field of proton lung MRI is advancing on a variety of fronts. In the realm of functional imaging, it is now possible to use arterial spin labeling (ASL) and oxygen-enhanced imaging techniques to quantify regional perfusion and ventilation, respectively, in standard units of measurement. By combining these techniques into a single scan, it is also possible to quantify the local ventilation-perfusion ratio, which is the most important determinant of gas-exchange efficiency in the lung. To demonstrate potential for accurate and meaningful measurements of lung function, this technique was used

Mixed-expired and end-tidal CO2 distinguish between ventilation and perfusion defects during exercise testing in patients with lung and heart diseases.

BACKGROUND: Mismatching of ventilation to perfusion is found in patients with COPD, left ventricular failure (LVF), and pulmonary vascular diseases. Such mismatching may be due to ventilation or perfusion defects or both. Our primary hypothesis was that pressures of mixed-expired CO2 pressure (Peco(2)), end-tidal Pco(2) pressure (Petco(2)), and their ratios would differ between groups during exercise testing, depending on whether the ventilation/perfusion (V/Q) abnormality was dominantly caused by airways or perfusion defects. METHODS: We administered incremental cycle ergometry tests to 25 n

True shunt in relation to venous admixture in an experimental porcine model of early ARDS.

Using the multiple inert gas elimination technique, ventilation/perfusion (V/Q) relationships were studied in an experimental porcine model of the early Adult Respiratory Distress Syndrome (ARDS) to establish the nature of the increased venous admixture. Six animals served as controls and revealed no major changes apart from a 10% decrease in cardiac output during the 4-h observation period. All control animals showed a shift to a higher mean V/Q of perfusion (Qmean) and a maintained log standard deviation (QSD) throughout the experiment. The distribution was unimodal and centered around a V/Q

Ventilation and cardiac related impedance changes in children undergoing corrective open heart surgery.

Electrical impedance tomography (EIT) can determine ventilation and perfusion relationship. Most of the data obtained so far originates from experimental settings and in healthy subjects. The aim of this study was to demonstrate that EIT measures the perioperative changes in pulmonary blood flow after repair of a ventricular septum defect in children with haemodynamic relevant septal defects undergoing open heart surgery. In a 19 bed intensive care unit in a tertiary children's hospital ventilation and cardiac related impedance changes were measured using EIT before and after surgery in 18 spo

[Pulmonary ventilation indices of patients following the surgical correction of mitral stenosis].

The method of spiroveloergometry was used in 97 patients after operation of mitral commissurotomy for studying reserves of pulmonary ventilation and its relation to the level of physical working capacity. The results obtained have shown that in patients of worse functional classes the ventilation potency of the lungs deteriorated which resulted in the restricted physical working capacity. It should be necessarily taken into consideration when making programs for the rehabilitation of cardiosurgical patients.

Predictors of outcome of prematurely born infants with pulmonary interstitial emphysema.

AIM: To determine how oxygenation, ventilation efficiency and tidal volume requirements changed with the development of pulmonary interstitial emphysema (PIE) and whether in affected patients a composite gas exchange index predicted death or bronchopulmonary dysplasia (BPD). METHODS: Infants who developed PIE from 2010 to 2016 were identified. The oxygenation index (OI), ventilation efficiency index (VEI), ventilation to perfusion ratio and inspiratory tidal volume were calculated before radiological evidence of PIE (pre-PIE) and at the worst PIE radiographic appearance (PIE-worst).
